Rehabilitacija bolesnika nakon osteoporotskog prijeloma

Osteoporoza je bolest karakterizirana niskom mineralnom gustoćom kosti i promjenama u koštanoj strukturi koje dovode do povećanog rizika za prijelom. Najčešći osteoporotski prijelomi su prijelomi ...
kuka, distalne podlaktice, proksimalnog humerusa i prijelomi kralježaka. Vježbe koje se koriste u rehabilitaciji nakon osteoporotskih prijeloma koriste princip koji je baziran na mogućnosti poboljšanja koštane gustoće mehaničkim opterećenjem koje napreže kost i potiče koštano stvaranje. Uobičajeno se koriste vježbe s progresivnim otporom, trening s opterećenjem i trening balansa. Rehabilitacija nakon vertebralnih prijeloma uz primjenu vježbi može poboljšati kvalitetu života, smanjiti kroničnu bol, poboljšati držanje, obrazac hoda, funkcionalni status i kvalitetu kosti. Ključ uspjeha rehabilitacije gerijatrijske populacije nakon prijeloma u području kuka je uključivanje multidisciplinarnog tima i rana mobilizacija. Zbog niske mišićne mase i snage osobe sa sarkopenijom mogu imati poteškoće s pokretljivošću i promjene u držanju koje mogu povećati rizik od padova i prijeloma. Među važne uzroke učestalih padova u starijoj populaciji ubrajaju se razni komorbiditeti, kognitivno oštećenje, lijekovi i prepreke iz okoliša. Okolišni čimbenici uključuju tip površine po kojoj se osoba kreće, rasvjetu, namještaj, različite prepreke i nepostojanje rukohvata. Tjelesna aktivnost, posebno prilagođene vježbe snaženja mišića, vježbe ravnoteže i korigiranje obrasca hoda, mogu smanjiti rizik od pada. Vertebralni prijelomi i prijelomi kuka imaju znatno veći i dugotrajniji utjecaj na kvalitetu života bolesnika u odnosu na ostale prijelome. Prijelomi kuka u gerijatrijskog populaciji imaju negativni utjecaj na sve aspekte života i značajan negativni učinak na kvalitetu života. U prilog tome govore podaci da se samo 40 – 60 % bolesnika oporavi na razinu pokretljivosti prije prijeloma, a do 70 % na razinu aktivnosti u osnovnim aktivnostima dnevnog života.

Preuranjena ovarijska insuficijencija (POI) podrazumijeva prekid ovarijske funkcije prije 40. godine života. Bolest se prezentira amenorejom, hipergonadotropnim hipogonadizmom i neplodnošću. S ovom ...
dijagnozom žene reproduktivne dobi teško se suočavaju. Klinička slika u ovih bolesnica je vrlo raznolika, isto kao i mogući uzroci. POI je povezan s ozbiljnim posljedicama za zdravlje, uključujući psihološko opterećenje, osteoporozu, autoimune poremećaje, ishemijsku koronarnu bolest i povećan mortalitet. Liječenje treba započeti odmah kako bi se spriječile dugotrajne posljedice. Nefarmakološke strategije i promjena životnog stila prva su linija liječenja u slučaju smanjene mineralne gustoće kosti (BMD), zajedno sa suplementima kalcija i D vitamina. Hormonsko nadomjesno liječenje (HNL) omogućava uspostavu hormonske ravnoteže i oponaša normalnu funkciju jajnika.

