La maladie associée aux anticorps anti-MOG (MOGAD) est une affection démyélinisante rare du système nerveux central ayant un tropisme optico-médullaire, notamment chez l’adulte, pouvant être ...monophasique ou à rechutes.
L’objectif était d’évaluer la présence de BOC dans le LCR comme un possible facteur de risque de sévérité des poussées et/ou de rechutes de la maladie.
Les données de 190 patients issus du registre français des NMO (NOMADMUS) ont été analysées après répartition en deux groupes selon la présence de bandes oligoclonales au sein du LCR. Nous avons décrit et comparé entre les groupes les caractéristiques générales, cliniques et d’imagerie, et avons également précisé l’évolution radiologique. Des courbes de survies ont été réalisées pour comparer le risque de rechute entre les groupes.
La présence de BOC dans le LCR n’était pas associée à un pronostic fonctionnel plus sévère, après mesure de l’EDSS ou de l’acuité visuelle. Le taux de rechutes était plus élevé dans le groupe BOC+(p=0,049) avec une différence se mettant en place essentiellement lors de la première année (p=0,007). Un phénotype de SEP sur l’IRM cérébrale était plus souvent décrit dans le groupe BOC+(p=0,014).
Les patients MOGAD BOC+soulèvent la question du risque d’erreur diagnostique pour une SEP. Cependant, malgré la plus forte proportion de phénotype de SEP sur l’IRM cérébrale initiale, près de 2/3 des patients du groupe BOC+décrivaient une amélioration ou disparition des lésions à l’imagerie, constituant un drapeau rouge pour le diagnostic de SEP.
Les patients atteints de MOGAD BOC+sont plus à risque de rechute. Une vigilance particulière reste nécessaire pour l’exclusion du diagnostic de SEP.
After cerebral venous thrombosis (CVT), the risk of venous thrombotic events was estimated at 2% to 3% for a new CVT and 3% to 8% for extracranial events. However, because of the paucity of ...prospective studies, the clinical course of CVT is still largely unknown. We aimed to prospectively evaluate the rate of thrombosis recurrence in a cohort of CVT patients with a long-term follow-up and to detect predisposing factors for recurrence.
Consecutive CVT patients with complete clinical, radiological, biological, and genetic data were systematically followed up. New venous thrombotic events were detected after hospital readmission and imaging confirmation.
One-hundred eighty-seven patients (mean age 45±18 years, 67% women) with angiographically confirmed CVT were included. Cause was found in 73% of patients. Coagulation abnormality and JAK2 gene mutation were detected in 20% and 9%, respectively. Median follow-up length was 73 months (range 1-247 months). Mean duration of the oral anticoagulant treatment was 14 months. Mortality rate was 2.5% per year, with 2% in-hospital mortality. During follow-up, CVT reoccurred in 6 patients, whereas 19 subjects had a symptomatic extracranial venous thrombotic event, with cumulative venous thrombotic recurrence rates of 3% at 1 year, 8% at 2 years, 12% at 5 years, and 18% at 10 years. A previous venous thrombotic event (hazard ratio, 2.8; P=0.018), presence of cancer or malignant hemopathies (hazard ratio, 3.2; P=0.039), and unknown CVT causes (hazard ratio, 2.81; P=0.024) were independently associated with recurrence.
In our cohort of CVT patients followed on average for >6 years, subjects with a previous venous thrombotic event, cancer/malignant hemopathies, and unknown CVT causes were found to be at higher risk of recurrence.
