Mesenchymal stem cells (MSCs) are powerful sources for cell therapy in regenerative medicine because they can be isolated from various tissues, expanded, and induced into multiple-lineages. Of note, ...their immunomodulatory effects maximize the therapeutic effects of stem cells engrafted on host, making them an especially attractive choice. Recently, several varieties of placenta-derived stem cells (PDSCs) including chorionic plate-derived MSCs (CP-MSCs) have been suggested as alternative sources of stem cells. However, comparative studies of immunomodulatory effects for CP-MSCs among various MSCs are largely lacking. We examined and compared immunomodulatory function of CP-MSCs with that of BM-MSCs and AD-MSCs using co-culture system with activated T-cells derived from human umbilical cord blood (UCB) exposed to anti-CD3 and anti-CD28 which are T-cell activating monoclonal antibodies. All MSCs expressed markers of stem cells and three germ layers by RT-PCR. These cells also exhibited comparable immunomodulatory effects when they were co-cultured with activated T-cells in dose-dependent manner. However, expression of HLA-ABC and HLA-G was highly positive in CP-MSCs compared to other MSCs, and higher levels of cytokines of IL-2, IL-4, IL-13, and GM-CSF were detected in dose-dependent manner in CP-MSCs. Taken together, the results of the present study suggest that while CP-MSCs, BM-MSCs, and AD-MSCs all have immunomodulatory effects, CP-MSCs may have additional advantage over the other MSCs in terms of immunomodulation. In conjunction with other previous studies, CP-MSCs are suggested to be a useful stem cell source in cell therapy.

Gastrectomy is a proxy of malnutrition, which may lead to increased risk for developing pulmonary tuberculosis (TB). Malabsorption in gastrectomy patients could lead to low serum levels of ...rifampicin, which may be related to higher treatment failure. However, there is limited information on treatment outcomes of TB in patients who have undergone gastrectomy. This study aims to determine treatment outcomes and adverse effects in patients treated for TB after undergoing gastrectomy for gastric cancer. During the study period, 112 patients were treated for active TB that developed after gastrectomy for gastric cancer. Among them, we selected 15 patients who were culture positive at initial diagnosis and had evidence of active TB on imaging studies; namely, the remaining 97 patients without initial culture or imaging studies were excluded. We thus performed a case-control study of gastric cancer patients treated for TB after undergoing gastrectomy (n = 15). The control group was defined as age- and sex-matched TB patients who had not received gastrectomy (n = 45). Treatment failure in clinical, microbiological aspects, and adverse events were analyzed. Patients who had undergone gastrectomy exhibited higher 4-month clinical failure rates, compared to non-gastrectomy patient: 4 (26.7%) vs. 1 (2.2%), P = 0.012. Gastrointestinal adverse effects were more frequent in patients with gastrectomy, compared to non-gastrectomy patients: 9 (60%) vs. 5 (11.1%), P < 0.001. In conclusion, patients treated for TB after undergoing gastrectomy are associated with higher rates of gastrointestinal adverse events and treatment failure.
Background/Aims: Tegoprazan is a novel potassium-competitive acid blocker that has beneficial effects on acid-related disorders such as gastroesophageal reflux and peptic ulcer diseases. This study ...aimed to validate the effect of tegoprazan on endoscopic submucosal dissection (ESD)-induced artificial ulcers.
Methods: Patients from 16 centers in Korea who underwent ESD for gastric neoplasia were enrolled. After ESD, pantoprazole was administered intravenously for 48 hours. The patients were randomly allocated to either the tegoprazan or esomeprazole group. Tegoprazan 50 mg or esomeprazole 40 mg were administered for 4 weeks, after which gastroscopic evaluation was performed. If the artificial ulcer had not healed, the same dose of tegoprazan or esomeprazole was administered for an additional 4 weeks, and a gastroscopic evaluation was performed.
Results: One hundred sixty patients were enrolled in this study. The healing rates of artificial ulcers at 4 weeks were 30.3% (23/76) and 22.1% (15/68) in the tegoprazan and esomeprazole groups, respectively (p=0.006). At 8 weeks after ESD, the cumulative ulcer healing rates were 73.7% (56/76) and 77.9% (53/68) in the tegoprazan and esomeprazole groups, respectively (p=0.210). Delayed bleeding occurred in two patients in the tegoprazan group (2.6%) and in one patient in the esomeprazole group (1.5%). Other adverse events were negligible in both groups.
Conclusions: Tegoprazan showed similar effects on post-ESD artificial ulcer healing in comparison with esomeprazole. (Gut Liver 2024;18:257-264)
Several studies have evaluated the impact of matrix-assisted laser desorption/ionization time-of-flight (MALDI-TOF) mass spectrometry (MS) combined with antimicrobial stewardship in patients with ...positive blood cultures; clinical outcomes improved. However, in many hospitals, antimicrobial stewardship is not available because of restricted medical resources. Thus, we investigated the impact of evaluation by MALDI-TOF MS on the clinical outcomes of patients with bacteremia and fungemia treated in a clinical setting lacking an antimicrobial stewardship program (ASP).
