The current study aimed to evaluate factors affecting the long-term stability of microimplants using removal torque and the correlation between removal torque and clinical variables.
...Materials and methods
This research evaluated 703 microimplants placed in 354 patients (mean age: 30.4 ± 12.1 years). The removal torque was evaluated according to various clinical variables including sex, age, placement site, microimplant size, and placement method (self-drilling versus pre-drilling). Pearson correlation and stepwise multiple linear regression analyses were performed to investigate different variables and their association with removal torque.
Results
The mean removal torque was significantly higher in the mandible (4.46 N cm) than in the maxilla (3.73 N cm). The values in the posterior teeth/retromolar areas were significantly higher than those in the anterior teeth area. There were no significant difference in terms of sex. Teenagers had a lower removal torque than older adults in the mandible, but not in the maxilla. Microimplants with a greater length and diameter, except for those with a greater diameter in the maxilla, was associated with a higher removal torque. Regardless of placement torque, the removal torque convergently reached approximately 4 N cm in both placement methods. The removal torque was significantly correlated with screw length in the self-drilling group and with diameter in the pre-drilling group.
Conclusions
Removal torque was related with placement site, age, placement method, and length and diameter of microimplants.
Niemann–Pick disease type C (NPC) is a rare, autosomal recessive, lysosomal storage disease, resulting from mutations in the cholesterol trafficking proteins NPC1 or NPC2, which is characterized by ...progressive neurodegeneration and hepatic dysfunction. The hepatic involvement in NPC is usually neonatal cholestasis and hepatosplenomegaly. Only a few cases of severe hepatic complications were reported including acute liver failure, cirrhosis, and hepatocellular carcinoma (HCC). We described the case of a 6‐year‐old male with NPC with HCC. He had a history of neonatal cholestasis and motor delay. At the age of 6 months, he was diagnosed with NPC, which was confirmed by the detection of a compound heterozygous NPC1 mutation (p.C113Y/p.A927V). He presented recurrent hypoglycemia and abdominal distension. An ultrasound, computed tomography scan, and biopsy revealed that he had a stage IV HCC with pulmonary metastasis. With the literature review and this case, HCC can be a rare fatal comorbid condition in patients with NPC, particularly infantile‐onset, male patients with a relatively long disease history, necessitating appropriate HCC surveillance.
Recent studies have suggested that breathing type may affect balance ability. However, most of these studies were conducted on the elderly and patients with musculoskeletal or neurological disorders. ...Therefore, the effect of voluntary breathing, such as thoracic and abdominal breathing, on the balance ability of people in various age groups is not clearly understood. The purpose of this study was to investigate the differences in balance ability according to the type of breathing in healthy young adults. This study included 78 healthy, young adults. All subjects were assessed for balance ability in neutral breathing, thoracic breathing, and abdominal breathing through a crossover design. Balance ability was assessed during static standing using a force plate. Participants were trained in voluntary breathing, evaluated using electromyography. During voluntary breathing, sway velocity, anterior-posterior difference, and anterior-posterior standard deviation increased while anterior-posterior sample entropy decreased compared to neutral breathing (
< 0.05). Compared with thoracic breathing, abdominal breathing increased sway velocity and variability, and reduced complexity (
< 0.05). These findings show that balance ability is affected by breathing, even in healthy young adults.
A large body of evidence suggests that B-cell lymphomas with enhanced Myc expression are associated with an aggressive phenotype and poor prognosis, which makes Myc a compelling therapeutic target. ...Phosphodiesterase 4B (PDE4B), a main hydrolyzer of cyclic AMP (cAMP) in B cells, was shown to be involved in cell survival and drug resistance in diffuse large B cell lymphomas (DLBCL). However, the interrelationship between Myc and PDE4B remains unclear. Here, we first demonstrate the presence of the Myc-PDE4B feed-forward loop, in which Myc and PDE4B mutually reinforce the expression of each other. Next, the combined targeting of Myc and PDE4 synergistically prevented the proliferation and survival of B lymphoma cells in vitro and in a mouse xenograft model. We finally recapitulated this combinatorial effect in Eμ-myc transgenic mice; co-inhibition of Myc and PDE4 suppressed lymphomagenesis and restored B cell development to the wild type level that was associated with marked reduction in Myc levels, unveiling the critical role of the Myc-PDE4B amplification loop in the regulation of Myc expression and the pathogenesis of B cell lymphoma. These findings suggest that the disruption of the Myc-PDE4B circuitry can be exploited in the treatment of B cell malignancies.
