A case of massive choroidal detachment during cataract extraction is described. Some thoughts about the pathophysiology of expulsive detachment in this case are discussed (Fuchs' heterochromy ...cyclitis and high blood pressure). The prognosis is good without need of posterior sclerotomy but with steroid treatment for a longer period.
A recessively inherited syndrome of blepharophimosis and ptosis with weakness of extraocular and frontal muscles is reported in six members of three related kindreds. Prognathism, synophrys and thick ...eyebrows added to a typical facial appearance. Additional findings included short stature, borderline head circumference and toe syndactyly. Borderline mental retardation and anosmia were found in one patient. The clinical features and the mode of inheritance distinguish this syndrome from other blepharophimosis and ptosis syndromes.
A case of metastatic Serratia marcescens (SM) endophthalmitis is described in a 57-year-old diabetic woman, after amputation of her leg above the knee because of peripheral vascular disease. SM ...cultured from the infected surgical stump was the source of septic emboli to her right eye and lungs, causing endophthalmitis and pneumonia. The ocular infection did not respond to appropriate antibiotic therapy and evisceration was required. SM infection can cause endophthalmitis refractory to antibiotics, and it should be aggressively treated when SM is cultured from any infected site.
Two cases are described of scleral perforation due to the needle of the bridle suture done before cataract extraction in 1,280 consecutive cases. The immediate diagnosis is important and is based on ...acute hypotony of the globe, the appearance of vitreous fluid under the conjunctiva, or both. Cryopexy with scleral buckling followed immediately by the usual cataract extraction is recommended. This procedure allows judgment of the actual state and avoids a second operation.
We report the results of six years' experience in the treatment of keratoconjunctivitis sicca with bromhexine. We found the drug to be efficacious in long-term use in patients with partially involved ...lacrimal glands.
