Teoricamente, nos apoiamos na compreensão do self em sua dimensão relacional. Metodologicamente, utilizamos a teoria do posicionamento como um recurso para dar visibilidade para o processo discursivo em terapia familiar, no qual uma mãe e um filho negociam posições alternativas para seu relacionamento. Concluímos, pela utilidade da teoria do posicionamento na experiência clínica, por permitir colocar em ação a ampliação da noção de identidade, substituindo o enfoque na unicidade pela possibilidade de múltiplas descrições de self.

To investigate if liver transplants were cost effective at a University Hospital of Ceará.
We evaluated data from the medical records of 62 liver transplant patients performed in 2007, from the day ...of admission for the transplant to the date of discharge or death. Indicators such as length of hospital stay and location ,quantity of medical material and medication used, as well as the investigations and procedures performed were determined.
The population was mostly male from the Northeast of Brazil with an mean age of 45 years. The mean total cost of a liver transplant was US$20,605.01. The largest part was payment for the professional team involved in the liver removal and transplant, followed by hospital stay , medication used during surgery and hospitalization, and products (medication and materials) used in the liver removal. According to the current schedule, the SUS made the following payments to HUWC; US$1,322.97 for liver removal, US$3,223.56 for medical fees and US$32,235.68 for the package including the complete surgical procedure, hospital expenses and care up to the seventh day of hospitalization.
Results were similar to those found by other Brazilian transplant centers. Liver transplantation in Ceará is an economically viable procedure based upon the financial transfer from SUS.
Elucidation of the structural properties of colloids is paramount for a successful formulation. However, the intrinsic dynamism of colloidal systems makes their characterization a difficult task and, ...in particular, there is a lack of physicochemical techniques that can be correlated to their biological performance. Nanoparticle tracking analysis (NTA) allows measurements of size distribution and nanoparticle concentration in real time. Its analysis over time also enables the early detection of physical instability in the systems not assessed by subtle changes in size distribution. Nanoparticle concentration is a parameter with the potential to bridge the gap between in vitro characterization and biological performance of colloids, and therefore should be monitored in stability studies of formulations. To demonstrate this, we have followed two systems: extruded liposomes exposed to increasing CHCl
concentrations, and solid lipid nanoparticles prepared with decreasing amounts of poloxamer 188. NTA and dynamic light scattering (DLS) were used to monitor changes in nanoparticle number and size, and to estimate the number of lipid components per particle. The results revealed a strong negative correlation between particle size (determined by DLS) and concentration (assessed by NTA) in diluted samples, which should be adopted to monitor nanocolloidal stability, especially in drug delivery.

Objective
To investigate the association between the severity of periodontitis (exposure) and dyslipidemia (outcome).
Methods
This was a cross‐sectional study of users of public health services. ...Periodontitis was defined using the Center for Disease Prevention and Control and the American Academy of Periodontology criteria. Lipid evaluation used data on systemic biomarkers. Dyslipidemia diagnosis was based on the Guidelines of total cardiovascular risk of the World Health Organization. Weight, height, waist circumference, and blood pressure were measured, and socioeconomic–demographic, lifestyle behavior factors, general and oral health conditions of the participants were collected. Hierarchical and logistic regression analyzes were used to determine the association between the exposures and the outcome. Odds Ratios, unadjusted and adjusted, and 95% confidence intervals were estimated.
Results
Of 1,011 individuals examined, 75.17% had dyslipidemia, and 84.17% had periodontitis, 0.2% with mild, 48.56% moderate, and 35.41% severe disease. The association between periodontitis and dyslipidemia was maintained through hierarchical analysis and in the multiple regression modeling, showing that the occurrences of dyslipidemia in the group with periodontitis, and its moderate and severe levels, were, respectively, 14%, 30%, and 16% higher compared with those without periodontitis.
Conclusions
The results showed a positive association between moderate and severe periodontitis and dyslipidemia.
This study reports the development of nanostructured hydrogels for the sustained release of the eutectic mixture of lidocaine and prilocaine (both at 2.5%) for intraoral topical use. The local ...anesthetics, free or encapsulated in poly(ε-caprolactone) nanocapsules, were incorporated into CARBOPOL hydrogel. The nanoparticle suspensions were characterized in vitro in terms of particle size, polydispersity, and surface charge, using dynamic light scattering measurements. The nanoparticle concentrations were determined by nanoparticle tracking analysis. Evaluation was made of physicochemical stability, structural features, encapsulation efficiency, and in vitro release kinetics. The CARBOPOL hydrogels were submitted to rheological, accelerated stability, and in vitro release tests, as well as determination of mechanical and mucoadhesive properties, in vitro cytotoxicity towards FGH and HaCaT cells, and in vitro permeation across buccal and palatal mucosa. Anesthetic efficacy was evaluated using Wistar rats. Nanocapsules were successfully developed that presented desirable physicochemical properties and a sustained release profile. The hydrogel formulations were stable for up to 6 months under critical conditions and exhibited non-Newtonian pseudoplastic flows, satisfactory mucoadhesive strength, non-cytotoxicity, and slow permeation across oral mucosa. In vivo assays revealed higher anesthetic efficacy in tail-flick tests, compared to a commercially available product. In conclusion, the proposed hydrogel has potential for provision of effective and longer-lasting superficial anesthesia at oral mucosa during medical and dental procedures. These results open perspectives for future clinical trials.
