Stigma has been identified as a key determinant of health and health inequities because of its effects on access to health-enabling resources and stress exposure. Though existing reports offer ...in-depth summaries of the mechanisms through which stigma influences health, a review of evidence on the upstream drivers of stigma across health and social conditions has been missing. The objective of this review is to summarize known structural determinants of stigma experienced across health and social conditions in developed country settings.
We conducted a rapid review of the literature. English- and French-language peer-reviewed and grey literature works published after 2008 were identified using MEDLINE, Embase, PsycINFO, Google and Google Scholar. Titles and abstracts were independently screened by two reviewers. Information from relevant publications was extracted, and a thematic analysis of identified determinants was conducted to identify broad domains of structural determinants. A narrative synthesis of study characteristics and identified determinants was conducted.
Of 657 publications identified, 53 were included. Ten domains of structural determinants of stigma were identified: legal frameworks, welfare policies, economic policies, social and built environments, media and marketing, pedagogical factors, health care policies and practices, biomedical technology, diagnostic frameworks and public health interventions. Each domain is defined and summarized, and a conceptual framework for how the identified domains relate to the stigma process is proposed.
At least 10 domains of structural factors influence the occurrence of stigma across health and social conditions. These domains can be used to structure policy discussions centred on ways to reduce stigma at the population level.
The coronavirus disease 2019 (COVID-19) pandemic in Canada has evolved rapidly. Since late 2020, COVID-19 vaccines have been relied on to protect against severe outcomes in the presence of ...circulating variants of concern (VOC).
This surveillance report provides a retrospective descriptive analysis of national trends in COVID-19 cases and severe outcomes by vaccination status, contextualizing trends against case demographics and circulating VOCs, from December 2020 to January 2022.
Case and vaccination coverage surveillance data were obtained from the National COVID-19 Case Dataset and the Canadian COVID-19 Vaccination Coverage Surveillance System for 12 of 13 provinces and territories. Descriptive analyses were produced to describe trends over time among individuals aged 12 years and older by COVID-19 outcome, vaccination status, and demographics. Age-standardized and age-stratified incidence rates and incidence rate ratios were computed for cases, hospitalizations, and deaths.
From mid to late-2021, incidence rates for cases and severe outcomes were consistently lowest among those with a completed primary series and highest among those who were unvaccinated. Unvaccinated individuals were much more likely to be hospitalized or to die compared to those with a completed primary series in all variant periods. Age-specific rates of severe outcomes were consistently highest among those aged 80 years and older across all vaccination statuses.
Vaccination remains one of the most important public health interventions, particularly among older adults, to protect against COVID-19 severe outcomes as the pandemic evolves. Routine monitoring of COVID-19 outcomes by vaccination status can identify changes in COVID-19 epidemiology and inform public health action and policy.
Introduction
On sait que la stigmatisation est un déterminant clé de la santé et des inégalités en matière de santé en raison de ses effets sur l’accès aux ressources favorables à la santé et sur ...l’exposition au stress. Alors que divers rapports offrent des synthèses approfondies des mécanismes par lesquels la stigmatisation influence la santé, il n’existe aucun examen des données probantes sur les facteurs en amont de la stigmatisation touchant les conditions sanitaires et sociales. L’objectif de cette étude consiste à réaliser une synthèse des déterminants structurels connus de la stigmatisation touchant les conditions sanitaires et sociales dans les pays développés.
Méthodologie
Nous avons procédé à une revue rapide de la littérature. Nous avons sélectionné des ouvrages de littérature grise et de littérature en anglais et en français évalués par des pairs et publiés après 2008 à l’aide de MEDLINE, Embase, PsycINFO, Google et Google Scholar. Les titres et les résumés ont été examinés indépendamment par deux évaluateurs. Nous avons extrait divers renseignements des publications pertinentes et nous avons mené une analyse thématique de ces déterminants afin de cerner les grands domaines des déterminants structurels. Nous avons réalisé une synthèse narrative des caractéristiques de chaque étude et des déterminants ciblés.
