La TM réalisée ou non après une thrombolyse intraveineuse (TIV) permet de recanaliser une artère cérébrale occluse à la phase aiguë d’AVC ischémique. Ceci ne peut être réalisé que dans des centres ...labélisés avec NRI.
L’objectif de cette étude est d’évaluer la prise en charge des AVC par TM dans un centre sans NRI.
De façon rétrospective, l’ensemble des patients ayant eu une TM dans le centre ont été sélectionnés entre 2011 et 2022, soit 172 dossiers sélectionnés et 160 dossiers inclus. Les caractéristiques des patients, les délais de prise en charge et leur évolution ont été analysés. Les TM sont réalisées dans deux centres de recours situés respectivement à 22 et 39kms.
L’âge médian est de 72 ans, le sex-ratio H/F=1. Le NIHSS médian pré-TM de 15,5. Parmi les 160 patients ayant eu une TM, 67,5 % ont eu une TIV. Le délai médian admission/IRM était de 30min, le délai IRM/TIV de 47min, le délai TIV/TM de 1h 45 et le délai TM/recanalisation de 40min. Quatre-vingt-un pour cent des patients avaient un TICI 2b-3. Le NIH H24 médian était de 11. Les complications rencontrées étaient de 30 % d’extension ischémique, 51 % de transformation hémorragique et 30 % autres. Le taux de décès était de 3 %.
Si nous comparons ces résultats avec ceux des grandes études princeps réalisées dans les centres de recours, le délai médian entre AVC et TIV est de 180min vs 87–145min, le délai entre l’AVC et la TM de 269 minutes vs 210 à 269 minutes, le pourcentage de TICI 2b-3 (81 %) est comparable (58,7 à 88 %), le taux de mortalité est plus faible (3 % vs 10 %). Ces résultats sont similaires à ceux de la littérature.
L’organisation du parcours patient en IDF et la possibilité d’accéder à des centres de recours assurent une excellente qualité de prise en charge.
La sclérose solitaire est une entité rare, affectant la moelle cervicale ou la jonction bulbo-médullaire ; entraînant un syndrome évolutif. Nous rapportons le cas et l’évolution d’un patient ayant ...une sclérose solitaire et ayant subi une greffe de moelle osseuse autologue.
Patient de 54 ans, suivi pour sclérose en plaques (SEP) solitaire de la jonction bulbo-médullaire d’aggravation progressive suivi depuis 2014. À l’examen clinique, patient confiné au fauteuil roulant, syndrome tétrapyramidal à prédominance gauche, spasticité marquée. ROT vifs, diffus, polycinétiques avec trépidation épileptoïde. Babinski et Hoffman bilatéral. Sensibilité tactile thermo-algique normale, hypopallesthésie. Pas de troubles de l’oculomotricité, ni de la déglutition, ni vésicosphinctériens. EDSS à 6,5. Le reste de l’examen clinique normal. IRM encéphalique sans particularité en dehors d’un large kyste arachnoïdien frontal inter-hémisphérique gauche (Fig. 1). IRM médullaire lésion de la jonction bulbo-médullaire médiane antéropostérieur et une très discrète prise de contraste antérieur. Les PEV étaient normaux. PES anomalies compatibles avec une atteinte latéralisée à gauche des cordinales postérieures au niveau cervical C8. PEM altérés à partir des recueils abdominaux bas témoignant d’une atteinte des voies pyramidales d’un niveau dorsal D11-D12. PL : protéinorachie : 0,34g/L ; glycorachie : 3,4g/L. Isoélectrofocalisation : profil normal. Index IgG à 0,49. Présence de pic étroit au niveau des gammaglobulines à l’IEP sériques. Auto-anticorps anti-aquaporin-4 (NMO) négatifs ; anticorps anti-MOG négatifs. Patient initialement traité par bolus mensuels de SOLUMEDROL 1g, puis Rituximab sans amélioration. Devant l’aggravation clinique, une autogreffe réalisée en Russie en 02/2019.
Suite à l’autogreffe le patient à présenté une thrombopénie à 19 G/L ; neutropénie probablement de margination ou ethnique, car initialement le caryotype était normal ; gammapathie monoclonale Lambda. EDSS post-greffe à 7,0. À 3 ans de la greffe, le score EDSS est évalué à 7,5.
L’autogreffe de moelle a été bien tolérée, mais n’a pas permis d’enrayer l’évolution chez ce patient présentant une sclérose solitaire de la jonction bulbo-médullaire. Aucun traitement n’est reconnu dans cette forme et l’évolution se fait vers un handicap sévère.

To describe the clinical characteristics and outcomes in patients with MS and COVID-19 and identify factors associated with COVID-19 severity.
