Amaç: Bu çalışmanın amacı normal bireylerde parasingulat sulkus (PCS) uzunluğunun, yaş, cinsiyete göre hemisferler arasındaki farklılıklarını ve PCS uzunluğu ile amigdala, talamus ve hipokampus ...hacimleri arasındaki ilişkiyi araştırmaktır.
Gereç ve Yöntem: 3T manyetik rezonans görüntüleme (MR) cihazında çekilen kranial MR’de patoloji saptanmamış, 20-40 yaşlarında, 48 hasta (26 K, 22 E) çalışmaya alındı. PCS uzunluğu, her iki hemisferde manuel çizildi. PCS uzunluğu 40 mm’den yüksek olanlar belirgin, 20-40 mm arasındakiler kısa, 20 mm’den düşükler yok olarak sınıflandırıldı. Üç grup, hemisferler arasında, yaş ve cinsiyete göre karşılaştırıldı. Amigdala, talamus, hipokampus hacimleri, 3D T1A sagital görüntülerden BrainSuite yazılımıyla hesaplandı. Hacimler ile PCS uzunlukları istatistiksel olarak karşılaştırıldı.
Bulgular: PCS uzunlukları belirgin, kısa, yok gruplarında hemisferler arasında, cinsiyet ve yaşa bağlı anlamlı fark saptanmadı. Sağ hemisferde PCS belirginlerde, sağ amigdala hacmi anlamlı yüksek; PCS kısalarda, sağ hipokampus hacmi en düşük; PCS-belirginlerde sağ hipokampus hacmi yüksekti.
Sonuç: Normal bireylerde PCS uzunluğunda, hemisferler arasında, cinsiyet ve yaşa bağlı fark görülmemesi, PCS’nin, hastalıklarda değişmesinin güvenilir bir bulgu olduğu sonucuna varılabilir. Sağ hemisferde PCS uzunluğu ile amigdala, hipokampus hacminin korele olması, anterior singulat korteks, medial prefrontal korteks bağlantılarının, PCS uzunluğu ile ilişkili olabileceğini düşündürmektedir.
Saf duyusal inme, derin ya da yüzeyel duyuların ayrı ayrı veya beraber tutulduğu, duysal veya irritatif bozukluklar ile kendini gösteren klinik sendromdur. Bu sendromda en sık talamusun ...ventro-posterior nükleusu etkilenir. Talamik etkilenmeleri olan hastalarda yukarı bakış felci, hemiparezi, hemihipoestezi, konfüzyon, uykuya meyil, duygusal labilite olabilmektedir. Talamus tüm duyu modalitelerinin koku hariç ortak kesişim noktası olduğundan tat, işitme, dokunma ve ağrı gibi duyusal modaliteler saf duyusal inmelerde etkilenebilmektedir. 56 yaşında erkek hasta sol kol, bacak, yüz ve gövde yarısında uyuşma yakınması ile başvurdu. Manyetik rezonans görüntülemede sağ talamusta iskemi ile uyumlu lezyon belirlendi. Sonrasında santral nörojenik ağrı sendromu gelişti. Nadir görülmesi, ek belirti olmadan hemihipoestezi ile başvurabilmesi ve hızla tanınıp tedavi başlanması gerektiğinden sunulmaya değerli görülmüştür
The neurotrophins are a family of secreted proteins that mediate numerous
functions in both the developing and mature nervous system, including growth,
survival, differentiation and synaptic ...plasticity of postmitotic neurons. The
binding of neurotrophins (BDNF, NGF) to Trk receptors induces their
dimerization which is followed by autophosphorylation of tyrosine residues
within the intracellular kinase domain, that leads to the activation of
signaling pathways such as the PI3K/Akt and MAPK/ERK pathways. Akt and ERK
kinases play a crucial role in regulating various processes in the brain,
including neuronal proliferation, differentiation, development, migration,
survival and long-term synaptic plasticity. Phosphorylated Akt can protect
cells from apoptosis via stimulation of the expression of proteins that favor
cell survival and by inhibiting executor caspases. Activation of ERK1/2
generally promotes cell survival, although under certain conditions, ERK1/2
can possess proapoptotic properties. Since several studies have revealed a
role for neurotrophins in anesthesia-induced neurotoxicity in the developing
brain, the goal of this study was to explore the potential of anesthetic dose
