Niobium is commonly used for superconducting quantum systems as readout resonators, capacitors, and interconnects. Structural defects at the Nb/Si and air/Nb interface may be a major source of ...two-level systems (TLS), which are detrimental to the device's coherence time. Thus, identifying and understanding the microscopic origin of possible TLS in Nb-based devices and their relationship to processing is key to superconducting qubit performance improvement. This work studied the structure and thermal stability of surface oxide on physical vapor deposited Nb films on Si wafers, using aberration-corrected (scanning) transmission electron microscopy and spectroscopy. Here, all Nb films exhibit columnar growth with strong 110 textures. After in-situ heating of the heterostructure at 360 °C inside the microscope, the initial amorphous niobium surface oxides decompose into face-centered cubic Nb nanograins in the amorphous Nb-O matrix, which may reduce microwave dissipation. Despite changes in the microstructure and chemistry of the niobium oxide surface layer due to heat treatment, the interface between the Nb and the surface oxide layer remains almost unchanged. Our comprehensive study of the Nb surface oxide decomposition mechanism may guide future superconducting qubit device optimization through interfacial scattering center and TLS minimization.
Abstract Study Objective Characterize the incidence of elevated aPTT results in patients treated with prophylactic, subcutaneous unfractionated heparin (UFH). Design Retrospective, cohort analysis. ...Setting Single-center, university hospital. Measurements Evaluation of 257 patients with activated partial thromboplastin time (aPTT) testing both prior to and following subcutaneous (SC) unfractionated heparin (UFH) therapy. Main Results Evaluated patients received UFH 5000 units every 8 hours. Baseline aPTT values were within the normal range (mean ± SD, 32.0 ± 8.5 seconds). After initiation of UFH, aPTT values increased (mean ± SD, 37.6 ± 15.2 seconds). After 24 hours of SC UFH, mean aPTT values (mean ± SD, 38.6 ± 15.5) exceeded the normal laboratory range (23.3–35.7 seconds). An elevated aPTT result after UFH was associated with baseline aPTT, length of therapy, and weight-based UFH dose. A significant association was not identified between aPTT elevation and age, race, sex, history of liver disease, type of admission, or transfusion of blood products. Conclusions Treatment with UFH resulted in a small, but significant, increase in aPTT.
Chronic cough in interstitial lung disease (ILD) causes significant impairment in quality of life. Effective treatment approaches are needed for cough associated with ILD.
This systematic review ...asked: Is there evidence of clinically relevant treatment effects for therapies for cough in ILD? Studies of adults aged > 18 years with a chronic cough ≥ 8 weeks' duration were included and assessed for relevance and quality. Based on the systematic review, guideline suggestions were developed and voted on by using CHEST guideline methodology.
Eight randomized controlled trials and two case series (≥ 10 patients) were included that reported data on patients with idiopathic pulmonary fibrosis, sarcoidosis, and scleroderma-related ILD who received a variety of interventions. Study quality was high in all eight randomized controlled trials. Inhaled corticosteroids were not supported for cough associated with sarcoidosis. Cyclophosphamide and mycophenolate were not supported for solely treating cough associated with scleroderma-associated ILD. A recommendation for thalidomide to treat cough associated with idiopathic pulmonary fibrosis did not pass the panel vote. In view of the paucity of antitussive treatment options for refractory cough in ILD, the guideline panel suggested that the CHEST unexplained chronic cough guideline be followed by considering options such as the neuromodulator gabapentin and speech pathology management. Opiates were also suggested for patients with cough refractory to alternative therapies.
The evidence supporting the management of chronic cough in ILD is limited. This guideline presents suggestions for managing and treating cough on the best available evidence, but future research is clearly needed.
The purpose of this state-of-the-art review is to update the American College of Chest Physicians 2006 guideline on global physiology and pathophysiology of cough. A review of the literature was ...conducted using PubMed and MEDLINE databases from 1951 to 2019 and using prespecified search terms. We describe the basic phenomenology of cough patterns, behaviors, and morphological features. We update the understanding of mechanical and physiological characteristics of cough, adding a contemporary view of the types of cough and their associated behaviors and sensations. New information about acoustic characteristics is presented, and recent insights into cough triggers and the patient cough hypersensitivity phenotype are explored. Lastly, because the clinical assessment of patients largely focuses on the duration rather than morphological features of cough, we review the morphological features of cough that can be measured in the clinic. This is the first of a two-part update to the American College of Chest Physicians 2006 cough guideline; it provides a more global consideration of cough phenomenology, beyond simply the mechanical aspects of a cough. A greater understanding of the typical features of cough, and their variations, may allow a more informed interpretation of cough measurements and the clinical relevance for patients.
Summary
A best evidence topic in cardiac surgery was written according to a structured protocol. The question addressed was whether surgical septal myectomy (SM) is more beneficial than alcohol ...septal ablation (ASA) in patients with hypertrophic obstructive cardiomyopathy. Altogether 218 articles were found using the reported search, of which 15 studies represented the best evidence to answer the clinical question. There were 14 observational studies and 1 meta-analysis study. The authors, journal, date and country of publication, patient group studied, study type, relevant outcomes and results of these articles are tabulated. Surgical SM was generally performed in younger patients whereas percutaneous ASA was favoured in patients with advanced age and significant co-morbidities. In a large study comprising 716 patients, the reduction of median residual left ventricular outflow tract (LVOT) gradient at 3 months was comparable after ASA (102 ± 52–10 mmHg) and SM (92 ± 39–9 mmHg). The New York Heart Association (NYHA) functional class and symptomatic improvement for either approach was comparable. Findings from the meta-analysis study showed that patients who underwent ASA had a higher incidence of post-procedure device implantation (odds ratio 3.09; P < 0.00001), as reported in 6 other studies. The risk of permanent pacemaker insertion during follow-up (FU) varied between 2.4–12.5% in SM and 1.7–22.0% in ASA. Isolated surgical myectomy and ASA are safe and effective in abolishing outflow obstruction, although the resolution of LVOT pressure gradient is more complete with surgery. The post-procedural and late mortality rates between the 2 groups are consistently low and comparable in carefully selected patients. Nonetheless, ASA is associated with the increased likelihood of complications such as permanent pacemaker implantation, early sustained-VT and VF, and re-intervention. Overall, when performed by experienced cardiologists and surgeons, both techniques are safe and effective in most cases and therefore treatment should be offered based on patient choice.

