La spasticité, un des symptômes le plus souvent rencontré dans la sclérose en plaques, touche plus particulièrement la mobilité et la marche. L’évaluation de ce symptôme fait appel le plus souvent à ...des échelles cliniques ne prenant pas en compte le retentissement sur les activités de vie quotidienne et la qualité de vie. La vie quotidienne des patients est affectée surtout dans les actes impliquant la mobilité et la marche, tels que l’utilisation des transports, le jardinage, les activités ménagères, et finalement assez peu les activités de base comme la toilette et l’habillage. À ce jour, aucune étude n’a décrit l’impact de la spasticité sur la vie quotidienne des patients atteints de sclérose en plaques. Cependant, évaluer l’impact de la spasticité dans cette population jeune semble primordial pour répondre aux besoins des patients par des actions thérapeutiques adaptées.
Spasticity is one of the most commonly seen symptoms in patients with multiple sclerosis. However, evaluation of the symptom often uses clinical scales that do not incorporate its impact on activities of daily living and quality of life. The everyday life of patients is affected primarily in actions related to mobility and walking capacity, such as the use of transport, gardening, household activities and, ultimately, basic activities such as bathing and dressing. Yet, so far, no study has described the impact of spasticity on the daily life of patients with multiple sclerosis. Nevertheless, assessing the effects of spasticity on such a young population would appear to be essential for meeting the needs of these patients with appropriate therapeutic interventions.
La prescription de bolus de méthylprednisolone lors d’une poussée de sclérose en plaques (SEP) implique une stérilité des urines. Une étude observationnelle a été conduite pour préciser l’intérêt de ...l’antibioprophylaxie devant une bactériurie (BA) avant bolus de méthylprednisolone. Quatre-vingt-dix-sept patients ont été inclus. Trente-deux patients présentaient une BA. Dix-sept patients ont été traités et 15 ne l’ont pas été. Le nombre d’infections urinaires dans le mois suivant les bolus est identique dans les deux groupes. Les résultats semblent en faveur d’une abstention thérapeutique. Une étude de plus grande ampleur devra être réalisée pour confirmer ces résultats et proposer des recommandations.
The prescription of methylprednisolone for multiple sclerosis acute relapse involves sterilization of urine. An observational study was conducted to clarify the benefit of antibiotic prophylaxis in case of asymptomatic bacteriuria found before methylprednisolone. Ninety-seven patients were included; 32patients had asymptomatic bacteriuria. Seventeen patients were treated and 15 were not. The number of urinary tract infections in the month following the methylprednisolone was the same in the two groups. The results seem in favor of a therapeutic abstention. A larger study will be performed to confirm these results and determine appropriate recommendations.

The recommendation attempts to answer four main questions about infections and MS. The French Group for Recommendations in Multiple Sclerosis (France4MS) did a systematic review of articles from PubMed and universities databases (from January 1975 to June 2020), using the RAND/UCLA formalized consensus method. The RAND/UCLA method has been developed to synthesize the scientific literature and expert opinions on health care topics and was used for reaching a formal agreement. Twenty-three experts contributed to the detailed review and a group of 63 multidisciplinary health professionals validated the final version of 36 recommendations.
It is recommended that MS patients undergo a minimal infectious workup, check their vaccination status at diagnosis, and repeat it during follow-up and before starting immunotherapy. Screening and preventive treatment of viral (group Herpes virus, HPV, JCV, HCV, HBV), bacterial (mycobacteria) and fungal (Cryptococcus) infections is recommended prior to the initiation of certain immuno-active MS therapies.
At diagnosis of MS and prior to the choice of therapeutic strategy, it is recommended to update the vaccination schedule of MS patients in reference to the HCSP vaccination schedule and the SFSEP recommendations. Before starting immunosuppressive treatment, it is recommended to inform patients of the risks of infections and to look for a constitutive or acquired immune deficiency. Health professionals and patients should be informed of the updated recommendations on infections and MS.
