Alveolar hemorrhage is a severe complication of systemic lupus erithematosus (SLe) associated with high mortality. Treatment includes administration of steroids and cyclophosphamide. There is a single case reported in the literature in which recombinant activatedfactor VII (rFVIIa) was used to control severe hemorrhage secondary to alveolitis unresponsive to standard treatment. We report the case of a patient with SLE who developed severe alveolar hemorrhage unresponsive to standard measures, but who was successfully treated with rFVIIa.
Rearrangement analysis of immunoglobulin genes is an exceptional opportunity to look back at the B lymphocyte differentiation during ontogeny and the subsequent immune response, and thus to study the ...selective pressures involved in autoimmune disorders. In a recent study to characterize the antigenic specificity of B lymphocytes during T1D progression, we generated hybridomas of islet-infiltrating B lymphocytes from NOD mice and other related strains developing insulitis, but with different degrees of susceptibility to T1D. We found that a sizable proportion of hybridomas produced monoclonal antibodies reactive to peripherin, an intermediate filament protein mainly found in the peripheral nervous system. Moreover, we found that anti-peripherin antibody-producing hybridomas originated from B lymphocytes that had undergone immunoglobulin class switch recombination, a characteristic of secondary immune response. Therefore, in the present study we performed immunoglobulin VL and VH analysis of these hybridomas to ascertain whether they were derived from B lymphocytes that had undergone antigen-driven selection. The results indicated that whereas some anti-peripherin hybridomas showed signs of oligoclonality, somatic hypermutation and/or secondary rearrangements (receptor edition and receptor revision), others seemed to directly derive from the preimmune repertoire. In view of these results, we conclude that anti-peripherin B lymphocytes are positively selected and primed in the course of T1D development in NOD mice, and reinforce the idea that peripherin is a relevant autoantigen targeted during T1D development in this animal model.
Objetivo: Determinar si el acortamiento del cuello uterino en pacientes sintomáticas es predictor de parto prematuro en o antes de las 35 semanas y dentro de 7 días. Método: Se estudiaron 852 ...pacientes que consultaron por dinámica uterina, a las que se midió el cuello uterino al ingreso. Las pacientes fueron manejadas de acuerdo a las Guías Clínicas del Hospital. Se excluyeron las pacientes en trabajo de parto, con membranas rotas o con cerclaje. Resultados: La edad gestacional promedio al ingreso fue 31,5 semanas (rango: 24,0-34,9). Se presentó parto prematuro a las 35 semanas o antes en 61 casos (7,2%), y dentro de los 7 días del ingreso en 14 pacientes (1,6%). Fueron predictores independientes significativos (OR; IC95%) para un parto <35 semanas las siguientes variables: antecedente de prematurez (2,03; 1,06-3,89), metrorragia (6,87; 2,83-16,65) y canal cervical <25 mm (3,31; 1,92-5,7). El análisis con curvas ROC demostró que el valor de corte que mejor predice un parto <35 semanas y dentro de 7 días de ingresada, es un cuello uterino <19,5 mm (p<0,05). Conclusiones: El acortamiento del canal cervical en pacientes sintomáticas, particularmente con un canal <20 mm, se asocia con un riesgo significativo de parto prematuro <35 semanas y dentro de 7 días de ingresada. La medición de canal cervical ayuda a distinguir a las pacientes en riesgo, con el fin de concentrar esfuerzos en ese grupo de embarazadas.Objective: To determine if the shortening of cervical length (CL) in women with threatened preterm labor could predict delivery at or before 35 weeks' gestation and within 7 days of presentation. Method: Sonogra-phic measurement of CL was done in 852 women with singleton pregnancies presenting with painful uterine contractions. Women in labor, with ruptured membranes or those with cervical cerclage were excluded. Re-sults: Median gestational age at presentation was 31.5 (range: 24.0-34.9) weeks and median cervical length was 31.5 (range: 3-61) mm. Delivery within 7 days occurred in 14 (1.6%) and delivery at or before 35 weeks, in 61 (7.2%) cases. Significant (OR; 95%CI) independent predictors of delivery at or before 35 weeks were vaginal bleeding (6.87; 2.83-16.65), cervical length (3.31; Cl 1.92-5.70) and a historyof preterm birth (2.03; 1.06-3.89). ROC curve analysis showed that a cervical length shorter than 19.5 mm (p<0.05) was the best cutoff valué for predicting a delivery at or before 35 weeks' gestation and within 7 days of presentation. Con-clusions: Shortening of cervical length in threatened preterm labor, especially when shorter than 19.5 mm, is associated with a significant risk of preterm labor <35 weeks' gestation and within 7 days of admission. Sonographic measurement of cervical length helps to recognize patients at risk for preterm birth, in order to focus interventions in that group of patients.
