Objectives:
The aim of this study was to identify CMV drug resistance mutations (DRM) in solid organ transplant (SOT) recipients with suspected resistance comparing next-generation sequencing (NGS) ...with Sanger sequencing and assessing risk factors and the clinical impact of resistance.
Methods:
Using Sanger sequencing as the reference method, we prospectively assessed the ability of NGS to detect CMV DRM in the UL97 and UL54 genes in a nationwide observational study from September 2013 to August 2016.
Results:
Among 44 patients recruited, 14 DRM were detected by Sanger in 12 patients (27%) and 20 DRM were detected by NGS, in 16 (36%). NGS confirmed all the DRM detected by Sanger. The additional six mutations detected by NGS were present in <20% of the sequenced population, being located in the UL97 gene and conferring high-level resistance to ganciclovir. The presence of DRM by NGS was associated with lung transplantation (p = 0.050), the administration of prophylaxis (p = 0.039), a higher mean time between transplantation and suspicion of resistance (p = 0.038) and longer antiviral treatment duration before suspicion (p = 0.024). However, the latter was the only factor independently associated with the presence of DRM by NGS in the multivariate analysis (OR 2.24, 95% CI 1.03 to 4.87).
Conclusions:
NGS showed a higher yield than Sanger sequencing for detecting CMV resistance mutations in SOT recipients. The presence of DRM detected by NGS was independently associated with longer antiviral treatment.
Human cytomegalovirus (HCMV) establishes a highly prevalent life-long latent infection. Though generally subclinical, HCMV infection may have severe consequences during fetal development and in ...immunocompromised individuals. Based on epidemiological studies HCMV(+) serology has been associated with the development of atherosclerosis, immune senescence and an increase mortality rate in elderly people. Such long-term detrimental effects of the viral infection presumably result from an inefficient immune control of the pathogen, depending on the quality and evolution of the individual host-pathogen relationship. Together with antigen-specific T lymphocytes, NK cells play an important role in anti-viral immune defense. HCMV promotes in some individuals the differentiation and persistent steady state expansion of an NK cell subset bearing the CD94/NKG2C activating receptor. The relationship between this adaptive NK cell response to HCMV and aging is overviewed.

La infección congénita por citomegalovirus (CMV) es una causa conocida de morbi-mortalidad perinatal. Se estima que afecta al 0,2-2,5% de los recién nacidos vivos, la tasa de transmisión aumenta en ...el transcurso del embarazo: 20-40% en el primer trimestre y 40-70% en el tercero, siendo que en los 2 primeros trimestres tienen más probabilidad de presentar secuelas. Con respecto a las manifestaciones clínicas se evidencia a un recién nacido con restricción de crecimiento, prematurez con ictericia, hepato-esplenomegalia y petequias, la mortalidad puede llegar al 30% ocasionada por la alteración hepática o por la presencia de infecciones bacterianas secundarias. Para llegar al diagnóstico se requiere de la detección del virus por medio de cultivo o PCR. El tratamiento con ganciclovir intravenoso durante 6 semanas previene el desarrollo de hipoacusia progresiva. El tratamiento con valganciclovir oral puede ser excelente alternativa debido a su buena biodisponibilidad, lo que le permite conseguir niveles plasmáticos similares a los alcanzados con ganciclovir intravenoso.
Introducción: el citomegalovirus (CMV) es un virus de importante presencia a nivel mundial. La detección precoz y seriada de la carga viral para CMV ayuda a tratar precozmente la infección y evita la ...enfermedad que en muchos casos puede ser letal en pacientes inmunodeprimidos.Objetivo: determinar la prevalencia de CMV en niños inmunodeprimidos sometidos a trasplante de médula ósea, por medio de Reacción de Polimerasa en Cadena en tiempo real.Métodos: estudio retrospectivo, descriptivo y transversal. La carga viral fue determinada en muestras de plasma recolectadas entre 2009 y 2012 en niños que recibieron trasplante de médula ósea en el Hospital de la Sociedad de Lucha contra el Cáncer SOLCA en Guayaquil, Ecuador.Resultados: fueron analizadas 38 muestras. La edad promedio fue de 7.2 años siendo 57.9% (n=22) varones y 42.1% (n=16) mujeres, con una relación de masculino/femenino 1.4. Cinco pacientes presentaron resultados positivos por la técnica mencionada, siendo el 80% del género femenino. Y las provincias que obtuvieron el mayor número decasos con resultados positivos fueron Los Ríos, Guayas y Esmeraldas. Los pacientes que fueron trasplantados a causa de leucemia linfoblástica aguda y linfoma leucemizado a leucemia linfoblástica aguda tuvieron