    Bronchoalveolar lavage (BAL) is a diagnostic approach done during fiber bronchoscopy. It enables counting and differentiating of cells, ratois betweenthem, measuring concentrations of cellular ... products and proteins. It isa standard diagnostic procedure in almost every interstitial lung disease. Procedure is safe, minimally invasive and reveals specific information in somedisorders (pulmonary alveolar proteinosis, Langerhans cell histiocytosis, alveolar hemorrhage, asbestosis, carcinosis). Information derived from the differential cell count is in general non-specific and thus just supportive indiagnostic procedure. In sarcoidosis and hypersensitivity pneumonitis in accordance with adequate clinical signs and radiological findings, BAL can confirm the diagnose. Biochemical parameters, including cytokines in the BAL have been frequently described. Till now, none of them has been of importance for clinical evaluation of diseses. BAL is not the recomended procedure to routine performance or serial use in patients with ILD.