Disease modifying therapies compromise immune response to SARS-Cov2 or its vaccine in patients with immune system diseases (ISD). Therefore, analysis of the humoral and cellular responses against ...Spike is of utmost importance to manage ISD patients. A single-center retrospective study was conducted to evaluate the impact of COVID-19 immunization in 87 ISD patients and 81 healthy controls. We performed a whole blood interferon gamma release assay using SARS-Cov2 Spike and Nucleocapsid recombinant proteins in order to evaluate T-cell memory response, and an IgG anti-Spike ELISA to evaluate humoral response. Cellular (26.4%) and humoral (44.8%) responses were negative against Spike in ISD patients following COVID-19 immunization. In univariate analysis, an anti-Spike T cell defective response was associated with the use of glucocorticoids (Odds ratio OR = 10.0; p < 10−4), serum albumin level ≤40 g/L (OR = 18.9; p < 10−4), age over 55 years old (OR = 3.9, p = 0.009) and ≤2 vaccine injections (OR = 4.9; p = 0.001). The impact of glucocorticoids persisted after adjustment for age and number of vaccine injections (OR = 8.38, p < 0.001). In contrast, the humoral response was impacted by the use of anti-CD20 mAb (OR = 24.8, p < 10−4), and an extended time since immunization (≥75 days; OR = 4.3, p = 0.002). Double defective cellular/humoral responses (6.9%) were typically encountered in glucocorticoids and/or anti-CD20 mAb treated ISD with a serum albumin level ≤40 g/L (OR = 17.5; p = 0.002). Glucocorticoid usage, B cell depleting therapies, and a low serum albumin level were the main factors associated with a non-response to COVID-19 immunization in ISD patients. These results need further confirmation in larger studies.

Exit strategies such as de-escalations have not been evaluated for rituximab in patients with neuromyelitis optica spectrum disorder (NMOSD). We hypothesized that they are associated with disease ...reactivations and aimed to estimate this risk.
We describe a case series of real-world de-escalations from the French NMOSD registry (NOMADMUS). All patients met the 2015 International Panel for NMO Diagnosis (IPND) diagnostic criteria for NMOSD. A computerized screening of the registry extracted patients with rituximab de-escalations and at least 12 months of subsequent follow-up. We searched for 7 de-escalation regimens: scheduled discontinuations or switches to an oral treatment after single infusion cycles, scheduled discontinuations or switches to an oral treatment after periodic infusions, de-escalations before pregnancies, de-escalations after tolerance issues, and increased infusion intervals. Rituximab discontinuations motivated by inefficacy or for unknown purposes were excluded. The primary outcome was the absolute risk of NMOSD reactivation (one or more relapses) at 12 months. AQP4+ and AQP4- serotypes were analyzed separately.
We identified 137 rituximab de-escalations between 2006 and 2019 that corresponded to a predefined group: 13 discontinuations after a single infusion cycle, 6 switches to an oral treatment after a single infusion cycle, 9 discontinuations after periodic infusions, 5 switches to an oral treatment after periodic infusions, 4 de-escalations before pregnancies, 9 de-escalations after tolerance issues, and 91 increased infusion intervals. No group remained relapse-free over the whole de-escalation follow-up (mean: 3.2 years; range: 0.79-9.5), except pregnancies in AQP+ patients. In all groups combined and within 12 months, reactivations occurred after 11/119 de-escalations in patients with AQP4+ NMOSD (9.2%, 95% CI 4.7-15.9), from 0.69 to 10.0 months, and in 5/18 de-escalations in patients with AQP4- NMOSD (27.8%, 95% CI 9.7-53.5), from 1.1 to 9.9 months.
There is a risk of NMOSD reactivation whatever the rituximab de-escalation regimen.

To report the clinical, biological, and imaging features and clinical course of a French cohort of patients with glial fibrillary acidic protein (GFAP) autoantibodies.
We retrospectively included all ...patients who tested positive for GFAP antibodies in the CSF by immunohistochemistry and confirmed by cell-based assay using cells expressing human GFAPα since 2017 from 2 French referral centers.
We identified 46 patients with GFAP antibodies. Median age at onset was 43 years, and 65% were men. Infectious prodromal symptoms were found in 82%. Other autoimmune diseases were found in 22% of patients, and coexisting neural autoantibodies in 11%. Tumors were present in 24%, and T-cell dysfunction in 23%. The most frequent presentation was subacute meningoencephalitis (85%), with cerebellar dysfunction in 57% of cases. Other clinical presentations included myelitis (30%) and visual (35%) and peripheral nervous system involvement (24%). MRI showed perivascular radial enhancement in 32%, periventricular T2 hyperintensity in 41%, brainstem involvement in 31%, leptomeningeal enhancement in 26%, and reversible splenial lesions in 4 cases. A total of 33 of 40 patients had a monophasic course, associated with a good outcome at last follow-up (Rankin Score ≤2: 89%), despite a severe clinical presentation. Adult and pediatric features are similar. Thirty-two patients were treated with immunotherapy. A total of 11/22 patients showed negative conversion of GFAP antibodies.
