Purpose: This study aimed to investigate the dyadic adjustment and sexual functions in patients with bipolar disorder (BD) during the remission period, and examine the effect of a history of ...childhood trauma on these.
Materials and Methods: This cross-sectional study is consisted of 80 (46 female, 34 male) patients with bipolar disorder-1 and 40 (25 female, 15 male) healthy controls. The Dyadic Adjustment Scale (DAS), Childhood Trauma Questionnaire (CTQ-28), Arizona Sexual Questionnaire (ASEX) were administered to the participants.
Results: The patient group showed significantly higher trauma scores in all subscales and total scores of the CTQ-28, and lower scores in the subscales and total score of DAS compared to the control group. 82.5% of bipolar patients and 57.5% of healthy controls were found to have sexual dysfunction (SED). According to the results of linear regression and multiple regression analyses, age and DAS-total score predicted SED, and male gender, absence of SED and CTQ-total score predicted DAS score.
Conclusion: Patients with BD have more dyadic problems, sexual dysfunction, and trauma history than healthy controls. Sexual function and dyadic adjustment affect each other in two ways and a history of childhood trauma has a negative effect on both sexual function and dyadic adjustment.
Amaç: Bu çalışmada, remisyon dönemindeki bipolar bozukluk tanılı hastaların çift uyumunun ve cinsel işlevlerinin araştırılması ve çocukluk çağı travması öyküsünün çift uyumuna ve cinsel işlevlere olan etkisinin incelenmesi amaçlanmıştır.
Gereç ve Yöntem: Kesitsel tipteki bu çalışma, remisyon döneminde bipolar bozukluğu olan 80 (46 kadın, 34 erkek) hasta ve 40 (25 kadın, 15 erkek) sağlıklı kontrolden oluşmaktadır. Katılımcılara sosyodemografik veri formu, çiftler uyum ölçeği (ÇUÖ), çocukluk çağı ruhsal travma ölçeği (CTQ-28), Arizona cinsel yaşantılar ölçeği (ASYÖ) uygulanmıştır.
Bulgular: Hasta grubunda kontrol grubuna göre CTQ-28'in tüm alt ölçeklerinde ve toplam puanlarında istatistiksel olarak anlamlı derecede yüksek, ÇUÖ alt ölçek ve toplam puanlarında ise istatiksel olarak anlamlı daha düşük puanlar saptandı. Bipolar bozukluk tanılı hastaların %82,5'inde ve sağlıklı kontrollerin %57,5'inde cinsel işlev bozukluğu saptandı. Lineer regresyon analizi sonuçlarına göre yaş ve ÇUÖ-toplam puanı cinsel işlev bozukluğunu yordamaktadır.
Sonuç: Bipolar bozukluk tanılı hastalarda sağlıklı kontrollere göre daha fazla çift sorunu, cinsel işlev bozukluğu ve travma öyküsü bulunmaktadır. Cinsel işlev ve çift uyumu karşılıklı olarak birbirini etkilemektedir. Çocukluk çağı travma öyküsü hem cinsel yaşamı hem de çift uyumunu olumsuz yönde etkilemektedir.
Purpose: The purpose of this study was to assess analgesic applications accompanied by numerical and verbal pain scores, radiological imaging, and consultation requests for genital traumas, which ...affect a small percentage of middle and high school students.
Materials and Methods: This retrospective study included 237 middle school and high school age, 10-18 years old patients who applied to the emergency department due to genital trauma between January 2019 and December 2020. The types of trauma, radiological imaging, genital organ injuries, analgesia applications, consultations, and verbal and numerical pain scale scores of patients were all evaluated.
Results: Of the 237 patients included in the study, 90 (38%) were female, and the mean age was 13.80±2.37 years (range, 10-18 years). Of the patients, 136(57.4%) were admitted with falls, 63(26.6%) traffic accidents, 35(14.8%) penetrating and 3(1.3%) iatrogenic injuries. The numerical pain score was 6.63±1.43 in falls, 6.09±1.66 in traffic accidents, 4.80±1.53 in penetrating injuries, and 3.67±1.15 in iatrogenic traumas. Analgesics were administered to 93(39.2%) of all patients. The severity of pain was mild in 43(18.1%) patients. It was moderate in 140(59.1%) and severe in 54(22.8%).
Conclusion: Genital traumas that occur alone are extremely rare and are frequently associated with multiple organ injuries. Genital traumas have a wide variety of etiological causes and complications. In cases of pediatric genital trauma, the use of verbal and numerical pain scales may be beneficial in evaluating patients, determining imaging requirements, and determining analgesia applications.
