    Background. Portable instruments that measure the prothrombin time (PT) and automatically calculate the international normalised ratio (INR) with the use of drop of whole blood have simplified the ... treatment of patients who are receiving warfarin therapy. We performed this study to determine the agreementbetween portable monitor and laboratory results. Methods. This study encompassed 133 patients (61 women and 72 men, aged 45 to years) on long-term anticoagulant therapy and 35 patients (19 women and 16 men, aged 55 to 80 years) starting anticoagulant therapy. Capillary blood samples were measured with CoaguChek (Boehringer Mannheim, Germany) instrument. Results were compared to those found with ACL-3000IL (Instrumentation laboratory) using citrated venous plasma. International sensitivity index of the thromboplastin used with ACL-3000 instrument was 1. 4. The difference in INR values between the two methods was calculated as the monitor value minus the clinical laboratory value. Agreement was achieved if the portable monitor and laboratory results were either within or outside the patients targeted therapeutie range or if the two results were within 0.4 international normalized ratio unites of each other. Results. The relationship between INR results from capillary blood samples measured with CoaguChek and venous plasmasamples measured with ACL-3000 instrument in 133 patients on long-term anticoagulant therapy was expressed by r = 0,98, p < 0,001; y =1,04x +0,05 INRand in 35 patients starting anticoagulant therapy was expressed by r = 0,83, p < 0,05; y = 0,84x+0,61 INR. Agreement was observed in 97% patients on long-term anticoagulant therapy and in 54% patients starting anticoagulant therapy. (Abstract truncated at 2000 characters).
