Coronary arterial disease is the main cause of morbidity in western countries and coronary angiography is the most important diagnosis test. Lately, coronary angiotomography has been introduced for ...studying coronary arteries in a non-invasive way. Our goal was to assess the diagnostic accuracy of coronary angiotomography in the diagnosis of coronary arterial disease.
Cross-sectional study for assessing diagnostic test comparing coronary angiotomography to invasive angiography in patients without previous coronary disease. All the patients underwent both tests consecutively in a blind way. Coronary arteries were studied according to the modified reference model of coronary segmentary analysis of the American Heart Association. Tomography validity indexes (sensitivity, specificity, positive--PPV--and negative--NPV--predictive values and global value) by patient, artery, and segment were calculated.
We studied 121 patients (47 females--38.8%--and 74 males--61.2%-). Prevalence of coronary arterial disease was 38%. The most frequent risk factor detected was hypertension. We obtained a 100% sensitivity and NPV studying patients as a whole by patient, a specificity of 92% (95% CI: 86.0-99.0) and a global value of 94% (95% CI: 89.0-99.0). The higher validity by artery was for anterior descending artery with a specificity of 98% (CI95%: 90.0-96.0) and NPV of 97% (IC95%: 96.0-100). The segment with higher validity was distal right coronary artery with 100% specificity and PPV.
A high diagnostic validity of coronary angiotomography was obtained and it could be considered as another diagnostic test for studying coronary arterial disease.
Tuberculosis is a major public health problem and most cases are concentrated in vulnerable populations. The objective was to describe the incidence rates trend in native and foreign population ...(2009-2018) in Madrid Region.
Retrospective analysis of cases from the Tuberculosis Regional Registry of cases of Madrid Region 2009-2018. Annual incidence rates were calculated by country of birth (Spain, other), sex and age group (<15, 15-34, 35-44, 45-64, >64), using the annual January 1st continuous register population. The infection rate trend and the annual percentage change (APC) were calculated, along with the best jointpoint adjustment using Jointpoint regression.
7,696 cases were analyzed, 48.2% were foreign-born individuals. Average age in native population was 50 years old (SD: 23.96) and 35 (DS: 36.64) in foreign-born individuals (p<0.001). The overall incidence rate decreased from 17.30 in 2009 to 9.00 per 100,000 in 2018 and was higher in men. Pulmonary tuberculosis reduced from 11.90 to 6.55. Among native population, the incidence of TB fell from 10.29 to 5.24 with an APC of -7.3% (95%IC: -8.9; -5.7) (p<0.05), no jointpoint was identified. Among foreign-born individuals the incidence of tuberculosis declined from 46.54 to 25.49, a joint point was identified in 2013, observing an incidence decrease for the period 2009-2013 and APC of -13.8% (IC95%: -17.5; -10.0).
The global incidence rate in this period has decreased by approximately 7% per year. However, this reduction occurred mainly in native population. In foreign-born individuals the incidence decreased by approximately 14% during the 2009-2013 period, after this period there have been no significant incidence changes.

In this paper the cliff shoreline erosion on the coastline between Punta Montijo and the Chipiona Port (Cadiz, Spain) is studied. The reasons which cause this erosive phenomenon and the future ...erosive tendency of these cliffs are estimated, obtaining magnitudes of recession and the rate at which it will occur. Analysis of the current situation has been carried out by determining its plan shape, the study of the theoretical erosion profile of the cliffs and the verification of their failure. The future evolution of the cliffs shoreline has been analysed through the simulation of its recession and the study of the profile response to storm wave action. Finally, is presented a model to estimate the recession of the cliff shoreline.