of propofol to influence neurotrophic signaling pathway in the cortex and
thalamus of 14-day-old (PND14) rats, and also to investigate whether same
treatment is able to induce neurodegeneration and/or changes in synaptic
plasticity. In this study we monitored the spatial and temporal expression of
neurotrophic signaling molecules (BDNF, NGF, TrkA, TrkB, p75, Akt and ERK) in
the brain of PND14 Wistar rats after the application of a single propofol
dose (25 mg/kg i.p). The potential of propofol treatment to induce cell death
was also examined, following the expression of TNF-α, TNFR1, cleaved
caspase-3 fragment, XIAP and NFκB. Finally, we investigated the changes in
the markers of synaptic plasticity (MAP-2, drebrin, GAP-43, synaptophysin,
synuclein-1 and N-cadherin) after the propofol treatment. The structures of
interest were the cortex and thalamus as the primary areas of anesthetic
actions. Changes of the levels of mRNA and proteins of interest were assessed
by RT- or Real Time PCR and Western immunoblot analysis at different time
points during the first 24 h after the treatment. Fluoro-Jade B staining was
used to follow the appearance of degenerating neurons. The obtained results
show that the treatment caused marked alterations in levels of the examined
neurotrophins, their receptors and downstream effector kinases. These changes
were not associated with increased neurodegeneration in either the cortex or
the thalamus, however significant changes in the expression of synaptic
plasticity markers was observed. Our results show that the expression of BDNF
and NGF is differentially regulated after propofol treatment. We consider
that BDNF overexpression in our experimental model is not strictly limited on
neuronal survival, but also on some specific aspects of synaptic plasticity.
Propofol treatment induced region-specific changes in the expression of Akt
and ERK kinases. These results indicate that in the brain of PND14 rats, the
interaction between Akt/ERK signaling might be one of important part of
endogenous defense mechanisms which the developing brain utilizes to protect
itself from potential anesthesia-induced damage. Even though the expression
of cleaved caspase-3 fragment was detected in both examined structures,
Fluoro-Jade B staining did not reveal neurodegeneration. Nevertheless,
significant region-specific changes in the expression of synaptic plasticity
markers was observed. We can conclude that general anesthetic propofol exerts
complex effects on neurotrophic signaling pathways in the postnatal rat
brain. Together, our results contribute towards an improved understanding of
the age-dependent component of anesthesia-induced neurotoxicity and
neuroplasticity; they also bare potential clinical implications.
Neurotrofini čine familiju signalnih molekula sa brojnim uticajima na rast,
preživljavanje, diferencijaciju i sinaptičku plastičnost postmitotskih
neurona, kako u adultnom mozgu, tako i u mozgu tokom razvića. Neurotrofini
(BDNF, NGF) se sa određenim afinitetom vezuju za Trk receptore, iniciraju
njihovu dimerizaciju i autofosforilaciju i na taj način se pokreće aktivacija
signalnih puteva kao što su PI3K/Akt i MAPK/ERK. Akt i ERK kinaze imaju
ključne regulatorne uloge u mozgu u procesima kao što su neuronalna
proliferacija, diferencijacija, razviće, migracija, preživljavanje i
dugotrajna sinaptička plastičnost. Aktivan (fosforilisan) Akt štiti ćeliju od
apoptoze tako što stimuliše ekspresiju proteina koji favorizuju
preživljavanje ćelija i sa druge strane inhibira egzekutorske kaspaze.
