This study was performed to establish an experimental method for the investigation of interactions between ethanol and drugs under predictable and controlled conditions. The model was tested with ...flumazenil (Ro 15-1788), a short-acting benzodiazepine antagonist with an elimination half-life of 1 h. Six healthy volunteers (5 males, 1 female) were administered ethanol by intravenous infusion with stepwise changing rates. The infusion rates were adapted to each subject on the basis of individual disposition parameters of ethanol, which were derived from preceding short-term infusions of 120 min duration (1.0 mg/kg in males, 0.8 mg/kg in the female). This two-step procedure led to individual ethanol plasma levels between 1.47 +/- 0.04 and 1.71 +/- 0.03 g/L, which were reached after 2.5 h and thereafter maintained over another 6 h. Within the period of constant ethanol levels, single doses of flumazenil and placebo, respectively, were injected intravenously as a bolus (2 min) in a double-blind fashion according to a randomized two-way crossover design. Three subjects received a dose of 0.10 mg/kg of flumazenil, and the remaining three subjects received a dose of 0.20 mg/kg. Evaluation of the plasma concentration time curves of flumazenil did not reveal evidence of an effect of ethanol on the pharmacokinetics of this drug.
Efavirenz (EFV) has been shown to be a highly effective HIV therapy in antiretroviral-naïve patients when used with nucleoside reverse transcriptase inhibitors.
The study participants were 314 ...patients, 45 of whom had not been previously treated with any antiretroviral medication. The other patients were heavily pretreated for about 3 years (1,047 days); 34 with two nucleoside reverse transcriptase inhibitors, 147 with triple therapy, and 88 with a quadruple regimen.
Suppression of plasma HIV-1 RNA to <50 copies/mL and <500 copies/mL was achieved in 56% and 72% of the pretreated patients and in 82% and 91% of the naïve patients, respectively, at week 80 (intention-to-treat analysis: noncompleters = failure: 10% and 15% and 20% and 22%, respectively). The viral load reduction at week 80 was 0.7 log(10) for the pretreated patients and 2.6 log(10) for the naïve patients. CD4 cell counts increased from 386 to 474 cells/microL at week 80 in the pretreated group and from 264 to 431 in the naïve patients. 118 patients discontinued the treatment due to adverse events (37 patients due to nervous system symptoms and 15 patients because of exanthema). There were no AIDS-defining events in the group of antiretroviral-treated patients.
EFV in combination with nucleoside reverse transcriptase inhibitors as antiretroviral therapy was potent and effective in reducing viral load, mainly in treating therapy-naïve patients and in preventing AIDS-defining events.

The piezoelectric (and electrostrictive) responses in these sections have been ...measured by means of an interferometric setup and the pyroeletric properties have been determined using the laser intensity modulation method (LIMM).
We report about the 6-month follow-up of 28 consecutive patients treated with a new tantalum stent (Wiktor¿ stent, Medtronic, Inc.). Indication for stenting was the prevention of restenosis in eight ...patients (restenosis group), and threatening or acute closure after PTCA in 20 patients (acute closure group). Twenty-eight of 30 stents were successfully positioned in 27 of 28 patients (96%), whereas implantation failed twice in one patient. Immediate stent occlusion developed in two patients in the acute closure group (7.4%) Subacute stent occlusion was observed in three patients (11%), one in the restenosis group, two in the acute closure group, between 3 and 5 days after implantation. Coronary bypass surgery had to be performed in four patients (15%): one patient after failed stent placement, two after acute, and one after subacute stent thrombosis. Major bleeding complications related to the anticoagulative drug regimen occurred in nine patients (33%). Three patients (11%) died for reasons most probably not related to stent implantation. A 6-month angiographic follow-up revealed restenosis in two of 19 patients (11%), one patient in each group. Sixteen of the 27 stented patients (59%) reached 6-month follow-up without death, acute or subacute stent thrombosis, or restenosis. It is concluded that the Wiktor stent can be placed with a high rate of success. It may also reduce the risk of restenosis. The stent also offers the possibility to circumvent emergency bypass surgery in case of PTCA related vessel occlusion. Acute and subacute stent occlusion still remains an unsolved topic.
It is widely expected that the coming decade will witness the first direct detection of gravitational waves (GWs). The ground-based LIGO and Virgo GW observatories are being upgraded to advanced ...sensitivity, and are expected to observe a significant binary merger rate. The launch of The Laser Interferometer Space Antenna (LISA) would extend the GW window to low frequencies, opening new vistas on dynamical processes involving massive (M >~ 10^5 M_Sun) black holes. GW events are likely to be accompanied by electromagnetic (EM) counterparts and, since information carried electromagnetically is complementary to that carried gravitationally, a great deal can be learned about an event and its environment if it becomes possible to measure both forms of radiation in concert. Measurements of this kind will mark the dawn of trans-spectral astrophysics, bridging two distinct spectral bands of information. The aim of this whitepaper is to articulate future directions in both theory and observation that are likely to impact broad astrophysical inquiries of general interest. What will EM observations reflect on the nature and diversity of GW sources? Can GW sources be exploited as complementary probes of cosmology? What cross-facility coordination will expand the science returns of gravitational and electromagnetic observations?
The effect of a new, powerful diuretic on biochemical parameters, urine output, central venous pressure, blood pressure, and cerebrospinal fluid pressure in patients with supratentorial intracerebral ...tumors who showed signs and symptoms of increased intracranial pressure was tested. When compared to an untreated control group and to the steady-state data of each patient, CSF pressure was significantly reduced using a dose of 240 mg of the diuretic. The 120 mg dosage did not produce significant results. Normalization of increased cerebrospinal fluid pressure was not completely obtained using either dose. Used alone, this substance is not suitable for treatment of increased intracranial pressure due to brain edema in patients with intra-cerebral tumors. It might, however, be useful in combination with other medications.