Malgré les progrès thérapeutiques, l’évolution de la sclérose en plaques, traitée ou non, peut tôt ou tard conduire à l’installation d’un handicap ne se limitant pas aux seuls troubles locomoteurs. ...Générées par des années de poussées suivies de l’installation insidieuse, mais continue, des déficits moteurs, sensitifs, génito-sphinctériens, cérébelleux, sensoriels ou cognitifs, les conséquences sont à la fois physiques, psychologiques et même socio-économiques. La rééducation fonctionnelle fait donc partie des traitements proposés et son efficacité semble maintenant établie. Différentes modalités de prise en charge sont possibles : en hospitalisation ou à domicile, impliquant de nombreux professionnels de santé (kinésithérapeutes, ergothérapeutes, orthophonistes, infirmières, orthoptiste…), dont la coordination est assurée par le neurologue et/ou le médecin de rééducation. Les objectifs doivent être déterminés de façon précise selon le niveau de handicap et être adaptés aux besoins de chaque patient. Les programmes sont le plus souvent pluridisciplinaires, associant des techniques de rééducation neurologiques spécifiques à des exercices plus fonctionnels voire du réentraînement à l’effort. Dans l’avenir, la rééducation ne devrait plus se limiter à la prise en charge de patients avec un handicap sévère mais être proposée plus tôt, afin d’avoir une action préventive sur les symptômes et leurs conséquences.
Multiple sclerosis (MS) is a chronic progressive disease which is the leading cause, after road traffic accidents, of handicap in young subjects. The large range of symptoms associated with MS lead to continuing decline in mood and quality of life. Despite therapeutic advances, functional impairments have significant consequences. Neurorehabilitation can be highly contributive in this disease with the goals of increasing independence and quality-of-life and improving functional capacities. Individualized programs elaborated by a multidisciplinary team of experts are the key to success of rehabilitation. Assessment is difficult because of the underlying conflict between the philosophies of rehabilitation and evidence-based medicine. The aim of this paper is to provide an overview of MS rehabilitation. Physical exercise is safe and should be encouraged for people with MS. Some studies have shown that supervised exercises have a beneficial effect on MS disability and quality of life. Inpatient rehabilitation for MS yields short-term benefits in function, mobility and quality of life; periodic hospitalization may be needed. In the future, rehabilitation professionals will have to learn how to anticipate patient needs and lay the groundwork for services and equipment in advance. Rehabilitation is one of the treatments of MS patients and should be viewed as an ongoing process to maintain and restore maximum function and quality of life.
Introduction La scoliose idiopathique de l’adolescent (SIA) est une déformation tridimentionnelle du rachis, d’étiologie inconnue. Nous posons l’hypothèse pathogénique suivante : la SIA s’accompagne ...d’une représentation centrale erronée de la verticalité à l’origine d’une adaptation posturale du tronc dans les 3 plans de l’espace. Pour le vérifier, nous avons débuté une étude dont l’objectif est d’évaluer le sens de la verticalité dans la SIA, à la fois dans ses composantes top down et bottom up. Matériel et méthodes Trois types de mesure ont été réalisées : – Verticale visuelle subjective (VVS) réalisée en conditions visuelles statique et dynamique (stimulation optocinétique 40°/sec) chez 83 SIA versus 15 contrôles non scoliotiques ; – Verticale posturale subjective (VPS) chez 12 SIA ; – évaluation de l’image du tronc déformé par le Spinal Appareance Questionnaire (SAQ) chez 19 SIA versus 5 hypercyphoses thoraciques. Résultats Exactement 51,8 % des SIA ont une VVS anormale, surtout en condition visuelle dynamique versus 0 % dans le groupe contrôle. Les valeurs de VPS dans la SIA sont proches de celles trouvées chez les hémiplégiques pusher (exemple : tilt droit = 8,28°4,05). Les SIA avec une scoliose majeure ont un SAQ significativement détérioré par rapport aux hypercyphoses thoraciques. Discussion–conclusion Un sous-groupe de patients SIA présente une perturbation du sens de la verticalité. Cette anomalie peut être secondaire à la déformation scoliotique mais peut également être un facteur d’évolutivité de la SIA.
