While this has been demonstrated in randomized controlled trials, there is a need to confirm these findings in an everyday clinical setting. The A(1)chieve study will evaluate adverse events and effectiveness of premix (biphasic insulin aspart 30 NovoMix 30), basal (insulin detemir Levemir), and meal-time (insulin aspart NovoRapid) insulin analogs in people with type 2 diabetes in near-routine clinical practice. A(1)chieve is an international, prospective, multi-center, open-label, non-interventional, 24-week study of people with type 2 diabetes using an insulin analog. The study will recruit 60 000 people from 30 countries across four continents (Asia, Africa, South America, and Europe). The primary aim of the study is to assess the adverse event profile of the study insulins in routine clinical practice, including rates of hypoglycemia. In addition, effectiveness (HbA(1c), fasting plasma glucose, and postprandial plasma glucose) and patient quality of life outcomes will be measured. Comprehensive epidemiological data will be collected at baseline, including recent plasma glucose results and hypoglycemic episodes, prevalence of diabetes-related complications, and measures of current standards of care. Thus, A(1)chieve should provide important information about how insulin analogs perform in daily clinical practice.

En los últimos años se reconoce un nuevo mecanismo involucrado en la fisiopatología de la Diabetes Mellitus tipo 2: el déficit de producción y/o acción de las incretinas. Las incretinas son ...enterohormonas que estimulan la secreción de insulina en respuesta a la ingesta de nutrientes. Glucagon like péptido-1 (GLP1) y Polipéptido insulinotrópico glucosa dependiente (GIP) son las principales incretinas descubiertas hasta hoy. Ambas presentan también efecto trófico sobre las células beta de los islotes pancreáticos. GLP-1 presenta otras acciones como son la inhibición de la secreción de glucagón, enlentecimiento del vaciamiento gástrico e inhibición del apetito. Ambas incretinas son rápidamente clivadas por la enzima dipeptidil peptidasa 4 (DPP-4). Nuevas drogas como los incretinomiméticos, análogos y los inhibidores de DPP-4 se presentan como una terapéutica prometedora para los pacientes con diabetes tipo 2.
Diabetes Mellitus and Depression are two increasing pathologies. Several articles indicate that Depression in Diabetes is twice as important as in the general population. Yet, Depression in diabetic ...patients is underdiagnosed. On the other hand, Alexithymia might be a factor associated to Depression. We therefore carried out a research in order to evaluate the relationship between Depression and Alexithymia in diabetic patients at the Italian Hospital of Buenos Aires. We performed psychiatric interviews, which led to diagnoses based on DSM-IV, and applied the following scales: HAM-D, TAS-26, DBI and M.I.N.I. We evaluated 59 patients who had been diagnosed Type II Diabetes. Crossing variables, we observed a negative correlation between the results of Beck's Depression Inventory and those of Hamilton's Depression Scale. On the contrary, these last results showed a positive correlation with results obtained through TAS-26, which reveals a significant relationship between Depression and the presence of Alexithymia. According to the link between Depression, Diabetes Mellitus II and Alexithymia and considering that Alexithymia has consequences on the illness evolution and the treatment of both pathologies, it seems important to determine when and how to include its evaluation.
