La physiopathologie des infections déterminées par les espèces du complexe Scedosporium apiospermum est encore mal connue. La capacité de ces champignons à accéder aux ressources en fer constitue ...cependant un facteur important de leur virulence, le fer étant impliqué dans de nombreux processus biologiques essentiels. Néanmoins, malgré son abondance dans l’environnement, il ne peut être assimilé directement par les microorganismes, qui ont donc développé diverses stratégies pour son acquisition. Le mécanisme le plus répandu consiste dans la sécrétion de chélateurs du fer appelés sidérophores. Ces métabolites secondaires séquestrent le fer extracellulaire et permettent son assimilation par le microorganisme via des transporteurs membranaires spécifiques (TMS).
Chez les Ascomycètes, les sidérophores sont produits par synthèse peptidique non-ribosomale, qui s’effectue via des protéines appelées non-ribosomal peptide synthetases (NRPS). Il a été montré dans notre unité que, dans des conditions de carence en fer, S. apiospermum est capable de produire un sidérophore, le N α-méthylcoprogène B. Cependant, les gènes assurant la synthèse de cet hydroxamate restent inconnus, et l’implication d’autres sidérophores dans le métabolisme du fer n’a jamais été investiguée, notamment lorsque le champignon se développe dans un environnement riche en fer lié à des protéines comme c’est le cas chez l’hôte. L’analyse bioinformatique du génome de S. apiospermum nous a permis d’identifier 7 orthologues des gènes SID impliqués dans la synthèse de la fusarinine C et de la ferricrocine chez Aspergillus fumigatus, ainsi que 9 gènes codant des protéines de type MFS potentiellement impliquées dans l’assimilation des complexes ferrisidérophores, et 13 codant des perméases à Fe++ ou Fe+++ ou des enzymes potentiellement impliquées dans des systèmes d’oxydo-réduction du fer. De manière intéressante, il s’avère que les gènes potentiellement impliqués dans la synthèse du N α-méthylcoprogène B sont regroupés en cluster comme il est fréquemment observé pour les sidérophores des Ascomycètes. Les gènes régulant le transport transmembranaire du fer sont, quant à eux, répartis de façon hétérogène dans le génome.
Pour confirmer le rôle de ces gènes, nous avons étudié leur niveau d’expression par RT-qPCR après 5 jours de culture en milieu carencé en fer ou au contraire en présence d’un excès de fer libre (Fe++ ou Fe+++) ou lié à la transferrine. Contrairement à nos attentes, l’expression des orthologues des gènes SID n’est pas affectée lors d’une carence martiale. Il est cependant possible que ces gènes soient induits seulement lors de la phase linéaire de croissance et ces expériences seront reproduites avec des temps d’incubation plus courts. Néanmoins, nous pouvons déjà affirmer que les gênes contr6lant la biosynthèse des sidérophores et des TMS sont réprimés en présence d’un excès de fer lie a la transferrine, et que leur expression reste inchangée en présence d’un excès de fer libre.
La compréhension des mécanismes régulant le métabolisme du fer via les NRPS permettra de mieux appréhender la physiopathologie des infections a S. apiospermum et pourra servir de base a la définition de nouvelles stratégies antifongiques. En effet, les NRPS qui sont absentes chez l’homme, pourraient constituer des cibles thérapeutiques pour lutter contre les infections à S. apiospermum.

Background Candida haemulonii complex-related species are pathogenic yeasts closely related to Candida auris with intrinsic antifungal resistance, but few epidemiological data are available. ...Methodology/Principal findings We analyzed clinical and demographic characteristics of patients with fungemia due to C . haemulonii complex and related species ( C . pseudohaemulonii , C . vulturna ) reported in France during 2002–2021, and compared them to data of C . parapsilosis fungemia, as they all can be commensal of the skin. We also conducted a study on adult inpatients and outpatients colonized by C . haemulonii complex, managed at the University Hospital of Martinique during 2014–2020. Finally, we performed a literature review of fungemia due to C . haemulonii complex and related species reported in Medline (1962–2022). In total, we identified 28 fungemia due to C . haemulonii complex in France. These episodes were frequently associated with bacterial infection (38%) and high mortality rate (44%), and differed from C . parapsilosis fungemia by their tropical origin, mainly from Caribbean and Latin America. All isolates showed decreased in vitro susceptibility to amphotericin B and fluconazole. In Martinique, we found that skin colonization was frequent in the community population, while colonization was strongly associated with the presence of foreign devices in ICU patients. The literature review identified 274 fungemia episodes, of which 56 were individually described. As in our national series, published cases originated mainly from tropical regions and exhibited high crude mortality. Conclusions/Significance Multidrug-resistant C . haemulonii complex-related species are responsible for fungemia and colonization in community and hospital settings, especially in tropical regions, warranting closer epidemiological surveillance to prevent a potential C . auris- like threat.

A. terreus
spondylodiscitis in a 20-year-old male with a past history of recurrent, incompletely treated pulmonary tuberculosis. Clinical signs at the time of admission included cough, low-grade fever, general weakness and left-sided back pain. Histological examination of spinal biopsy samples revealed lesions of necrosis, granulomatous inflammation and septate hyphae with acute-angle branching.
A. terreus
was recovered from culture. The patient received antifungal therapy with voriconazole plus caspofungin and underwent surgical debridement. Further investigations revealed no cause of primary immunodeficiency such as chronic granulomatous disease, severe combined immunodeficiency syndrome or disorders of the IL-12/IFNγ signaling pathway. Moreover, HIV serological tests resulted negative and the patient was not under immunosuppressive therapy. Unfortunately, owing to precarity and medication non-adherence, vertebral sequelae occurred. This new report emphasizes the need to consider a fungal infection in patients with spondylodiscitis, regardless of the immune status.
A 36-year-old woman returning from holiday in Portugal presented with bilateral ocular pain and acute visual loss. Eye examination revealed an inflammation in the anterior segment associated with ...corneal keratitis. During her trip, she wore contact lenses and rinsed the storage case using tap water. Corneal scrapings, contact lenses and cleaning solution from the case were inoculated onto agar plates supplemented with heat-inactivated bacteria. Bilateral Acanthamoeba keratitis was diagnosed, as microscopic examination of cultures from the last two samples revealed typical, spherical double-walled cystic structures (Fig. 1).
