Men living in refugee settings are often exposed to violence, poverty, and social instability, which impacts physical and mental health and increases the risk of perpetrating sexual and gender-based ...violence. Healthy Men Healthy Communities was developed as a male-led health promotion program to address men's physical and mental health and their role in creating healthy relationships and families. Three community leaders from the settlements were trained to facilitate the program, which was implemented among six groups consisting of twelve men in each group. Pre/post surveys and feedback were collected among the facilitators and participants. Facilitators suggested culturally appropriate ways to present physical activities as a stress reduction technique and the importance of spacing out births. The small group setting facilitated open conversations on topics such as birth spacing and healthy partner communication. Participants experienced an increase in knowledge and confidence in practicing the program content, such as stress-reduction techniques and healthy communication strategies. Participants recommended additional topics such as fertility and sexually transmitted infections. The Healthy Men Healthy Communities program has the potential for wider implementation among male South Sudanese refugees to promote their health as well as the health of their families.
Women seeking care during the perinatal period often face delays or long waits at healthcare facilities due to lack of providers and/or resources, leading to sub-optimal outcomes. We implemented a ...program whereby patients with concerns could receive same-day care virtually from a midwife rather than presenting to the clinic or hospital for care. Implementation strategies included virtual training, a staged increase in patient volume, and frequent communication between the midwives via text, email, and monthly meetings. Virtual visits included a variety of complaints, the five most common being to establish care, first-trimester bleeding, nausea and vomiting, mental health concerns, and postnatal breast problems. There was a threefold increase in virtual visits during the first 6 months with 92% of patients not requiring urgent face-to-face follow-up. Midwives were able to provide high-quality telehealth care that met the patients’ needs and decreased the demand on hospital-based services. With the growing ubiquity of mobile phones and internet access, this strategy may be effective in providing quality care while decreasing demands on physical infrastructure. More research is needed to assess acceptability in other contexts. Reproducibility in low-resource settings may be limited if women lack access to video conferencing on phones or laptops.
Introduction
Nutrition impacts health outcomes of pregnant people and their fetuses. Discussing nutrition with patients may be challenging for health care providers. In this vacuum, patients use ...social media for health information during pregnancy. Little is known about the type and quality of nutrition information that is available on social media. The aim of this study was to qualitatively explore the pregnancy nutrition messages contained in social media videos for content and evaluate the information's concordance with evidence‐based guidelines.
Methods
A review of current social media video content for nutrition in pregnancy was conducted across 4 platforms: YouTube, Instagram, Facebook, and TikTok. Content analysis was used to descriptively and categorically analyze the social media video content.
Results
In the 62 social media videos reviewed, most were not consistent with established guidelines, were inaccurate, or presented a homogenous and unrealistic image of the pregnant person. Four categories of nutrition in pregnancy social media information emerged: what to eat, what not to eat, what I ate: aspirational, and what I ate: rebellion. Few videos were produced by health care organizations. Some were produced by anonymous organizations, with the majority made by pregnant or postpartum people. Many were testimonial or confessional in style. Despite regulations requiring identification of paid posts and advertisements, it was difficult to discern if videos were authentic sharing of patient experiences or advertisements.
Discussion
There is incorrect and incomplete information on social media surrounding nutrition in pregnancy. Professional guidelines for nutrition in pregnancy are available but are not as accessible to patients as social media videos. Social media use is associated with disordered eating and increased risk of anxiety and depression. However, targeted interventions by health care providers that use social media platforms have been successful in the distribution of credible health information. Midwives and other health care providers should share evidence‐based nutrition recommendation with patients to facilitate meaningful conversations around nutrition in pregnancy during in person and online interactions.
Cardiac conditions are the leading cause of pregnancy-related deaths and disproportionately affect non-Hispanic Black people. Multidisciplinary maternal mortality review committees have found that ...most people who died from cardiac conditions during pregnancy or postpartum were not diagnosed with a cardiovascular disease before death and that more than 80% of all pregnancy-related deaths, regardless of cause, were preventable. In addition, other obstetric complications, such as preeclampsia and gestational diabetes, are associated with future cardiovascular disease risk. Those with cardiac risk factors and those with congenital and acquired heart disease require specialized care during pregnancy and postpartum to minimize risk of preventable morbidity and mortality. This bundle provides guidance for health care teams to develop coordinated, multidisciplinary care for pregnant and postpartum people with cardiac conditions and to respond to cardio-obstetric emergencies. This bundle is one of several core patient safety bundles developed by the Alliance for Innovation on Maternal Health that provide condition- or event-specific clinical practices for implementation in appropriate care settings. The Cardiac Conditions in Obstetric Care bundle is organized into five domains: 1) Readiness , 2) Recognition and Prevention , 3) Response , 4) Reporting and Systems Learning , and 5) Respectful Care . This bundle is the first by the Alliance to be developed with the fifth domain of Respectful Care . The Respectful Care domain provides essential best practices to support respectful, equitable, and supportive care to all patients. Further health equity considerations are integrated into elements in each domain.
