Introduction: Performance anxiety is a transdiagnostic construct similar in both sports and clinical medical examinations. Given that acceptance and commitment therapy (ACT) and other similar ...therapies have transdiagnostic evidence in improving the performance of athletes, the same approach can be adopted for medical students in improving their performance during clinical examinations. This pilot study aimed to assess the efficacy of a brief ACT-based intervention in improving transdiagnostic performance anxiety in clinical medical students through assessing changes in psychopathology scores (depression, anxiety, and stress) and psychological process variables (psychological flexibility, cognitive fusion, and mindfulness). Methods: Final-year medical students were randomized into intervention and control groups. A one-day ACT-based intervention was delivered to an intervention group, with the control group crossing over one month post intervention. Both groups filled in sociodemographic questionnaires and questionnaires measuring psychological flexibility, cognitive fusion, mindfulness, depression, anxiety, and stress at three time points: T1 (before intervention), T2 (immediately after intervention), and T3 (one month post intervention). Repeated measures ANOVA was employed to assess the change between the intervention and control groups over time. Results and Conclusions: There was a significant change in anxiety scores from T1 to T3. In addition, there were significant improvements in mindfulness, cognitive fusion, and psychological flexibility scores over time in the intervention group compared with the control group. This pilot study builds on small single-sample evidence bases for the efficacy of an ACT-based intervention in non-sports performance enhancement, suggesting that larger-scale randomized trials of similar interventions in clinical medical students may prove equally efficacious.
COVID-19 has affected almost every country in the world, which causing many negative implications in terms of education, economy and mental health. Worryingly, the trend of second or third wave of ...the pandemic has been noted in multiple regions despite early success of flattening the curve, such as in the case of Malaysia, post Sabah state election in September 2020. Hence, it is imperative to predict ongoing trend of COVID-19 to assist crucial policymaking in curbing the transmission.
Generalized logistic growth modelling (GLM) approach was adopted to make prediction of growth of cases according to each state in Malaysia. The data was obtained from official Ministry of Health Malaysia daily report, starting from 26 September 2020 until 1 January 2021.
Sabah, Johor, Selangor and Kuala Lumpur are predicted to exceed 10,000 cumulative cases by 2 February 2021. Nationally, the growth factor has been shown to range between 0.25 to a peak of 3.1 throughout the current Movement Control Order (MCO). The growth factor range for Sabah ranged from 1.00 to 1.25, while Selangor, the state which has the highest case, has a mean growth factor ranging from 1.22 to 1.52. The highest growth rates reported were in WP Labuan for the time periods of 22 Nov - 5 Dec 2020 with growth rates of 4.77. States with higher population densities were predicted to have higher cases of COVID-19.
GLM is helpful to provide governments and policymakers with accurate and helpful forecasts on magnitude of epidemic and peak time. This forecast could assist government in devising short- and long-term plan to tackle the ongoing pandemic.
The COVID-19 pandemic has had considerable psychological health impacts across the globe. This study aimed to establish the psychological process variables underlying psychopathology in Malaysian ...public university students during the national Movement Control Order (MCO). The aim was to craft structured and sustainable psychological support programs with these students. We conducted a cross-sectional study involving Malaysian university students subjected to the Malaysian MCO. Structured questionnaires measuring sociodemographic factors, measures of depression, anxiety, stress, psychological mindedness, psychological flexibility and state mindfulness were employed. A total of 515 students participated in this study with 12 students (2.3%) being quarantined at the time. Many of them scored ‘moderate’ or above on the Depression, Anxiety and Stress Scale (DASS) with 20.2%, 25.0% and 14.2%, respectively. Quarantined students had higher depressive symptoms, with female students scoring significantly higher for depression, anxiety, and stress. Multiple regressions suggested gender and quarantine status predicted depression scores. However, only gender significantly predicted anxiety and stress. Psychological flexibility and psychological mindedness (Insight subscale) are significantly correlated with depression, anxiety, and stress, with psychological mindedness predicting all three psychopathologies. This study demonstrates that gender, psychological flexibility, and psychological mindedness are key demographic and psychological factors impacting students. Targeting psychological flexibility and psychological mindedness may enable timely prevention and intervention programs for our students to support their mental and physical health as we move through, and out of, the pandemic.