The Covisep registry is a multicenter, retrospective, observational cohort study conducted in MS expert centers and general hospitals and with neurologists collaborating with MS expert centers and members of the Société Francophone de la Sclérose en Plaques. The study included patients with MS presenting with a confirmed or highly suspected diagnosis of COVID-19 between March 1, 2020, and May 21, 2020.
COVID-19 diagnosed with a polymerase chain reaction test on a nasopharyngeal swab, thoracic computed tomography, or typical symptoms.
The main outcome was COVID-19 severity assessed on a 7-point ordinal scale (ranging from 1 not hospitalized with no limitations on activities to 7 death) with a cutoff at 3 (hospitalized and not requiring supplemental oxygen). We collected demographics, neurological history, Expanded Disability Severity Scale score (EDSS; ranging from 0 to 10, with cutoffs at 3 and 6), comorbidities, COVID-19 characteristics, and outcomes. Univariate and multivariate logistic regression models were used to estimate the association of collected variables with COVID-19 outcomes.
A total of 347 patients (mean SD age, 44.6 12.8 years, 249 women; mean SD disease duration, 13.5 10.0 years) were analyzed. Seventy-three patients (21.0%) had a COVID-19 severity score of 3 or more, and 12 patients (3.5%) died of COVID-19. The median EDSS was 2.0 (range, 0-9.5), and 284 patients (81.8%) were receiving DMT. There was a higher proportion of patients with a COVID-19 severity score of 3 or more among patients with no DMT relative to patients receiving DMTs (46.0% vs 15.5%; P < .001). Multivariate logistic regression models determined that age (odds ratio per 10 years: 1.9 95% CI, 1.4-2.5), EDSS (OR for EDSS ≥6, 6.3 95% CI. 2.8-14.4), and obesity (OR, 3.0 95% CI, 1.0-8.7) were independent risk factors for a COVID-19 severity score of 3 or more (indicating hospitalization or higher severity). The EDSS was associated with the highest variability of COVID-19 severe outcome (R2, 0.2), followed by age (R2, 0.06) and obesity (R2, 0.01).
In this registry-based cohort study of patients with MS, age, EDSS, and obesity were independent risk factors for severe COVID-19; there was no association found between DMTs exposure and COVID-19 severity. The identification of these risk factors should provide the rationale for an individual strategy regarding clinical management of patients with MS during the COVID-19 pandemic.
Fatigue is a disabling symptom of multiple sclerosis (MS). The lack of effective therapeutics has promoted the development of cognitive behavioural therapy (CBT)-based fatigue management programmes. ...However, their efficacy does not sustain over time. We proposed to test the long-term effectiveness of a 6-week fatigue programme supplemented with four booster sessions ('FACETS+') in patients with relapsing remitting MS (RRMS) and fatigue.
This multicentre, randomised, controlled, open-label, parallel-group trial versus standard care enrolled patients with RRMS and fatigue. Participants were randomised to either FACETS+ plus standard care or standard care alone. The primary outcome measure was fatigue impact (Modified Fatigue Impact Scale (MFIS) at 12 months) based on intention-to-treat analyses.
From May 2017 to September 2020, 162 patients were screened; 105 were randomly assigned to FACETS+ (n=57) or standard care (n=48) and 88 completed the primary outcome assessment for the MFIS. At month 12, participants showed improved MFIS compared with baseline in the intervention group (mean difference (MD)=14.0 points; (95% CI 6.45 to 21.5)) and the control group (MD=6.1 points; (95% CI -0.30 to 12.5)) with a significant between-group difference in favour of the intervention group (adjusted MD=7.89 points; (95% CI 1.26 to 14.52), standardised effect size=0.52, p=0.021). No trial-related serious adverse events were reported.
A 6-week CBT-based programme with four booster sessions is superior to standard care alone to treat MS-related fatigue in the long term (12 months follow-up). The results support the use of the FACETS+ programme for the treatment of MS-related fatigue.

Le diagnostic et le traitement de la SEP reposent de plus en plus sur l’analyse de l’IRM. L’IA pourrait permettre une meilleure détection des nouvelles lésions (NL) et des lésions à expansion lente ...(SEL).
Comparer l’analyse des IRM par IA basée sur l’apprentissage profond au rapport du radiologue. Évaluer l’impact de cette comparaison sur la décision thérapeutique.
Nous avons inclus rétrospectivement 31 patients atteints de SEP ayant réalisé deux IRM dans le même établissement. Les séquences 3D FLAIR, 3D T1W avant et après injection, ont été analysées par le logiciel Jazz (IA). Deux neuroradiologues ont revu indépendamment tous les cas : nombre de NL, SEL et lésions rehaussées par le produit de contraste et ont comparé le rapport Jazz avec le rapport initial du radiologue. La décision thérapeutique initiale a été rediscutée dans le cadre d’une RCP en tenant compte du rapport Jazz.