Abstract
Background
Based on the controversy surrounding pulmonary artery catheterization (PAC) in surgical patients, we investigated the interchangeability of cardiac index (CI) and systemic ...vascular resistance (SVR) measurements between ClearSight™ and PAC during living-donor liver transplantation (LDLT).
Methods
This prospective study included consecutively selected LDLT patients. ClearSight™-based CI and SVR measurements were compared with those from PAC at seven LDLT-stage time points. ClearSight™-based systolic (SAP), mean (MAP), and diastolic (DAP) arterial pressures were also compared with those from femoral arterial catheterization (FAC). For the comparison and analysis of ClearSight™ and the reference method, Bland-Altman analysis was used to analyze accuracy while polar and four-quadrant plots were used to analyze the trending ability.
Results
From 27 patients, 189 pairs of ClearSight™ and reference values were analyzed. The CI and SVR performance errors (PEs) exhibited poor accuracy between the two methods (51.52 and 51.73%, respectively) in the Bland-Altman analysis. CI and SVR also exhibited unacceptable trending abilities in both the polar and four-quadrant plot analyses. SAP, MAP, and DAP PEs between the two methods displayed favorable accuracy (24.28, 21.18, and 26.26%, respectively). SAP and MAP exhibited acceptable trending ability in the four-quadrant plot between the two methods, but not in the polar plot analyses.
Conclusions
During LDLT, CI and SVR demonstrated poor interchangeability, while SAP and MAP exhibited acceptable interchangeability between ClearSight™ and FAC.
To investigate the difference in labial and palatal alveolar bone thickness and height during the retention period after incisor retraction treatment with microimplant.
A sample of 21 patients (mean ...age: 17.80 ± 4.38 years) who underwent incisor retraction treatment using microimplants after premolar extraction was investigated. The cone-beam computed tomography images at pretreatment, posttreatment, and retention were used to measure anterior alveolar bone thickness (labial, palatal, and total; at three vertical levels) and height (labial and palatal) and differences in the incisor position during treatment or retention. Repeated-measures analysis of variance with Bonferroni correction was performed to compare the variables at T0, T1, and T2.
The maxillary central incisor moved posteriorly by approximately 8.0 mm along with intrusive movement of 1.8 mm after treatment. The alveolar bone thickness significantly decreased on the palatal side and increased on the labial side after treatment. Thereafter, the palatal bone thickness significantly increased and labial bone thickness decreased during the retention period. The palatal interdental bone depressed by incisor retraction showed substantial bone deposition after retention.
Radiographic palatal bone dehiscences on the incisor root and palatal bone depression between the incisor roots were apparent after treatment. This palatal bone loss around the incisor roots noticeably recovered with newly formed bone during retention.
We aimed to compare seroprevalence of anti-myelin oligodendrocyte glycoprotein (MOG) and anti-aquaporin-4 (AQP4) antibodies in Korean adults with inflammatory demyelinating diseases (IDDs) of the ...central nervous system (CNS), based on a multicenter nationwide database. Sera were analyzed using a live cell–based assay for MOG and AQP4 antibodies. Of 586 Korean adults with IDDs of the CNS, 36 (6.1%) and 185 (31.6%) tested positive for MOG and AQP4 antibodies, respectively. No participant showed double positivity. Seroprevalence of MOG antibodies was about five times lower than that of AQP4 antibodies in a large cohort of Korean adults with IDDs of the CNS.
