Lobomycosis or lacaziosis is a chronic granulomatous fungal infection caused by Lacazia loboi. Most cases are restricted to tropical regions. Transmission is believed to occur through traumatic ...inoculation in the skin, mainly in exposed areas. It is characterized by keloid-like nodules. There are only a few hundred cases reported. The differential diagnoses include many skin conditions, and treatment is difficult. The reported case, initially diagnosed as keloid, proved to be refractory to surgical treatment alone. It was subsequently approached with extensive surgery, cryotherapy every three months and a combination of itraconazole and clofazimine for two years. No signs of clinical and histopathological activity were detected during follow-up.

Também são escassos os ...trabalhos com seguimento dos pacientes. OBJETIVOS: Relatar nove casos observados no período de 1981 a 2004, confrontando seus achados com os 20 casos da literatura. MÉTODOS: Foram analisadas variáveis epidemiológicas, clínicas, histopatológicas e terapêuticas. RESULTADOS: Foram observados o predomínio na raça negra (8:1), no sexo feminino (4:1) e nos trópicos (9:1); idade de início até os quatro anos (93%); deficiência de alfa 1-antitripsina (7%); aortite (7%); dermatoses relacionadas (62%); aspecto facial envelhecido (97%), fase atrófica estável na infância após período variável de meses a anos com lesões inflamatórias (97%). A histopatologia das lesões recentes mostrou infiltrado mais intenso e perda elástica menor do que nas lesões tardias. A cirurgia de reconstrução deu bons resultados na fase atrófica, enquanto o ácido retinóico tópico a 0,05% não foi efetivo. CONCLUSÕES: A elastólise pós-inflamatória e cutis laxa na fase aguda correlacionaram-se com diversas dermatoses inflamatórias, que promoveram elastólise (62%). A abordagem adequada dessas doenças pode diminuir a extensão do quadro atrófico final, que tem como boa opção terapêutica a cirurgia reconstrutora. Sugere-se pesquisar a deficiência de alfa 1-antitripsina.
BACKGROUND: Although rare diseases, one third of all cases of post-inflammatory elastolysis and cutis laxa published in the international literature are from the state of Minas Gerais, in Brazil. Reports with long term follow-up of these patients are also scarce. OBJECTIVES: To report nine cases observed from 1981 to 2004 and compare the findings with 20 cases described in the literature. METHODS: Epidemiological, clinical, pathological and therapeutical variables were assessed. RESULTS: The onset of the disease is usually up to the age of four years (93%), and it is more common in African Americans (8:1), females (4:1) and in tropical climate (9:1). Other features include alpha-1-antitrypsin deficiency (7%), aortitis (7%), related skin conditions (62%), premature facial ageing (97%) and stable atrophic phase during childhood after months or years with inflammatory lesions (97%). Biopsies of recent lesions showed a more intense inflammatory infiltrate and less elastolysis than those obtained from older lesions. Reconstructive surgery yielded good results during atrophic phase and 0.05% topical tretinoin was ineffective. CONCLUSIONS: In 62% of patients the acute phase of post-inflammatory elastolysis and cutis laxa is associated with different elastolysis-promoting inflammatoy dermatoses. The correct management of these conditions may reduce severity of atrophic lesions, which can also be treated by reconstructive surgery. Alpha-1-antitrypsin deficiency should be investigated.