KOPB je često udružen s komorbiditetima s kojima dijeli iste rizične faktore i ima negativan utjecaj na druge bolesti. KOPB i
njegove komorbiditete uzrokuje i pogoršava upala i mehanizam oksidacije u ...
rasponu od 20 do 81 %. Najveću incidenciju i najveći smrtni rizik nose kardiovaskularne bolesti. Uobičajeni patofiziološki mehanizam je endotelna disfunkcija, prokoagulantno i proinflamatorno stanje te pretilost. Najčešći kardiovaskularni komorbiditeti su koronarna bolest srca, srčano zatajivanje, arterijska hipertenzija i periferna vaskularna bolest. Smanjenje FEV1 povećava učestalost koronarnih događaja i mortalitet. U bolesnika s KOPB-om neophodna je pravovremena dijagnoza kardiovaskularne bolesti kao i dijagnoza KOPB-a u kardiovaskularnih bolesnika. Invazivna i neinvazivna procjena kroničkoga srčanog zatajivanja je otežana u bolesnika s KOPB-om jer bolesnik može imati ograničenja respiracije za vrijeme vježbanja i stresa. Arterijska hipertenzija i periferna vaskularna bolest su česte u bolesnika s KOPB-om zbog endotelne disfunkcije. Funkcija pluća je negativno povezana s krvnim tlakom i u zdravoj populaciji i kod bolesnika s KOPB-om. U bolesnika s KOPB-om prevalencija poremećaja srčanog ritma iznosi 12 do 14 %, a među njima je najčešća fibrilacija atrija. Inhalacija dugodjelujućeg beta 2 agonista ima prihvatljivu podnošljivost kod kardiovaskularnih bolesnika. Osteoporoza je glavni komorbiditet kod bolesnika s KOPB-om, ali je često nedovoljno dijagnosticirana i povezana s cjelokupno lošim zdravstvenim statusom i prognozom KOPB-a. Nedostatak vitamina D povezan je s KOPB-om i komorbiditetima koji smanjuju urođeni i stečeni imunitet. Karcinom pluća, metabolički sindrom, pretilost, malnutricija, depresija, opstruktivna apneja za vrijeme spavanja, disfunkcija mišića i anemija također su komorbiditeti KOPB-a.

Jest chorobą obarczoną ryzykiem trwałego kalectwa i z powodu powikłań wcześniejszej śmiertelności. Ryzyko zachorowania, dotyczące kobiet wzrasta głównie po 65 roku życia. Szacuje się, że na całym świecie osteoporoza dotyka 200 mln kobiet, z których około 20 – 25% dozna urazu w postaci złamania kości. Biorąc pod uwagę fakt wydłużenia życia statystycznej Polki, należy zauważyć, że spowoduje to wzrost liczby kobiet chorujących na osteoporozę oraz złamań będących jej następstwem. Zwiększą się koszta medyczne i społeczne związane z leczeniem choroby i jej następstw. Bezsprzecznie możemy stwierdzić, że wiedza na temat osteoporozy a zwłaszcza czynników jej ryzyka i profilaktyki powinien być dobrze znany środowiskom związanym z opieką zdrowotną, a przede wszystkim osobom narażonym na jej rozwój i konsekwencje. W związku z powyższym zajmowanie się tematyką osteoporozy wydaje się być jak najbardziej zasadne, na co wskazują liczne dane dotyczące zachorowalności, świadczące o fakcie zwiększania się liczby osób chorych na osteoporozę w krajach Unii Europejskiej i w Polsce. Cel pracy: Celem pracy było zbadanie ogólnego poziomu wiedzy o chorobie i czynnikach jej ryzyka wśród kobiet leczących się z powodu osteoporozy Materiał i metody: Grupę badaną stanowiło 137 pacjentek poradni leczenia osteoporozy w Lublinie. Wiek badanych mieści się w przedziale od 38 do 91 lat (M= 61,6; SD=10,6 ). Badania prowadzone były w Wojewódzkim Szpitalu Specjalistycznym w Lublinie oraz Niepublicznym Zakładzie Opieki Zdrowotnej REUMED w Lublinie w okresie od lutego do kwietnia 2015 roku. Do pomiaru zmiennych wykorzystano kwestionariusz ankiety własnego autorstwa zawierający pytania pozwalające ocenić ogólną wiedzę badanych na temat istoty choroby oraz wiedzę na temat czynników ryzyka. Wyniki: Badane wykazują się średnim poziomem wiedzy o osteoporozie. Miejsce zamieszkania różnicuje istotnie poziom wiedzy respondentek o osteoporozie. Osoby mieszkające na wsi charakteryzują się słabszą wiedzą, niż mieszkanki miast. Badane wykazywały się przeciętną wiedzą o czynnikach ryzyka wpływających na rozwój osteoporozy. Najczęściej podawanymi przez badane czynnikami ryzyka było małe pokrycie wapnia w pożywieniu, płeć żeńska oraz wczesna menopauza. Summary Osteoporosis is one of the important health, social and