Introduction and objective: Retropharyngeal abscess are uncommon. They usually occur
in children younger than 5 years and most of them are polymicrobial infections. Method: We report a case
of a ...six-month-old boy with an abscess of 7 cm3 that required incision and transoral surgical drainage. We
used the CARE checklist when writing our report 8. Parents of patient gave their consent to use clinical
data for publication. Discussion: Computed tomography of the neck is the diagnostic technique of choice.
It has to be done when deep neck infections are suspected. Conservative treatment with intravenous antibiotics
would be the first choice. Prompt surgical drainage would be needed if symptoms worsen or in large
abscesses with risk of airway obstruction. Conclusions: Deep neck abscesses are uncommon in infants at
present, but potentially serious. An early diagnosis and treatment are important to avoid severe complications.
Introducción y objetivo: Los abscesos retrofaríngeos son actualmente poco frecuentes
en niños. Ocurren normalmente en menores de 5 años y son en su mayoría infecciones polimicrobianas.
Método: Se presenta el caso de un niño de 6 meses que presentó un absceso de 7 cm3 de volumen que requirió
incisión y drenaje por vía transoral. Se usó la lista de comprobación CARE para la redacción del caso. Los
padres del paciente consintieron el uso de sus datos clínicos. Discusión: La tomografía axial computarizada
es la prueba diagnóstica de elección. Se debe realizar siempre que se sospechen infecciones profundas del
cuello. El tratamiento puede ser conservador en su inicio mediante terapia antibiótica intravenosa. Requerirán
drenaje quirúrgico si empeoran clínicamente y en aquellos abscesos de gran tamaño con riesgo de
obstrucción de la vía aérea. Conclusiones: Los abscesos cervicales profundos de gran tamaño en lactantes
son una patología rara en la actualidad, pero potencialmente grave. Es importante realizar un diagnóstico
y tratamiento precoz para evitar complicaciones serias.
Introducción y objetivo: La osteonecrosis benigna del conducto auditivo externo (CAE)
es un efecto secundario bastante desconocido del tratamiento con bifosfonatos (BFS). Nuestro objetivo es
...describir la forma de presentación y el manejo terapéutico de los casos descritos en la literatura. Método:
Se realizó una revisión sistemática de la literatura de los casos de osteonecrosis del CAE secundaria a BFS
desde septiembre 2003 a febrero 2019. Se siguieron los criterios PRISMA evaluando las bases de datos
PubMED. Resultados: Se han identificado 7 estudios que incluyen un total de 17 pacientes con osteonecrosis
del CAE. El 76% fueron mujeres y el 24% restante varones, con edades comprendidas entre 51 y 89.
El tiempo de evolución hasta la aparición de complicaciones osciló entre 15 meses y 20 años. La forma de
presentación fue la úlcera sobreinfectada con exposición ósea unilateral en 41% de los casos. La zona de
aparición inicial fue el suelo del CAE en el 100%. Se realizó tomografía axial computarizada al 47% de los
casos observando signos de necrosis ósea; y dos biopsias con resultado de osteomielitis. 9 pacientes (53%)
fueron tratados con éxito con antibioterapia ± corticoides tópicos, mientras que los 8 restantes (47%) precisaron
tratamiento quirúrgico además de la retirada de BFS. Discusión/Conclusiones: La osteonecrosis de
CAE es un efecto secundario muy infrecuente del uso de BFS. La forma de presentación más común es la úlcera indolora en el suelo del conducto. Para su tratamiento con frecuencia es suficiente con la limpieza
local y administración de antibióticos tópicos.
Introduction and objective: Benign osteonecrosis of the external auditory canal (EAC)
is a side effect pretty unknown due to bisphosphonate (BPs) therapy. Our aim is to describe the way of
presentation and treatment of cases published in literature. Method: We did a systematic review of patients
with diagnose of osteonecrosis of the external ear canal secondary to bisphosphonates from September
2003 till February 2019. We followed PRISMA criteria and evaluated PubMed database. Results: 7 articles
were identified including a total of 17 patients with osteonecrosis of EAC. 76% were women and 24% men,
within ages between 51 and 89. The duration time till complications appeared ranged from 15 months to
20 years. The most common presentation was as a unilateral ulceration with bone exposure in 41% of cases.
Floor of the EAC was the initial place in 100%. Computed Tomography was performed in 47% of patients
showing bone necrosis; and two biopsies with osteomyelitis signs. 9 patients (53%) were successfully treated
with antibiotics and topical corticosteroids, while 8 cases (47%) needed surgical treatment in addition to
the withdrawal of BFS. Discussion/Conclusions: Osteonecrosis of EAC is a rare side effect of BPs treatment.
Painless ulceration on the EAC floor is the most common onset. Topical antibiotics and local debridement
are frequently enough for complete healing.