Résultats
Sur les 657 publications sélectionnées, nous en avons retenu 53. Nous avons relevé 10 domaines de déterminants structurels de la stigmatisation : cadres juridiques, politiques d’aide sociale, politiques économiques, environnements social et bâti, médias et marketing, facteurs pédagogiques, politiques et pratiques en matière de soins de santé, technologie biomédicale, cadres diagnostiques et interventions en santé publique. Chaque domaine a été circonscrit et synthétisé, et nous proposons un cadre conceptuel de la manière dont les domaines cernés sont liés au processus de stigmatisation.
Conclusion
Au moins 10 domaines de facteurs structurels ont une influence sur la stigmatisation touchant les conditions sanitaires et sociales. On peut se servir de ces domaines pour structurer des discussions stratégiques centrées sur les moyens de réduire la stigmatisation au sein de la population.

Starting in 2014, biennial clusters of acute flaccid myelitis (AFM), frequently described as "polio-like" illness, have been reported across the United States and elsewhere, often linked to ...enteroviruses. To assess AFM trends in Canada, we reviewed the Canadian Acute Flaccid Paralysis Surveillance System (CAFPSS) for cases reported during the 2018 and 2019 calendar years that meet the Centers for Disease Control and Prevention case definitions for AFM. A total of 10 cases (8 in 2018 and 2 in 2019) met the confirmed AFM case definition and 30 (26 in 2018 and 4 in 2019) met the probable AFM case definition. Sixty percent of confirmed and probable cases were younger than five years old, and all cases had symptom onset between the months of July and October. Enteroviruses were detected in 50% of confirmed cases. At the time of writing this report, 2020 AFM data were not yet available; it is unknown if a spike in AFM cases will be seen in 2020.
Canada has maintained a low incidence of toxigenic diphtheria since the 1990s, supported by continued commitment to publicly funded vaccination programs.
To determine whether hospitalization data, ...complemented with notifiable disease data, can describe the toxigenic respiratory and cutaneous diphtheria burden in Canada, and to assess if Canada is meeting its diphtheria vaccine-preventable disease-reduction target of zero annual cases of locally transmitted respiratory diphtheria.
Diphtheria-related hospital discharge data from 2006 to 2017 were extracted from the Discharge Abstract Database (DAD), and diphtheria case counts for the same period were retrieved from the Canadian Notifiable Disease Surveillance System (CNDSS), for descriptive analyses. As data from the province of Québec are not included in the DAD, CNDSS cases from Québec were excluded.
A total of 233 diphtheria-related hospitalizations were recorded in the DAD. Of these, diphtheria was the most responsible diagnosis in 23. Half the patients were male (52%), and 57% were 60 years and older. Central region (Ontario) accounted for the most discharge records (61%), followed by Prairie region (Alberta, Manitoba and Saskatchewan; 23%). Cutaneous diphtheria accounted for 43% of records, and respiratory diphtheria accounted for 3%, with the remainder being other diphtheria complications or site unspecified. Two records with diphtheria as the most responsible diagnosis resulted in inpatient deaths. Eighteen cases of diphtheria were reported through CNDSS. Cases occurred in all age groups, with the largest proportions among those aged 20 to 59 years (39%) and those aged 19 years and younger (33%). Cases were only reported in the Prairie (89%) and West Coast (British Columbia; 11%) regions.
Hospital administrative data are consistent with the low incidence of diphtheria reported in CNDSS, and a low burden of respiratory diphtheria in Canada. Although Canada appears to be on track to meet its disease-reduction target, information on endemic transmission is not available.

Pour estimer les tendances de la myélite flasque aiguë au Canada, nous avons examiné le Système canadien de surveillance de la paralysie flasque aiguë (SCSPFA) pour passer en revue les cas déclarés au cours des années civiles 2018 et 2019 qui répondent aux définitions de cas de la myélite flasque aiguë des Centers for Disease Control and Prevention. Au total, 10 cas (8 en 2018 et 2 en 2019) répondaient à la définition de cas confirmé de myélite flasque aiguë et 30 (26 en 2018 et 4 en 2019) répondaient à la définition de cas probable de myélite flasque aiguë. Soixante pour cent des cas confirmés et probables avaient moins de cinq ans, et la date de début des symptômes se situait entre les mois de juillet et d’octobre pour tous les cas. Des entérovirus ont été détectés dans 50 % des cas confirmés. Au moment de la rédaction du présent rapport, les données de la myélite flasque aiguë pour 2020 n’étaient pas encore disponibles; il n’est pour le moment pas possible de déterminer si une hausse du nombre de cas de myélite flasque aiguë sera observée en 2020.
