Studies assessing personality dimensions by the "Temperament and Character Inventory" (TCI) have previously found an association between Parkinson's disease (PD) and lower Novelty Seeking and higher ...Harm Avoidance scores. Here, we aimed to describe personality dimensions of PD patients with motor fluctuations and compare them to a normative population and other PD populations.
All PD patients awaiting Deep Brain Stimulation (DBS) answered the TCI before neurosurgery. Their results were compared to those of historical cohorts (a French normative population, a de novo PD population, and a PD population with motor fluctuations).
Most personality dimensions of our 333 included PD patients with motor fluctuations who are candidates for DBS were different from those of the normative population and some were also different from those of the De Novo PD population, whereas they were similar to those of another population of PD patients with motor fluctuations.
During the course of PD, personality dimensions can change in parallel with the development of motor fluctuations, either due to the evolution of the disease and/or dopaminergic treatments.
It can be difficult to differentiate multiple system atrophy (MSA) with predominant parkinsonism (MSA-P), a very disabling but rare disease, from Parkinson's disease (PD). Autonomic dysfunction, ...particularly cardiovascular autonomic neuropathy (CAN), is classically more pronounced in MSA. We investigated whether testing for CAN combined with sweat function assessment was helpful to differentiate patients with MSA from those with PD.
In this retrospective study, 62 patients with MSA-P and 96 with PD, comparable in age, BMI and sex ratio with disease duration of 4.3±2.5 years for MSA vs. 11.5±6 years for PD (P<0.0001) were tested for CAN using heart rate changes with deep breathing (HR-DB), stand test (HR-ST) and Valsalva maneuver (HR-VM) and blood pressure changes during stand test (BPs-ST and BPd-ST), Valsalva maneuver (BPs-VM-II and IV), hand grip (BPd-HG) and the total “Ewing” score tests (EwS), and for sweating function using electrochemical skin conductance for hands and feet (HESC and FESC).
In MSA-P abnormal EwS, HR-DB, BP-ST and ESC were more frequent compared to PD, despite longer disease duration for PD. Using a model adjusted for sex, BMI, age, disease duration and treatment, the Odds Ratio for having MSA-P vs. PD based on EwS was 2.71, on HR-DB 2.36, on BP-ST 2.87 and on ESC 4.94 while it was 11.68 (2.17–62.79) for the combination of the three last tests.
Assessment of HR-DB, BP-ST and ESC, that is, rapid and non-invasive could be helpful in combination as a first simple screening to differentiate MSA-P from PD.
Background: Leucine-rich glioma-inactivated 1 (LGI1) encephalitis is a rare entity. Its typical features are seizures, faciobrachial dystonic seizures (FBDS), cognitive impairment, and personality ...changes. Case report: We report the case of a 66-year-old man with an unusual presentation, consisting of two types of FBDS, one starting in the foot and the other consisting of asynchronous myoclonic and dystonic jerks of the face triggered by noise and chin stimulation. The patient displayed no personality changes or cognitive impairment. Discussion: LGI1 encephalitis is a heterogeneous disease. Many different forms of FBDS may be observed, and these seizures can be the only symptom. This type of encephalitis should be suspected in presenting very frequent episodic events with dystonic features, regardless of the part of the body affected.
The added value of dopamine transporter SPECT (DAT-SPECT) for the diagnosis of “possible” multiple system atrophy of the cerebellar type (MSA-C) remains unknown.
We reviewed retrospectively the ...charts of 128 consecutive patients with a clinical diagnosis of MSA-C who were seen between 2007 and 2016 at the French Reference Center for MSA. The main objective was to evaluate the proportion of patients for whom the diagnosis of “possible” MSA-C was made because of a positive DAT-SPECT.