Amaç: Ortaokul ve lise çağının küçük bir kısmını oluşturan genital travma olgularında, sayısal ve sözel ağrı skoru eşliğindeki analjezik uygulamalarının, radyolojik görüntülemelerinin ve konsültasyon istemlerinin değerlendirilmesi amaçlandı.
Gereç ve Yöntem: Bu retrospektif çalışmaya Ocak 2019-Aralık 2020 tarihleri arasında genital travma nedeniyle acil servise başvuran ortaokul ve lise çağındaki 10-18 yaş arası 237 hasta dahil edildi. Hastaların travma sınıflamaları, radyolojik görüntülemeleri, genital yaralanma şekilleri, analjezi uygulamaları, konsültasyonları, sözel ve sayısal ağrı skalası sonuçları değerlendirildi.
Bulgular: Çalışmaya alınan 237 hastanın 90'ı (%38) kadın olup, yaş ortalaması 13.80±2.37 yıl (dağılım, 10-18 yıl) idi. Hastaların 136'sı (%57,4) düşme, 63'ü (%26,6) trafik kazası, 35'i (%14,8) penetran ve 3'ü (%1,3) iyatrojenik yaralanma ile başvurdu. Sayısal ağrı skoru düşmelerde 6.63±1.43, trafik kazalarında 6.09±1.66, penetran yaralanmalarda 4.80±1.53 ve iyatrojenik travmalarda 3.67±1.15 idi. Tüm hastaların 93'üne (%39,2) analjezik uygulandı. 43 (%18.1) hastada ağrı şiddeti hafifti. 140'ında (%59.1) orta, 54'ünde (%22.8) şiddetli idi.
Sonuç: İzole genital travmalar çok nadirdir ve sıklıkla çoklu organ yaralanmaları ile ilişkilidir. Genital travmalarda; etiyolojik nedenler ve komplikasyonlar oldukça çeşitlilik gösterir. Pediatrik genital travma olgularında hastaların değerlendirilmesinde, görüntüleme gereksinimlerinde ve analjezi uygulamalarında sözel ve sayısal ağrı skalalarının kullanılması faydalı olabilir.
Purpose: We aimed to analyze imaging tests and findings in a series of geriatric trauma patients admitted to emergency department (ED) at different time intervals.
Materials and Methods: Two groups ...of 300 randomly selected patients over consecutive five-year periods were compared. Patients admitted in the first and second five-year periods were recorded as group I and II, respectively. A comprehensive comparison was carried out between two groups regarding the age, sex, reason for admission, comorbidities, Revised Trauma Score (RTS), Glasgow Coma Scale (GCS), radiological findings and number of radiological examinations, dose length product (DLP) values and duration of hospital stay.
Results: The number of patients who underwent X-ray and CT examinations and total number of X-rays and CT scans in group II was higher than those in group I. CT scans were negative for a trauma-related finding in 49% of patients in group I and 55% of patients in group II. In patients with radiological evidence of trauma, no significant difference was observed between two groups regarding the major trauma related change. However, the trauma-related minor findings on CT were more common in group II than in group I.
Conclusion: Despite the increasing use of imaging tests, there was no difference in imaging findings suggesting major trauma or requiring a longer inpatient stay. This may be related to the increasing use of radiological examinations over the years as a result of the orientation towards defensive medicine.
Purpose: This study was designed to determine the transformations that occur during the Covid-19 pandemic trauma process experienced by individuals. In this context, it is aimed to determine the ...awareness level of the Turkish community about Covid-19, to obtain information about the ways individuals deal with the process, and to reveal the psychological, social and economic effects of the pandemic process on individuals.
Matherials and Methods: Within this study, the “Coronavirus Effect Scale” was developed and the scale, which was developed during the pandemic period, was applied to 2115 participants, who were selected to equally represent each region in Turkey.
Results: It was revealed that, during this coronavirus pandemic process that was discussed as a trauma, the participants had a high level of awareness about coronavirus, experienced difficulties in economic and psychological terms, attempted to cope with the process through positive thoughts and spiritual means, and improved their philosophies of life with an awareness on themselves.
Conclusion: Within the framework of the research, "Coronavirus Effect Scale" was developed. As a result of the application of the developed scale, it was observed that the pandemic process, which is being experienced, affects the individuals forming the Turkish society in psychological, social and economic dimensions. People experience changes and transformations with the traumatic process.
Amaç: Bu araştırma Covid-19 pandemi sürecinde bulunan bireylerin yaşadıkları travma süreciyle gerçekleşen dönüşümleri tespit etmek amacıyla kurgulanmıştır. Bu bağlamda araştırmada Türk toplumunun Covid-19’a ilişkin farkındalık seviyesinin belirlenmesi, bireylerin süreçle başa çıkma yollarına ilişkin bilgiler elde edilmesi ve pandemi sürecinin bireylerdeki psikolojik ve sosyal etkilerinin ortaya konulması amaçlanmaktadır.
