We selected 94 patients with schizophrenia and investigated the seropositivity rate for anti-T. gondii IgG antibodies by ELISA. Clinical parameters of schizophrenic patients such as illness type and status, clinical course, awareness of the illness and need for electroconvulsive therapy (ECT) were compared with their serological status. Anti-T. gondii IgG antibodies were detected in 43 (46%) of schizophrenic patients. Chronic patients had a rate of 34 (72%) seropositivity, whereas 9 (22%) of the patients with partial remission showed evidence of latent toxoplasmosis. Of continuous patients, 35 (81%) were found to be seropositive and this rate was significantly more than in the other groups. The rate of latent toxoplasmosis was detected significantly higher in patients who lack awareness of schizophrenia (36, i.e. 72%) than the patients who were aware of their illnesses (7, i.e. 16%). Anti-T. gondii IgG antibodies were detected in 38 (70%) of ECT performed patients while this percentage was 13% in the ones who had never been treated with ECT. This difference was also statistically significant. We showed that Toxoplasma-infected subjects had 15× higher probability of having continuous course of disease than Toxoplasma-free subjects. Včasnou detekcí malnutrice ve stáří se můžeme vyhnout zhoršení funkční výkonnosti, závažným zdravotním komplikacím, zvýšené nemocnosti, úmrtnosti a zhoršení kvality života. Vyšší věk je spojen s častějším výskytem poruch výživy, především s malnutricí. Prevalence malnutrice u hospitalizovaných geriatrických pacientů se nachází v rozmezí mezi 30–60 % a u dlouhodobě institucionalizovaných dosahuje 52–85 %. Nepřesné posuzování a sledování nutričního stavu seniorů ve zdravotnických zařízeních zdravotnickým personálem může mít zásadní vliv na vznik malnutrice během hospitalizace. Cílem příspěvku je zhodnotit stav výživy seniorů na gerontopsychiatrickém oddělení a posoudit vliv faktorů jako deprese, úroveň kognitivního stavu a soběstačnosti na vznik malnutrice.- The quality of nutrition in old age affects significantly the overall condition of the body, and significant deficits may contribute to multimorbidity in elderly patients. Early detection of malnutrition in old age can avoid worsening functional performance, serious medical complications, increased morbidity, mortality and impaired quality of life. Increasing age is associated with a greater incidence of nutritional disorders, especially with malnutrition. The prevalence of undernutrition in hospitalized geriatric patients is in the range between 30 to 60% and in the long-term institutionalized it reaches 52–85%. Inaccurate assessment and monitoring of nutritional status of older people in healthcare facilities healthcare professionals can have a significant influence on the occurrence of malnutrition during hospitalization. Proto je třeba v klinické praxi věnovat poruchám výživy větší pozornost a ve stáří je aktivně vyhledávat. To se týká především rizikových skupin populace například hospitalizovaných, dlouhodobě ústavně léčených a nesoběstačných seniorů. Hodnocení stavu výživy je nedílnou součástí funkčního geriatrického vyšetření. V praxi to znamená provádět screening nutričních poruch při preventivní prohlídce starších osob, u všech hospitalizovaných seniorů a těch, kteří nastupují ústavní léčbu nebo u nichž je zaváděna domácí ošetřovatelská péče. V práci je podán přehled nově vyvinutých standardizovaných testů. Podrobně jsou popsány Škála pro hodnocení stavu výživy (Mini-nutritional Assessment, MNA) a její zkrácená verze (MNA-SF), zhodnoceny jsou psychometrické charakteristiky obou škál a možnosti i omezení jejich využití.- Ageing is often accompanied by increasing prevalence of nutritional disorders particularly of undernutrition which pose a serious risk of complications, increase morbidity and mortality, deteriorates patient's functional status and performance and his/her quality of life. Therefore, it is necessary to focus clinical attention on nutritional disorders and to screen actively elderly subjects at nutritional risk e.g. hospitalized elderly, residents of long-term care facilities and disabled. Nutritional assessment became an integral part of comprehensive geriatric assessment. In daily practice every elderly should be screened for nutritional deficiencies during bi-annual preventive visit, at every hospitalization, at entering long-term institution and home care service. Bohužel výsledky intenzivní léčby u nemocných starších 60 let s AML, tedy pacientů s nejvyššími riziky rozvoje AML, jsou neuspokojivé. Důvodem těchto neuspokojivých výsledků jsou zvýšená rizika mortality při samotné intenzivní chemoterapii u starších nemocných a dále samotná biologická podstata AML ve vyšším věku, která je spojena se zvýšenou rezistencí k intenzivní léčbě a s vyšším rizikem relapsu. Možnost provedení alogenní transplantace krvetvorných buněk po redukované přípravě v rámci konzolidační léčby za současně se zlepšující podpůrné péče v potransplantačním období může dle publikovaných výsledků zlepšit nepříznivou prognózu AML u nemocných starších 60 let. Limitací uvedeného postupu je fakt, že alogenní transplantaci v současné době nemůže podstoupit většina starších pacientů kurativně léčených pro AML z důvodu nedosažení kontroly základního onemocnění nebo z důvodu neuspokojivého celkového stavu neumožňujícího provedení transplantace. Zavedení nových léčebných postupů do léčby AML a další zlepšení samotných transplantačních postupů může v budoucnu potenciálně zvýšit možnost zařazení transplantace do léčby pacientů starších 60 let s AML.- Acute myeloid leukemia (AML) is disease of older age with the median of age at diagnosis 69 years. Unfortunately results of intensive treatment of patients older than 60 years with AML, thus patients with high-risk of AML development, are unsatisfactory. The reasons for this unsatisfactory results are higher risks of treatment-related mortality among older patients treated with intensive chemotherapy and biological characteristics of AML in older age with higher risk of AML resistance to intensive treatment and higher risk of AML relapse. According to published data reduced-intensity allogeneic transplantation as part of consolidation treatment and advancement in supportive care after transplantation can improve outcome of patients older than 60 years with AML. The limitation of this strategy is a fact that majority of older patients cannot undergo allogeneic transplantation due to unsatisfactory control of AML or due to diminished performance status which does not alow transplant procedure.