resultados positivos por infección activa por CMV, 80 y 20%, respectivamente.Conclusión: la prevalencia de CMV en niños sometidos a trasplante de médula ósea mediante PCR en tiempo real fue del 13%, mayor en el género femenino. Human cytomegalovirus (HCMV) infection promotes the differentiation and persistent expansion of a mature NK cell subset, which displays high surface levels of the activating CD94/NKG2C NK cell ...receptor, together with additional distinctive phenotypic and functional features. The mechanisms underlying the development of adaptive NK cells remain uncertain but some observations support the involvement of a cognate interaction of CD94/NKG2C with ligand(s) displayed by HCMV-infected cells. To approach this issue, the heterodimer and its adaptor (DAP12) were expressed in the human Jurkat leukemia T cell line; signaling was detected by transfection of a reporter plasmid encoding for Luciferase (Luc) under NFAT/AP1-dependent control. Engagement of the receptor by solid-phase bound CD94- or NKG2C-specific monoclonal antibodies (mAbs) triggered Luc expression. Moreover, reporter activation was detectable upon interaction with HLA-E+ 721.221 (.221-AEH) cells, as well as with 721.221 cells incubated with synthetic peptides, which stabilized surface expression of endogenous HLA-E; the response was specifically antagonized by soluble NKG2C- and HLA-E-specific mAbs. By contrast, activation of Jurkat-NKG2C+ was undetectable upon interaction with Human Fetal Foreskin Fibroblasts (HFFF) infected with HCMV laboratory strains (i.e., AD169, Towne), regardless of their differential ability to preserve surface HLA-E expression. On the other hand, infection with two clinical isolates or with the endotheliotropic TB40/E strain triggered Jurkat-NKG2C+ activation; yet, this response was not inhibited by blocking mAbs and was independent of CD94/NKG2C expression. The results are discussed in the framework of previous observations supporting the hypothetical existence of specific ligand(s) for CD94/NKG2C in HCMV-infected cells.
Introducción: la enfermedad de Menetrier es poco frecuente en pediatría, sin etiología establecida, se le ha asociado a infecciones en su mayoría virales y bacterianas capaces de generar un proceso ...inflamatorio que desencadena pérdida de proteínas con clínica de dolor abdominal, náuseas, vómitos y edemas. Con resolución espontánea luego de4-6 semanas sin secuelas. El objetivo es describir un caso de enfermedad de Menetrieren pediatría.Caso clínico: preescolar masculino de tres años edad, sin antecedentes perinatales, con cuadro de cinco días de vómitos, diarrea abundante, no disentérica y fiebre; en los últimos dos días con edema bipalpebral. Luego del examen coprológico presentó: quistes de Giardia lamblia, manejado con metronidazol. Es remitido al Hospital Infantil Napoleón Franco Pareja en Cartagena-Colombia por evidencia de edema bipalpebral.En valoración de urgencias se encontró niño en buenas condiciones generales, con facie de enfermedad aguda, con signos vitales estables, hemograma con ligera leucocitosis, extendido de sangre periférica con granulaciones tóxicas en los neutrófilos, parcial de orina sin proteinuria, albúmina sérica 1.26 gr/dl, transaminasas normal, ionograma normal, perfil lipídico normal, ecografía abdominal con moderada distensión de asas intestinales, endoscopia digestiva alta con hipertrofia de pliegues en mucosa de cuerpogástrico, hiperemia y friabilidad sin erosiones. La biopsia mostró gastropatía crónica activa con marcada hiperplasia foveolar, compatible con enfermedad de Menetrier. Durante su estancia se administró albúmina 1 gr/kg por cuatro días con resolución del edema y proteinograma, se da egreso con niveles de albúmina normales y en sus controles posteriores, el niño continuó asintomático. Con resolución completa de su cuadro clínico.Conclusión: la enfermedad de Menetrier en pediatría se autolimita con una duración de 4 – 6 semanas puede cursar con manifestaciones de dolor abdominal, vómitos, náuseas y edemas. Con las manifestaciones clínicas y hallazgo endoscópico de hipertrofia de pliegues de fondo y cuerpo gástrico se puede presumir el diagnóstico. Se confirma con reporte histopatológico, el curso natural de esta enfermedad es benigno, con resolución completa de los síntomas. Rev.cienc.biomed. 2015; 6(2):354-359

El citomegalovirus (CMV) es una de las infecciones congénitas más frecuentes, con una prevalencia del 0,3-2,4%. En España, al no formar parte del cribado gestacional, se realiza ...screening de los recién nacidos con factores de riesgo y, en muchos centros, de los que presentan bajo peso para la edad gestacional (BPEG). Para ello se realiza, generalmente, determinación de Polymerase Chain Reaction (PCR) del virus en orina y/o ecografía transfontanelar en busca de imágenes compatibles.