COVID-19 has affected Malaysia in multitude aspects. Although Malaysia is on the right track to flatten the curve attributed to swift and decisive actions by the government, it is important to assess ...the psychological after effect that caused by the pandemic and the movement control order. This study performed in Borneo, Malaysia, was conducted using principles of snowballing, and by invitation through university students and staffs mailing list to participate. The survey collected data on sociodemographic, along with measuring psychological impact by using the newly validated Malay version Fear of COVID-19 Scale and DASS-21 scale. There were a total of 255 respondents. The majority of the respondents’ age was 25 years and below, with a female male ratio of 2:1 and around 70% respondents were students. Below 25-year-old age group, and females are having significantly higher levels of fear of COVID-19, depression, anxiety, and stress. Single people have higher depression level. There was a significant difference between students and non-students for depression, anxiety, and stress, but not for fear of COVID-19. It is imperative to allay the fears and psychological sequalae of COVID-19, especially those individuals at higher risks. Psychological interventions as well as telepsychiatry mobilisation could prove useful in dealing with the distress.

Apart from the worry of contracting or spreading COVID-19, Malaysian university ...students were also locked down on their campuses, suffering significant psychological distress. Hence, an online mindfulness intervention was proposed to alleviate psychological distress and improve psychological flexibility and mindfulness.
This was a quasi-experimental study with university students as participants. Intervention group participants were instructed to complete online questionnaires which covered basic demographics and instruments assessing depression, anxiety, stress, mindfulness, psychological flexibility, and fear of COVID-19 before and after the one-hour intervention. The control group also completed before and after questionnaires and were subsequently crossed over to the intervention group. Repeated measures ANOVA was conducted to assess time*group effects.
118 participants were involved in this study. There were significant differences in anxiety (F(1,116) = 34.361, p < 0.001, partial eta-squared = 0.229) and psychological flexibility between the two groups (F(1,116) = 11.010, p = 0.001, partial eta-squared = 0.087), while there were no differences in depression, stress, mindfulness, or fear of COVID-19.
The results of this study corroborate the efficacy of online single-session mindfulness therapy as a viable short-term psychological intervention under financial and time constraints. Since university students are in the age group with the highest incidence of depressive and anxiety disorders, it is crucial to utilize resources to address as many students as possible to ensure maximum benefit.
The COVID-19 pandemic has resulted in multiple physical and psychological stressors, which require quantification and establishment of association with other psychological process variables. The ...Coronavirus Stress Measure (CSM) is a validated instrument with acceptable validity and reliability. This study aimed to examine the psychometric properties of the CSM in a Malaysian population. University participants were recruited via convenience sampling using snowball methods. The reliability and validity of the Malay CSM (CSM-M) were rigorously evaluated, utilising both confirmatory factor analysis and Rasch analysis, in relation to sociodemographic variables and response to the depression, anxiety, and stress subscales of the Malay validation of the DASS-21, and also perceived stress (measured by the PSS) and psychological flexibility (AAQ-II). The sample comprised of 247 Malaysian participants. The McDonald’s omega value for the Malay CSM was 0.935 indicating very good internal reliability. The CSM was significantly correlated with stress, anxiety, depression, perceived stress, and psychological flexibility. The Malay CSM properties were examined also with Rasch analysis, with satisfactory outcomes. There was positive correlated error between items 1 and 3, as well as negative correlated error between items 1 and 4. Hence, item 1 was excluded, leaving with 4 items. Confirmatory factor analysis demonstrated good data-model fit, and model fit statistics confirmed that Malay CSM showed a single-factor model. The Malay CSM hence demonstrates good validity and reliability, with both classical and modern psychometric methods demonstrating robust outcomes. It is therefore crucial in operational and research settings in establishing the true extent of stress levels as a consequence of the COVID-19 pandemic.
