Summary
Cow's milk is a common cause of food allergy in children. Children usually outgrow cow's milk allergy by the age of 3–5 years, but some will have persistent symptoms beyond childhood. We ...performed a systematic review of randomized controlled trials (RCTs) and observational studies to assess the evidence supporting the use of oral immunotherapy in IgE‐mediated cow's milk allergy to inform the World Allergy Organization guidelines. Of 1034 screened articles published until May 2011, five RCTs and five observational studies fulfilled a priori specified inclusion criteria. RCTs including 218 patients showed that oral immunotherapy, compared to elimination diet alone, increased the likelihood of achieving full tolerance of cow's milk relative risk: 10.0 (95% CI: 4.1–24.2). Adverse effects of immunotherapy include frequent local symptoms (16% of doses), mild laryngospasm relative risk: 12.9 (1.7–98.6), mild asthma rate ratio: 3.8 (2.9–5.0), reactions requiring oral glucocorticosteroids relative risk: 11.3 (2.7–46.5) or intramuscular epinephrine injection rate ratio 5.8 (1.6–21.9). Results of observational studies were consistent with those of RCTs. Despite the availability of RCTs, the overall low quality of evidence leaves important uncertainty about anticipated effects of immunotherapy due to very serious imprecision of the estimates of effects and the likelihood of publication bias for some of the critical outcomes. A potentially large benefit of oral immunotherapy in patients with cow's milk allergy may be counterbalanced by frequent and sometimes serious adverse effects. Additional, larger RCTs measuring all patient‐important outcomes are still needed.
Introduction L'utilisation croissante de sucres ajoutés dans les aliments transformés a contribué, ces dernières décennies, à fortement augmenter la consommation quotidienne de fructose. Il a bien ...été démontré chez l'animal et chez l'Homme qu'une surconsommation de fructose induit une lipogenèse et une insulino-résistance hépatique. Néanmoins peu d'études se sont intéressées à l'impact d'une surconsommation en fructose sur l'expression génique du muscle squelettique. Le but de cette étude a été d'évaluer les effets d'une alimentation riche en fructose sur l'expression génique du muscle squelettique chez des sujets apparentés diabétiques de type 2. Patients et Méthodes 10 sujets sains apparentés à des diabétiques de type 2 ont été soumis à deux types de régimes pendant 7 jours : a) un régime contrôle et b) une surnutrition en fructose (régime contrôle +3,5 g de fructose par kg de masse maigre par jour). Les sensibilités hépatique et périphérique à l'insuline ont été mesurées par clamp euglycémique hyperinsulinémique. L'oxydation des substrats a été déterminée par calorimétrie indirecte. Les contenus en lipides du foie et du muscle squelettique ont été évalués par spectroscopie par résonance magnétique. L'expression génique du muscle squelettique a été évaluée par puce à ADN à partir de biopsies musculaires. Résultats La surnutrition en fructose a induit une augmentation significative du pourcentage en lipides du foie (+74 %) et du muscle squelettique (+25 %) associée à l'établissement d'une insulino-résistance hépatique. Ces effets sont associés à de profonds changements de l'oxydation des substrats à jeun, avec une oxydation préférentielle des glucides (+42 %) aux détriments de lipides (− 32 %). Les analyses transcriptomiques ont montré une sous-expression de nombreux gènes liés au métabolisme énergétique, à la fonction mitochondriale et à l'oxydation des lipides. Les analyses bioinformatiques ont permis de suspecter les facteurs de transcription Nurr1 et PPARα dans la régulation de ces réseaux de gènes en réponse à la diète riche en fructose. Conclusions Notre étude montre qu'en plus des altérations hépatiques, la surnutrition en fructose est capable d'altérer le métabolisme musculaire.

Background
Prevalence of allergic diseases in infants is approximately 10% reaching 20 to 30% in those with an allergic first‐degree relative. Prebiotics are selectively fermented food ...ingredients that allow specific changes in composition/activity of the gastrointestinal microflora. They modulate immune responses, and their supplementation has been proposed as an intervention to prevent allergies.