El objetivo del estudio es evaluar el rendimiento de la PCR de CMV en orina y ecografía transfontanelar, en recién nacidos >34 semanas asintomáticos, sin factores de riesgo, con BPEG. El objetivo secundario es evaluar el coste-efectividad.
Estudio observacional, descriptivo y retrospectivo, entre enero y diciembre de 2019, en un hospital de tercer nivel (IIIC). Incluye recién nacidos >34 semanas, sin factores de riesgo con BPEG, con PCR de CMV en orina y/o ecografía transfontanelar realizada.
Se incluyeron en el estudio 259 pacientes. Se dividió la muestra en grupo 1 (pacientes con percentil menor de 10 para talla, peso y perímetro cefálico 53 pacientes, 20,5%) y grupo 2 (percentil menor de 10 para peso 206 pacientes, 79,5%). En el grupo 1 se objetivó mayor incidencia de prematuridad tardía, embarazo gemelar, ingreso y consumo de drogas ilegales durante el embarazo. Se realizaron 186 PCR de CMV en orina y 223 ecografías transfontanelares, siendo más frecuente la realización de ambas pruebas en el grupo 1 (p=0,002). Se obtuvo un único resultado positivo en orina en el total de la muestra (0,54%) (grupo 2), sin alteraciones en la ecografía y sin sintomatología a lo largo de su evolución. Se encontraron 2 ecografías transfontanelares alteradas con hallazgos no relacionados con infección congénita por CMV. Se realizó un estudio de coste-efectividad, en el cual el coste del total de ecografías transfontanelares y CMV en orina realizadas en nuestra muestra ascendió a 17.000€, para detección de un caso positivo asintomático.
En nuestra población no parece coste efectivo realizar pruebas de cribado de infección congénita por CMV a recién nacidos a término, y casi a término asintomáticos, cuyo único factor de riesgo es BPEG. Sería necesario ampliar la muestra a otras poblaciones.
Infection by cytomegalovirus (CMV) is one of the most common congenital infections, with a global prevalence of 0.3%-2.4%. In Spain, CMV screening is not performed during pregnancy, but rather in neonates with risk factors, and, in many hospitals, in those born small for gestational age (SGA). Screening is usually performed by measurement of the viral load in urine by polymerase chain reaction (PCR) and/or head ultrasound in search of compatible features.
The aim of the study was to assess the yield of the CMV PCR test in urine and head ultrasound examination in asymptomatic neonates born SGA after 34 weeks’ gestation. The secondary objective was to assess the cost-effectiveness of this strategy.
We conducted an observational and retrospective study between January and December 2019 in a tertiary care hospital. It included neonates delivered after 34 weeks, SGA and without additional risk factors assessed with a CMV PCR test in urine and/or head ultrasound.
The sample included 259 patients. It was divided in two groups: group 1, patients with a head circumference, weight and length below the 10th percentile (53 patients; 20.5%), and group 2, patients in whom only the weight was below the 10th percentile (206 patients; 79.5%). The incidence of late preterm birth, twin pregnancy, neonatal admission and exposure to illicit drugs during gestation was higher in group 1. A total of 186 urine PCR tests and 223 head ultrasounds were performed overall, and both tests were performed more frequently in group 1 (P=.002). There was only one positive CMV PCR test result in the sample (0.54%), corresponding to a patient in group 2 with no abnormal sonographic findings who remained asymptomatic throughout the followup. Two head ultrasound examinations yielded abnormal findings, in both cases unrelated to congenital CMV infection. We performed a cost-effectiveness analysis and determined that the cumulative cost of head ultrasound examinations and urine CMV PCR tests in our sample amounted to €17000 for the detection of a single asymptomatic positive case.
In our population, screening for congenital CMV infection in asymptomatic late preterm and term newborns whose only risk factor is SGA does not seem to be cost effective. It would be necessary to expand the sample to other populations.
Introduction: human immunodeficiency virus infection has gradually increased worldwide in recent years and has become a public health problem, mainly for developing countries. Objective: to carry out ...a review of the most frequent alterations of the posterior segment in these patients with the aim of updating the existing knowledge on the subject. Methods: review of scientific articles in Google Academic and other mega search engines for systematic review using Trip Database and Cochrane. Results: retinopathy associated to human immunodeficiency virus (non-infectious) is the most frequent posterior segment alteration in these patients and cotton wool exudates its most representative clinical finding. Cytomegalovirus retinitis is the most common opportunistic eye infection in patients with very low CD4 T-cell counts; it is seen in up to 40% and is the most frequent cause of blindness in these patients. Conclusions: Toxoplasma chorioretinitis presents in an atypical way in these patients and has a close relationship with cerebral toxoplasmosis, so a complete neurological study should be performed. Sometimes, it is a diagnostic challenge for the Ophthalmology Specialist.