LHC@home has provided computing capacity for simulations under BOINC since 2005. Following the introduction of virtualisation with BOINC to run HEP Linux software in a virtual machine on volunteer ...desktops, initially started on test BOINC projects, like Test4Theory and ATLAS@home, all CERN applications distributed to volunteers have been consolidated under a single LHC@home BOINC project. As part of an effort to unite CERN’s batch, cloud, grid and volunteer computing efforts, the BOINC service has been integrated with the other compute services at CERN, notably HTCondor, in terms job submission and accounting. The paper will also address contributions to improve the BOINC software and community effort to evolve BOINC for a sustainable volunteer computing environment. Furthermore, we discuss future challenges to reduce the effort required by volunteers to run virtual machines for experiment simulations and improvements to BOINC to widen the appeal of volunteer computing.

In vitro, PF4 increased the differentiation of hematopoietic progenitors to B cell lineage cells, indicating that PF4 has a direct effect on B cell differentiation. STAT5 activation is essential in early B cell development and PF4 increased the phosphorylation of STAT5. Taken together, these data demonstrate that PF4 has an important role in increasing B cell differentiation in the bone marrow environment.
Acute cough associated with the common cold (CACC) causes significant impairment in quality of life. Effective treatment approaches are needed for CACC. We conducted a systematic review on the ...management of CACC to update the recommendations and suggestions of the CHEST 2006 guideline on this topic.
This systematic review of randomized controlled trials (RCTs) asked the question: Is there evidence of clinically relevant treatment effects for pharmacologic or nonpharmacologic therapies in reducing the duration/severity of acute CACC? Studies of adults and pediatric patients with CACC were included and assessed for relevance and quality. Based on the systematic review, guideline suggestions were developed and voted on using the American College of Chest Physicians organization methodology.
Six systematic reviews and four primary studies identified from updated literature searches for each of the reviews or from hand searching were included and reported data on 6,496 participants with CACC who received one or more of a variety of interventions. The studies used an assortment of descriptors and assessments to identify CACC.
The evidence supporting the management of CACC is overall of low quality. This document provides treatment suggestions based on the best currently available evidence and identifies gaps in our knowledge and areas for future research.

Soil Science is a unique discipline concerning a complex material that is part of many natural and utilitarian systems. As such, the teaching of Soil Science requires principles that reflect the ...nature of soil and the practices of soil scientists. Because no discipline-specific teaching principles could be found for Soil Science in the literature, an iterative approach was used to develop them, which involved input from students, academics, employers, graduates in the workplace, as well as published generic teaching principles. The synthesis of these perspectives was achieved via a series of cycles that first involved student feedback on Soil Science teaching from five Australian universities, combined with academic reflections on learning and teaching. The outcome of this activity was subject to perspectives provided by employers of soil scientists and practising soil scientists in the workplace. Quantitative and qualitative analyses of these sources and published generic teaching materials were blended into a set of 11 teaching principles of Soil Science that reflect the unique nature of soil and the outcomes required of graduates who have majored in Soil Science.
