Our goal was to evaluate the efficacy of internal medicine consulting (CIM) in primary care (PC) for the care of patients with dyspepsia.
Prospective case-control study of a population of 87,016 ...inhabitants during a 2 years period. We included patients with dyspepsia (319) jointly attended by a family physician (FP) and consulting internists (CI). The CI went weekly to the primary care center (PCC) to attend patients who would have been referred to the hospital outpatient clinic for specialized care (SC). As control group, 775 patients with dyspepsia were randomly and simultaneously chosen among a total of 29,317 first medical referrals during the period of the study.
5.36% of the consultations for SC were patients with dyspepsia (annual incidence of 183 patients/1,000 inhabitants). We observed a significant reduction in: mean time for SC (26.4 days; 22.4-30.4), mean number of tests per patient ordered by SC (0.77; 0.42-1.12), mean time for patient information on the tests ordered by SC (27.3 days; 18.7-35,9), mean time for the resolution of the process (27.3 days; 18.4-36.2) and rate of specialized follow-up visits (25.2%; 18.5-31.9); p < 0.005 in all cases. The resolution rate of the process (37.2%; 25.7-48.7) and the discharge rate (38.7%; 28.9-48.5) were significantly higher for CIM (p < 0.01 in both cases). The referral rate for SC showed a negative correlation (r = -0.97; p < 0.01) with the quality of the management; this rate related to the type of training, age and years of medical practice of the GPs. Satisfaction of patients with the CIM was very high.
The CIM with PC improves the efficacy of the specialized medical care of patients with dyspepsia.
Se analiza la estructura de tamanos de la comunidad planctonica en una estacion de caracter frontal en el Mar de Alboran. El rango de tamanos analizado incluye fundamentalmente picoplancton y ...nanoplancton. Se emplean tres tecnicas (Utermohl-microscopia invertida; epifluorescencia y citometria de flujo) de forma que se consigue un solapamiento en la cobertura del rango de tamanos estudiado y se analizan los posibles artefactos y errores metodologicos asi como su efecto sobre el modelado del espectro de tamanos y abundancias. La variabilidad asociada al caracter frontal de la estacion se refleja de forma mas acusada en la escala secundaria del espectro: la invasion de celulas grandes provoca un decrecimiento del caracter negativo de la pendiente del espectro correspondiente al nanoplancton, mientras que el picoplancton permanece proximo al valor de -1.0 (escala fisiologica). Desde un punto de vista operativo, parece mas rentable extender el metodo de epifluorescencia hacia la enumeracion de celulas relativamente grandes, que forzar la tecnica de Utermohol para la enumeracion de organismos relativamente pequenos.

The smallest marine phytoplankton, collectively termed picophytoplankton, have been routinely enumerated by flow cytometry since the late 1980s, during cruises throughout most of the world ocean. We ...compiled a database of 40 946 data points, with separate 5 abundance entries for Prochlorococcus, Synechococcus and picoeukaryotes. We use average conversion factors for each of the three groups to convert the abundance data to carbon biomass. After gridding with 1 spacing, the database covers 2.4% of the ocean surface area, with the best data coverage in the North Atlantic, the South Pacific and North Indian basins. The average picophytoplankton biomass is 12±22 μgCl−1 or 10 1.9 gCm−2. We estimate a total global picophytoplankton biomass of 0.53–0.74PgC (17–39% Prochlorococcus, 12–15% Synechococcus and 49–69% picoeukaryotes).Future efforts in this area of research should focus on reporting calibrated cell size, and collecting data in undersampled regions.
En este trabajo se procedió al reconocimiento de lipopolisacáridos de diferentes serotipos de Pseusodomas aeruginosa por sueros humanos. Los lipopolisacáridos fueron aislados por el método Wespthal y ...Jann. Se utilizaron un total de 104 sueros de pacientes infectados y donantes voluntarios, provenientes de instalaciones hospitalarias y del Banco de Sangre, para el reconocimiento de antígenos lipopolisacarídicos, mediante el ensayo de Dott Blot. Los resultados mostraron que los serotipos de mayor reconocimiento, por los sueros testados, fueron el O11, O13, O16 y el O15, existiendo diferencias significativas entre ellos (p< 0,05).