Recurrent epistaxis is the most frequent clinical manifestation of hereditary haemorrhagic telangiectasia (HHT). Its treatment is difficult. Our objective was to assess the use of tranexamic acid ...(TA), an antifibrinolytic drug, for the treatment of epistaxis in HHT patients and to investigate in vitro the effects of TA over endoglin and ALK-1 expression and activity in endothelial cells. A prospective study was carried out on patients with epistaxis treated with oral TA in the HHT Unit of Sierrallana Hospital (Cantabria, Spain). Primary cultures of endothelial cells were treated with TA to measure the levels of endoglin and ALK-1 at the cell surface by flow cytometry. RNA levels were also measured by real-time PCR, and the transcriptional effects of TA on reporters for endoglin, ALK-1 and the endoglin/ALK-1 TGF-beta pathway were assessed. The results showed that the fourteen HHT patients treated orally with TA improved, and the frequency and severity of their epistaxis were decreased. No complications derived from the treatment were observed. Cultured endothelial cells incubated with TA exhibited increased levels of endoglin and ALK-1 at the protein and mRNA levels, enhanced TGF-beta signaling, and improved endothelial cell functions like tubulogenesis and migration. In summary, oral administration of TA proved beneficial for epistaxis treatment in selected patients with HHT. In addition to its already reported antifibrinolytic effects, TA stimulates the expression ofALK-1 and endoglin, as well as the activity of the ALK-1/endoglin pathway.
Liver transplantation is the only alternative treatment in patients in end-stage liver function. In many cases the need for liver transplantation is a consequence of the toxic effects of alcohol.
The ...aim of our study was to determine if patients who are candidates for a liver transplant require a systematic exploration of the head and neck to rule out malignant lesions in this area.
All the candidates for a liver transplant, between 2011 and 2017, were included in our study. All of them underwent a systematic exploration of the head and neck by an otolaryngologist to rule out malignancy of the head and neck.
A total of 141 patients were included in our study. In two of them (1.3%) a carcinoma of the head and neck was detected (one of the oropharynx and the other of the larynx). Both patients were asymptomatic from an ENT point of view. They were treated with chemoradiotherapy. One of them died during follow-up due to the appearance of a metachronous lung tumour. The other is free of disease 2 years after diagnosis but without having been transplanted. Both patients had a history of smoking and high alcohol intake.
In patients who are candidates for a liver transplant who have a history of smoking and / or high alcohol intake, a pretransplant head and neck study including pharyngo-laryngeal fibroscopy is indicated.
El trasplante hepático constituye la única alternativa de tratamiento en pacientes con hepatopatías crónicas avanzadas. La etiología más frecuente en nuestro medio es la viral y la alcohólica.
El objetivo de nuestro estudio fue determinar si los pacientes que son candidatos a un trasplante hepático precisan una exploración de cabeza y cuello de forma sistemática para descartar neoplasias de dicha área.
Todos los pacientes candidatos a la realización de un trasplante hepático entre los años 2011 y 2017, fueron incluidos en nuestro estudio. A todos ellos se les realizó de forma sistemática una exploración de cabeza y cuello por un otorrinolaringólogo para descartar neoplasias de cabeza y cuello.
Un total de 141 pacientes fueron incluidos en nuestro estudio. En dos de ellos (1,3%) se detectó un carcinoma de cabeza y cuello (uno de orofaringe y otro de laringe), estando ambos pacientes asintomáticos. Fueron tratados con quimiorradioterapia, falleciendo uno de ellos durante el seguimiento por la aparición de un tumor metacrónico pulmonar. El otro paciente está libre de enfermedad 2 años después del diagnóstico, pero no ha recibido trasplante hepático hasta la fecha. Ambos pacientes tenían antecedentes de tabaquismo y alta ingesta de alcohol.
En pacientes que son candidatos a un trasplante hepático, y que presentan antecedentes de tabaquismo y/o alta ingesta alcohólica recomendamos la realización de un estudio de cabeza y cuello pretrasplante que incluya exploración nasofibroscópica de las regiones faringo-laríngeas para despistaje de tumores sincrónicos.
Introduction and Objective: The objective of this study was to describe the clinic-epidemiological
characteristics of the nasopharyngeal carcinoma in Cantabria (Spain) and to compare it with
previous ...studies performed. Material and methods: A retrospective study of nasopharyngeal carcinomas
diagnosed in Marqués de Valdecilla Hospital (Santander, Cantabria, Spain) between 2004 and 2015 was
made, determining the following variables: age, occupation, toxics, outcome symptoms, treatment and
clinical evolution. Results: 45 patients with ages range between 35 and 85 were diagnosed of nasopharyngeal
carcinoma, with an incidence of 0,71/100000H/year. 38 were men (84,5%) and 7 were women
(15,6%). The fifties decade was the most frequent period of diagnosis. Otological symptoms were the main
clinical outcome (40%), followed by cervical adenopathy (26,6%). Undifferentiated carcinoma was the most
common histological type (68,9%), being diagnosed in advanced stages. Conclusions: Nasopharyngeal
carcinoma is an uncommon tumour in Cantabria being more frequent in males in their fifties. Its clinical
presentation, often nonspecific, results in advanced stages diagnose. The most common histological type
(undifferentiated carcinoma).
