To describe the impact of media content, timing, and use behaviors on child sleep.
These data were collected in the baseline survey and media diary of a randomized controlled trial on media use in children aged 3 to 5 years. Sleep measures were derived from the Children's Sleep Habits Questionnaire. Media diaries captured time, content title, and co-use of television, video-game, and computer usage; titles were coded for ratings, violence, scariness, and pacing. Nested linear regression models were built to examine the impact of timing, content, and co-use on the sleep problem score.
On average, children consumed 72.9 minutes of media screen time daily, with 14.1 minutes occurring after 7:00 pm. Eighteen percent of parents reported at least 1 sleep problem; children with a bedroom television consumed more media and were more likely to have a sleep problem. In regression models, each additional hour of evening media use was associated with a significant increase in the sleep problem score (0.743 95% confidence interval: 0.373-1.114), as was daytime use with violent content (0.398 95% confidence interval: 0.121-0.676). There was a trend toward greater impact of daytime violent use in the context of a bedroom television (P=.098) and in low-income children (P=.07).
Violent content and evening media use were associated with increased sleep problems. However, no such effects were observed with nonviolent daytime media use.
Describe the outcome of adenovirus pneumonia in a pediatric intensive care unit (PICU) over a 3-year period, to identify the risk factors that may be associated with worse outcome.
A retrospective ...observational study was performed in the PICU of children's hospital in Shanghai from July 2016 to June 2019. Sixty-seven children over 29 days to 14 years old with adenovirus pneumonia who were admitted to PICU with acute hypoxemic respiratory failure were included in this study. The primary outcome was hospital mortality, and secondary outcomes were hospital and PICU length of stay (LOS), and risk factors of worse outcome.
Of 67 children with severe adenovirus pneumonia, the hospital mortality was 16.42% (11/67) and 28-day mortality was 14.93% (10/67). Median Pediatric Risk of Mortality III (PRISM III) score at admission was 13 (interquartile range IQR, 10-15). Median PICU LOS stay was 11 days (8-18d) and hospital LOS was 22 days (16-31d). Among children with extracorporeal membrane oxygenation (n = 9), 6 cases survived and 3 cases died. The patients who need renal replacement therapy, neuromuscular blockade, parenteral nutrition, and packed red blood cell perfusion had higher hospital mortality (p < 0.001, p = 0.041, p = < 0.001, p = 0.012, respectively). Multivariate logistic analysis indicated that liver dysfunction and nosocomial infection were associated with high risk of mortality.
The hospital mortality of adenovirus pneumonia in our PICU was 16.42%. Patients complicated liver dysfunction and co-infection & nosocomial infection were associated with poor outcome.
Television and video/DVD viewing among very young children has become both pervasive and heavy. Previous studies have reported an association between early media exposure and problems with attention ...regulation but did not have data on the content type that children watched. We tested the hypothesis that early television viewing of 3 content types is associated with subsequent attentional problems. The 3 different content types are educational, nonviolent entertainment, and violent entertainment.
Participants were children in a nationally representative sample collected in 1997 and reassessed in 2002. The analysis was a logistic regression of a high score on a validated parent-reported measure of attentional problems, regressed on early television exposure by content and several important sociodemographic control variables.
Viewing of educational television before age 3 was not associated with attentional problems 5 years later. However, viewing of either violent or non-violent entertainment television before age 3 was significantly associated with subsequent attentional problems, and the magnitude of the association was large. Viewing of any content type at ages 4 to 5 was not associated with subsequent problems.
The association between early television viewing and subsequent attentional problems is specific to noneducational viewing and to viewing before age 3.
This article analyses how digitisation and screen policy reform altered the production of domestic drama and children's programmes in Australia. Focusing on dynamics that developed before widespread ...use of streaming services, it maps the disruptions and evolution that digital ‘multichannels' caused and how they challenged audiovisual policy frameworks intended to safeguard local television including drama on advertiser-funded broadcasters. The article reveals how the effects of fragmentation undermined commercial television’s business model and eroded investment in scripted content. Shifting policy priorities also brought new support mechanisms for local programmes and led to adjustments to the ABC’s drama funding practices, with significant effects on the form, content, and cultural visibility of Australian drama. This initial stage of digital disruption - spanning roughly 2001-2014 - is often overlooked but is crucial for appreciating the challenges facing Australian television drama production in the 2020s.