Seventy-eight MSA-C patients had at least one DAT-SPECT. Fifty-nine of them were considered for the final analysis. In these, 22 had “possible” MSA-C and 23 “probable” MSA-C before DAT-SPECT, while 14 did not reach diagnosis criteria at that time. In those with “possible” MSA-C, DAT-SPECT was positive in 64%. In patients with “probable” MSA-C, 83% showed nigrostriatal denervation. Six out of 14 (43%) received a diagnosis of “possible” MSA-C because of positive DAT-SPECT. These patients had mean disease duration of 2.3 years at the time of DAT-SPECT compared to 3.5 years of the entire cohort of MSA-C patients with DAT-SPECT. Of the eight remaining, one had positive DAT-SPECT but also pons atrophy on magnetic resonance imaging, and seven progressed to “probable” MSA based on clinical features.
Our results suggest that DAT-SPECT significantly contributes to the diagnosis of “possible” MSA-C (43% of patients not reaching consensus diagnosis criteria before DAT-SPECT). DAT-SPECT seems especially useful in patients with shorter disease duration, while a negative result does not exclude a diagnosis of MSA.
•Positive DAT-SPECT is an additional feature for the diagnosis of “possible” MSA-C.•DAT-SPECT contributed in 43% of patients to the diagnosis of “possible” MSA-C.•A negative DAT-SPECT does not exclude a diagnosis of MSA-C.
La qualité de vie (QdV) est altérée dans la maladie de Parkinson et elle pourrait être influencée par la personnalité des patients.
Nous voulions évaluer l’association entre les dimensions de ...personnalité et la QdV de patients parkinsoniens avec fluctuations motrices en attente de stimulation cérébrale profonde des noyaux sous-thalamiques (SCP-NST).
Les données proviennent de la cohorte française PREDI-STIM incluant des patients parkinsoniens en attente de SCP-NST. Tous les patients ont répondu aux questionnaires « Temperament and Character Inventory » (TCI) et PDQ-39 (Parkinson Disease Questionnaire-39) avant la stimulation. Les analyses ont été réalisées avec des modèles de régression linéaire généralisés univariés et ajustés pour évaluer une potentielle association entre les différentes dimensions de personnalité (TCI) et les différents scores de QdV (PDQ-39).
Chez les 363 patients, il existait une association négative significative entre le tempérament d’Évitement du Danger et la QdV (p=3e-11, R2=0,18), et une association positive significative entre les caractères d’Auto-Détermination et de Coopération et la QdV (respectivement, p=2e-11, R2=0,19 ; p=1e-3, R2=0,1). Ces associations étaient principalement élevées avec les composantes mentales de cette QdV.
Trois dimensions de personnalité (Évitement du Danger, Auto-Détermination et Coopération) sont associées avec la QdV des patients particulièrement aux niveaux émotionnel et social. L’Évitement du Danger étant associé à la dépression et l’Auto-Détermination et la Coopération à la maturité personnelle et sociale, l’éducation thérapeutique renforçant les ressources personnelles de ces patients semble importante pour améliorer leur bien-être.
Des scores bas en Évitement du Danger et élevés en Auto-Détermination et Coopération sont associés à une meilleure QdV chez les patients parkinsoniens au stade des fluctuations motrices.
Diagnosis of Parkinson's disease (PD) is often traumatic. It is then important to organize this announcement in order to support the patient in this major step of his care course. Anxiety and ...depression are present in about 50% of PD patients. Besides pharmacological treatment, cognitive-behavioral therapy induces improvement in anxio-depressive symptoms. We have to promote physical activity for PD patients, from the beginning of the disease, because it prevents from deconditioning, improves motor and non-motor symptoms, quality of life, and decreases loneliness. Rehabilitation may be also proposed (physiotherapy, speech therapy, occupational therapy). But there is no specific recommendation in PD, and usual activities can be proposed. Therapeutic education is an answer for PD patients who need to know and understand their disease and treatment. Organization of therapeutic education is different depending of the teams: group workshop and/or individual sessions, conduct by doctors or paramedical staff (nurse, physiotherapist, psychologist…). Therapeutic education programs are always evolving. It will soon be proposed therapeutic education for care partners, and some patients may join therapeutic education staff.
