Summary Objective To investigate the efficacy of different electrical stimulation (ES) therapies in pain relief of patients with knee osteoarthritis (OA). Method Electronic databases including ...MEDLINE, Embase and Cochrane Library were searched through for randomized controlled trials (RCTs) comparing any ES therapies with control interventions (sham or blank) or with each other. Bayesian network meta-analysis was used to combine both the direct and indirect evidence on treatment effectiveness. Results 27 trials and six kinds of ES therapies, including high-frequency transcutaneous electrical nerve stimulation (h-TENS), low-frequency transcutaneous electrical nerve stimulation (l-TENS), neuromuscular electrical stimulation (NMES), interferential current (IFC), pulsed electrical stimulation (PES), and noninvasive interactive neurostimulation (NIN), were included. IFC is the only significantly effective treatment in terms of both pain intensity and change pain score at last follow-up time point when compared with the control group. Meanwhile, IFC showed the greatest probability of being the best option among the six treatment methods in pain relief. These estimates barely changed in sensitivity analysis. However, the evidence of heterogeneity and the limitation in sample size of some studies could be a potential threat to the validity of results. Conclusion IFC seems to be the most promising pain relief treatment for the management of knee OA. However, evidence was limited due to the heterogeneity and small number of included trials. Although the recommendation level of the other ES therapies is either uncertain (h-TENS) or not appropriate (l-TENS, NMES, PES and NIN) for pain relief, it is likely that none of the interventions is dangerous. Design This systematic review and network meta-analysis covered ...12 trials in total. Electronic databases including MEDLINE, Embase and Cochrane Library were searched through to identify randomized controlled trials comparing the two modes of US with control interventions (sham or blank) or with each other. Bayesian network meta-analysis was used to integrate both the direct and indirect evidences on treatment effectiveness. Results Pulsed US (PUS) is more effective in both pain relief and function improvement when compared with the control group; but for continuous US (CUS), there is only a significant difference in pain relief in comparison with the control group. In addition, no matter in terms of pain intensity or function at the last follow-up time point, PUS always exhibited a greater probability of being the preferred mode. However, the evidence of heterogeneity and the limitation in sample size of some studies could be a potential threat to the validity of results. Conclusions Our findings indicated that PUS, with a greater probability of being the preferred mode, is more effective in both pain relief and function improvement when compared with the control group. However, CUS could only be considered as a pain relief treatment in the management of knee OA. The findings also confirmed that none of these modes is dangerous. The traditional approaches for evaluating the stability of slopes or earth dams subjected to water drawdown are performed under plane-strain two-dimensional (2D) condition. Three-dimensional (3D) ...effect is neglected in assessment of the safety of a slope limited by rigid structures or a dam constrained by a narrow valley. Based on the kinematic approach of limit analysis, a 3D rotational failure mechanism is adopted here to investigate the influence of water drawdown on stability of 3D slopes. Several stability charts are presented to conveniently estimate the safety factor of 3D slopes under four different types of drawdown processes. An example is given to demonstrate the difference in the safety factors obtained from 2D and 3D analyses. When a slope is constrained to a large width (the ratio of the width to the height B/H ≥ 10.0), the 3D effect can be neglected and the plane-strain analysis is appropriate to assess its safety.
Significant improvements to the thermoelectric figure of merit ZT have emerged in recent years, primarily due to the engineering of material composition and nanostructure in inorganic semiconductors ...(ISCs). However, many present high-ZT materials are based on low-abundance elements that pose challenges for scale-up, as they entail high material costs in addition to brittleness and difficulty in large-area deposition. Here we demonstrate a strategy to improve ZT in conductive polymers and other organic semiconductors (OSCs) for which the base elements are earth-abundant. By minimizing total dopant volume, we show that all three parameters constituting ZT vary in a manner so that ZT increases; this stands in sharp contrast to ISCs, for which these parameters have trade-offs. Reducing dopant volume is found to be as important as optimizing carrier concentration when maximizing ZT in OSCs. Implementing this strategy with the dopant poly(styrenesulphonate) in poly(3,4-ethylenedioxythiophene), we achieve ZT = 0.42 at room temperature.
Degeneration of articular cartilage (AC) tissue is the most common cause of osteoarthritis (OA) and rheumatoid arthritis. Bone morphogenetic proteins (BMPs) play important roles in bone and cartilage ...formation. This article reviews the experimental and clinical applications of BMPs in cartilage regeneration. Experimental evidence indicates that BMPs play an important role in protection against cartilage damage caused by inflammation or trauma, by binding to different receptor combinations and, consequently, activating different intracellular signaling pathways. Loss of function of BMP-related receptors contributes to the decreased intrinsic repair capacity of damaged cartilage and, thus, the multifunctional effects of BMPs make them attractive tools for the treatment of cartilage damage in patients with degenerative diseases. However, the development of BMP therapy as a treatment modality for cartilage regeneration has been hampered by certain factors, such as the eligibility of participants in clinical trials, financial support, drug delivery carrier safety, availabilities of effective scaffolds, appropriate selection of optimal dose and timing of administration, and side effects. Further research is needed to overcome these issues for future routine clinical applications. Research and development leading to the successful application of BMPs can initiate a new era in the treatment of cartilage degenerative diseases like OA.