Antecedentes: La gran mayoría de las curvas ultrasonográficas de crecimiento intrauterino han sido basadas en una edad gestacional (EG) determinada por fecha de última menstruación segura y ...confiable, la que es discordante con la ecografía de primer trimestre hasta en un 20%. Objetivo: Presentación de un normograma para diámetro biparietal (DBP), circunferencia abdominal (CA) y fémur (F), en embarazos únicos con EG calculada por ecografía de primer trimestre. Métodos: En la Unidad de Ultrasonografía de la Clínica Alemana de Santiago, durante 2000 y 2004, se analizaron todos los embarazos simples con eco-grafía de primer trimestre, definida por embrión vivo, único, con longitud céfalo-nalgas entre 3 y 84 mm. Se revisaron todos los informes ecográficos realizados durante el mismo embarazo, con informe de DBP, CA y F. Se seleccionaron 4.762 ecografías realizadas a 2.614 embarazadas. Se comparó los resultados con curvas de referencia nacional e internacional. Resultados: El DBP de nuestra población a las 22 semanas es significativamente mayor que las publicadas por Chitty y Vaccaro, y similares a las de Kurmanavicius y Snijders; mientras que la CA es mayor y el fémur es similar a las demás curvas de referencia. Conclusión: Las curvas obtenidas y determinadas por la ecografía de primer trimestre son diferentes con las curvas de referencias comparadas. Esto deberá considerarse dado que la ecografía precoz para el cálculo de la edad gestacional se ha generalizado en la práctica obstétrica actual.

The choice may affect directly on diagnosis of IUGR. Most of these charts defined gestational age by last menstrual period (LMP), nevertheless, LMP has shown to be discordant with first trimester biometry in up to 20% of the cases in the best of clinical situations. Objective: To build a normogram of fetal biometry based on first trimester CRL and not on LMP. Methods: Between 2000 and 2004, 20400 scans were performed. First trimester CRL was used for dating second and third trimester biometries. 4762 scans in 2614 pregnant women were used to build our normal biometry charts. Our results were compared to other reference charts. Results: BDP at 22 weeks'GA form our population was significantly greater than that from Chitty and Vaccaro and similar to Kurmanavicius y Snijders. The AC of our population was greater and FL was similar to those of reference charts. Conclusión: The fetal biometry of our population was statistically different from that of local and in-ternational reference charts. This is important as gestational age is determined by first trimester ultrasound for obstetrical use.
Most of our knowledge of the antigenic repertoire of autoreactive B lymphocytes in type 1 diabetes (T1D) comes from studies on the antigenic specificity of both circulating islet-reactive ...autoantibodies and peripheral B lymphocyte hybridomas generated from human blood or rodent spleen. In a recent study, we generated hybridoma cell lines of infiltrating B lymphocytes from different mouse strains developing insulitis, but with different degrees of susceptibility to T1D, to characterize the antigenic specificity of islet-infiltrating B lymphocytes during progression of the disease. We found that many hybridomas produced mAbs restricted to the peripheral nervous system (PNS), thus indicating an active B lymphocyte response against PNS elements in the pancreatic islet during disease development. The aim of this study was to identify the autoantigen recognized by these anti-PNS mAbs. Our results showed that peripherin is the autoantigen recognized by all anti-PNS mAbs, and, therefore, a relevant neuroendocrine autoantigen targeted by islet-infiltrating B lymphocytes. Moreover, we discovered that the immune dominant epitope of this B lymphocyte immune response is found at the C-terminal end of Per58 and Per61 isoforms. In conclusion, our study strongly suggests that peripherin is a major autoantigen targeted during T1D development and poses a new question on why peripherin-specific B lymphocytes are mainly attracted to the islet during disease.
The paper describes the organization, goals, and results of the sEXism Identification in Social neTworks (EXIST) challenge, a shared task proposed for the first time at IberLEF 2021. EXIST 2021 ...proposes two challenges: sexism identification and sexism categorization of tweets and gabs, both in Spanish and English. We have received a total of 70 runs for the sexism identification task and 61 for the sexism categorization challenge, submitted by 31 different teams from 11 countries. We present the dataset, the evaluation methodology, an overview of the proposed systems, and the results obtained. The final dataset consists of more than 11,000 annotated texts from two social networks (Twitter and Gab) and its development has been supervised and monitored by experts in gender issues.
