    Background: Developmental dysplasia of the hip (DDH) is the most frequent pathology affecting juvenile hip. In last two decades DDH is well-controlled disorder and does not pose any significant ... health problem. Nowadays we are frequently facing late consequence of DDH in a form of hip osteoarthritis. Even though there are specific guidelines in a literature about conservative and operative management of DDH, data concerning the late consequences of DDH treatment is sparse. A reason for this is a long follow up between the occurrence of DDH and development of hip osteoarthritis, mostly lasting few decades. Hip osteoarthritis is a common disorder affecting substantial amount of population and causing significant morbidity. Although mechanical and chemical reasons are implicated in occurrence of hip osteoarthritis, the exactmechanism remains elusive. Aim: The aim of this work was to find out which is the target geometry of the hip in DDH by means of conservative or operative treatment in order to improve longterm result as much as possible. The goal was also to establish the relevance of different biomechanical parameters in determination of long-term results of DDH treatment. Methods: The first part embraced 27 patients (45 hips), who were treated conservativelyfor DDH in Dept. of Orthopaedic Surgery between 1955 and 1965. In all hips geometrical and biomechanical status of the hip was determined on basis of pelvic X-rays taken after closing of triradiate cartilage. Patients were examined clinically using Harris and D`Aubigne scoring system on average 29 years after X-rays were taken. The second part comprised 33 patients (38 hips) in whom Salter's osteotomy was performed for DDH in Dept. of OrthopaedicSurgery between 1974 and 1983. Hips were analysed geometrically soon after surgery and biomechanically on average 10 years after surgery. (Abstract truncated at 2000 characters)
