Background and purpose
This study was undertaken to assess skin biopsy as a marker of disease onset and severity in hereditary transthyretin amyloidosis with polyneuropathy (ATTRv‐PN), a treatable ...disease.
Methods
In this single center retrospective study, skin Congo red staining and intraepidermal nerve fiber density (IENFD) were evaluated in symptomatic ATTRv‐PN patients and asymptomatic TTR gene mutation carriers between 2012 and 2019. Non‐ATTRv subjects with suspected small fiber neuropathy who underwent skin biopsy during the same timespan were used as controls.
Results
One hundred eighty‐three symptomatic ATTRv‐PN patients, 36 asymptomatic carriers, and 537 non‐ATTRv patients were included. Skin biopsy demonstrated amyloid depositions in 80% of the 183 symptomatic cases. Skin amyloid deposits were found in 75% of early stage ATTRv‐PN patients, and in 14% of asymptomatic carriers. All 183 symptomatic and 34 of 36 asymptomatic patients displayed decreased ankle IENFD with a proximal–distal gradient distribution, and reduced IEFND correlated with disease severity and duration.
Conclusions
Our study demonstrates skin amyloid deposits are a marker of ATTRv‐PN disease onset, and decreased IENFD a marker of disease progression. These results are of major importance for the early identification of ATTRv‐PN patients in need of disease‐modifying treatments.
Skin biopsy provides useful information in hereditary transthyretin amyloidosis with polyneuropathy. Amyloid deposition in skin is a biomarker of disease onset in paucisymptomatic subjects, especially in early onset Val30Met. Small nerve fiber reduction, which often precedes disease onset, correlates with disease severity.
Hereditary transthyretin amyloidosis is a life-threatening autosomal dominant systemic disease due to pathogenic
variants (ATTRv), mostly affecting the peripheral nerves and heart. The disease is ...characterised by a combination of symptoms, organ involvement and histological amyloid deposition. The available disease-modifying ATTRv treatments (DMTs) are more effective if initiated early. Pathological nerve conduction studies (NCS) results are the cornerstone of large-fibre polyneuropathy diagnosis, but this anomaly occurs late in the disease. We investigated the utility of a multimodal neurological and cardiac evaluation for detecting early disease onset in ATTRv carriers.
We retrospectively analysed a cohort of ATTRv carriers with normal NCS results regardless of symptoms. Multimodal denervation and infiltration evaluations included a clinical questionnaire (Lauria and New York Heart Association (NYHA)) and examination, intra-epidermal nerve fibre density assessment, autonomic assessment based on heart rate variability, Sudoscan, meta-iodo-benzyl-guanidine scintigraphy, cardiac biomarkers, echocardiography, MRI and searches for amyloidosis on skin biopsy and bone scintigraphy.
We included 130 ATTRv carriers (40.8% men, age: 43.6±13.5 years), with 18 amyloidogenic
gene mutations, the majority of which was the late-onset Val30Met variant (42.3%). Amyloidosis was detected in 16.9% of mutation carriers, including 9 (6.9%) with overt disease (Lauria>2 or NYHA>1) and 13 asymptomatic carriers (10%) with organ involvement (small-fibre neuropathy or cardiomyopathy). Most of these patients received DMT. Abnormal test results of unknown significance were obtained for 105 carriers (80.8%). Investigations were normal in only three carriers (2.3%).
Multimodal neurological and cardiac investigation of TTRv carriers is crucial for the early detection of ATTRv amyloidosis and initiation of DMT.
Paraneoplastic motor neuron disorders (MND) are rare conditions; their exact clinical and electrophysiological phenotype have not been exhaustively described yet. The purpose of this study is to ...depict the main characteristics of paraneoplastic MND to highlight the features that may allow its diagnosis. Based on the description of eight original cases, and on the revision of 21 patients identified from a systematic review of the literature, the main features of paraneoplastic MND can be summarized as follows: (1) subacute; (2) lower motor neuron syndrome, associated or not with upper motor neuron involvement; (3) predominant asymmetric upper limb involvement; (4) presence of other non-motor neurological manifestations, including sensory neuronopathy; (5) signs of inflammation in the cerebrospinal fluid (CSF); (6) neurological improvement or stabilization after immunotherapy and tumor treatment. The diagnosis of paraneoplastic MND may be difficult because of its rarity, the absence of pathognomonic clinical features, and the frequent absence of prior tumor history. However, it is of capital importance to correctly identify patients with paraneoplastic MND, as this represents a potentially treatable condition. In the presence of subacute lower motor neuron impairment, especially when atypical clinical features for degenerative MND or other non-motor neurological manifestations are present, we recommend testing for onconeural antibodies. In the case, the search for onconeural antibodies is negative, but it exists a strong clinical suspicion for a paraneoplastic etiology; CSF analysis and total-body 18FDG-PET/CT imaging should be performed to circumstantiate diagnosis.
