Abstract A case is reported of a patient presenting to clinic after a sudden spontaneous ‘cracking’ sensation in the jugulo-digastric region. This proved, following clinical and radiological ...investigation, to be a fractured elongated styloid process(ESP). This extraordinary presentation of Eagle’s syndrome has not previously been published.
A number of methods have been developed to assist subjects in providing an estimate of portion size but their application in improving portion size estimation by children has not been investigated ...systematically. The aim was to develop portion size assessment tools for use with children and to assess the accuracy of children's estimates of portion size using the tools. The tools were food photographs, food models and an interactive portion size assessment system (IPSAS). Children (n 201), aged 4-16 years, were supplied with known quantities of food to eat, in school. Food leftovers were weighed. Children estimated the amount of each food using each tool, 24 h after consuming the food. The age-specific portion sizes represented were based on portion sizes consumed by children in a national survey. Significant differences were found between the accuracy of estimates using the three tools. Children of all ages performed well using the IPSAS and food photographs. The accuracy and precision of estimates made using the food models were poor. For all tools, estimates of the amount of food served were more accurate than estimates of the amount consumed. Issues relating to reporting of foods left over which impact on estimates of the amounts of foods actually consumed require further study. The IPSAS has shown potential for assessment of dietary intake with children. Before practical application in assessment of dietary intake of children the tool would need to be expanded to cover a wider range of foods and to be validated in a 'real-life' situation.
Human campylobacteriosis exhibits a distinctive seasonality in temperate regions. This paper aims to identify the origins of this seasonality. Clinical isolates typed by multi-locus sequence typing ...(MLST) and epidemiological data were collected from Scotland. Young rural children were found to have an increased burden of disease in the late spring due to strains of non-chicken origin (e.g. ruminant and wild bird strains from environmental sources). In contrast the adult population had an extended summer peak associated with chicken strains. Travel abroad and UK mainland travel were associated with up to 17% and 18% of cases, respectively. International strains were associated with chicken, had a higher diversity than indigenous strains and a different spectrum of MLST types representative of these countries. Integrating empirical epidemiology and molecular subtyping can successfully elucidate the seasonal components of human campylobacteriosis. The findings will enable public health officials to focus strategies to reduce the disease burden.
Cathodoluminescence (CL) zoning in quartz crystals from rhyolitic pumices in two ignimbrite members of the ~340-ka Whakamaru super-eruption deposits, Taupo Volcanic Zone, New Zealand, is investigated ...in conjunction with the analysis of Ti concentration in quartz to reconstruct the history of changing magma chamber conditions and to elucidate the eruption-triggering processes. CL intensity images are taken as a proxy for Ti concentration and thus temperature and/or pressure and/or compositional variations during crystal growth history. Estimates of the maximum temperature changes (i.e., assuming other factors influencing Ti uptake remain constant) are made using the TitaniQ geothermometer based on the Ti concentration in quartz. These results are reviewed in comparison with Fe–Ti oxide, feldspar-melt and amphibole geothermometry. Core-to-rim quartz Ti profiles record a marked change in conditions (temperature increase and/or pressure decrease and/or change in melt composition) causing and then following a significant resorption horizon in the outer parts of the crystals. Two alternative models that could explain the quartz Ti zonation invoke a temperature increase caused by mafic recharge and/or a pressure decrease due to magma ponding and re-equilibration at shallow crustal levels. Concomitant changes in melt composition and Ti activity may, however, also have strongly influenced Ti uptake into the quartz. Some crystals also show other marked increases in CL brightness internally, but any accompanying magmatic changes did not result in eruption. Diffusion modelling indicates that this significant change in conditions occurred over ~10–85 years prior to caldera-forming eruption. This rapid thermal pulse or pressure change is interpreted as evidence for open-system processes, and appears to record a magma chamber recharge event that rejuvenated the Whakamaru magma system (melt-dominant magma plus crystal mush), and potentially acted as a trigger for processes that led to eruption.
We report the use of an electron light source (ELS) located at the Telescope Array Observatory in Utah, USA, to measure the isotropic microwave radiation from air showers. To simulate extensive air ...showers, the ELS emits an electron beam into the atmosphere and a parabola antenna system for the satellite communication is used to measure the microwave radiation from the electron beam. Based on this measurement, an upper limit on the intensity of a 12.5GHz microwave radiation at 0.5m from a 1018 eV air shower was estimated to be 3.96×10−16 W m−2 Hz−1 with a 95% confidence level.

Children with a first UTI in the 2000s compared to those in the 1990s, were referred younger, were half as likely to have a renal scar (girls OR 0.47, 95% CI 0.29 to 0.76; boys 0.35, 0.16 to 0.81), and were about 12 times more likely to have vesicoureteric reflux without scarring (girls 11.9, 4.3 to 33.5; boys 14.4, 4.3 to 47.6). In the 2000s, general practitioners treated about half the children at first consultation. Children who were treated within 3 days of their symptoms starting were one-third as likely to scar as those whose symptoms lasted longer (0.33, 0.12 to 0.72).
Most kidney defects seen in children after UTIs, are acquired scars, and in Newcastle, active management in primary care has halved this rate.

