    Background. Heart failure is an important cause of morbidity and mortality that affects mainly elderly people. With retrospective analysis of hospitalized patients with heart failure we wanted to ... establish the major differences in management of elderly patients with heart failure in our hospital. Methods. We studied records of 205 patients with heart failure admitted to General hospital Izola during the period between 1. January and 31. June 2004. We compared data on hospital management of 131 patients > 75 years old (mean age 82 5 years) and 74 patients <= 75 years old (mean age 66 8 years). Results. Among elderly patients there were significantly more women (58% vs. 41%, p = 0,02). Elderly patients were more likely to have hypertensive heart disease (44% vs. 20%, p < 0,001) and less likely ischemic heart disease (42% vs. 32%, p = 0,16). They had higher rates of arterial hypertension (66% vs. 54%, p = 0,1), chronic atrial fibrillation (47% vs. 24%,p < 0,01), impaired renal function (65% vs. 49%, p = 0,02) and anemia (51%vs. 28%, p < 0,01). Echocardiography was performed in higher percentage among younger patients (82% vs. 62 %, p < 0,01), who had higher prevalence of systolic dysfunction (52% vs. 37%, p = 0,07). Elderly patients were less likely to receive beta-adrenergic blockers (30% vs. 42%, p = 0,08), spironolactone (19% vs. 38%, p < 0,01) and anticoagulation therapy (31% vs. 45%, p = 0,06). Conclusions. Elderly patients with heart failure are more likely to be female with hypertensive heart disease and preserved left ventricular systolic function. The differences in management between older andyounger patients with heart failure in our hospital certainly exist but aresmaller than we expected.