economic world today. It is burdened with the risk of disease and permanent disability due to complications early mortality. The risk relating to women increases mainly after 65 years. It is estimated that worldwide osteoporosis affects 200 million women, of which about 20 - 25% will suffer a fracture of the bone. Given increasing life statistical Pole, it should be noted that this will increase the number of women suffering from osteoporosis and fractures that are its consequence. Will increase the cost of medical and social treatment-related disease and its consequences. Undoubtedly, we can say that knowledge of osteoporosis and in particular its risk factors and prevention should be familiar environments related to health care, and above all persons exposed to its development and consequences. Therefore, dealing with issues of osteoporosis seems to be the most reasonable, as evidenced by the numerous incidence data testifying to the fact of increasing the number of people suffering from osteoporosis in the European Union and in Poland. The aim of the study was to assess the general level of knowledge about the disease and its risk factors among women treated for osteoporosis. Materials and methods: The study group consisted of 137 patients, outpatient treatment of osteoporosis in Lublin. The age of patients is in the range from 38 to 91 years (M = 61.6; SD = 10.6). The research was conducted at the Provincial Specialist Hospital in Lublin and the Non-public Health Care REUMED in Lublin in the period from February to April 2015. For the measurement of the variables used by his own questionnaire containing questions to assess general knowledge of the respondents about the nature of the disease and knowledge of risk factors. Results: Both show the average level of knowledge about osteoporosis. Place of residence differentiate significantly the level of knowledge respondents about osteoporosis. People living in rural areas are characterized by a weaker knowledge than urban residents. The studied showed a significant knowledge of the risk factors affecting the development of osteoporosis. The most commonly quoted by the examined risk factors was little coverage of calcium in the diet, female gender, and early menopause.

Sak Jarosław Abstrakt Wprowadzenie: Współczesna medycyna oraz psychologia coraz więcej czasu poświęcają problemom związanym z osteoporozą oraz przystosowaniu się do niej. Przystosowanie się i akceptacja osteoporozy to skomplikowany proces, na, który wpływają obciążenia chorobowe, uwarunkowania zewnętrzne oraz właściwości jednostki. Akceptacja choroby dla wielu pacjentek jest problemem, z którym zmagają się przez cały okres jej trwania, zmuszając je do akceptacji niedomagań przez nią narzuconych choroby. Pojawienie się choroby ma duże znaczenie, dla jakości życia kobiet. Przede wszystkim zmuszając je do akceptacji niedomagań narzuconych przez chorobę. Cel pracy: Celem pracy było zbadanie stopnia akceptacji choroby kobiet chorujących na osteoporozę. Materiał i metody: W badaniu uczestniczyło 137 pacjentek leczących się z powodu osteoporozy. Grupę badaną stanowiły kobiety od 38-go do 91-g roku życia. Badania przeprowadzono od lutego do kwietnia 2015 roku. W pracy zastosowano kwestionariusz ankiety własnego autorstwa oraz Skalę Akceptacji Choroby (AIS) autorstwa B. J. Felton, T. A. Revenson i G. A. Hinrichsen w adaptacji Z Jurczyńskiego. Wyniki: Kobiety chorujące na osteoporozę wykazują się średnim stopniem akceptacji choroby. Wiek zdiagnozowania choroby oraz wykształcenie różnicuje istotnie badane ze względu na stopień akceptacji choroby oraz problemów narzuconych przez chorobę. Wnioski: Rosnąca liczba osób chorych z powodu osteoporozy stawia przed specjalistami służby zdrowia ogromne wyzwanie. Wdrożenie skutecznych programów profilaktycznych oraz oddziaływanie na zwiększenie świadomości chorujących byłoby pomocne w zapobieganiu chorobie, ewentualnie jej akceptacji w sytuacji wystąpienia. Adaptation and acceptance of osteoporosis is a complex process, for which affect disease burden, external conditions and the characteristics of the individual. Acceptance of