Abstract
Introduction/Background
COVID-19 vaccinations for those aged 6 months to 5 years were announced in the summer of 2022. Specifically, Moderna Spikevax and Pfizer-BioNTech Comirnaty were ...authorized on July 14, 2022, and September 9, 2022, respectively. Given that this was the last age group to become eligible for vaccination against COVID-19, promoting and measuring vaccination coverage in this vulnerable population is of public health significance.
Objectives
This analysis describes and compares the trend in vaccination coverage (VC) over the initial weeks of eligibility in those 0-17 years old.
Design/Methods
Data were obtained from the Canadian COVID-19 Vaccination Coverage Surveillance System and were analyzed to describe and compare VC of those <5, 5-11, and 12-17-year-olds over the first 16 weeks of eligibility. VC is defined as the percent of the population that have received at least 1 dose of a COVID-19 vaccine.
Results
Sixteen weeks after those <5 years became eligible for vaccination, coverage was 10.9% in those aged 2-4 and 6.3% in those aged <2. Compared to other paediatric age groups, VC in those <5 is markedly lower, where, after 16 weeks of eligibility, coverage in those aged 5–11 was 54.4% and 81.5% in those 12-17. There was an immediate spike in coverage for both 5–11-year-olds and 12–17-year-olds over the first 4 weeks following eligibility, where VC was 40.1% and 63.3%, respectively. A similar increase was not observed 4 weeks following eligibility in those aged <5, where VC was 7.4% in those aged 2-4 and 4.0% in those <2. The weekly increase in VC decreased more rapidly as age groups decreased; weekly increase in VC slowed to less than 1% in 28 weeks, 20 weeks, and 16 weeks after eligibility in those aged 12-17, 5-11, and <5, respectively.
Conclusion
These data suggest that VC over time in the <5 population has been slow and has not followed the trajectory of other paediatric populations, when compared to the 5-11 and 12-17 age groups. Reasons for this may include but are not limited to parents and guardians’ knowledge, attitudes, and beliefs toward getting their infant children vaccinated, as well as pandemic fatigue, as eligibility for the infant population occurred after the other paediatric age groups.

Objectif : ...Déterminer si les données d’hospitalisation, complétées par les données sur les maladies à déclaration obligatoire, peuvent décrire le fardeau de la diphtérie respiratoire et cutanée toxigène au Canada, ainsi qu’estimer si le Canada est en voie d’atteindre sa cible de réduction des maladies évitables par la vaccination pour la diphtérie, qui est de zéro cas par année de diphtérie respiratoire transmise localement. Méthodes : Les données sur les congés des personnes hospitalisées liés à la diphtérie de 2006 à 2017 ont été extraites de la base de données sur les congés des patients (BDCP), et les données sur les cas de diphtérie pour la même période ont été extraites du Système canadien de surveillance des maladies à déclaration obligatoire (SCSMDO), afin de procéder à des analyses descriptives. Étant donné que les données de la province du Québec ne sont pas incluses dans la BDCP, les cas du SCSMDO du Québec ont été exclus de l’analyse. Résultats : Au total, 233 hospitalisations liées à la diphtérie ont été enregistrées dans la BDCP. De ce nombre, la diphtérie était le diagnostic principal dans 23 cas. La diphtérie cutanée représentait 43 % des enregistrements, la diphtérie respiratoire 3 %, et le reste était d’autres complications liées à la diphtérie ou la diphtérie dont la localisation n’était pas spécifiée. Deux patients pour lesquels la diphtérie était le diagnostic principal ont entraîné le décès des patients hospitalisés. Dix-huit cas de diphtérie ont été déclarés par l’entremise du SCSMDO. Les cas étaient observés dans tous les groupes d’âge, dont la plus grande proportion de cas chez ceux de 20 à 59 ans (39 %) et ceux de 19 ans et moins (33 %). Les cas n’ont été rapportés que dans les Prairies (89 %) et les régions de la côte Ouest (Colombie-Britannique 11 %). Conclusion : Les données administratives des hôpitaux sont conformes à la faible incidence de diphtérie déclarée dans le SCSMDO et le faible fardeau de la diphtérie respiratoire au Canada.