Aktivacija ERK1/2 kinaze pomoviše preživljavanje, međutim u određenim
uslovima ERK1/2 može imati i pro-apoptotske odlike. S obzirom da je uloga
neurotrofina u neurotoksičnosti koja je indukovana anestezijom tokom ranog
postnatalnog razvića pokazana u nekoliko studija, cilj ove doktorske
disertacije je bio da se na molekulskom nivou ispitaju promene u signalnom
putu neurotrofina koje nastaju u kori i talamusu postnatalnih pacova nakon
primene anestetske doze propofola, kao i da se utvrdi potencijal tretmana da
indukuje ćelijsku smrt i/ili promene u sinaptičkoj plastičnosti. Nakon
jednokratne primene propofola (25 mg/kg i.p.) ispitana je vremenska i
prostorna ekspresija neurotrofina BDNF i NGF, njihovih aktiviranih receptora
TrkB, TrkA i p75 receptora, kao i nishodnih kinaza Akt i ERK kod 14 dana
starih (PND14) pacova. Ispitan je i potencijal propofolskog tretman da
indukuje ćelijsku smrt, praćenjem ekspresije TNF-α, TNFR1, aktivnog fragmenta
kaspaze-3, njegovog inhibitora XIAP proteina, kao i transkripcionog faktora
NFκB. Finalno je ispitivan potencijal propofolskog tretmana da indukuje
promene u ekspresiji markera sinaptičke plastičnosti (MAP-2, drebrina,
GAP-43, sinaptofizina, sinukleina-1 i N-kadherina). Promene su praćene u kori
i talamusu kao primarnim ciljevima dejstva anestetskog delovanja. Primenom
sledećih metoda Western blot analize, RT- i PCR-a u realnom vremenu
ispitivane su promene u ekspresiji ciljnih proteina i iRNK, a primenom
Fluoro-žad B histološkog bojenja analizirana je pojava neurona u
degeneraciji. Dobijeni rezultati su pokazali da je propofolski tretman doveo
do značajnih promena u nivoima ispitivanih neurotrofina, njihovih receptora i
nishodnih efektorskih kinaza. Ove promene nisu povezane sa povećanom
neurodegeneracijom ni u kori, ni u talamusu, ali su detektovane značajne
promene u zastupljenosti ispitivanih markera sinaptičke plastičnosti. Naši
podaci pokazuju da je ekspresija neurotrofina BDNF i NGF različito regulisana
u odgovoru na propofolski tretman i da se najznačajnije promene u PND14 mozgu
odigravaju preko BDNF signalnog puta i u znatno manjoj meri preko NGF
signalnog puta. Smatramo da prekomerna ekspresjia BDNF-a u našem
eksperimentalnom modelu nije isključivo ograničena na neuronalno
preživljavanje, već i na određene aspekte sinaptičke plastičnosti. Pored
promene u ekspresiji neurotrofina, propofolski tretman je indukovao promene u
ekspresiji Akt i ERK kinaza na regionalno-specifičan način. Fluoro-žad B
histohemijsko bojenje nije pokazalo neuronalnu degeneraciju, uprkos prolaznom
povećanju u ekspresiji aktivnog fragmenta kaspaze-3. Detektovane su značajne
regionalno-specifične promene u ekspresiji markera sinaptičke plastičnosti
nakon propofolskog tretmana. Možemo zaključiti da opšti anestetik propofol
ispoljava kompleksne efekte na neurotrofine i njihove nishodne signalne
puteve u mozgu pacova tokom ranog postnatalnog razvića. Dobijeni rezultati
ukazuju da u mozgu PND14 životinja aktivacija Akt i ERK signalnih puteva
predstavlja važan aspekt endogenih odbrambenih mehanizama kojim se mladi
mozak štiti od potencijalnih neželjenih efekata izazvanih jednokratnom
primenom propofola. Balansirana ekspresija ove dve kinaze u istom regionu
mozga je važna za odgovarajući izlazni signal i povoljan fiziološki ishod.