Knowledge of non-breeding distributions is a crucial component to seabird conservation, as conditions during the non-breeding period can play an important role in regulating seabird populations. ...Specifically, if seabirds from different colonies share the same wintering grounds, conditions in that shared region could have a widespread impact on multiple breeding populations. Red-legged kittiwakes (Rissa brevirostris) are endemic to the Bering Sea and may be especially susceptible to effects of climate change due to a restricted breeding range, small population size, and specialized diet. To examine whether red-legged kittiwakes from different breeding colonies overlapped in winter distribution and activity patterns, we used geolocation loggers to simultaneously track individuals from the two largest red-legged kittiwake breeding colonies in Alaska (separated by over 1000 km) during two consecutive non-breeding periods. We found that non-breeding activity patterns were generally similar between birds originating from the two colonies, but birds employed different migratory strategies during the early winter. Kittiwakes from Buldir Island in the western Aleutian Islands left the colony in September and immediately headed west, spending October through December around the Sea of Okhotsk and the Kuril Islands. In contrast, birds from St. George Island in the Pribilof Islands remained in the eastern Bering Sea or around the eastern Aleutian Islands for a couple months before traveling farther west. During late winter however, from January through March, birds from both colonies converged south of Kamchatka and east of the Kuril Islands over the Kuril-Kamchatka Trench and in the Western Subarctic Gyre before returning to their respective colonies in the spring. This late winter overlap in distributions along the Kuril-Kamchatka Trench suggests the region is a winter hotspot for red-legged kittiwakes and highlights the importance of this region for the global kittiwake population.
Regional and local studies suggest that the Tufted Puffin Fratercula cirrhata in North America is declining in portions of its range. However, whether the overall population is declining, or its ...range is contracting with little change to the overall population size, is unknown. To examine population trends throughout its North American range, we assembled 11 datasets that spanned 115 years (1905–2019) and included at-sea density and encounter estimates and at-colony burrow and bird counts. We assessed trends for the California Current, Gulf of Alaska, and Bering Sea/Aleutian Islands large marine ecosystems (LME). We found: (1) nearly uniform and long-term declines of Puffins breeding in the California Current ecosystem, with most ecosystem colonies surveyed, (2) declining trends at two large colonies and in one at-sea dataset in the Gulf of Alaska LME, with the fourth smaller colony exhibiting no significant trend, and (3) positive trends at four out of five colonies in the Bering Sea/Aleutian Islands ecosystem complex, with no detectable trend at the fifth very large colony. The general pattern of Tufted Puffin declines across the California Current and Gulf of Alaska LMEs may be attributable to a variety of factors, but additional study is needed to evaluate the relative influence of potential population drivers both independently and synergistically. Potential mechanisms driving population increases in the Bering Sea/Aleutian Islands ecosystem include reduced depredation and bycatch, intrinsic population growth, and immigration. We found strong evidence for declines in two of the three LMEs evaluated representing approximately three quarters of the species’ North American range. This region of decline includes the Gulf of Alaska LME, which contains a significant portion of the species’ estimated total North American population. Despite data limitations, our analysis coupled with more focused and local studies indicates that the Tufted Puffin is a species of conservation concern.
Some of the longest and most comprehensive marine ecosystem monitoring programs were established in the Gulf of Alaska following the environmental disaster of the Exxon Valdez oil spill over 30 years ...ago. These monitoring programs have been successful in assessing recovery from oil spill impacts, and their continuation decades later has now provided an unparalleled assessment of ecosystem responses to another newly emerging global threat, marine heatwaves. The 2014-2016 northeast Pacific marine heatwave (PMH) in the Gulf of Alaska was the longest lasting heatwave globally over the past decade, with some cooling, but also continued warm conditions through 2019. Our analysis of 187 time series from primary production to commercial fisheries and nearshore intertidal to offshore oceanic domains demonstrate abrupt changes across trophic levels, with many responses persisting up to at least 5 years after the onset of the heatwave. Furthermore, our suite of metrics showed novel community-level groupings relative to at least a decade prior to the heatwave. Given anticipated increases in marine heatwaves under current climate projections, it remains uncertain when or if the Gulf of Alaska ecosystem will return to a pre-PMH state.

A high dietary intake of polyphenols has been associated with a decreased risk of cardiovascular disease and cancer, attributed in part to their antioxidant activity and pro-apoptotic effects. ...Aquamin is a multi-mineral algal extract that enhances bone mineralization, relieves osteoarthritis, and aids digestion; however, Aquamin has not demonstrated antioxidant activity. In the present study, Aquamin was supplemented with 8% Enzogenol, a pine bark extract with a high phenolic content, and 2% Sunphenon, a green tea extract that also has a high phenolic content to produce a mixed product (A:E:S). The antioxidant activity of A:E:S was compared with that of its constituent compounds and also with catechin and epigallocatechin by measuring total phenol content, ferric-reducing antioxidant potential, and 2,2-diphenyl-2-picrylhydrazyl hydrate. The cytotoxic and apoptotic effects of the compounds were also measured in the U937, human monocytic blood cell line. A:E:S demonstrated an antioxidant activity that was equal to that of the compounds used in its preparation. Aquamin was not cytotoxic in the U937 cell line; however, A:E:S was cytotoxic and the primary mechanism of cell death was apoptosis. The biological effects of Aquamin were enhanced by supplementation with Enzogenol and Sunphenon to include antioxidant effects and the ability to induce apoptosis in U937 cells.