Objective
To assess in pregnant women, breastfeeding mothers, and infants (populations) the effect of supplementing prebiotics (intervention) versus no prebiotics (comparison) on the development of allergic diseases and to inform the World Allergy Organization guidelines.
Methods
We performed a systematic review of studies assessing the effects of prebiotic supplementation with an intention to prevent the development of allergies.
Results
Of 446 unique records published until November 2016 in Cochrane, MEDLINE, and EMBASE, 22 studies fulfilled a priori specified criteria. We did not find any studies of prebiotics given to pregnant women or breastfeeding mothers. Prebiotic supplementation in infants, compared to placebo, had the following effects: risk of developing eczema (RR: 0.68, 95% CI: 0.40 to 1.15), wheezing/asthma (RR, 0.37; 95% CI: 0.17 to 0.80), and food allergy (RR: 0.28, 95% CI: 0.08 to 1.00). There was no evidence of an increased risk of any adverse effects (RR: 1.01, 95% CI: 0.92 to 1.10). Prebiotic supplementation had little influence growth rate (MD: 0.92 g per day faster with prebiotics, 95% CI: 0 to 1.84) and the final infant weight (MD: 0.10 kg higher with prebiotics, 95% CI: −0.09 to 0.29). The certainty of these estimates is very low due to risk of bias and imprecision of the results.
Conclusions
Currently available evidence on prebiotic supplementation to reduce the risk of developing allergies is very uncertain.
The GRADE (Grades of Recommendation, Assessment, Development, and Evaluation) approach provides guidance to grading the quality of underlying evidence and the strength of recommendations in health ...care. The GRADE system's conceptual underpinnings allow for a detailed stepwise process that defines what role the quality of the available evidence plays in the development of health care recommendations. The merit of GRADE is not that it eliminates judgments or disagreements about evidence and recommendations, but rather that it makes them transparent. This first article in a three-part series describes the GRADE framework in relation to grading the quality of evidence about interventions based on examples from the field of allergy and asthma. In the GRADE system, the quality of evidence reflects the extent to which a guideline panel's confidence in an estimate of the effect is adequate to support a particular recommendation. The system classifies quality of evidence as high, moderate, low, or very low according to factors that include the study methodology, consistency and precision of the results, and directness of the evidence.
The GRADE approach to grading the quality of evidence and strength of recommendations provides a comprehensive and transparent approach for developing clinical recommendations about using diagnostic ...tests or diagnostic strategies. Although grading the quality of evidence and strength of recommendations about using tests shares the logic of grading recommendations for treatment, it presents unique challenges. Guideline panels and clinicians should be alert to these special challenges when using the evidence about the accuracy of tests as the basis for clinical decisions. In the GRADE system, valid diagnostic accuracy studies can provide high quality evidence of test accuracy. However, such studies often provide only low quality evidence for the development of recommendations about diagnostic testing, as test accuracy is a surrogate for patient-important outcomes at best. Inferring from data on accuracy that using a test improves outcomes that are important to patients requires availability of an effective treatment, improved patients' wellbeing through prognostic information, or - by excluding an ominous diagnosis - reduction of anxiety and the opportunity for earlier search for an alternative diagnosis for which beneficial treatment can be available. Assessing the directness of evidence supporting the use of a diagnostic test requires judgments about the relationship between test results and patient-important consequences. Well-designed and conducted studies of allergy tests in parallel with efforts to evaluate allergy treatments critically will encourage improved guideline development for allergic diseases.
