The purpose of this article was to review the literature concerning the need to splint implants together when restoring them with a provisional restoration immediately after implant placement.
The ...literature is reviewed concerning the rationale for splinting teeth and reports concerning the efficacy of splinting implants together. Based on this team’s experience with a prospective series of consecutive 2 to 5 unit provisionalization cases, guidelines are included with case examples for understanding the technique.
As long as canine guidance is present, and occlusion is stable, multiunit single quadrant restorations do not need to be splinted when provisionalized.
There is insufficient knowledge of the strength of all-ceramic crowns bonded to natural teeth to warrant the use of all-ceramic crowns in place of metal-ceramic crowns.
The purpose of this study was ...to evaluate and compare fracture resistance of crowns made of 3 different types of 2 all-ceramic crown systems—0.4-mm and 0.6-mm aluminum oxide coping crowns and zirconia ceramic coping crowns—and metal-ceramic crowns.
Forty intact, noncarious human maxillary central incisors were divided into 4 groups (n=10): Group MCC (control), metal-ceramic crown (JRVT High Noble Alloy); Group AC4, crown with 0.4-mm aluminum oxide coping (Procera AllCeram); Group AC6, crown with 0.6-mm aluminum oxide coping (Procera AllCeram); and Group ZC6, crown with 0.6-mm zirconia ceramic coping (Procera AllZirkon). Teeth were prepared for complete-coverage all-ceramic crowns so that a final dimension of 5.5 ± 0.5 mm was achieved incisocervically, mesiodistally, and faciolingually. A 1.0-mm deep shoulder finish line was used with a rounded internal line angle. All restorations were treated with bonding agent (Clearfil SE Bond) and luted with phosphate-monomer–modified adhesive cement (Panavia 21). Fracture strength was tested with a universal testing machine at a crosshead speed of 2 mm per minute with an angle of 30 degrees to the long axis of the tooth after restorations were stored in 100% relative humidity of a normal saline solution for 7 days. The mode of fracture was examined visually. Means were calculated and analyzed with 1-way ANOVA and Tukey's HSD (α=.05).
The means of fracture strength were: Group MCC, 405 ± 130 N; Group AC4, 447 ± 123 N; Group AC6, 476 ± 174 N; and Group ZC6, 381 ± 166 N. There was no significant difference between groups (
P=.501). The mode of failure for all specimens was fracture of the natural tooth.
There was no significant difference in the fracture strength of the teeth restored with all-ceramic crowns with 0.4- and 0.6-mm aluminum oxide copings, 0.6-mm zirconia ceramic copings, and metal ceramic crowns.

Conventional complete denture prosthetics require several appointments to register the maxillomandibular relationship and evaluate the esthetics. The fabrication of milled complete dental prostheses ...with digital scanning technology may decrease the number of appointments. The step-by-step method necessary to obtain impressions, maxillomandibular relation records, and anterior tooth position with an anatomic measuring device is described. The technique allows the generation of a virtual denture, which is milled to exact specifications without the use of conventional stone casts, flasking, or processing techniques.

The authors conducted a literature search using PUBMED and Ovid databases. They considered for review 31 articles in English from 1969 to 2003 that pertained to immediate loading of the anterior mandible. The authors developed a technique to provide a bar-supported prosthesis on the day of surgery. They treated five patients and followed them up for at least six months. The preliminary results are presented.
This literature review demonstrated that immediate loading of anterior mandibular implants is an acceptable method, with predictable results. This case series demonstrates the potential for delivering a final bar on the day of surgery, based on the current evidence and clinical application.
The method described provides patients with immediate prosthetic restorations and a decreased treatment time compared with that for the traditional two-stage implant approach.

This prospective clinical trial examined the effect on teeth and implants when rigidly or non-rigidly connected in a cross-arch model.
Thirty patients received 2 implants, 1 on each side of the ...mandible, and were restored with 3-unit fixed partial dentures connected either rigidly or non-rigidly to an abutment tooth. Patients were followed for at least 5 years post-restoration.
Repeated-measures analysis revealed no significant difference in crestal bone loss at implants (rigid versus non-rigid methods). An overall significant difference (P < .001) was found comparing methods for teeth. Paired t tests revealed no significant differences in crestal bone levels for implants or teeth at the 5-year recall. Kaplan-Meier methods and the Cox proportional hazards model showed no differences between attachment methods with regard to success based on survival and bone loss criteria. During the 5-year recall period, 1 implant (rigid side) was removed. Four implants developed bone loss greater than 2 mm during the course of this trial. One tooth on the rigid side and 2 teeth on the non-rigid side had greater than 2 mm of crestal bone loss and were removed secondary to fractures. In all, 5 abutment teeth were removed, all of which had been treated with root canal therapy and fractured at the interface of the post within the tooth. There was no clear relationship of tooth fracture to attachment. Repeated-measures analysis of mobility values revealed no significant changes over the time course of this study, and paired t tests revealed no statistically significant differences between implants for mobility. Repeated-measures analysis and paired t tests for probing depth revealed no significant changes over the time course of this study. There were no significant differences in soft tissue indices for either attachment method. The percentage of patients who had measurable intrusion was 66% for the non-rigid group, and 44% for the rigid group; 25% of the non-rigid teeth had greater than 0.5 mm intrusion, compared with 12.5% for the rigid group. For the 2 time periods evaluated, there was no significant increase in intrusion over time. The non-rigid-side implant required more nonscheduled visits to treat problems than the rigid implant and the teeth.
Most patients were treated successfully with rigid or non-rigid attachment of implants to teeth.
The high incidence of intrusion and non-scheduled patient visits suggest that alternative treatments without connecting implants to teeth may be indicated.
Edentulous patients desire restoration of their chewing ability as soon as possible after placement of dental implants. The purpose of this article is to provide clinicians with evidence that ...immediate loading of implants placed into the anterior mandible can predictably provide the patient with early functional rehabilitation. Two methods are presented that illustrate delivery of the final restoration or a provisional implant-borne prosthesis immediately after implant placement.
A literature search was performed which produced 14 articles in the English literature that provided sufficient evidence that immediate loading is now not experimental and can be recommended as an acceptable treatment alternative.
Two practical methods to achieve immediate function are presented in a step-wise manner to illustrate how to deliver this service to the patient.
Based on our literature review, immediate loading of the edentulous mandible with an implant-borne restoration is an acceptable and predictable method to deliver efficient return of function for the edentulous patient.

The indications for implant dentistry continue to increase, enabling the restoration of partially and totally edentulous patients with greater success and predictability. Recent goals for implant ...dentistry include simplifying the involved procedures, reducing the duration of therapy for the patient and clinician, and enabling the use of conventional prosthodontic techniques for implant-supported restorations. This article reviews key developments in implantology and highlights the various design characteristics of internal abutment connection implants, demonstrating their clinical application in a detailed case presentation.