the disease for many patients is a problem faced by the whole period of its duration, forcing them to accept the shortcomings imposed by her illness. The emergence of the disease is important for the quality of life of women. First of all, forcing them to accept the shortcomings imposed by the disease. The aim of the study was to examine the degree of acceptance of the disease women suffering from osteoporosis. Material and Methods: The study included 137 patients treated for osteoporosis. The study group were women from 38 th to 91th years. The study was conducted from February to April 2015. The study used a questionnaire survey of his own authorship and Acceptance of Illness Scale (AIS) by B. J. Felton, Revenson T. A. and G. A. Hinrichsen adaptation With Jurczyński. Results: Women suffering from osteoporosis have an average degree of acceptance of the disease. Age diagnose the disease and education differentiates significantly tested due to the degree of acceptance of the disease and the problems imposed by the disease. Conclusions: The increasing number of patients suffering from osteoporosis poses to health care professionals a huge challenge. The implementation of effective prevention programs and impact on increasing awareness of suffering would be helpful in the prevention of disease, or to accept it in a situation occur. Osteoporoza jest chorobą charakteryzującą się niską masą kostną i zniszczeniami mikroarchitektury tkanki kostnej, która prowadzi do kruchości kości i zwiększa ryzyko złamań. Osteoporoza i związane z nią złamania kości mają ogromne znaczenie społeczne i gospodarcze na całym świecie. Badania sugerują, że maksymalna masa kostna i szybkość jej utraty związane ze starzeniem, są zdeterminowane przez czynniki genetyczne w podobnym stopniu jak przez czynniki środowiskowe. Kumulacja homocysteiny jest toksyczna dla układu kostnego (osteoporoza), oka (dyslokacja soczewki) i centralnego układu nerwowego (drgawki, zaburzenia psychiatryczne). Wywołuje także zaburzenia układu naczyniowego (udar i inne zdarzenia sercowo naczyniowe). Cel. Wpływ poziomu homocysteiny w osoczu na ryzyko wystąpienia osteoporozy i jej powikłań. Materiały i metody. Posługując się słowami kluczowymi przeszukano zagraniczne bazy bibliograficzne: Embase, Medline, ScienceDirect, Web of Science. Przeanalizowano badania kliniczne, opublikowane w języku angielskim w międzynarodowych czasopismach. Wyniki. Zaobserwowano związek pomiędzy stężeniem homocysteiny, a utratą masy kostnej. Poziom homocysteiny w osoczu jest znacząco wyższy u kobiet z osteoporozą i odwrotnie proporcjonalny do gęstości kości kręgów lędźwiowych i szyjki kości udowej. Wnioski. Osoczowy poziom homocysteiny jest czynnikiem ryzyka wystąpienia złamań w przebiegu osteoporozy u ludzi w wieku podeszłym. Abstract Admission. Osteoporosis is a disease characterized by low bone mass and microarchitectural damage of bone tissue which leads to bone fragility and increased risk of fracture. Osteoporosis and related fractures are of great social and economic importance throughout the world. Studies suggest that the maximum bone mass and the rate of loss associated with aging are determined by genetic factors to a similar as by environmental factors. Accumulation of homocysteine is toxic to the bone (osteoporosis), the eye (the dislocation of the lens), and central nervous system disorders (seizures, psychiatric disorders). Also causes vascular disorders (stroke and other cardiovascular events). Objective. Effect of plasma homocysteine level at risk for osteoporosis and its complications. Systematic review. Materials and methods. Using the keywords searched foreign bibliographic databases: Embase, Medline, ScienceDirect, Web of Science. We analyzed clinical trials published in English in international journals. Results. A correlation between homocysteine and bone loss. Homocysteine levels in plasma are significantly higher in women with osteoporosis and inversely proportional to the bone density of lumbar vertebrae and hip. Conclusions. The plasma levels of homocysteine is a risk factor for fractures in osteoporosis in the elderly.

