Coronavirus disease was first reported in December 2019, and the World Health Organization declared it as a pandemic on March 11, 2020. The virus is known to attack various vital organs, including ...the respiratory system. Patients sometimes require positive pressure ventilation and tracheostomy. Because tracheostomy is a droplet-spreading procedure, medical staff should protect themselves against the risk of transmission of this contagious viral disease. In our case, we performed tracheostomy for a 70-year-old man with coronavirus disease 2019 (COVID-19) who had required more oxygen with gradual weakness of respiratory muscle to maintain his arterial oxygen saturation. We focused on the risks of the medical staffs and patients, and minimized them at the same time using temporary balloon over-inflation, pre-operative adjustment of endotracheal tube position, and attachment of a transparent film dressing to the surgical field without stopping the ventilator while following routine safety measures. Fourteen days after the tracheostomy, all participating medical staff members were healthy and asymptomatic. The patient was discharged 105 days after the COVID-19 diagnosis.
The Kumaraswamy distribution is very similar to the Beta distribution, but has the important advantage of an invertible closed form cumulative distribution function. The parameterization of the ...distribution in terms of shape parameters and the lack of simple expressions for its mean and variance hinder, however, its utilization with modeling purposes. The paper presents two median-dispersion re-parameterizations of the Kumaraswamy distribution aimed at facilitating its use in regression models in which both the location and the dispersion parameters are functions of their own distinct sets of covariates, and in latent-variable and other models estimated through simulation-based methods. In both re-parameterizations the dispersion parameter establishes a quantile-spread order among Kumaraswamy distributions with the same median and support. The study also describes the behavior of the re-parameterized distributions, determines some of their limiting distributions, and discusses the potential comparative advantages of using them in the context of regression modeling and simulation-based estimation.
Trauma with prolonged shock can cause systemic capillary leak syndrome regardless of the site of injury and a transfusion can aggravate it. The systemic capillary leak induces both an abdominal ...compartment syndrome and pulmonary edema, and a transfusion can aggra-vate these sequelae within hours. In our case, 21-year-old man with a penetrating injury in his left thorax experienced delay in rescue and definitive surgery. To manage life-threatening shock, massive blood transfusion and crystalloids had been infused. Cardiopulmonary cerebral resuscitations were performed 2 times during the surgery. Massive amount of pulmonary secretions emitted from his airways with severe hypoxia along with development of massive ascites causing abdominal compartment syndrome, while the surgery was underway. After temporary abdominal closure, he was moved to the intensive care unit and underwent venovenous extracorporeal membranous oxygenation. He recovered without any notable complications. It is important to prevent and correct the shock rapidly by appropriate rescue, controlling the source and infusing less amount of crystalloid and transfusion.
When a patient with severe trauma is admitted to the emergency room (ER), they are evaluated before transfer to either the intensive care unit (ICU) or operating room. To minimize the time until a ...definitive treatment can be provided, direct operating room resuscitation can be performed. In this hospital the ER was closed during the hospital’s transition to a coronavirus disease 2019-dedicated hospital, and direct ICU resuscitation for patients with trauma was performed for a short period. To perform effective trauma resuscitation, all ICU beds were reorganized to achieve a modified, experienced nurse: patient ratio (1:2-3) and 2 beds were assigned for trauma ICU resuscitation alone. The equipment for initial resuscitation was installed and ICU nurses received training. Consultations with the hospital administration, nursing, and pharmaceutical departments were completed in advance to avoid formal problems. Conversion of the ICU for direct resuscitation procedures was performed in 4 patients.
In August, 2011, the Korean Public Health Surveillance declared an outbreak of pulmonary disease due to the inhalation of humidifier disinfectants (HDs), which led to approximately 20,000 deaths. In ...March, 2020, the World Health Organization declared coronavirus disease-2019 (COVID-19) a pandemic. In this Case Report, we present a rare case of a patient who inhaled toxic HDs and developed COVID-19. He was young and had a low risk of severe COVID-19, however, he had a critical course to recovery. He was admitted to the intensive care unit and administered high-flow oxygen via a nasal cannula. He received dexamethasone injections each day for 10 days and his condition began to improve on hospital Day 6, although radiographical findings revealed no improvement. He was discharged on hospital Day 26. Despite the patient’s chronic lung disease becoming asymptomatic, HDs could be an important risk factor affecting the clinical course of COVID-19.
This paper proposes the code acquisition scheme on a MIMO channel for preamble search in a MIMO-CDMA uplink system. The performance of a ML code acquisition technique based on transmit and receive ...beamforming is analyzed by considering the detection probability. The acquisition performance and MAT for a MIMO code acquisition system are numerically evaluated. It is shown that the multiple transmit antennas can give the code acquisition system a transmit beamforming gain and result in much better performance than the corresponding scheme with a SIMO case.

The goal of this study was to improve ...post-thaw motility of boar semen by supplementing the thawing medium with a phosphodiesterase inhibitor, Zardaverine.
Thawed boar semen samples were treated with different concentrations of Zardaverine (0, 20, 50, 75, 100 µM) and the motility was evaluated using a computer-assisted sperm analyser. Toxic effects (sperm viability, DNA integrity, mitochondrial activity) were examined by eosin-nigrosin staining, terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) assay and MitoTracker.
Sperm motility values included curvilinear velocity, rectilinear speed, average value, linearity index, straightness index, and progressive motility. The kinetic values were significantly higher with the 50 uM Zardaverine supplementation compared to non-treated control. Furthermore, there were no toxic effects of the Zardaverine treatment.
The dual phosphodiesterase inhibitor (type 3/4) Zardaverine significantly enhanced the motility of thawed spermatozoa without adverse effects.
A wide range of techniques have been developed to separate X or Y- chromosome-bearing sperm. In particular, bovine semen sex-sorted by using flow cytometry based on differences in the amount of DNA ...between X and Y chromosome bearing sperm is used in dairy farms. The first piglets were produced using sex-sorted sperm 30 years ago. However, sexed sperm have not been commercially available in pigs because the flow cytometry technique is not capable of sorting the high number of sperm required for porcine artificial insemination (AI), and the prolonged exposure to an electrical filed might damage to the DNA in sperm. The purpose of this study was to evaluate a boar sperm sorting method based on magnetic nanoparticles. A flow cytometer assay verified the efficacy of the magnetic nanoparticles (> 90% of sex-sorted sperm). In addition, a duplex polymerase chain reaction (PCR) assay using sex chromosome specific genes including SRY (sex-determining region Y; male), ZFY (zinc finger protein Y-linked; male), and ZFX (zinc finger protein X-linked; female) showed that in vitro fertilized porcine embryos by X and Y-chromosome bearing sperm were 100% female (40/40) and 72% female (35/48), respectively, at 8-cell or morula stages, suggesting that the sex-sorted sperm were fertile. In conclusion, our findings suggest that the sex-sorted method based on magnetic nanoparticles can be utilized for porcine sex-sorted AI.