Introduction L’obésité est une pathologie inflammatoire chronique qui prédispose au diabète et qui est associée à une infiltration de macrophages au sein du tissu adipeux. Les récepteurs nucléaires ...LXR sont exprimés dans les macrophages vasculaires où ils exercent des propriétés anti-inflammatoires. Compte tenu de cette observation, la fonction des LXR dans les macrophages du tissu adipeux a été examinée pour savoir si des réponses pharmacologiques spécifiques aux agonistes de LXR existent et différencient les macrophages du tissu adipeux et les macrophages vasculaires. De plus, la modulation par LXR de la connexion entre les macrophages du tissu adipeux et les adipocytes a été évaluée. Matériels et Méthodes Pour évaluer la fonctionnalité des LXR, les macrophages du tissu adipeux viscéral provenant d’individus obèses ont été traités avec des agonistes des LXR, l’expression des gènes cibles connus de LXR a été évaluée par Q-PCR. Les profils d’expressions géniques des macrophages vasculaires et ceux du tissu adipeux issus d’un même individu obèse ont été comparés à l’état basal et après activation des LXR. Pour évaluer le rôle des LXR dans la modulation de la connexion entre macrophages et adipocytes, ces derniers ont été incubés avec du milieu conditionné provenant de la culture des macrophages du tissu adipeux préalablement traités ou non par les agonistes synthétiques des LXR. Résultats La comparaison des profils d’expressions au niveau basal a permis d’identifier plusieurs voies de signalisation différemment exprimées entre ces deux types de macrophages. De façon intéressante, de nouveaux gènes cibles de LXR spécifiques des macrophages du tissu adipeux ont été identifiés. La « co-culture indirecte » des macrophages du tissu adipeux et des adipocytes montrent que les LXR permettent la restauration partielle de l’expression adipocytaire de gènes impliqués dans la lipogenèse, le métabolisme du glucose et la signalisation de l’insuline. De plus, une diminution de l’expression des gènes impliqués dans la réponse inflammatoire au niveau adipocytaire est observée. Conclusion L’identification de nouveaux gènes cibles de LXR spécifiques des macrophages du tissu adipeux pourra aider à l’évaluation d’un rôle potentiel des agonistes LXR dans le traitement des pathologies associées à l’obésité.
The objective of Integrated Care Pathways for Airway Diseases (AIRWAYS-ICPs) is to launch a collaboration to develop multi-sectoral care pathways for chronic respiratory diseases in European ...countries and regions. AIRWAYS-ICPs has strategic relevance to the European Union Health Strategy and will add value to existing public health knowledge by: 1) proposing a common framework of care pathways for chronic respiratory diseases, which will facilitate comparability and trans-national initiatives; 2) informing cost-effective policy development, strengthening in particular those on smoking and environmental exposure; 3) aiding risk stratification in chronic disease patients, using a common strategy; 4) having a significant impact on the health of citizens in the short term (reduction of morbidity, improvement of education in children and of work in adults) and in the long-term (healthy ageing); 5) proposing a common simulation tool to assist physicians; and 6) ultimately reducing the healthcare burden (emergency visits, avoidable hospitalisations, disability and costs) while improving quality of life. In the longer term, the incidence of disease may be reduced by innovative prevention strategies. AIRWAYSICPs was initiated by Area 5 of the Action Plan B3 of the European Innovation Partnership on Active and Healthy Ageing. All stakeholders are involved (health and social care, patients, and policy makers).
Severe or therapy-resistant asthma is increasingly recognised as a major unmet need. A Task Force, supported by the European Respiratory Society and American Thoracic Society, reviewed the definition ...and provided recommendations and guidelines on the evaluation and treatment of severe asthma in children and adults. A literature review was performed, followed by discussion by an expert committee according to the GRADE (Grading of Recommendations, Assessment, Development and Evaluation) approach for development of specific clinical recommendations. When the diagnosis of asthma is confirmed and comorbidities addressed, severe asthma is defined as asthma that requires treatment with high dose inhaled corticosteroids plus a second controller and/or systemic corticosteroids to prevent it from becoming "uncontrolled" or that remains "uncontrolled" despite this therapy. Severe asthma is a heterogeneous condition consisting of phenotypes such as eosinophilic asthma. Specific recommendations on the use of sputum eosinophil count and exhaled nitric oxide to guide therapy, as well as treatment with anti-IgE antibody, methotrexate, macrolide antibiotics, antifungal agents and bronchial thermoplasty are provided. Coordinated research efforts for improved phenotyping will provide safe and effective biomarker-driven approaches to severe asthma therapy.
