Gas accretion onto some massive black holes (MBHs) at the centers of galaxies actively powers luminous emission, but most MBHs are considered dormant. Occasionally, a star passing too near an MBH is ...torn apart by gravitational forces, leading to a bright tidal disruption flare (TDF). Although the high-energy transient Sw 1644+57 initially displayed none of the theoretically anticipated (nor previously observed) TDF characteristics, we show that observations suggest a sudden accretion event onto a central MBH of mass about 10 6 to 10 7 solar masses. There is evidence for a mildly relativistic outflow, jet collimation, and a spectrum characterized by synchrotron and inverse Compton processes; this leads to a natural analogy of Sw 1644+57 to a temporary smaller-scale blazar.
Pulmonary hypertension (PH) is highly heterogeneous and despite treatment advances it remains a life-shortening condition. There have been significant advances in imaging technologies, but despite ...evidence of their potential clinical utility, practice remains variable, dependent in part on imaging availability and expertise. This statement summarizes current and emerging imaging modalities and their potential role in the diagnosis and assessment of suspected PH. It also includes a review of commonly encountered clinical and radiological scenarios, and imaging and modeling-based biomarkers. An expert panel was formed including clinicians, radiologists, imaging scientists, and computational modelers. Section editors generated a series of summary statements based on a review of the literature and professional experience and, following consensus review, a diagnostic algorithm and 55 statements were agreed. The diagnostic algorithm and summary statements emphasize the key role and added value of imaging in the diagnosis and assessment of PH and highlight areas requiring further research.
Background.Treatment of invasive mold infection in immunocompromised patients remains challenging. Voriconazole has been shown to have efficacy and survival benefits over amphotericin B deoxycholate, ...but its utility is limited by drug interactions. Liposomal amphotericin B achieves maximum plasma levels at a dosage of 10 mg/kg per day, but clinical efficacy data for higher doses are lacking. Methods.In a double-blind trial, patients with proven or probable invasive mold infection were randomized to receive liposomal amphotericin B at either 3 or 10 mg/kg per day for 14 days, followed by 3 mg/kg per day. The primary end point was favorable (i.e., complete or partial) response at the end of study drug treatment. Survival and safety outcomes were also evaluated. Results.Of 201 patients with confirmed invasive mold infection, 107 received the 3-mg/kg daily dose, and 94 received the 10-mg/kg daily dose. Invasive aspergillosis accounted for 97% of cases. Hematological malignancies were present in 93% of patients, and 73% of patients were neutropenic at baseline. A favorable response was achieved in 50% and 46% of patients in the 3- and 10-mg/kg groups, respectively (difference, 4%; 95% confidence interval, -10% to 18%; P > .05); the respective survival rates at 12 weeks were 72% and 59% (difference, 13%; 95% confidence interval, -0.2% to 26%; P > .05). Significantly higher rates of nephrotoxicity and hypokalemia were seen in the high-dose group. Conclusions.In highly immunocompromised patients, the effectiveness of 3 mg/kg of liposomal amphotericin B per day as first-line therapy for invasive aspergillosis is demonstrated, with a response rate of 50% and a 12-week survival rate of 72%. The regimen of 10 mg/kg per day demonstrated no additional benefit and higher rates of nephrotoxicity.
Abstract
We present the first results from Karl G. Jansky Very Large Array (VLA) observations as a part of the Fundamental Reference Active Galactic Nucleus (AGN) Monitoring Experiment, a program to ...understand the relationship between AGN accretion physics and wavelength-dependent position as a function of time. With this VLA survey, we investigate the radio properties from a volume-complete sample of 25 hard X-ray-selected AGNs using the VLA in its wideband mode. We observed the targets in the A-array configuration at 4–12 GHz with all polarization products. In this work, we introduce our calibration and imaging methods for this survey, and we present our results and analysis for the radio-quiet AGN NGC 4388. We calibrated and imaged these data using the multiterm, multifrequency synthesis imaging algorithm to determine its spatial, spectral, and polarization structure across a continuous 4–12 GHz band. In the AGN, we measure a broken power law spectrum with
α
= − 0.06 below a break frequency of 7.3 GHz and
α
= − 0.34 above. We detect polarization at subarcsecond resolution across both the AGN and a secondary radio knot. We compare our results to ancillary data and find that the VLA radio continuum is likely due to AGN winds interacting with the local interstellar medium that gets resolved away at subparsec spatial scales as probed by the Very Long Baseline Array. A well-known ionization cone to the southwest of the AGN appears likely to be projected material onto the underside of the disk of the host galaxy.
The marginal areas of the Greenland ice sheet develop streams and lakes each summer, documenting that surface runoff of meltwater is a major component of ice-sheet mass balance. Here we map the slush ...limit, a proxy for the extent of surface runoff, using daily MODIS data for the years 2000–2021. We develop an automated algorithm capable of detecting daily slush limits, provided sufficient image quality. The algorithm is applied to the ice sheet's western flank (61.7 $^{\circ }$N to 76.5 $^{\circ }$N). We find significant increasing trends in maximum slush limits until the year 2012, but not thereafter. We show that the slush limit typically rises quickly early in the ablation season but stabilizes before melting ceases. The data provide evidence that upward migration of surface runoff in summer 2012 stopped early at the upper margin of the ice slabs. These thick and continuous ice layers are located close to the surface, in the firn, and impede percolation of melt into deeper pore space. Had the ice slabs extended higher, the summer 2012 provided sufficient energy to raise the slush limit by another $\sim$300 m in elevation.
In response to a warming climate, temperature extremes are changing in many regions of the world. Therefore, understanding how the fluxes of sensible heat, latent heat and net ecosystem exchange ...respond and contribute to these changes is important. We examined 216 sites from the open access Tier 1 FLUXNET2015 and free fair-use La Thuile data sets, focussing only on observed (non-gap-filled) data periods. We examined the availability of sensible heat, latent heat and net ecosystem exchange observations coincident in time with measured temperature for all temperatures, and separately for the upper and lower tail of the temperature distribution, and expressed this availability as a measurement ratio. We showed that the measurement ratios for both sensible and latent heat fluxes are generally lower (0.79 and 0.73 respectively) than for temperature measurements, and the measurement ratio of net ecosystem exchange measurements are appreciably lower (0.42). However, sites do exist with a high proportion of measured sensible and latent heat fluxes, mostly over the United States, Europe and Australia. Few sites have a high proportion of measured fluxes at the lower tail of the temperature distribution over very cold regions (e.g. Alaska, Russia) or at the upper tail in many warm regions (e.g. Central America and the majority of the Mediterranean region), and many of the world's coldest and hottest regions are not represented in the freely available FLUXNET data at all (e.g. India, the Gulf States, Greenland and Antarctica). However, some sites do provide measured fluxes at extreme temperatures, suggesting an opportunity for the FLUXNET community to share strategies to increase measurement availability at the tails of the temperature distribution. We also highlight a wide discrepancy between the measurement ratios across FLUXNET sites that is not related to the actual temperature or rainfall regimes at the site, which we cannot explain. Our analysis provides guidance to help select eddy covariance sites for researchers interested in understanding and/or modelling responses to temperature extremes.