Transthyretin Familial Amyloid Polyneuropathy (TTR-FAP) is a rare disease with autosomal dominant transmission due to a point mutation of the TTR gene. By removing the main source of systemic mutant ...TTR, liver transplantation (LT) has become the reference therapy of this severe and fatal polyneuropathy of adult-onset, stopping disease progression in subgroup of patients. Recently, new therapeutic strategies have emerged, which intend to stabilize TTR or to silence the TTR gene. Amongst them, the TTR kinetic stabilizer tafamidis is the first drug approved in the EU.
We shall review the natural history of TTR-FAP and the best indications for LT. Data on the efficacy, safety and tolerability of the TTR kinetic stabilizers, tafamidis and diflunisal, have been reviewed, from the pivotal Phase III clinical trials published in PubMed medical journals or presented at international meetings. We will review the ongoing phase III clinical trials of TTR gene silencing with RNAi therapeutics and ASO published in clinicaltrialgov.
Due to the data on efficacy, tolerability, safety, tafamidis and diflunisal became the first line anti-amyloid treatment in stage 1 TTR-FAP. Both drugs slow progression of the disease. Only tafamidis got marketing authorization. We are waiting for results of the 2 phase III clinical trials of TTR gene silencing in varied stages of the disease.
Les critères diagnostiques électrophysiologiques de polyradiculoneuropathie inflammatoire démyélinisante chronique (PIDC) publiés en 2010 incluent des réponses retardées des potentiels évoqués ...somesthésiques (PES). Différentes composantes ont été décrites comme d’intérêt. L’objectif de ce travail est de tester les différents critères décrits sur une population de PIDC par comparaison à des témoins appariés. Un recueil rétrospectif sur 3 centres (GH Pitié-Salpétrière, HP Saint-Joseph, CHU Bicêtre) a permis de sélectionner 10 patients (7 hommes), de moyenne d’âge au début des symptômes de 59 ans (34–83) ; la moyenne d’âge au diagnostic est de 70 ans (41–84). Les présentations cliniques sont majoritairement des formes atypiques : 4 formes sensitives pures, 1 atteinte monomélique du membre supérieur, 2 atteintes sensitivomotrices des 4 membres, des épisodes répétitifs de diplopie et troubles sensitifs, 1 présentation motrice pure sans bloc de conduction, 1 ataxie à la marche. Tous répondaient à un traitement immunomodulateur de première ou deuxième intention. Dix témoins appariés pour l’âge et la taille ont été sélectionnés. Ont été étudiés les latences des ondes N9 (point d’Erb) et N13 (médullaire), le temps de conduction (TC) N9-N13 aux membres supérieurs ; les latences des ondes N8 (creux poplité), N18 (radiculaire) et N22 (médullaire) ainsi que les TC N8-N22, N8-N18 et N18-N22 aux membres inférieurs. Les amplitudes de N9 via les rapports N5/N9, N9/N14 et P9/N13 étaient également testées. Enfin, a été analysée l’onde P15 périphérique proximale (crête iliaque-creux glutéal) par sa latence et ses TC. Les éléments qui, après correction de Bonferroni, restaient significativement différents entre la population de PIDC et de témoins sont les latences N9, N13, N18, N22 et le TC P15-N22. En revanche, aucun des rapports d’amplitude testés ne ressortait. Au total, les PES peuvent apporter des éléments d’atteinte proximale via l’étude des latences ; des critères diagnostiques précis restent encore à établir.