Background and objectives
There is a paucity of evidence on the value of intraperitoneal chemotherapy (IPC) following cytoreductive surgery (CRS) for colorectal peritoneal metastasis. This study ...aimed to evaluate the association between mitomycin C‐IPC and survival outcomes following CRS.
Methods
The institutional databases of two tertiary hospitals were reviewed to identify patients who underwent CRS for colorectal peritoneal metastasis. The outcomes of patients who underwent CRS without IPC were compared with those of patients who underwent CRS plus early postoperative intraperitoneal chemotherapy (EPIC) or CRS plus hyperthermic intraperitoneal chemotherapy (HIPEC). The primary endpoints were cancer‐specific survival (CSS), progression‐free survival (PFS), and peritoneal PFS (P‐PFS).
Results
In 149 patients with peritoneal metastasis alone, EPIC and HIPEC use was significantly associated with better CSS, PFS, and P‐PFS in the multivariate analysis. CSS was also significantly associated with perioperative systemic chemotherapy. Among 42 patients with both peritoneal and extraperitoneal metastases, CSS was independently related to the completeness of cytoreduction score, location of extraperitoneal metastasis, and grade 3–4 complications.
Conclusions
Mitomycin C‐IPC after CRS was associated with better survival outcomes than CRS alone in patients with resectable peritoneal metastasis of colorectal cancer. This study found that IPC had beneficial effects regarding P‐PFS in patients with both peritoneal and extraperitoneal metastases.
Tremendous efforts have been made to establish effective therapeutic neovascularization using adipose tissue-derived stromal vascular fraction (SVF), but the efficiency is low, and underlying ...mechanisms and their interaction with the host in a new microenvironment are poorly understood.
Here we demonstrate that direct implantation of SVF derived from donor adipose tissue can create a profound vascular network through the disassembly and reassembly of blood endothelial cells at the site of implantation. This neovasculature successfully established connection with recipient blood vessels to form a functionally perfused circuit. Addition of vascular growth factors to the SVF implant improved the efficiency of functional neovasculature formation. In contrast, spheroid culture of SVF before implantation reduced the capacity of vasculature formation, possibly because of cellular alteration. Implanting SVF into the mouse ischemic hindlimb induced the robust formation of a local neovascular network and salvaged the limb. Moreover, the coimplantation of SVF prevented fat absorption in the subcutaneous adipose tissue graft model.
Freshly isolated SVF can effectively induce new vessel formation through the dynamic reassembly of blood endothelial cells and could be applied to achieve therapeutic neovascularization for relieving ischemia and preventing fat absorption in an autologous manner.
The revised International Staging System (R‐ISS) has recently been developed to improve the risk stratification of multiple myeloma (MM) patients over the ISS. We assessed the R‐ISS in MM patients ...who were treated with novel agents as a primary therapy and evaluated its discriminative power and ability to reclassify patients from the ISS. A total of 514 newly diagnosed MM patients treated with novel agents including thalidomide, bortezomib, and lenalidomide as a primary therapy were included in this retrospective analysis. With a median follow‐up duration of 42.3 months (range, 40.5–44.1), the median overall survival (OS) was 61.0 months. There was a significant difference in median OS (not reached, 60.9, and 50.1 months for stages 1, 2, and 3, respectively, P < 0.001) among the three stages of R‐ISS. The C‐statistic was significantly greater for R‐ISS than for ISS (0.769 vs. 0.696, P < 0.001). The event NRI was −0.08 (95% confidence interval CI, −0.18–0.01) and the non‐event NRI was 0.05 (95% CI, −0.03–0.10), resulting in a total NRI of −0.03 (95% CI, −0.14–0.08, P = 0.602). The R‐ISS performs well and has significantly better discriminative power than the ISS in MM patients treated with novel agents as a primary therapy. However, it does not better reclassify patients from the ISS, suggesting that there is still room to improve the staging system. Moreover, new statistical measures for assessing and quantifying the risk prediction of new prognostic models are necessary in future studies.