Preciznije razjašnjenje molekulskih mehanizama koji se pokreću nakon
izlaganja mladih eksperimentalnih životinja opštoj anesteziji može pomoći u
boljem razumevanju starosno-zavisne komponente neurotoksičnosti i
neuroplastičnosti koje izaziva primena opštih anestetika u kliničkoj
pedijatriji.
No volumetric brain magnetic resonance imaging (MRI) study has been performed in hypochondriac patients to date. In the present study, we aimed to volumetrically evaluate the orbito-frontal cortex ...(OFC), anterior cingulate, caudate nucleus, and thalamus. The volumes of the OFC, thalamus, caudate nucleus, and anterior cingulate cortex were measured in 16 treatment-naive hypochondriac patients and 16 healthy control subjects. Volumetric measurements were made with T1-weighted coronal MRI images, with 1.5-mm-thick slices, at 1.5T and were done blindly. Hypochondriac patients had significantly smaller mean left and right OFC, and greater left but not right thalamus volumes compared with healthy controls. There was no difference in regard to caudate and anterior cingulate volumes for either side between groups. The Hamilton Depression Rating Scale (HDRS) scores were not associated with any volumetric parameter studied. Age at onset showed a significant relationship to left OFC, and left thalamus volumes. Duration of illness also exhibited a significant association with left OFC, and left thalamus volumes. In conclusion, this morphometric MRI study showed that patients with hypochondriasis had smaller left and right OFC and significantly greater left thalamus volumes compared to healthy controls. Taken together, our findings suggest that abnormalities in the OFC and thalamus seem to play an important role in the pathophysiology of hypochondriasis.
U ovom doktorskom radu proučavani su opseg i vrijeme pojave pokazatelja oštećenja i
oporavka posljedično traumatskoj ozljedi mozga (engl. traumatic brain injury, TBI) u dosad
nedovoljno istraženim ...regijama mozga (talamus, mali mozak, rostralni dio velikog mozga).
Pokusi su rađeni na odraslim Hannover-Wistar štakorima muškog spola. TBI je inducirana
metodom lateralne ozljede mozga tlakom tekućine. Štakori su žrtvovani prvog, trećeg ili
sedmog dana nakon TBI, a kontrolnu skupinu činile su životinje žrtvovane prvog dana nakon
lažne ozljede. Uzorci mozgova proučavani su Western blot i TUNEL metodama,
histološkim/imunohistološkim analizama te Fluoro-Jade B bojenjem. U talamusu ozlijeđenih
životinja pronađene su progresivna degeneracija i gubitak neurona, programirana stanična smrt
neovisna o aktivnoj kaspazi-3, a posredovana čimbenikom indukcije apoptoze (AIF), reaktivna
astrocitoza i mikroglioza, te ograničene neuroplastične promjene tijekom prvog tjedna nakon
TBI. U malom mozgu ozlijeđenih štakora, prvog dana nakon TBI, neurodegenerativne
promjene zabilježene su u stražnjim režnjićima, a potom su ponovno uočene sedmog dana
nakon traume u širem području ove strukture. U IX. režnjiću bio je detektiran i značajan gubitak
Purkinjeovih stanica u svim istraživanim vremenskim točkama. Najjače izražena apoptoza, koja
nije bila posredovana aktivacijom kaspaze-3 niti AIF-om, zabilježena je prvog dana nakon
ozljede u svim slojevima kore IX. režnjića. Pojačani AIF signal pronađen je u bijeloj tvari
malog mozga trećeg i sedmog dana nakon TBI. Iako su ekspresije markera astrocita i mikroglije
bile smanjene u ipsilateralnoj polovici malog mozga nakon TBI, na mjestima oštećenja
Purkinjeovih stanica zabilježena je njihova aktivacija. Diskretne neuroplastične promjene
nakon TBI detektirane su u IX. režnjiću. U strukturama rostralnog dijela velikog mozga
degeneracija i gubitak neurona nisu zabilježeni u prvom tjednu nakon TBI. U frontalnom
korteksu ozlijeđenih štakora pronađene su reaktivna astrocitoza i mikroglioza, te stanice
pozitivne na DCX, marker neuroblasta, a neuroplastične promjene nisu uočene. Promjene
imunoreaktivnosti DCX-a detektirane su i u subventrikularnoj zoni i rostralnom migratornom
putu ozlijeđenih životinja. Sumarno, rezultati ovog rada pokazali su kako TBI u štakora
uzrokuje pojavu znakova oštećenja i oporavka u istraživanim moždanim strukturama u
različitim vremenskim točkama tijekom prvog tjedna nakon ozljede.