Introduction: there are few data available about pityriasis versicolor (PV) in pregnant women and/or infected by human immunodeficiency virus (HIV) and the cutaneous manifestations related to HIV are ...based predominantly in men. Objectives: to estimate prevalence and variables associated whit PV, seborrheic dermatitis (DS), xerosis (XE) and pruritus (PR) in 816 women: pregnant or nonpregnant and infected or uninfected with HIV. Methods: cross-sectional study including patients of public services in the city of Belo Horizonte, Brazil, examined by dermatologists from July 2008 to April 2011. The dermatoses were diagnosed based on their clinical findings, and the PV was confirmed by mycological examination. There were evaluated several variables of interest for each of these diseases, such as sociodemographic and behavior characteristics, the use of antiretroviral (ARV) and the markers of HIV progression. For the statistical analysis there were used the Kruskal-Wallis test to check the homogeneity between the groups, the X2 test and Fisher's Exact test, for evaluating the qualitative effect of the predictors, and the Mann-Whitney test for verifying quantitative predictors. The multivariate analysis was performed by logistic regression, keeping only the significant variables in the final model (p <0.05). Results: the prevalence of PV was 4.9%, increasing your chance of four times when the family history of PV (HFPV) was positive, in 10 times when there was previous history for PV (HPPV) and decrease your chance in 0.9 times for each year increase in age. DS prevalence was 3.6%, with three times more chance to occur in patients with oily skin, two more chance in HIV-infected women and, in these women, 2.8 more chance when viral load was greater than or equal to 400 copies/mL. The prevalence of XE was 18.4%, with a increasing chance of 20 times in pregnant women, 250 times in those HIV-infected and, in these women, at 5.7 times when they were using ARV. PR prevalence was 15.6%, with a greater chance of 18 times in the pregnant women, seven times in XE and two times in atopic. Conclusions: the pregnancy was significantly associated with XE and PR, but not with PV and DS. HIV infection was significantly associated with DS and XE, but not with PV and PR. HFPV and HPPV positive were associated with a greater chance of PV, while advancing age associated with a lower chance. High chance of PR was still associated with the presence of XE and atopy.
Introdução: existem poucos dados disponíveis sobre pitiríase versicolor (PV) em mulheres grávidas e/ou infectadas pelo vírus da imunodeficiência humana (HIV) e as manifestações cutâneas relacionadas ao HIV se baseiam predominantemente em homens. Objetivos: estimar a prevalência e as variáveis associadas à PV, dermatite seborreica (DS), xerodermia (XE) e prurido (PR) em 816 mulheres: grávidas ou não grávidas, infectadas ou não infectadas pelo HIV. Métodos: estudo transversal que incluiu pacientes de serviços públicos da cidade de Belo Horizonte, Brasil, examinadas por dermatologistas no período de julho de 2008 a abril de 2011. As dermatoses foram diagnosticadas com base nos seus achados clínicos, sendo a PV confirmada pelo exame micológico. Foram avaliadas diversas variáveis de interesse para cada uma dessas doenças, tais como características sociodemográficas e de comportamento, uso de antirretroviral (ARV) e marcadores de progressão do HIV. Para a análise estatística foram utilizados: o teste de Kruskal-Wallis, para verificar a homogeneidade entre os grupos; o teste X2 e o teste Exato de Fisher para avaliar o efeito dos preditores qualitativos; e o teste de Mann-Whitney para verificar os preditores quantitativos. A análise multivariada foi feita por regressão logística, mantendo-se somente as variáveis significantes no modelo final (p<0,05). Resultados: a prevalência da PV foi de 4,9%, com aumento da sua chance em quatro vezes quando a história familiar para PV (HFPV) foi positiva, em 10 vezes quando havia a história pregressa para PV (HPPV) e diminuição da sua chance em 0,9 vezes para cada ano de aumento na idade. A prevalência da DS foi de 3,6%, com três vezes maior chance de ocorrer em pacientes de pele oleosa, duas vezes maior chance nas infectadas pelo HIV e nestas mulheres, 2,8 vezes maior chance quando a carga viral foi igual ou superior a 400 cópias/mL. A prevalência da XE foi de 18,4 %, com aumento da sua chance em 20 vezes nas grávidas, em 250 vezes nas infectadas pelo HIV e, nestas, em 5,7 vezes quando usavam ARV. A prevalência do PR foi de 15,6%, com aumento da sua chance em 18 vezes nas grávidas, em sete vezes nas xerodérmicas e em duas vezes nas atópicas. Conclusões: a gravidez apresentou associação significativa com a XE e o PR, mas não com a PV e a DS. O HIV associou-se significativamente a DS e XE, mas não a PV e PR. HFPV e HPPV positivas se associaram a alta chance de PV e o avanço da idade a menos chance. Alta chance de PR se associou ainda à presença de XE e atopia.
