    Rhinosinusitis is one of the most common diseases of the upper respiratory tract in general practice. Most cases of acute rhinosinusitis are caused by viruses, rarely by bacteria and fungi. The ... common bacteria seen in acute bacterial rhinosinusitis include Streptococcus pneumoniae, Haemophilus influenzae and Moraxella catarrhalis. Acute bacterial rhinosinusitis is usually a secondary infection following acute viral upper respiratory tract infection that impairs the mucus clearance mechanism and results in sinus ostia obstruction. Clinical signs and symptoms o f acute viral and bacterial rhinosinusitis are extraordinarily similar and non-specific. The diagnosis is made clinically. Sinus radiography is not recommended for the diagnosis in routine cases, especially not in children. The majority of viral rhinosinusitis cases resolves without speci fic treatment. For the initial empirical treatment o f acute bacterial rhinosinusitis, amoxicillin is still recommended. Patients who do not respond to initial antibiotic treatment, who have recurrent rhinosinusitis or in whom signs and symptoms of complication are present should be refered for further evaluation in hospital.