Les troubles génito-sexuels sont fréquents dans la sclérose en plaques et altèrent de façon significative la qualité de vie. Classiquement décrits après quelques années d’évolution, ils peuvent ...cependant survenir dès le début de la maladie, même en cas d’EDSS faible. Les troubles urinaires, qui ont été les plus étudiés, ont fait l’objet de recommandations concernant leur dépistage et leur suivi. Les troubles sexuels sont fréquemment présents dès les premières années de la maladie avec un retentissement important sur la qualité de vie. Ils se manifestent chez la femme par des troubles de la libido et de l’orgasme, et chez l’homme par des troubles de l’érection. Leur dépistage devrait se généraliser. Leur prise en charge doit être à la fois médicamenteuse et psychologique; il est également indispensable de prendre en compte les symptômes aggravant tels que la fatigue, la dépression, les douleurs…, limitant l’activité sexuelle. Les troubles anorectaux ont été peu étudiés et sont sous-estimés ; les quelques études publiées concernent uniquement des petites cohortes de patients. Néanmoins, leur prévalence est élevée et leurs retentissements non négligeables. La constipation et les fuites fécales sont les symptômes les plus fréquents et sont souvent associées. Tout comme les troubles vésico-sphinctériens, ils représentent les principales épines irritatives de la maladie et doivent faire l’objet d’un dépistage systématique. Leur prise en charge est diététique, médicamenteuse et parfois rééducative.
Most multiple sclerosis (MS) patients experience some bladder, sexual, and/or bowel dysfunction during the course of the disease, which has been closely associated with the quality-of-life scores, including in patients with otherwise low disability. This underlines the need for identifying and treating these problems. Bladder dysfunction produces symptoms of urgency, frequency, and urge incontinence (due to bladder over activity and incomplete emptying), and is found in up to 80% of patients with MS. These symptoms occur in the early stage of the disease and require screening and rigorous monitoring. For many patients, treatment is effective and has been shown to reduce the risks of urologic complications, improve levels of continence, and enhance the patient's quality of life. Sexual disturbances are also present from the beginning and have and greatly influence the quality of life of the MS patient. The findings in the literature show substantial evidence that people with MS experience high levels of sexual dysfunction, most of them with hypoactive sexual behavior often associated with dissatisfaction in the relationship. The most common problems in women are lack of sexual interest and decreased libido, often with problems in orgasmic capacity, while men report erectile dysfunction and also lack of sexual interest. Unlike bladder dysfunction, there have been rather limited advances in the treatment of fecal incontinence and constipation specifically for patients with MS, despite a prevalence of up to 50%. Until now, the management of these problems has been empirical, with a lack of evaluated therapeutic regimes. Biofeedback retraining is an effective treatment in some patients with MS complaining of constipation or fecal incontinence. A response is more likely in patients with limited disability and a nonprogressive disease course. Since treatments and preventive strategies can manage many of these problems, we suggest increasing the focus on these aspects of the disease when consulting patients, including at early stages.
Objective To study the risk factors for falls in patients with multiple sclerosis presenting gait disturbances. Methods This is a retrospective review of prospectively collected data from a cohort of ...MS patients followed for gait disturbances between April 2013 and June 2014 in the neurological rehabilitation department. The history of falls, trauma, current treatments and clinical data (disease duration, clinical form, EDSS, motor skills, spasticity, pain) were collected. Univariate and multivariate analyzes were performed to predict the risk of falls. Results One hundred and ninety-six patients (median EDSS: 6; age: 55 years ±9.3) were divided into 2 groups: fallers ( n = 92), non-fallers ( n = 103). In our model, taking antidepressants, previous fractures (upper limbs, spine), sprains of ankles and knees, impaired visual and brain functions (or mental) of EDSS were predictors of falls (AUC = 0.73; sensitivity = 0.74; specificity = 0.59). Discussion Balance disorders and falls are among the most disabling symptoms of MS, affecting about 3/4 of patients during the course of the disease. In our study, 47.1% of patients had falled. These results are similar to those of Cattaneo et al. 1 , (54%) and of Finlayson et al. 2 , (52.2%). If visual disturbances and antidepressants have been identified as risk factors for falling, prior fracture and sprain were not included in those studies. This could explain the fear of falling in this population. Investigation of a history of fracture or sprain is recommended in the pervention of falls in MS patients with gait disturbances.