GFAP autoimmunity is mainly associated with acute/subacute meningoencephalomyelitis with prodromal symptoms, for which tumors and T-cell dysfunction are frequent triggers. The majority of patients followed a monophasic course with a good outcome.
To characterise the response to treatment of inaugural optic neuritis (ON) in patients with myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD).
We searched the French MOGAD ...database for adults with inaugural ON with a detailed report of acute treatment modalities and measures of high-contrast best-corrected visual acuity (BCVA) at nadir and after 3 months. Predictors of visual outcomes were assessed by multivariable analysis.
Among 245 patients with at least one episode of ON, 82 fulfilled all criteria, and data on the peripapillary retinal nerve fibre layer (pRNFL) were available for 44. All patients received methylprednisolone (MP), combined with plasma exchange in 18. After 3 months, 75 of 82 (91%) patients retained full BCVA recovery, and median (range) pRNFL of the affected eye was 72 µm (40-102). Failure to regain 0.0 logarithmic minimum angle of resolution vision (Snellen 20/20) at 3 months was associated with time to first MP treatment ≥10 days (OR 16, 95% CI 1.14 to 213, p=0.01). pRNFL thickness after 3 months was related to better BCVA at nadir and time to first MP treatment <10 days (r2=19%, p=0.004 and r2=11%, p=0.03, respectively).
Time to MP affects functional but also structural visual outcomes of ON in MOGAD.

Décrire les pratiques de prise en charge thérapeutique précoce de la sclérose en plaques chez le patient naïf de traitement en identifiant et quantifiant les critères influençant celles-ci.
Les neurologues français partageront leur approche thérapeutique face à 6 situations cliniques, dits cas-vignettes. Ils seront présentés au travers d’une conversation entre le neurologue participant et les patients fictifs via un chatbot. Ils seront constitués par la combinaison aléatoire de différentes modalités des critères suivants : démographiques, cliniques et IRM. Un nombre de 150 réponses est attendu. Cette méthodologie est validée par la HAS. Ce questionnaire est accessible en ligne via un code à flasher.
Recueil en cours, ils devraient être disponibles à la fin du premier trimestre 2023.
Ces dernières années, la prise en charge thérapeutique a évolué avec un arsenal thérapeutique grandissant, comprenant notamment des traitements dits de haute efficacité. Devant ce constat, il paraît intéressant de dresser un nouvel état des lieux des pratiques de prise en charge précoce du patient naïf atteint de SEP en France.
Cette étude permet d’identifier et de mesurer les critères de décisions impactant significativement cette prise en charge.
La sarcoïdose est une maladie inflammatoire systémique qui touche dans 5–50 % des cas le système nerveux central. Son diagnostic reste difficile du fait d’une présentation hétérogène.
Notre objectif ...est la description clinique, biologique et radiologique ainsi que la prise en charge des patients présentant une neurosarcoïdose (NS) avec atteinte du système nerveux central (SNC) en France.
C’est une étude française, rétrospective, multicentrique décrivant des patients remplissant les critères de Zajicek de NS définie ou probable entre 2010 et 2019 et présentant une atteinte du SNC. Les dossiers ont été sélectionnés par le PMSI et les données démographiques, cliniques et radiologiques ont été recueillies.
Nous avons inclus 118 patients, dont 38 montrant une atteinte isolée du SNC. La NS est initiale dans 78 % des cas principalement avec atteinte des nerfs crâniens (36 %), médullaire (23 %) ou une épilepsie (21 %). La BGSA est positive chez 8 % des patients prélevés. La ponction lombaire retrouve des BOC dans 28 %. À l’IRM, le rehaussement leptoméningé est la plus fréquente anomalie (34 %). Les corticoïdes restent le traitement le plus utilisé, suivi par le méthotrexate et cyclophosphamide.