Gereç ve Yöntem: Araştırma kapsamında “Koronavirüs Etkisi Ölçeği” geliştirilmiş ve pandemi süreci devam ederken geliştirilen ölçeğin çevrimiçi ortamda Türkiye’de bulunun her bölgeyi temsil eder nitelikte 2115 katılımcıya uygulaması yapılmıştır.
Bulgular: Bir travma olarak ele alınan koronavirüs pandemisi süreci ile birlikte katılımcıların koronavirüse ilişkin farkındalık düzeylerinin yüksek seviyede bulunduğu, ekonomik ve psikolojik anlamda zorluklar yaşadıkları, pozitif düşünceler ve manevi yollarla süreçle başa çıkma yoluna gittikleri ve kendilerine ilişkin farkındalıklarıyla birlikte, yaşam felsefelerinde gelişim yaşadıkları ortaya konulmuştur.
Sonuç: Araştırma çerçevesinde "Coronavirus Etki Ölçeği" geliştirilmiştir. Geliştirilen ölçeğin uygulanması sonucunda yaşanmakta olan pandemi sürecinin Türk toplumunu oluşturan bireyleri psikolojik, sosyal ve ekonomik boyutlarda etkilediği görülmüştür. Bireylerin travmatik süreçle birlikte değişim ve dönüşümler yaşadığı anlaşılmıştır.
Bu araştırma Karabük üniversitesi öğrencilerinde çocukluk çağı travmalarının suçluluk-utanç duyguları ve benlik saygısı ile ilişkisinin incelenmesi amacıyla planlandı. Araştırma Karabük ...üniversitesinde 400 öğrenci ile gerçekleştirildi. Araştırmada elde edilen veriler çocukluk çağı travmaları ölçeği (CTQ-28, ÇÇTÖ), suçluluk ve utanç duyguları ölçeği (SUTÖ), Coopersmıth benlik saygısı ölçeği (BSÖ) yardımıyla elde edildi ve elde edilen puanlar istatistiksel paket programı kullanılarak analiz edildi. Araştırmada benlik saygısı ölçek puanı artıkça çocukluk çağı travma ölçeğinde ve çocukluk çağı suçluluk ölçeği alt boyutlarında azalma olduğu görüldü. Diğer yandan bütün Çocukluk çağı travma ölçeği alt boyutları ile Çocukluk çağı suçluluk ölçeği arasında istatistiksel olarak anlamlı ve pozitif yönlü güçlü bir ilişkinin olduğu görüldü (p<.001).

The study of the effects of social events experienced by an individual on epigenetic marks on the genome has created the ...field of social epigenetics. Social epigenetics examines the effects of psychosocial stress factors such as poverty, war trauma and childhood abuse on epigenetic mechanisms. Epigenetic mechanisms alter chemical markers in the genome structure without changing the DNA sequence. Among these mechanisms, DNA methylation in particular may have different phenotypic effects in response to stressors that may occur in the psychosocial environment. Post-traumatic stress disorder is one of the most significant proofs of the effects of epigenetic expressions altered due to traumatic events on the phenotype. The field of epigenetic inheritance has shown that epigenetic changes triggered by environmental influences can, in some cases, be transmitted through generations. This field provides a better understanding of the basis of many psychological disorders. This review provides an overview of social epigenetics, PTSD, and epigenetic inheritance.
Öz: Göç kişinin çevresinde oluşturduğu değişimler nedeniyle insan gelişimi için önem taşıyan bir olgudur. Bu derleme çalışmasında göç olgusu Bronfenbrenner’ın Ekolojik Sistemler Kuramı’na göre ...değerlendirilmektedir. Göç sonucu mikrosistem, mezosistem, ekzosistem ve makrosistemde değişimler ortaya çıkar ve bu değişimlerle birlikte sistemler arası etkileşimler göçmen çocuğun gelişimini bazen olumlu bazen de olumsuz etkilemektedir. Çocuk gelişimini incelemek için önemli alanlardan biri de çocuk oyunlarıdır. Göç deneyiminin çocuklar üzerindeki etkilerini ve çocukların göç deneyiminin oluşturduğu değişimin yarattığı stres ile baş etme biçimlerini değerlendirmek için çocuk oyunlarının incelenmesi önemlidir. Oyunlar, çocukların göç olgusuna açıklama getirmelerini ve olay üzerinde kontrol sahibi hissederek kaygılarını azaltmalarını sağlar. Literatürde göç deneyiminin çocuk gelişimi üzerindeki etkisini inceleyen çalışmalar sınırlı olmakla birlikte göç sonrası çocuk gelişimini değerlendirmek amacıyla çocuk oyunlarını inceleyen çalışmalar yok denecek kadar azdır. Bu derleme çalışmasında göç eden çocukların oyunlarını inceleyen çalışmalar gözden geçirilerek ilerideki araştırmalar için öneriler sunulmaktadır. Abstract: Migration is a very important phenomenon changing the whole life of an individual. Especially children are influenced most from this change. In this review article, migration is evaluated according to Bronfenbrenner’s theory. After migration, changes are occurred in Microsystem mesosystem, exosystem and macrosystem. Changes in ecological systems of children and their interactions may effect development of immigrant children through positively and negatively. This big change in the child’s life besides influencing their whole development affects their play behavior too. Evaluating children’ play is important for investigate to effects of migration on child development and children’ coping with this changes. These behaviors help children bring explanations to migration, feel in control and as a result decrease their anxiety. Besides there are a little research about effect of migration on child development, there are a few research evaluate children’s play after migration. In this review, studies that examine the play of immigrant children are reviewed and suggestions are offered for future studies.
