    Otitis externa maligna is a rare, but aggressive and often difficult to control, potentially fatal inferction of the meatus acusticus externus. At tthe Department of Otorhinolaryngoloty and ... Cervicofacial Surgery, Medical Center, Ljubljana, 18 patients were treated in the period 1973-1996, of which 17 were cured and one fatility was recorded. Because of the potentially severe complications, a correct and early diagnosis of the disease is of paramount importance for the outcome. New insights into the pathogenesis of otitis externa maligna made it possible to employ more successful methods in the treatment of the disease. At the Department of Otorhinolaryngology and Cervicofacial Surgery, Medical Centre, Ljubljana with considreations of our own clinical experience we employed these new approaches in the diagnosis and therapy of otitis externa maligna very early and successfully. While at the beginning a radical surgical approach alone was considered appropriate, this approach was soon superseded by a combination of surgical and systemic antibiotic therapy. Today a therapy of systemic and topical application of fluoroquinolones is the therapy of choice.