We designed a pre-post quasi experimental study and retrospectively reviewed the medical records of patients aged > 18 years old with bacteremia and fungemia during two periods: October-December 2012 and October-December 2013. Conventional methods were used to detect microbial pathogens in 2012, and MALDI-TOF MS was employed in 2013. Clinical outcomes compared between periods were the time to pathogen identification, time to effective therapy, 30-day all-cause mortality, time to microbiological clearance, length of ICU stay, and rate of recurrence of the same bloodstream infection (BSI).
A total of 556 patients were enrolled; 302 patients in 2012, and 254 in 2013. The use of MALDI-TOF MS without an ASP reduced the time to pathogen identification (86.4 vs. 63.5 h, P < 0.001) but did not significantly reduce the time to effective therapy (27.4 vs. 23.2 h, P = 0.187). Also, none of the following differed significantly between the two periods: mortality (17.5 vs. 15.7%, P = 0.571), the time to microbiological clearance (3.6 vs. 3.7 days, P = 0.675), the length of ICU stay (16.8 vs. 14.7 days, P = 0.706), and the recurrence rate of the same BSI (5.0 vs. 2.8%, P = 0.183).
The use of MALDI-TOF MS alone in a setting lacking an ASP did not afford clinical benefits. An ASP combined with MALDI-TOF MS is necessary to improve clinical outcomes.
A printed circuit steam generator (PCSG) is being considered as the component for pressurized water reactor (PWR) type small modular reactor (SMR) that can further reduce the physical size of the ...system. Since a steam generator in many PWR-type SMR generates superheated steam, it is expected that dryout front oscillation can potentially cause thermal fatigue failure due to cyclic thermal stresses induced by the transition in boiling regimes between convective evaporation and film boiling. To investigate the fatigue issue of a PCSG, a reference PCSG is designed in this study first using an in-house PCSG design tool. For the stress analysis, a finite element method analysis model is developed to obtain the temperature and stress fields of the designed PCSG. Fatigue estimation is performed based on ASME Boiler and pressure vessel code to identify the major parameters influencing the fatigue life time originating from the dryout front oscillation. As a result of this study, the limit on the temperature difference between the hot side and cold side fluids is obtained. Moreover, it is found that the heat transfer coefficient of convective evaporation and film boiling regimes play an essential role in the fatigue life cycle as well as the temperature difference.

Analysis of 368 clinical strains confirmed amoxicillin resistance in 31 strains (resistance rate of 8.7%). Abstract Background To determine a new classification system for medial meniscus root tears (MMRT) based on arthroscopic findings. Methods 24 knees (55%) belonged to the nondisplaced or overlapped ...group, and 20 knees (45%) to the widely displaced group. Absolute meniscal extrusion was defined as distance between outer edge of the articular cartilage of tibial plateau and meniscal outer edge. Relative extrusion was defined as extruded meniscus width divided by entire meniscal width, multiplied by 100. The proportion of knees with major (> 3 mm) extrusion were compared in two groups, as were the severity of chondral wear and osteoarthritic change. Results Absolute (4.6 mm vs. 3.7 mm, P = 0.006) and relative (46% vs. 39%, P = 0.042) extrusion of the medial meniscus were greater in widely displaced than in nondisplaced or overlapped group. Medial joint space width was significantly narrower in the widely displaced than in the nondisplaced or overlapped group (3.0 mm vs. 4.0 mm, P = 0.007). The widely displaced group had a 4° greater varus deformity, and higher rates of major extrusion (> 3 mm), grade III or IV chondral wear in the medial femoral condyle (60% vs. 29%, P = 0.039) and medial compartment osteoarthritis (75% vs. 21%, P = 0.001) than did the nondisplaced or overlapped group. Conclusions Widely displaced MMRT had greater meniscal extrusion and more severe chondral wear and osteoarthritis than did nondisplaced or overlapped MMRT. In this novel classification system, the stage of MMRT severity was associated with tear site displacement. Level of evidence Case series (level IV).
Chinese chive (Allium tuberosum Rottler) has potential risks associated with pathogenic bacterial contamination as it is usually consumed raw. In this study, we investigated the microbiota of Chinese ...chives purchased from traditional markets and grocery stores in March (Spring) and June (Summer) 2017. Differences in bacterial diversity were observed, and the microbial composition varied across sampling times and sites. In June, potential pathogenic genera, such as Escherichia, Enterobacter, and Pantoea, accounted for a high proportion of the microbiota in samples purchased from the traditional market. A large number of pathogenic bacteria (Acinetobacter lwoffii, Bacillus cereus, Klebsiella pneumoniae, and Serratia marcescens) were detected in the June samples at a relatively high rate. In addition, the influence of the washing treatment on Chinese chive microbiota was analyzed. After storage at 26℃, the washing treatment accelerated the growth of enterohemorrhagic Escherichia coli (EHEC) because it caused dynamic shifts in Chinese chive indigenous microbiota. These results expand our knowledge of the microbiota in Chinese chives and provide data for the prediction and prevention of food-borne illnesses.