Przystosowanie się i akceptacja osteoporozy to skomplikowany proces, na, który wpływają obciążenia chorobowe, uwarunkowania zewnętrzne oraz właściwości jednostki. Akceptacja choroby dla wielu pacjentek jest problemem, z którym zmagają się przez cały okres jej trwania, zmuszając je do akceptacji niedomagań przez nią narzuconych choroby. Pojawienie się choroby ma duże znaczenie, dla jakości życia kobiet. Przede wszystkim zmuszając je do akceptacji niedomagań narzuconych przez chorobę. Cel pracy: Celem pracy było zbadanie stopnia akceptacji choroby kobiet chorujących na osteoporozę. Materiał i metody: W badaniu uczestniczyło 137 pacjentek leczących się z powodu osteoporozy. Grupę badaną stanowiły kobiety od 38-go do 91-g roku życia. Badania przeprowadzono od lutego do kwietnia 2015 roku. W pracy zastosowano kwestionariusz ankiety własnego autorstwa oraz Skalę Akceptacji Choroby (AIS) autorstwa B. J. Felton, T. A. Revenson i G. A. Hinrichsen w adaptacji Z Jurczyńskiego. Wyniki: Kobiety chorujące na osteoporozę wykazują się średnim stopniem akceptacji choroby. Wiek zdiagnozowania choroby oraz wykształcenie różnicuje istotnie badane ze względu na stopień akceptacji choroby oraz problemów narzuconych przez chorobę. Wnioski: Rosnąca liczba osób chorych z powodu osteoporozy stawia przed specjalistami służby zdrowia ogromne wyzwanie. Wdrożenie skutecznych programów profilaktycznych oraz oddziaływanie na zwiększenie świadomości chorujących byłoby pomocne w zapobieganiu chorobie, ewentualnie jej akceptacji w sytuacji wystąpienia. Adaptation and acceptance of osteoporosis is a complex process, for which affect disease burden, external conditions and the characteristics of the individual. Acceptance of the disease for many patients is a problem faced by the whole period of its duration, forcing them to accept the shortcomings imposed by her illness. The emergence of the disease is important for the quality of life of women. First of all, forcing them to accept the shortcomings imposed by the disease. The aim of the study was to examine the degree of acceptance of the disease women suffering from osteoporosis. Material and Methods: The study included 137 patients treated for osteoporosis. The study group were women from 38 th to 91th years. The study was conducted from February to April 2015. The study used a questionnaire survey of his own authorship and Acceptance of Illness Scale (AIS) by B. J. Felton, Revenson T. A. and G. A. Hinrichsen adaptation With Jurczyński. Results: Women suffering from osteoporosis have an average degree of acceptance of the disease. Age diagnose the disease and education differentiates significantly tested due to the degree of acceptance of the disease and the problems imposed by the disease. Conclusions: The increasing number of patients suffering from osteoporosis poses to health care professionals a huge challenge. The implementation of effective prevention programs and impact on increasing awareness of suffering would be helpful in the prevention of disease, or to accept it in a situation occur. Jest chorobą obarczoną ryzykiem trwałego kalectwa i z powodu powikłań wcześniejszej śmiertelności. Ryzyko zachorowania, dotyczące kobiet wzrasta głównie po 65 roku życia. Szacuje się, że na całym świecie osteoporoza dotyka 200 mln kobiet, z których około 20 – 25% dozna urazu w postaci złamania kości. Biorąc pod uwagę fakt wydłużenia życia statystycznej Polki, należy zauważyć, że spowoduje to wzrost liczby kobiet chorujących na osteoporozę oraz złamań będących jej następstwem. Zwiększą się koszta medyczne i społeczne związane z leczeniem choroby i jej następstw. Bezsprzecznie możemy stwierdzić, że wiedza na temat osteoporozy a zwłaszcza czynników jej ryzyka i profilaktyki powinien być dobrze znany środowiskom związanym z opieką