L’administration intra duodénale de Lévodopa (Duodopa®) est associée à des cas de neuropathies périphériques. Le lien entre maladie de Parkinson, L-dopa et Duodopa®, et leur survenue, reste encore ...flou.
Décrire la fréquence de survenue et le type de neuropathies sous Duodopa® dans l’expérience toulousaine. Proposer au travers de la littérature un protocole de suivi et de prise en charge.
Dans cette étude monocentrique rétrospective, menée entre 2014 et 2015, tous les patients parkinsoniens au stade des complications motrices sévères et invalidantes de la dopathérapie, relevant d’un traitement par Duodopa®, ont été inclus. Ils ont été évalués avant Duodopa®, puis tous les 6 mois pendant 18 mois (T18), au moyen d’une évaluation standardisée comprenant : examen clinique, électromyographie et bilan biologique (dosage des vitamines B6, B12 et de l’homocystéine).
Sur 12 patients inclus, 3 avaient une neuropathie préexistante. À 18 mois, 6 patients (67 %), avaient une neuropathie à l’EMG dont 2 présentaient des signes cliniques. Dans ce groupe, la dose quotidienne de Lévodopa a été significativement augmentée (p=0,036) comparativement au groupe des patients sans neuropathie. Il n’y avait pas de différence significative concernant les dosages vitaminiques entre les 2 groupes. Chez les patients présentant une neuropathie avant traitement, un seul s’est aggravé sur l’EMG.
La survenue d’une neuropathie est fréquente et précoce sous Duodopa®, le plus souvent de type sensitive mais asymptomatique. Un suivi électromyographique tous les 6 mois pourrait être préconisé pour le suivi. Les doses de Lévodopa, seul facteur de risque retrouvé, sont à surveiller. Une supplémentation vitaminique doit être réalisée lorsqu’il existe une carence ou une hyper homocystéinémie.
Si le risque de neuropathie semble fréquent sous Duodopa®, les mécanismes demeurent flous et les stratégies préventives également. Un observatoire des neuropathies des patients sous Duodopa® en France est préconisé.

Les neuropathies survenant sous lévodopa pourraient être liées à la dose cumulée de lévodopa, aux carences en vitamine B12, en B6 et folates et à l’accumulation d’homocystéine et d’acide ...méthylmalonique.
Il s’agit d’une patiente parkinsonienne de 74 ans depuis 2005, au stade des complications motrices depuis 2010. En 2015 quelques semaines après son initiation, l’apomorphine en perfusion sous-cutanée, doit être arrêtée du fait d’une réaction cutanée allergique. En 2016 elle est traitée par perfusion intra duodénale de lévodopa (Duodopa®), son état neurologique s’améliore très favorablement. Alors qu’elle est traitée depuis 14 mois, son suivi vitaminique montre l’apparition d’une carence marquée et isolée en vitamine B6 (14,37nmol/L norme>30). Elle est asymptomatique (sans neuropathie) et son état neurologique est stable. Il est décidé de la traiter de façon systématique par chlorhydrate de pyridoxine. Au cours de 2 tentatives espacées d’un mois chacune, elle reçoit 250mg par jour de vitamine B6 pendant 48h. Après la deuxième prise, son état neurologique se détériore, elle marche difficilement été se décrit très bloquée et douloureuse. À l’arrêt de la vitamine B6 la patiente recouvre son état habituel.
L’administration concomitante de vitamine B6 et de DOPA diminue de 67 % la concentration plasmatique de DOPA alors que l’index d’activité de la dopa décarboxylase plasmatique augmente de 483 %. La vitamine B6 est un co-facteur activateur de la DOPA décarboxylase. En supplémentant les patients, elle pourrait entraîner une diminution de la concentration de DOPA cérébrale et expliquer l’aggravation motrice observée.
Bien que cette interaction soit pharmacologiquement connue, il n’existe pas de cas d’aggravation clinique décrit chez les patients parkinsoniens supplémentés en vitamine B6. Ce risque clinique doit être connu.
