We present a case of a postmenopausal woman diagnosed with an ovarian mass containing thyroid follicles and foci of papillary thyroid carcinoma during pathological examination. This patient referred ...having had a metachronous thyroid malignancy 10 years before. The differential diagnosis between a thyroid malignancy arising from a struma ovarii and a metastatic ovarian tumor originating from thyroid-cancer is challenging. Struma ovarii should be considered when thyroid components are the predominant element or when thyroid malignant tissue is identified within an ovarian lesion. Thyroid carcinoma arising from a struma ovarii is reported to occur in a minority of cases. Of these, papillary carcinoma is the most frequent subtype encountered. Regarding primary thyroid carcinomas, papillary carcinomas have a lower metastatic potential when compared to follicular carcinomas, and most of the metastases occur in the cervical lymph nodes. Ovarian metastases are exceedingly rare and generally associated with widespread disease. However, they must be considered in the presence of previous history of malignant thyroid carcinoma. The authors review the main clinical, imaging and therapeutic aspects of both these entities and present the most likely diagnosis.
Poncet's disease is a reactive polyarthritis associated to active tuberculosis (TB), with excluded presence of mycobacterium in the joints and bones affected, and no other cause for the clinical ...arthritis. It is a frequently forgotten entity, especially in countries with low tuberculosis prevalence. It is described a case report of a man of Guinea-Bissau that presented symmetrical generalized polyarthralgias and hands swelling, clinical sacroiliítis and multiple enthesopathies. Serum and urinary biochemical testing were normal. Immunologic studies were negative and serum levels of angiotensin-converting enzyme and calcium were normal. Sexual transmitted diseases were excluded. The initial diagnose was undifferentiated polyarthritis or reactive arthritis. A persistent and refractory monoarthritis of the right wrist demanded a direct and cultural exam of the synovial fluid, synovial biopsy and protein chain reaction for TB that were negative. After he developed right wrist arthritis with purulent synovial fluid positive to TB in the direct exam and the detection of M. tuberculosis in the cultural exam. Granula was found in his chest radiograph. He was diagnosed a Poncet's disease reactive to a pulmonary TB infection.

Implementation and development of promotional and educational oral health programs, it is critical to identify and intervene on the risk factors for oral health in adolescents. Research Question: ...Does the oral health status of adolescents in particular DMFT index, plaque index, oral health risk and oral hygiene habits, suffer modification after participation in an educational program ProSorriso? Purpose of the Study: Implement a program of oral health education, to evaluate its effectiveness in improving the oral health of adolescents. Methods: Experimental analytical study-before and after the intervention program on the risk factors for oral health. A sample of 200 adolescents attending schools of Portugal, aged 11-16 years (x=13.21±1.014). Material: Risk Quantifiers in Oral Health assessed before and after participation in “ProSorriso” program. This aims at the promotion and education for oral health: oral health, diet and oral hygiene. Results: In the evaluation after the program ProSorriso: adolescents decreased the percentage of decayed teeth 50,5% to 38,5%;improved eating habits (Z=-1,325,p=0,185); had higher percentage of teeth without plaque (Z=-5,465,p=0,000). There were more adolescents to brush their teeth twice a day (Z=-2,562,p=0,010). The assessment of risk classification showed that before the educational program ProSorriso had a higher percentage of adolescents who were at high risk of developing oral problems (p=0,001). Conclusion: The intervention “ProSorriso” shown to produce effect in changing behaviors that promote healthier life styles and to be determinant of oral health status.
To develop recommendations for the treatment of psoriatic arthritis (PsA) with biological therapies, endorsed by the Portuguese Society of Rheumatology.
These treatment recommendations were ...formulated by Portuguese rheumatologists based on literature evidence and consensus opinion. A draft of the recommendations was first circulated to all Portuguese rheumatologists and their suggestions were incorporated in the draft. At a national meeting the recommendations were discussed and all attending rheumatologists voted on the level of agreement for each recommendation. A second draft was again circulated before publication.