Le bortézomib (Velcade® ) est un inhibiteur du protéasome indiqué dans le traitement de première ou deuxième ligne des myélomes multiples. Il peut être administré en monothérapie ou associé au ...melphalan, à la prednisone ou à la thalidomide. Plus rarement, il est utilisé pour d’autres indications (protocoles de désimmunisation sur rejets de greffes notamment). Le risque de survenue d’une neuropathie induite par bortézomib est compris entre 31 et 45 % selon la littérature 1–3 . Les deux principaux facteurs de risque de neuropathies induites par bortézomib sont la dose cumulée totale reçue et l’existence d’une neuropathie sous-jacente. Ces neuropathies nécessitent parfois l’arrêt du bortézomib pourtant essentiel chez ces patients. Les neuropathies sous bortézomib sont classiquement décrites comme des polyneuropathies distales douloureuses symétriques cliniques associées à un électroneuromyogramme compatible avec une polyneuropathie axonale distale et symétrique à prédominance sensitive. Une implication motrice est décrite dans 10 % des cas et quelques cas de neuropathies démyélinisantes ont également été évoqués. Dans ce travail, nous rapportons les présentations clinico-électriques des neuropathies survenues sous bortézomib dans le service de neurophysiologie clinique du CHU Bicêtre. Ces 10 patients illustrent le véritable polymorphisme de ces neuropathies : douloureuses, ataxiantes, mononeuropathies multiples, polyradiculoneuropathies aiguës possiblement asymétriques et polyneuropathies axonales longueur dépendantes. Les facteurs favorisants retrouvés dans notre revue de cas sont concordants avec ceux de la littérature. Deux mécanismes physiopathologiques semblent se dégager : un mécanisme causal dose-dépendant et lié à une sensibilité individuelle et un mécanisme favorisant dépendant de l’existence d’une neuropathie sous-jacente. Il semble donc important de dépister précocement et surveiller les signes neurologiques et neurophysiologiques chez tous les patients devant recevoir du bortézomib quel qu’en soit la dose pour limiter le développement de ces neuropathies.
Les neuropathies carentielles peuvent prendre un aspect électro-clinique trompeur, pouvant mimer un syndrome de Guillain-Barré.
Nous présentons 3 cas de neuropathie rapidement évolutive dans un ...contexte de carence vitaminique.
Observations Trois patients âgés de 28 à 31 ans ont présenté un tableau de neuropathie périphérique sensitivomotrice des 4 membres d’installation subaiguë dans un contexte de carence vitaminique potentielle : une femme ayant bénéficié d’un by-pass gastrique avec suites opératoires compliquées de vomissements avec perte de 65kg, une patiente présentant une dépression avec anorexie avec amaigrissement de 25kg et un homme éthylique chronique avec vomissements récents et perte de 3kg. Les ENMG montrent une atteinte sensitivomotrice des 4 membres non-longueur dépendante, avec des éléments de démyélinisation sans remplir les critères officiels. L’étude du LCS est normale : absence de méningite et d’inflammation. Les bilans immunologiques et infectieux sont négatifs. Le bilan métabolique montre une carence nette en vitamines B1 et B9 dans 2 cas sur 3, qui a été retenue comme causale avec amélioration clinique et électrophysiologique lente après supplémentation. Dans le troisième cas, une carence en vitamine B9 isolée est retrouvée et le diagnostic de polyradiculopathie aiguë inflammatoire démyélinisante compliquant une polyneuropathie axonale ancienne est retenue avec amélioration après traitement par veinoglobulines.
Les 3 patients ont en commun un contexte pouvant favoriser une carence vitaminique. Dans ces 3 cas, l’ENMG ne permettait pas de trancher formellement entre les étiologies inflammatoire et carentielle. Il est important dans de telles situations de doser les vitamines et supplémenter sans délai mais aussi effectuer un bilan exhaustif pour ne pas méconnaître un diagnostic différentiel dysimmunitaire. La carence en vitamine B1 est une cause de neuropathie rapidement évolutive à traiter rapidement. Il faut savoir l’évoquer en contexte favorisant mais aussi ne pas méconnaître un diagnostic différentiel de traitement spécifique.

Motor unit number index (MUNIX) is proposed to monitor neuromuscular disorders. Our objective is to determine the intra-individual variability over time of the MUNIX.
In 11 different hospital centres, MUNIX was assessed twice, at least 3 months apart (range 90–360 days), in tibialis anterior (TA), abductor pollicis brevis (APB), abductor digiti minimi (ADM) and deltoid muscles in 118 healthy subjects. MUNIX sum score 2, 3 and 4 were respectively the sum of the MUNIX of the TA and ADM, of the TA, APB and ADM and of the TA, APB, ADM and deltoid muscles.
The repeatability of the MUNIX was better for sum scores than for single muscle recordings. The variability of the MUNIX was independent of sex, age, interval between measurements and was lower for experienced than non-experienced operators. The 95th percentile of the coefficient of variability of the MUNIX sum score 2, 3 and 4 were respectively 22%, 18% and 15% for experienced operators.
The MUNIX technique must be performed by experienced operators on several muscles to reduce its variability and improve its reliability.
A variation of the MUNIX sum score ≥20% can be interpreted as a significant change of muscle innervation.