Jazz a permis de détecter des NL chez 10/31 patients, vs 4/31 dans les rapports initiaux. Des SEL ont été détectées chez 28/31 patients, vs 0/31 dans les rapports initiaux. Des lésions rehaussées par le gadolinium ont été détectées chez 4/31 patients, vs 3/31 dans les rapports initiaux. À l’issue des RCP utilisant les résultats de l’IA Jazz, la stratégie thérapeutique a été modifiée pour 8/31 patients (25,8 %)
L’utilisation de l’IA Jazz a montré une meilleure évaluation des images IRM que l’analyse radiologique standard en particulier pour les SEL et les NL. Les SELs présentent une croissance lente de leur taille au fil du temps, et représentent un marqueur de l’activité chronique de la sclérose en plaque. Associé à l’interprétation clinique l’augmentation de précision de l’(IA) conduit à une modification de la stratégie thérapeutique dans ¼ des cas.
L’IA est un outil puissant de détection des NL et des SEL susceptible de modifier les stratégies thérapeutiques dans la SEP.

The safety and efficacy of switching from natalizumab to fingolimod have not yet been evaluated in a large cohort of patients with multiple sclerosis (MS) to our knowledge.

The Enquête Nationale sur l'Introduction du Fingolimod en Relais au Natalizumab (ENIGM) study, a survey-based, observational multicenter cohort study among MS tertiary referral centers. Participants were patients for whom a switch from natalizumab to fingolimod was planned. Clinical data were collected on natalizumab treatment, duration and management of the washout period (WP), and relapse or adverse events during the WP and after the initiation of fingolimod.
Occurrence of MS relapse during the WP or during a 6-month follow-up period after the initiation of fingolimod.
Thirty-six French MS tertiary referral centers participated. In total, 333 patients with MS switched from natalizumab to fingolimod after a mean of 31 natalizumab infusions (female to male ratio, 2.36; mean age, 41 years; and Expanded Disability Status Scale score at the initiation of natalizumab, 3.6). Seventy-one percent were seropositive for the JC polyomavirus. The Expanded Disability Status Scale score remained stable for patients receiving natalizumab. Twenty-seven percent of patients relapsed during the WP. A WP shorter than 3 months was associated with a lower risk of relapse (odds ratio, 0.23; P = .001) and with less disease activity before natalizumab initiation (P = .03). Patients who stopped natalizumab because of poor tolerance or lack of efficacy also had a higher risk of relapse (odds ratio, 3.20; P = .004). Twenty percent of patients relapsed during the first 6 months of fingolimod therapy. Three percent stopped fingolimod for efficacy, tolerance, or compliance issues. In the multivariate analysis, the occurrence of relapse during the WP was the only significant prognostic factor for relapse during fingolimod therapy (odds ratio, 3.80; P = .05).
In this study, switching from natalizumab to fingolimod was associated with a risk of MS reactivation during the WP or shortly after fingolimod initiation. The WP should be shorter than 3 months.
L’idarucizumab est l’antidote spécifique du dabigatran qui permet la neutralisation immédiate et complète de son effet anticoagulant. Nous rapportons un cas de thrombolyse intra-veineuse (TIV) chez ...un patient sous dabigatran.
Il s’agit d’un patient de 71 ans, sous dabigatran pour une fibrillation atriale. Admis dans notre service, dans le cadre d’une alerte TIV pour un déficit moteur de l’hémicorps gauche de survenue brutale. À l’admission il présente une hémiparésie ataxiante gauche et une dysarthrie avec un score de NIHSS à 6. Une IRM cérébrale réalisée à l’admission, en faveur d’un accident vasculaire cérébral ischémique sylvien profond droit, la séquence T2-flair est négative, absence d’occlusion d’artères proximales. Le dosage plasmatique du dabigatran était de 187ng/ml. L’indication d’une TIV posée, et l’inversion de l’effet du dabigatran réalisée par l’idarucizumab 5g en perfusion intra-veineuse (IV) rapide ; immédiatement après, la TIV par r-tpa réalisée avec succès. L’évolution a été marquée à 24heures de la TIV par une récupération complète de son déficit neurologique avec un NIHSS à 0. Le scanner cérébral de contrôle post TIV à 24heures en faveur d’un discret remaniement hémorragique, qui n’empêche pas l’introduction d’un antiagrégant plaquettaire. Le bilan d’hémostase post TIV était normal. Reprise d’une anticoagulation orale à sept jour de son accident vasculaire cérébral.