The COVID-19 crisis has had repercussions on global mental wellbeing. This study aimed: (1) to identify the mediating role of psychological process variables, namely psychological mindedness, ...psychological mindfulness, and psychological inflexibility on the relationship between dysfunctional coping and psychopathologies in Indonesian undergraduate students subjected to national quarantine orders throughout July, 2020 and (2) to compare the level of anxiety, depression, and anxiety between Indonesian and Malaysian undergraduate students. A cross-sectional study was performed with 869 Indonesian undergraduate students from Nahdlatul Ulama University of Surabaya (UNUSA) and 515 undergraduate students from Universiti Malaysia Sabah (UMS). The BIPM, MAAS, AAQ-I, DASS-21, and Brief COPE were used to assess the research variables. The proportion who scored “moderate” and above for depression, anxiety, and stress were 20.2%, 25.0%, and 14.2%, respectively, in Malaysian samples and 22.2%, 35.0%, and 23.48% in Indonesian samples. In Study 1, psychological mindedness, psychological mindfulness, and psychological inflexibility significantly mediated the relationship between dysfunctional coping and psychopathologies. In Study 2, Indonesians demonstrated significantly higher anxiety and stress compared to Malaysian samples. Despite the contrasting COVID-19 situations in Malaysia and Indonesia, psychopathologies were more affected in Indonesia. Hence, our study suggests how crucial it is for mental health providers to consider promoting psychological mindedness, psychological mindfulness, and psychological flexibility to alleviate the corresponding psychopathologies among undergraduate students.
Introduction and Methods: A man diagnosed with sarcoidosis developed partial retrograde amnesia followed by three months of depressive symptoms with panic attacks, which remitted with an ...antidepressant and a benzodiazepine. However, three months later hallucinations and overvalued persecutory ideations subsequently emerged, only to show partial response to antipsychotic treatment. Results: Sarcoidosis was treated with corticosteroids but serial thorax imaging shows minimal response. Corticosteroid was onboard throughout treatment, corresponding with non-remission of psychotic symptoms. There are persistent subjective cognitive difficulties but Mini Mental State Exam score is full. He is currently being treated with escitalopram, alprazolam and quetiapine. Conclusion: This case illustrated the dilemma between prolonging the corticosteroid therapy for sarcoidosis treatment and the psychiatric sequalae. While symptomatic antipsychotic treatment is effective, multi-disciplinary approach is imperative in determining the duration of the corticosteroid therapy.
An eating disorder presenting simultaneously in identical twins remains a rarity in the literature. In this case report, we describe monozygotic twins who presented simultaneously with anorexia ...nervosa as a psychological effect of COVID-19 home quarantine. Both twins were previously well but developed food restriction, exercise, and mutual surveillance behaviours in competition with each other over three months in early 2020. There were no features of other mood, anxiety or psychotic disorders, and there was no organic disease. However, a strong family history of anxiety disorders and highly permeable familial boundaries were identified. Non-pharmacological management modalities including family therapy and psychotherapy were implemented, and tailored to the twins. We discuss the unique genetic and psychological interactions that conceivably explain the aetiology of this simultaneous presentation of an eating disorder in a pair of twins and examine a simple psychodynamic formulation of this simultaneous emergence. Lastly, we look at the sequelae of COVID-19 related quarantines and isolation as potentiators of eating disorders in adolescents whose education and potential employment were drastically curtailed by movement restrictions.
INTRODUCTION: Psychological mindedness concept is widely accepted to be significantly correlated to psychological strength and mental health. There are multiple scales that attempted to measure ...psychological mindedness objectively, with the most recent one being Balanced Index of Psychological Mindedness (BIPM). Psychological mindedness is an area of interest in psychiatry and psychology in Malaysia. Hence, this study aims to validate the Bahasa Malaysia Version of Balanced Index of Psychological Mindedness (BIPM-M). MATERIALS AND METHODS: The 14-items scale was translated according to World Health Organization guidelines. 141 participants were recruited from the university population. Participants were required to fill the basic demographic information, as well as Bahasa Malaysia version of Balanced Index of Psychological Mindedness, and two other scales, namely Mindful Awareness Attention Scale (MAAS), and Acceptance and Action Questionnaire-II Scale (AAQ-II). RESULTS: BIPM-M showed excellent internal consistency with Cronbach α of 0.87. As per the original BIPM, the Bahasa Malaysia version also suggested two factor structures in this scale. BIPM-M was significantly correlated with AAQ-II. Conclusion: BIPM-M is a psychometrically sound instrument that can be utilized to assess the psychological mindedness among Malaysians, and certainly enhance the array of psychological assessments available as Malaysia faces the mental health after-effect of COVID-19.