A consensus was achieved regarding the initiation, assessment of response and switching biological therapies in patients with PsA. Specific recommendations were developed for several disease domains: peripheral arthritis, axial disease, enthesitis and dactylitis.
These recommendations may be used for guidance in deciding which patients with PsA should be treated with biological therapies. They cover a rapidly evolving area of therapeutic intervention. As more evidence becomes available and more biological therapies are licensed, these recommendations will have to be updated.
Invariant Natural Killer (iNKT) cells are T lymphocytes that recognize lipid antigens presented by CD1d molecules and have a semi-invariant T cell receptor. iNKT cells produce high quantities of ...cytokines after antigen recognition or activation. The regulation of iNKT cell activation is crucial for its role in infection and tumor control. CD5 and CD6 are transmembrane surface glycoproteins expressed by conventional T cells that regulate the activation of these cells, but their expression on human iNKTs has not been investigated yet. Here, we studied the basal expression of both CD5 and CD6 on human iNKTs and also their modulation after activation. The involvement of CD6 in antigen induced iNKT cell activation was also addressed. Our results show that human peripheral blood iNKT cells express both CD5 and CD6, at similar and higher levels, respectively, than conventional T cells. Activation of a human iNKT cell line with the non-specific stimulus Phytohaemagglutinin (PHA) down-regulated the expression of both CD5 and CD6, whereas when the prototypic antigen α-Galactosylceramide (α-GalCer) was used only a decrease in CD6 expression was observed. The use of Raji B cells, expressing or not CD166, a ligand for CD6, as antigen presenting cells did not disclose a major role for CD6 in α-GalCer induced iNKT cell activation

Here we show that SR-A deficient mice and wild type mice are equally susceptible to malaria infection both during liver and blood stages. Moreover, like wild type mice, SR-A deficient mice are able to mount a protective immune response against radiation attenuated sporozoites.
Our results do not reveal a function of SR-A I and II receptors in the Plasmodium berghei ANKA infection, both in the development of CM and parasitemia control. Moreover, these receptors appear not to be required for the establishment of a protective immune response against the malaria liver stages.
Introduction: Cardiovascular risk factors decrease after kidney transplantation, however cardiovascular disease remains one of the most common causes of death in children and young adults. The ...authors performed a comprehensive analysis of cardiovascular risk factors before and after kidney transplantation. Methods: Retrospective study, from 2007 to 2017, that included children and adolescents with a minimum of six months follow -up after kidney transplant. Demographic profile, chronic kidney disease characteristics and family and personal cardiovascular risk factors (hypertension, left ventricular hypertrophy, anemia, overweight/obesity, dyslipidemia, hypoalbuminemia and new onset diabetes after transplant) were assessed. Pre - and post -transplant cardiovascular risk factors were compared using the McNemar test. Results: We included 49 patients, with a mean age at the time of transplantation of 9.3 ± 4.2 years. Congenital anomalies of kidney and urinary tract (37.7%) and glomerulopathies (32.7%) were the most frequent causes of stage 5 chronic kidney disease. Transplantation was preemptive in five patients. Living donor transplant was performed in six cases. The immunosuppression regimen was identical in all patients and there were nine acute rejection episodes. The most frequent pre -transplant cardiovascular risk factors were dyslipidemia (90.3%), hypertension (70.8%) and anemia (61.9%). After transplant there was a decrease of all cardiovascular risk factors except for overweight/obesity. Although a better control of hypertension was granted, the decrease was not significant. Hypoalbuminemia and dyslipidemia decreased from the 6 -month evaluation on (p<0.001) and left ventricular hypertrophy and anemia from the second year on (p<0.05). Conclusions: We aimed to evaluate the evolution of cardiovascular risk factors in pediatric patients before and after kidney transplant. In our cohort there was a decrease in all studied cardiovascular risk factors after kidney transplant, except for overweight/obesity. This could mean that chronic kidney disease related factors decrease with time, but traditional risk factors may persist or increase, and need active prevention.