L’idarucizumab est un anticorps monoclonal dont l’affinité pour le dabigatran est 350 fois celle de la thrombine. Il permet une neutralisation immédiate et complète de l’action du dabigatran. À l’inverse des concentrés de complexe prothrombinique, du fait de son mécanisme d’action, l’idarucizumab ne possède théoriquement aucun effet pro-thrombotique. La dose standard de l’ideracizumab est de 5g en perfusion IV rapide.
Dans ce cas clinique, l’idarucizumab a neutralisé l’effet anticoagulant du dabigatran, et le traitement par r-tPA était sans danger, le patient complètement remis et satisfait de notre prise en charge.
Moderately effective therapies (METs) have been the main treatment in pediatric-onset multiple sclerosis (POMS) for years. Despite the expanding use of highly effective therapies (HETs), treatment ...strategies for POMS still lack consensus.
To assess the real-world association of HET as an index treatment compared with MET with disease activity.
This was a retrospective cohort study conducted from January 1, 2010, to December 8, 2022, until the last recorded visit. The median follow-up was 5.8 years. A total of 36 French MS centers participated in the Observatoire Français de la Sclérose en Plaques (OFSEP) cohort. Of the total participants in OFSEP, only treatment-naive children with relapsing-remitting POMS who received a first HET or MET before adulthood and at least 1 follow-up clinical visit were included in the study. All eligible participants were included in the study, and none declined to participate.

The primary outcome was the time to first relapse after treatment. Secondary outcomes were annualized relapse rate (ARR), magnetic resonance imaging (MRI) activity, time to Expanded Disability Status Scale (EDSS) progression, tertiary education attainment, and treatment safety/tolerability. An adapted statistical method was used to model the logarithm of event rate by penalized splines of time, allowing adjustment for effects of covariates that is sensitive to nonlinearity and interactions.
Of the 3841 children (5.2% of 74 367 total participants in OFSEP), 530 patients (mean SD age, 16.0 1.8 years; 364 female 68.7%) were included in the study. In study patients, both treatment strategies were associated with a reduced risk of first relapse within the first 2 years. HET dampened disease activity with a 54% reduction in first relapse risk (adjusted hazard ratio HR, 0.46; 95% CI, 0.31-0.67; P < .001) sustained over 5 years, confirmed on MRI activity (adjusted odds ratio OR, 0.34; 95% CI, 0.18-0.66; P = .001), and with a better tolerability pattern than MET. The risk of discontinuation at 2 years was 6 times higher with MET (HR, 5.97; 95% CI, 2.92-12.20). The primary reasons for treatment discontinuation were lack of efficacy and intolerance. Index treatment was not associated with EDSS progression or tertiary education attainment (adjusted OR, 0.51; 95% CI, 0.24-1.10; P = .09).
Results of this cohort study suggest that compared with MET, initial HET in POMS was associated with a reduction in the risk of first relapse with an optimal outcome within the first 2 years and was associated with a lower rate of treatment switching and a better midterm tolerance in children. These findings suggest prioritizing initial HET in POMS, although long-term safety studies are needed.
Purpose
The assessment of multiple sclerosis (MS) lesions on follow-up magnetic resonance imaging (MRI) is tedious, time-consuming, and error-prone. Automation of low-level tasks could enhance the ...radiologist in this work. We evaluate the intelligent automation software Jazz in a blinded three centers study, for the assessment of new, slowly expanding, and contrast-enhancing MS lesions.
Methods
In three separate centers, 117 MS follow-up MRIs were blindly analyzed on fluid attenuated inversion recovery (FLAIR), pre- and post-gadolinium T1-weighted images using Jazz by 2 neuroradiologists in each center. The reading time was recorded. The ground truth was defined in a second reading by side-by-side comparison of both reports from Jazz and the standard clinical report. The number of described new, slowly expanding, and contrast-enhancing lesions described with Jazz was compared to the lesions described in the standard clinical report.
Results
A total of 96 new lesions from 41 patients and 162 slowly expanding lesions (SELs) from 61 patients were described in the ground truth reading. A significantly larger number of new lesions were described using Jazz compared to the standard clinical report (63 versus 24). No SELs were reported in the standard clinical report, while 95 SELs were reported on average using Jazz. A total of 4 new contrast-enhancing lesions were found in all reports. The reading with Jazz was very time efficient, taking on average 2min33s ± 1min0s per case. Overall inter-reader agreement for new lesions between the readers using Jazz was moderate for new lesions (Cohen kappa = 0.5) and slight for SELs (0.08).
Conclusion
The quality and the productivity of neuroradiological reading of MS follow-up MRI scans can be significantly improved using the dedicated software Jazz.
