  • Bulozna keratopatija po operaciji katarakte na Oddelku za očesne bolezni v Mariboru v letih 1988-1998 = Bullous keratopathy after cataract surgery at the Maribor teaching hospital Department of ophthalmology between 1988-1998

    Background. Bullous keratopathy (BK) is a result of corneal decompensation. The aim of our paper is to show its incidence, its causes and the treatment ofpatients with BK after extracapsular cataract ... extraction (ECCE). Methods. Retrospectively the documentation of 43 patients with BK hospitalized at our department between 1988 and 1998 was analysed. The study includes 19 patients with BK after ECCE performed at our department. Results. Of the 29 patients (0.56% of all operated senile cataracts in this period) with BK after ECCE, 13(0.43%) had an intraocular lens implanted into the posterior chamber (IOL post.), 3 (1.36%) into the anterior chamber (IOL ant.), in 3 cases (2.38%) an intraocular lens was not implanted - postoperative aphakia. In patients with IOL post. implantation, postoperative inflammation was noted in 7 cases, in one case a preoperatively described corneal degeneration was confirmed - cornea guttata, in 5 patients it was not possible to establish the cause of BK. In the group of 3 patients with IOL ant. implantation, postoperative inflammation was noted in 2 cases and unstable IOL ant. in one case. Among thepatients with post operative aphakia there were 2 cases of lens mass remnants causing inflammation in the anterior chamber, in one case a mechanical lesion of the corneal endothelium had occurred during surgery. Conservative local therapy was applied in 9 patients (473.%), transplantation of the cornea -penetrating keratoplasty (PK) was done in 10 patients (52.7%). A total of 19 PK (11.9%) for BK after cataract surgery were performed during this period. Conclusions. At our department the incidence of BK after ECCE is 0.56%, in patients with IOL post. implantation it is only 0.45%, which corresponds to other European hospitals. The main reason is post operatave inflammation. PK was done in over one half of our patients.