Context:
Deciphering the early processes occurring in adipose tissue during weight gain is a major issue for understanding the development of fat mass and obesity. Experimental overfeeding in humans ...is a unique situation to tackle these events.
Objective:
Our aim was to identify the pathways involved in sc adipose tissue remodeling during the initial phase of weight gain.
Research Design and Methods:
Forty-four healthy men were involved in an overfeeding protocol with a lipid-enriched diet (+760 kcal/d) for 2 months. Subcutaneous abdominal adipose tissue biopsies were taken for histology, transcriptomics, and Western blotting in the basal state, after 14 d, and at the end of the protocol.
Results:
Overfeeding significantly increased body weight (+2.5 kg) and fat mass. Reorganization of gene expression patterns occurred in adipose tissue with an up-regulation of numerous genes involved in lipid metabolism and storage, followed by clusters of genes related to angiogenesis and extracellular matrix remodeling. Histological examination showed increased microvascular density and connective tissue deposition after 56 d of overfeeding, with no changes in the number of macrophages or inflammatory cells. Inhibition of the canonical Wnt/β-catenin signaling pathway and induction of the renin-angiotensin system might be implicated in the remodeling of sc adipose tissue.
Conclusions:
We characterize the coordinated and time-dependent processes that occur in human adipose tissue during the early phase of weight gain in healthy subjects and identify pathways representing potential targets in pathologies of adipose development, including obesity.
Introduction L’incidence de l’obésité et des maladies métaboliques en général ne cesse d’augmenter à travers le monde. Ces désordres métaboliques sont associés à des perturbations de l’expression des ...gènes impliqués dans le métabolisme lipidique et glucidique et l’inflammation. L’expression d’un certain nombre de ces gènes est régulée par les membres de la superfamille des récepteurs nucléaires. Le récepteur nucléaire Farnesoid X Receptor (FXR) est exprimé dans le tissu adipeux, cependant son rôle reste encore inconnu. L’objectif de ce projet est d’élucider le rôle de FXR dans la fonction adipocytaire et de déterminer les mécanismes moléculaires impliqués dans ce processus. Résultats Nous montrons ainsi que les cellules MEFs (mouse embryonic fibroblasts) déficientes en FXR (FXR-/-) présentent une augmentation de la lipolyse et une diminution de la néolipogenèse par rapport aux cellules MEFs contrôles (FXR+/+ ) ce qui a pour conséquence une diminution des triglycérides stockées. Ce phénotype est associé à une diminution de l’expression des gènes marqueurs de l’adipogenèse et les gènes codant pour les protéines des gouttelettes lipidiques à la fois dans les MEFs FXR-/- et les pré-adipocytes isolées à partir du tissu adipeux des souris FXR-/- . L’introduction de FXR, par la voie d’un rétrovirus sur-exprimant FXR, restaure une différentiation adipocytaire normale. L’absence de FXR induit une augmentation de l’expression des gènes de la voie Wnt/β-caténine et une augmentation de l’expression de la protéine β-caténine. D’ailleurs, l’expression des gènes codant pour les inhibiteurs de cette voie de signalisation sont inhibés dans les MEFs FXR-/-. Un traitement par l’agoniste synthétique de PPARγ, la rosiglitazone, ne restaure pas une différenciation adipocytaire normale et des gouttelettes lipidiques comparables aux MEFs FXR+/+ . De même, les souris FXR-/- sont résistantes au traitement rosiglitazone en comparaisons aux contrôles. Conclusion Cette étude montre clairement que FXR contrôle la différenciation adipocytaire en interférant avec la voie Wnt/β-caténine et la voie d’activation de PPARγ