Notre cohorte est une des plus importante publiée à ce jour. Les caractéristiques cliniques et paracliniques de nos patients sont similaires à la littérature. La BGSA est peu rentable contrairement aux biopsies des adénopathies médiastinales (89,5 %). Les traitements utilisés sont hétérogènes. Les données concernant le méthotrexate, le cyclophosphamide et les antiTNFα semblent montrer une bonne efficacité.
L’hétérogénéité, la rareté, de la NS rend le diagnostic et la prise en charge difficile. Une base de données nationales paraît indispensable à l’amélioration de nos pratiques et l’établissement de recommandations.
La maladie associée aux anticorps anti-MOG (MOGAD) est une maladie inflammatoire rare du système nerveux central touchant préférentiellement l’enfant et le jeune adulte.
L’objectif de cette étude ...était de décrire les caractères cliniques et paracliniques et d’évaluer le pronostic d’une population de patients présentant une MOGAD ayant débutée après l’âge de 50 ans.
Nous avons conduit une étude observationnelle, rétrospective, nationale, multicentrique, incluant les patients ayant présenté un premier épisode de MOGAD après l’âge de 50 ans, et pour lesquels le test sérique était réalisé au laboratoire de référence à Lyon. Nous avons inclus 77 patients, issus de la base NOMADMUS, pour lesquels les données cliniques et paracliniques étaient disponibles.
Au total, 77 patients ont été inclus. L’âge médian était de 57 ans avec 88 % de patients de moins de 70 ans. La névrite optique était l’atteinte initiale la plus fréquente. Les patients étaient fréquemment traités, en phase aiguë (88,2 %), au long cours (72 % ont reçu un traitement de fond), avec 40 effets indésirables identifiés, dont 12 graves. Au total, 44,1 % des patients ont rechuté après 2 ans de suivi et l’EDSS médian au dernier suivi était à 2.
Le pronostic était globalement comparable aux patients plus jeunes, avec un risque de rechute et un handicap modéré, et semblable entre les patients monophasiques et ceux à rechutes. Par ailleurs, des effets secondaires, parfois graves, liés aux traitements de fond ont été observés, devant faire discuter au cas par cas l’intérêt de traiter ces patients au long cours.
À la différence de la SEP ou la NMO, les patients débutant la maladie après 50 ans ont une présentation clinique et un risque évolutif similaire aux patients plus jeunes.
The intrathecal production of oligoclonal immunoglobulin bands (OCB) is a prognostic factor for multiple sclerosis (MS) evolution in clinically isolated syndrome (CIS) patients and a diagnostic ...factor for MS. The kappa free light chain (K)-index represents a quantitative automated alternative to OCB. We retrospectively evaluated OCB and K-index results in 274 patients with MS (n = 48) or CIS (n = 29) at diagnosis, non-MS inflammatory central nervous diseases (n = 35), and non-inflammatory central/peripheral nervous diseases (n = 162). Several cut-offs were established: a pathophysiological cut-off (K-index: 3.3) useful for differential diagnosis (negative predictive value for MS >99%), an optimised cut-off (K-index: 9.1) with better sensitivity and equivalent specificity than OCB for the diagnosis of MS, and a high-risk cut-off (K-index: >55.0) allowing prediction of MS (specificity 100%). We developed a scaled interpretation of the K-index and we discuss the usefulness of testing OCB only when the K-index is positive >3.3 to obtain a better specificity.
•K-index cut-off 3.3 performed better than OCB (higher sensitivity) for MS diagnosis.•K-index cut-off 9.9 displayed better sensitivity and equivalent specificity than OCB.•High K-index cut-off (>55) performed better than OCB in terms of specificity (100%).•We propose the use of 3 cut-offs allowing a scaled interpretation of K-index.•OCB testing could be avoided when K-index is <3.3.