We pooled data from 12 prospective US and European cohorts with self-reported physical activity (baseline, 1987-2004). We used multivariable Cox regression to estimate hazard ratios (HRs) and 95% confidence intervals for associations of leisure-time physical activity with incidence of 26 types of cancer. Leisure-time physical activity levels were modeled as cohort-specific percentiles on a continuous basis and cohort-specific results were synthesized by random-effects meta-analysis. Hazard ratios for high vs low levels of activity are based on a comparison of risk at the 90th vs 10th percentiles of activity. The data analysis was performed from January 1, 2014, to June 1, 2015.

A total of 1.44 million participants (median range age, 59 19-98 years; 57% female) and 186 932 cancers were included. High vs low levels of leisure-time physical activity were associated with lower risks of 13 cancers: esophageal adenocarcinoma (HR, 0.58; 95% CI, 0.37-0.89), liver (HR, 0.73; 95% CI, 0.55-0.98), lung (HR, 0.74; 95% CI, 0.71-0.77), kidney (HR, 0.77; 95% CI, 0.70-0.85), gastric cardia (HR, 0.78; 95% CI, 0.64-0.95), endometrial (HR, 0.79; 95% CI, 0.68-0.92), myeloid leukemia (HR, 0.80; 95% CI, 0.70-0.92), myeloma (HR, 0.83; 95% CI, 0.72-0.95), colon (HR, 0.84; 95% CI, 0.77-0.91), head and neck (HR, 0.85; 95% CI, 0.78-0.93), rectal (HR, 0.87; 95% CI, 0.80-0.95), bladder (HR, 0.87; 95% CI, 0.82-0.92), and breast (HR, 0.90; 95% CI, 0.87-0.93). Body mass index adjustment modestly attenuated associations for several cancers, but 10 of 13 inverse associations remained statistically significant after this adjustment. Leisure-time physical activity was associated with higher risks of malignant melanoma (HR, 1.27; 95% CI, 1.16-1.40) and prostate cancer (HR, 1.05; 95% CI, 1.03-1.08). Associations were generally similar between overweight/obese and normal-weight individuals. Smoking status modified the association for lung cancer but not other smoking-related cancers.
Leisure-time physical activity was associated with lower risks of many cancer types. Health care professionals counseling inactive adults should emphasize that most of these associations were evident regardless of body size or smoking history, supporting broad generalizability of findings.
Cyathostomins comprise a group of 50 species of parasitic nematodes that infect equids. Ribosomal DNA sequences, in particular the intergenic spacer (IGS) region, have been utilized via several ...methodologies to identify pre-parasitic stages of the commonest species that affect horses. These methods rely on the availability of accurate sequence information for each species, as well as detailed knowledge of the levels of intra- and inter-specific variation. Here, the IGS DNA region was amplified and sequenced from 10 cyathostomin species for which sequence was not previously available. Also, additional IGS DNA sequences were generated from individual worms of 8 species already studied. Comparative analysis of these sequences revealed a greater range of intra-specific variation than previously reported (up to 23%); whilst the level of inter-specific variation (3–62%) was similar to that identified in earlier studies. The reverse line blot (RLB) method has been used to exploit the cyathostomin IGS DNA region for species identification. Here, we report validation of novel and existing DNA probes for identification of cyathostomins using this method and highlight their application in differentiating life-cycle stages such as third-stage larvae that cannot be identified to species by morphological means.
We report studies of ultrahigh-energy cosmic-ray composition via analysis of depth of air shower maximum (X(max)), for air shower events collected by the High-Resolution Fly's Eye (HiRes) ...observatory. The HiRes data are consistent with a constant elongation rate d<X(max)>/dlog(E) of 47.9+/-6.0(stat)+/-3.2(syst) g/cm2/decade for energies between 1.6 and 63 EeV, and are consistent with a predominantly protonic composition of cosmic rays when interpreted via the QGSJET01 and QGSJET-II high-energy hadronic interaction models. These measurements constrain models in which the galactic-to-extragalactic transition is the cause of the energy spectrum ankle at 4x10(18) eV.
The spatial and temporal arrangement of crops is a conspicuous feature of rural landscapes. It has been identified as an important factor in many environmental issues, such as the coexistence of ...genetically modified (GM) and non-GM crops, and the mitigation of soil erosion. This paper examines a scenario-based approach for rapid generation and screening of crop allocations that meet user’s constraints without requiring mechanistic modelling. LandSFACTS (Landscape Scale Functional Allocation of Crops Temporally and Spatially) is a software application specifically designed to simulate such crop arrangement scenarios, whilst ensuring both spatial and temporal coherence with regard to the initial constraints. The software uses an empirical approach to allocate crops to fields (polygons in vector format) over a sequence of years, using a stochastic process (Markov chains) and rule-based constraints. Crop rotations are represented by transition probabilities complemented by other temporal constraints such as return period or prohibited sequences. Further spatial and temporal constraints on crop arrangement can be applied through separation distances, yearly proportions, and the application of statistical tests. The software outputs a crop allocation solution with a crop for every field for every year, respecting all user-defined constraints; the range of potential solutions can then be explored through multiple model runs. Metrics based upon the difficulty of obtaining such an allocation from the initial constraints are also generated. A case study is provided to demonstrate the use of combined agronomic and environmental criteria for exploring GM crop coexistence at the landscape scale.