BPurpose: This study aimed to analyze epidemiology and the treatment approaches for pediatric maxillofacial trauma patients who presented to the Department of Plastic, Reconstructive, and Aesthetic ...Surgery during 5 years period.
Materials and Methods: Age and gender distributions of the patients were determined. Duration of hospitalization, etiology of trauma, location of the fracture, and treatment methods applied was determined, and the results of these treatments and complications that developed were evaluated.
Results: The fractures were the most common at the age of 17 years (18.3%). The most common cause of trauma in patients operated for maxillofacial trauma was fall (48.3%). Most operations were performed in the age range of 12–17 years (50.6%), and panfacial fractures were the most frequently operated fracture localization (27.58%).
Conclusion: The simplest and most effective treatment should be applied for pediatric maxillofacial trauma. While conservative treatments may be sufficient in minimally displaced fractures, open reduction and internal fixation methods are applied in fractures with greater displacement. Fixation materials must necessarily be removed since they prevent bone growth. The use of bioabsorbable plates has been limited in recent years due to their high costs.
Amaç: Bu çalışmanın amacı beş yıl boyunca Plastik, Rekonstrüktif ve Estetik Cerrahi Anabilim Dalına başvuran pediatrik maksillofasiyal travma hastalarının epidemiyolojisinin ve tedavi yaklaşımlarının analiz edilmesidir.
Gereç ve Yöntem: Hastaların yaş ve cinsiyet dağılımları belirlendi. Hastanede yatış süresi, travmanın etyolojisi, fraktürün yeri, uygulanan tedavi yöntemleri tespit edilirken bu tedavilerin sonuçları ve gelişen komplikasyonlar değerlendirildi.
Bulgular: Kırıkların en sık görüldüğü yaş 17’ydi (%18,3). Maksillofasiyal travma nedeniyle opere edilen hastalarda en sık travma nedeni düşmeydi (%48,3). En fazla opere edilen yaş aralığı 12-18 (%50,6), en fazla opere edilen kırık lokalizasyonu panfasiyal kırıklardı (%27,58).
Sonuç: Çocukluk çağı maksillofasiyal travmalarda en basit ve en etkili tedavinin uygulanması gerekir. Az deplasmanlı kırıklarda konservatif tedaviler yeterli olabilirken daha fazla deplasman olan kırıklarda açık redüksiyon ve internal tespit yöntemleri uygulanmaktadır. Kemik büyümesini engellediğinden dolayı tespit materyallerin mutlaka çıkarılması gereklidir. Son yıllarda eriyebilen plakların yüksek maliyetleri nedeniyle kullanımı kısıtlı olmaktadır.
Son yıllarda ani kardiyak ölüm oranlarında artış görülmektedir. Günümüzde, implante edilebilir kardiyoverter defibrilatör (implantable cardioverter defibrilator, ICD) ani kardiyak ölümleri önlemede ...kullanılan bir cihazdır. Travma sonucunda cihazda birtakım problemler meydana gelebilmektedir. Kablo kırığı bu problemlerden biridir. Literatürde, künt travmaya bağlı ICD komplikasyonlarının, özellikle kablo kırığının oldukça nadir görüldüğü belirtilmektedir. Bu olgu sunumunda; eski eşi tarafından sol kolu çekilerek künt travmaya maruz kalan ve oluşan kablo kırığı nedeniyle uygunsuz şoklara maruz kalan kadın olgu medikolegal açıdan değerlendirildi. Olguda, göğse direkt bir travma olmadan, sol kolun çekilmesine bağlı olarak lead kırığı meydana gelmiştir. Hafif şiddetteki bir travmanın bile, yaşamsal tehlike oluşturabilecek, ciddi klinik sonuçlara neden olabileceğine dikkat çekmek amaçlandı.