Background The aim of this study is to evaluate analgesic efficacy of pericapsular nerve group (PENG) block in patients with intertrochanteric femur fracture (IFF). Methods This double-blinded ...randomized controlled trial in patients with IFF scheduled for proximal femoral nail antirotation (PFNA) between December 2020 and November 2021. The primary outcome was VAS scores during exercising at 6 h after surgery; secondary outcomes were pain during exercising and rest, intraoperative dose of remifentanil, cumulative dose of postoperative fentanyl, postoperative analgesia satisfaction scores, and ratio of quadriceps weakness. Results A total of 50 patients were randomly divided into PENG block group (n = 25) or fascia iliaca compartment block (FICB) group (n = 25). Exercising VAS scores at 6 h after surgery were significantly lower in PENG block group than that in FICB group (2 (2, 4) vs. 6 (4, 7), P < 0.001). The intraoperative dose of remifentanil and cumulative dose of postoperative fentanyl by patient-controlled intravenous analgesia within 24 h after surgery in PENG block group were significantly lower than in FICB group (both P < 0.001). Postoperative analgesia satisfaction scores in PENG block group were significantly higher than those in FICB group (P = 0.016). The ratio of quadriceps weakness at 6 h after surgery was significantly higher in FICB group than PENG block group (48% vs. 0%, P < 0.001). Conclusions Compared to FICB, ultrasound-guided PENG block may provide better postoperative pain relief in patients with IFF, with less pronounced quadriceps weakness.
Many constitutive models are available nowadays to predict soil–structure interaction problems. It is sometimes not very easy for engineers to select a suitable soil model to carry out their design ...analyses in terms of complexity versus accuracy. This paper describes the application of three constitutive models to back-analyse a well-instrumented centrifuge model test, in which the effect of basement excavation on an existing tunnel was simulated. These three models include a linear elastic – perfectly plastic model with the Mohr–Coulomb failure criterion (called MC model), a nonlinear elastic Duncan–Chang model (DC), and a hypoplastic model (HP), the last of which can capture the state-, strain-, and path-dependent soil stiffness even at small strains and path- and state-dependent soil strength. By comparing with measured data from the centrifuge model test, it is found that the HP model yielded the best predictions of tunnel heave among the three models. Not only the gradient, but also the magnitude of tunnel heave is well predicted by this HP model. This can be explained by the fact that the HP model can capture the state-, strain-, and path-dependent soil stiffness even at small strains and path- and state-dependent soil strength, but not the MC and DC models. However, all three models underestimated the change in tunnel diameter and the maximum tensile bending strain in the transverse direction.
Objective The incidence of breast cancer related lymphedema (BCRL) varies between 7-35% depending on the combination of treatment modalities. Early detection of BCRL is crucial in order to start an ...effective non-operative treatment program. Because of the lack of prospective research on this topic, this study was undertaken to prospectively determine the effect of Lympho Venous Anastomosis (LVA) on BCRL and to review the current literature. Study design and methods Ten patients who were previously treated for breast cancer by surgery, radiotherapy, and chemotherapy, and were unresponsive to 12-weeks of non-operative treatment, underwent an LVA procedure (Degni-Cordeiro). Objective measurements were gathered for circumferential measurement and water volumetry, and quality of life. Various types of lymphoscintigraphy were carried out pre-operatively and post-operatively at 3 and 12 months. Treatment was embedded in a multidisciplinary setting. Results Post-operative volume measurements initially showed a 4.8% reduction of lymphedema at 3 months and a 2% reduction after one year. Various scintigraphic parameters showed some improvement. Quality of life questionnaires reported minimal improvement. Reviewing the literature, only retrospective studies were found; these reported varying results for LVA procedures. The selection of patients, classification of lymphedema, indications and types of LVA, and additional therapeutic options were heterogeneous, not comparable, and lacked a validated method of effect-assessment. Conclusions Our results showed a minimal reduction in volume of lymphedema following LVA; in the literature, there was no convincing evidence of the success of LVA. Non-operative treatment and elastic stockings are still preferred by most patients with lymphedema, especially in early stages with few irreversible changes.
Basement excavation inevitably causes stress changes in the ground, leading to soil movements that may affect the serviceability and safety of adjacent tunnels. Despite paying much attention to the ...basement–tunnel interaction, previous research has mainly focused on the influence of tunnel location in relation to the basement, tunnel stiffness, and excavation geometry. The effects of sand density and basement wall stiffness on nearby tunnels due to excavation, however, have so far been neglected. A series of three-dimensional centrifuge tests were thus carried out in this study to investigate these effects on the complex basement–tunnel interaction. Moreover, three-dimensional numerical analyses and a parametric study by adopting a hypoplastic sand model were conducted to improve the fundamental understanding of this complex problem, and calculation charts were developed as a design tool. When the basement was constructed directly above the existing tunnel, excavation-induced heave and strain were more sensitive to a change in soil density in the transverse direction than that in the longitudinal direction of the tunnel. Because a looser sand possesses smaller soil stiffness around the tunnel, the maximum tunnel elongation and transverse tensile strain increased by more than 20% as the relative sand density decreased by 25%. Moreover, the tensile strain induced along the longitudinal direction was insensitive to the stiffness of the retaining wall, but the tensile strain induced along the transverse direction was significantly reduced by a stiff wall. When the basement was constructed at the side of the existing tunnel, the use of a diaphragm wall reduced the maximum settlements and tensile strains induced in the tunnel by up to 22% and 58%, respectively, compared with the use of a sheet pile wall. Under the same soil density and wall stiffness, excavation-induced maximum movement and tensile strains in the tunnel located at a side of the basement were about 30% of the measured values in the tunnel located directly beneath the basement centre.