zdrowotną, a przede wszystkim osobom narażonym na jej rozwój i konsekwencje. W związku z powyższym zajmowanie się tematyką osteoporozy wydaje się być jak najbardziej zasadne, na co wskazują liczne dane dotyczące zachorowalności, świadczące o fakcie zwiększania się liczby osób chorych na osteoporozę w krajach Unii Europejskiej i w Polsce. Cel pracy: Celem pracy było zbadanie ogólnego poziomu wiedzy o chorobie i czynnikach jej ryzyka wśród kobiet leczących się z powodu osteoporozy Materiał i metody: Grupę badaną stanowiło 137 pacjentek poradni leczenia osteoporozy w Lublinie. Wiek badanych mieści się w przedziale od 38 do 91 lat (M= 61,6; SD=10,6 ). Badania prowadzone były w Wojewódzkim Szpitalu Specjalistycznym w Lublinie oraz Niepublicznym Zakładzie Opieki Zdrowotnej REUMED w Lublinie w okresie od lutego do kwietnia 2015 roku. Do pomiaru zmiennych wykorzystano kwestionariusz ankiety własnego autorstwa zawierający pytania pozwalające ocenić ogólną wiedzę badanych na temat istoty choroby oraz wiedzę na temat czynników ryzyka. Wyniki: Badane wykazują się średnim poziomem wiedzy o osteoporozie. Miejsce zamieszkania różnicuje istotnie poziom wiedzy respondentek o osteoporozie. Osoby mieszkające na wsi charakteryzują się słabszą wiedzą, niż mieszkanki miast. Badane wykazywały się przeciętną wiedzą o czynnikach ryzyka wpływających na rozwój osteoporozy. Najczęściej podawanymi przez badane czynnikami ryzyka było małe pokrycie wapnia w pożywieniu, płeć żeńska oraz wczesna menopauza. Summary Osteoporosis is one of the important health, social and economic world today. It is burdened with the risk of disease and permanent disability due to complications early mortality. The risk relating to women increases mainly after 65 years. It is estimated that worldwide osteoporosis affects 200 million women, of which about 20 - 25% will suffer a fracture of the bone. Given increasing life statistical Pole, it should be noted that this will increase the number of women suffering from osteoporosis and fractures that are its consequence. Will increase the cost of medical and social treatment-related disease and its consequences. Undoubtedly, we can say that knowledge of osteoporosis and in particular its risk factors and prevention should be familiar environments related to health care, and above all persons exposed to its development and consequences. Therefore, dealing with issues of osteoporosis seems to be the most reasonable, as evidenced by the numerous incidence data testifying to the fact of increasing the number of people suffering from osteoporosis in the European Union and in Poland. The aim of the study was to assess the general level of knowledge about the disease and its risk factors among women treated for osteoporosis. Materials and methods: The study group consisted of 137 patients, outpatient treatment of osteoporosis in Lublin. The age of patients is in the range from 38 to 91 years (M = 61.6; SD = 10.6). The research was conducted at the Provincial Specialist Hospital in Lublin and the Non-public Health Care REUMED in Lublin in the period from February to April 2015. For the measurement of the variables used by his own questionnaire containing questions to assess general knowledge of the respondents about the nature of the disease and knowledge of risk factors. Results: Both show the average level of knowledge about osteoporosis. Place of residence differentiate significantly the level of knowledge respondents about osteoporosis. People living in rural areas are characterized by a weaker knowledge than urban residents. The studied showed a significant knowledge of the risk factors affecting the development of osteoporosis. The most commonly quoted by the examined risk factors was little coverage of calcium in the diet, female gender, and early menopause.