Introducción y objetivo: Analizar las características clínico-epidemiológicas de los carcinomas
de nasofaringe en Cantabria (España) y compararlas con estudios previos realizados. Material y
métodos: Se realizó un análisis retrospectivo de los casos diagnosticados de carcinoma de nasofaringe en
el Hospital Universitario Marqués de Valdecilla (Santander, Cantabria, España), entre los años 2004 y 2015.
De las historias clínicas se obtuvieron las siguientes variables: edad al diagnóstico, profesión, hábitos tóxicos,
clínica de presentación, tratamiento realizado y evolución clínica. Resultados: 45 pacientes con edades
comprendidas entre los 35 y los 85 años fueron diagnosticados de carcinoma de nasofaringe en el citado
periodo (incidencia 0,71/100000H/año); 38 eran varones (84,5%) y 7 eran mujeres (15,6%), encontrándose
la edad media de presentación en la década de los 50 (28,9%). Los síntomas más frecuentes de presentación
fueron los óticos (40%), seguidos de las adenopatías cervicales (26,6%). En cuanto al tipo histológico el
carcinoma indiferenciado (68,9%) fue el más habitual. En la mayor parte de los casos el diagnóstico se realizó
en estadio avanzado. Discusión y conclusiones: El carcinoma de nasofaringe es un tumor infrecuente en Cantabria, apareciendo sobre todo en varones en la quinta década de la vida. Su forma de presentación, a
menudo inespecífica, da lugar a que su diagnóstico se realice normalmente en fases avanzadas.
La epiglotitis aguda es una inflamación de la epiglotis y estructuras adyacentes rápidamente progresiva que puede producir obstrucción severa de la vía aérea. Objetivo: conocer las características ...clínico-epidemiológicas de las epiglotitis aguda en Cantabria. Método Estudio retrospectivo descriptivo de los pacientes diagnosticados de epiglotitis aguda desde enero de 1992 a abril de 2015 en la Comunidad Autónoma de Cantabria (España). Resultados: Cincuenta pacientes fueron incluidos en el estudio, con edades comprendidas entre 1 y 80 años, de los cuales cuarenta eran varones y diez eran mujeres. Sólo uno de ellos se trataba de un paciente pediátrico. Se diagnosticaron 2,7 casos por año, con una incidencia de 0,5 casos por 100000 habitantes y año. Dos pacientes tenían síndrome de Down. El síntoma principal de presentación fue odinofagia (88%) seguido de disnea (56%) y alteración en la voz (52%). Nueve pacientes requirieron ingreso en cuidados Intensivos. Cuatro pacientes precisaron intubación orotraqueal; a cinco se les realizó una traqueotomía de urgencia. Siete pacientes presentaron complicaciones graves, uno de ellos con secuelas neurológicas severas. Ninguno de ellos murió a consecuencia de la epiglotitis. Conclusiones: La epiglotitis aguda es una patología poco frecuente pero grave y que, por tanto, precisa un tratamiento precoz para evitar complicaciones. Objetivamos una práctica desaparición de la epiglotitis en pacientes pediátricos en nuestro medio, lo que se explica por la introducción en el calendario vacunal de la inmunización contra el Haemophilus Influenzae.
Acute epiglottitis is an inflammation of the epiglottis and other adjacent structures that can produce severe obstruction of the airway. Our objective is to know the clinical-epidemiological characteristics of acute epiglottitis in a Region of the north of Spain. Method: Retrospective descriptive study of all patients diagnosed with acute epiglottitis from January 1992 to April 2015 in the Region of Cantabria (Spain). Results: Fifty patients were included in our study, aged between 1 and 80 years, of whom 40 were men and 10 were women. Only one of them was a pediatric patient. 2.7 cases / year were diagnosed, with an incidence of 0.5 cases per 100000 habitants per year. Two patients had Down syndrome. The main symptom was odynophagia (88%) followed by dyspnea (56%) and voice alteration (52%). Nine patients required a stay in Intensive Care. Four patients required orotracheal intubation; five needed an emergency tracheotomy. Seven patients had serious complications, one of them with severe neurological damage. None of them died because of epiglottitis. Conclusions: Acute epiglottitis is a rare but serious disease and, therefore, requires early treatment to avoid complications. We have observed an almost total disappearance of epiglottitis in pediatric patients in our region, which is explained by the introduction in the vaccination calendar of immunization against Haemophilus influenzae.