To determine the correlation between higher-order aberrations (HOAs) and best-corrected visual acuity (BCVA) recovery speed after spectacles treatment using iDesign measurements in refractive ...amblyopic children.
This is a prospective case series. Children aged from 3 to 7 years with refractive amblyopia (Landolt C equivalent < 0.8) were recruited. All participants were followed for at least 6 months after full correction of the refraction error by spectacles. The HOAs were measured using iDesign before and after cycloplegia at first visit and at 3-month intervals. Then correlation between BCVA recovery after treatment for 6 months and HOAs was determined.
We analyzed 24 eyes of 12 children (mean age, 4.5 years). Baseline mean BCVA was logarithm of minimal angle of resolution (logMAR) 0.335 (Landolt C equivalent 0.46), which improved to logMAR 0.193 (Landolt C equivalent 0.64) after treatment with full-correction spectacles for 6 months. The amblyopic eye BCVA recovery was negatively correlated with tetrafoil with/without cycloplegia (P = 0.006 and 0.022, respectively) and trefoil with cycloplegia (P = 0.049).
trefoil and tetrafoil measured with iDesign negatively correlates with the BCVA recovery speed of refractive amblyopic eyes after spectacles treatment in this pilot study. The current study results may aid in further investigation for diagnosis and treatment of refractory refractive and idiopathic amblyopia.
This study, conducted in the Netherlands, evaluated the association between ethnic background and children's TV viewing time at multiple time points and its trajectory. We analyzed 4,833 children ...with a Dutch, Moroccan, Turkish, or Surinamese ethnic background from the Generation R Study, a population-based study in the Netherlands. Parent-reported television viewing time for children at ages 2, 3, 4, 6, and 9 years was collected by questionnaires sent from April 2004 until January 2015. Odds ratios of watching television ≥1 hour/day at each age were calculated for children from the various ethnic backgrounds. Generalized logistic mixed models (GLMMs) were used to assess the association between ethnic background and television viewing time trajectory. The effect modification by family socioeconomic status was examined in cross-sectional and longitudinal analyses. The percentage of children viewing television ≥1 hour/day increased from age 2 to 9 years for children from all ethnic backgrounds. After adjusting for maternal educational level and net household income, children from all ethnic subgroups had greater odds of watching television ≥1 hour/day at some time points compared with children with a Dutch background (Surinamese: all ages; Moroccan: at ages 4 and 6 years; Turkish: at ages 4 and 9 years). The GLMMs indicated that television viewing trajectories differed between ethnic subgroups. The associations between ethnic background and children's television viewing time were moderated by maternal educational level for child ages 4 and 6 years (p < 0.05). In longitudinal analyses, the ethnic differences in probability of watching television ≥1 hour/day were larger in children from high-educated mothers than in children from low-educated mothers. In conclusion, ethnic differences in television viewing time were present at all measuring time points. The discrepancy between children with a Dutch background and children with another background was larger in high maternal educational subgroups.
The goal was to quantify television viewing in day care settings and to investigate the characteristics of programs that predict viewing.
A telephone survey of licensed child care programs in ...Michigan, Washington, Florida, and Massachusetts was performed. The frequency and quantity of television viewing for infants, toddlers, and preschool-aged children were assessed.
With the exception of infants, children in home-based child care programs were exposed to significantly more television on an average day than were children in center-based programs (infants: 0.2 vs 0 hours; toddlers: 1.6 vs 0.1 hours; preschool-aged children: 2.4 vs 0.4 hours). In a regression analysis of daily television time for preschool-aged children in child care, center-based programs were found to have an average of 1.84 fewer hours of television each day, controlling for the other covariates. Significant effect modification was found, in that the impact of home-based versus center-based child care programs differed somewhat depending on educational levels for staff members; having a 2- or 4-year college degree was associated with 1.41 fewer hours of television per day in home-based programs, but no impact of staff education on television use was observed in center-based programs.
For many children, previous estimates of screen time significantly underestimated actual amounts. Pediatricians should council parents to minimize screen time in child care settings.
