    Impaired fibrinolysis, due to elevated levels of plasminogen activator inhibitor-1(PAI-1) is associated with increased risk of cardiovascular diseases. Excess of circulating PAI-1 may influence ... fibrin degradation and also directly influence vessel wall remodelling. In obesity levels of PAI-1 are increased and could account for high risk of cardiovascular disease in obese individuals. The mechanisms involved in increased PAI-1 production in obesity are far from being explained. Therefore, the aim of our studies was toelucidate the role of adipose tissue in plasma PAI-1 regulation. In the first 3 studies, plasma PAI-1, adipsin, leptin, insulin, lipoproteins and fibrinogen were determined in obese women before and after weight reduction program and after 5 months of follow up. Furthermore, intima-media thickness was measured at entry and after followup. In the fourth study, PAI-1 expression in abdominal and femoral subcutaneous fat was studied before and after weight reduction. Our studies showed that the decrease in PAI-1 after weight reduction was more closely related to adipose tissue changes, than to changes in metabolic variables. We found that relatively low serum leptin levels might be predictive for weight regain after successful weight reduction. By studying the extent of carotid atherosclerosis, we found that early atherosclerotic changes may be reversed by normalisation of body weight and its long-term maintenance. The fourth study showed that PAI-I expression is increased in abdominal, but not in femoral subcutaneous fat of obese subjects. However, after weight reduction the decrease in PAI-1 mRNA content in abdominal subcutaneous fat was not assoviated with decrease in plasma PAI-1. (Abstract truncated at 2000 characters).