In this doctoral thesis regional and temporal distribution of the parameters of damage and
repair following traumatic brain injury (TBI) were investigated in the rat brain structures which
have not been previously studied extensively (thalamus, cerebellum, rostral part of the
cerebrum). Experiments were done on adult male Hannover-Wistar rats. TBI was induced by
lateral fluid percussion injury method. Rats were sacrificed on the first, third or seventh day
after TBI, and the rats sacrificed on the first day after sham injury were used as the control
group. Brain samples were examined by Western blot and TUNEL methods,
histologic/immunohistologic analyses, and by Fluoro-Jade B staining. In the thalamus of
injured animals progressive degeneration and neuronal loss, programmed cell death, which was
caspase-independent, but mediated by apoptosis inducing factor (AIF), reactive astrocytosis
and microgliosis as well as limited neuroplastic changes were detected during the first week
after TBI. In the cerebellum of the injured rats, on the first day after TBI, neurodegenerative
changes were discovered in the posterior lobules, and on the seventh day after brain trauma,
diffusely through this brain structure. In the lobule IX, a significant loss of Purkinje cells was
detected at all the investigated time points. Most significant apoptosis, which was caspase-3
and AIF independent, was found on the first post-injury day in all the cortical layers within the
lobule IX. Enhanced AIF signal was detected in the cerebellar white matter on the third and
seventh day after TBI. Even though the expressions of astrocytic and microglial markers were
lower in the ipsilateral halves of the cerebellum after TBI, activation of these cells was
discovered in the areas of pronounced Purkinje cells’ loss. Discrete neuroplastic changes after
TBI were also detected in the lobule IX. In the structures of the rostral part of the cerebrum,
degeneration and neuronal loss were not detected in the first week after TBI. In the frontal
cortex of the injured animals reactive astrocytosis and microgliosis were found as well as the
cells which expressed DCX, a neuroblast marker, while other neuroplastic changes were not
revealed. Changes in the DCX immunoreactivity were also detected in the subventricular zone
and the rostral migratory stream in the injured animals. In conclusion, results of this thesis
demonstrated that TBI in the rat causes appearance of the sings of damage and repair in the
investigated brain structures in different time points within the first week after the injury.

The role of acetylcholinesterase (AChE) in the developing neocortex was reexamined by comparing its expression in rats, mice and guinea pigs, following the protocol for acetylcholinesterase ...histochemistry (described in Sendemir et al., 1996) in order to determine the suitability of the breeding colony at UludaÛ University for use as an animal model. A total of 103 pups as well as two adult animals of each species were used. In the rat pups, acetylcholinesterase-rich patches were distributed in a vibrissa-related array in the somatosensory cortex soon after birth, whereas regions of the cortex lying between individual patches and between rows of patches in the were depleted enzyme. Mice on postnatal day 3 and older mice revealed lightly stained, acetylcholinesterase positive spots in the center of barrel cores, while the barrel walls remained devoid of acetylcholinesterase; septae that divided the individual barrels were densely enzyme positive. The barrels in the guinea pigs formed before birth. Well- formed barrels were observed by postnatal day 0, and acetylcholinesterase activity gradually decreased by postnatal day 10 but did not fade away. The differential enzyme location in different rodents indicates that its role in the development of thalamocortical connectivity is distinctly different in rats as opposed to mice and guinea pigs.
