Abstract Purpose Grade 4 lymphopenia (G4L) during radiation therapy (RT) is associated with higher rates of distant metastasis and decreased overall survival in a number of malignancies, including ...esophageal cancer (EC). Through a reduction in integral radiation dose, proton RT (PRT) may reduce G4L relative to photon RT (XRT). The purpose of this study was to compare G4L in patients with EC undergoing PRT versus XRT. Methods and materials Patients receiving curative-intent RT and concurrent chemotherapy for EC were identified. Lymphocyte nadir was defined as the lowest lymphocyte count during RT. G4L was defined as absolute lymphocyte count <200/mm3 . Univariate and multivariable logistic regression analyses (MVA) were performed to assess patient and treatment factors associated with lymphopenia. A propensity-matched (PM) cohort was created using logistic regression, including baseline covariates. Results A total of 144 patients met the inclusion criteria. The median age was 66 years (range, 32-85 years). Of these patients, 79 received XRT (27% 3-dimensional chemo-RT and 73% intensity modulated RT) and 65 received PRT (100% pencil-beam scanning). Chemotherapy consisted of weekly carboplatin and paclitaxel (99%). There were no significant differences in baseline characteristics between the groups, except for age (median 4 years older in the PRT cohort). G4L was significantly higher in patients who received XRT versus those who received PRT (56% vs 22%; P < .01). On MVA, XRT (odds ratio OR: 5.13; 95% confidence interval CI, 2.35-11.18; P < .001) and stage III/IV (OR: 4.54; 95% CI, 1.87-11.00; P < .001) were associated with G4L. PM resulted in 50 PRT and 50 XRT patients. In the PM cohort, G4L occurred in 60% of patients who received XRT versus 24% of patients who received PRT. On MVA, XRT (OR: 5.28; 95% CI, 2.14-12.99; P < .001) and stage III/IV (OR: 3.77; 95% CI, 1.26-11.30; P = .02) were associated with G4L. Conclusions XRT was associated with a significantly higher risk of G4L in comparison with PRT. Further work is needed to evaluate a potential association between RT modality and antitumor immunity as well as long-term outcomes.
We report the discovery of two planetary systems around comoving stars: TOI-2076 (TIC 27491137) and TOI-1807 (TIC 180695581). TOI-2076 is a nearby (41.9 pc) multiplanetary system orbiting a young ...(204 ± 50 Myr), bright (K = 7.115 in TIC v8.1) start. TOI-1807 hosts a single transiting planet and is similarly nearby (42.58 pc), similarly young (180 ± 40 Myr ), and bright. Both targets exhibit significant, periodic variability due to starspots, characteristic of their young ages. Using photometric data collected by TESS we identify three transiting planets around TOI-2076 with radii of Rb = 3.3 ± 0.04 R⊕, Rc = 4.4 ± 0.05 R⊕, and Rd = 4.1 ± 0.07 R⊕. Planet TOI-2076b has a period of Pb = 10.356 days. For both TOI-2076c and d, TESS observed only two transits, separated by a 2 yr interval in which no data were collected, preventing a unique period determination. A range of long periods (<17 days) are consistent with the data. We identify a short-period planet around TOI-1807 with a radius of Rb = 1.8 ± 0.04 R⊕ and a period of Pb = 0.549 days. Their close proximity, and bright, cool host stars, and young ages make these planets excellent candidates for follow up. TOI-1807b is one of the best-known small (R < 2 ${R}_{\oplus }$) planets for characterization via eclipse spectroscopy and phase curves with JWST. TOI-1807b is the youngest ultra-short-period planet discovered to date, providing valuable constraints on formation timescales of short-period planets. Given the rarity of young planets, particularly in multiple-planet systems, these planets present an unprecedented opportunity to study and compare exoplanet formation, and young planet atmospheres, at a crucial transition age for formation theory.
Robust environmental decision support tools are critical to maximise the ecological benefit of management decisions. However the models that underpin these rarely undergo rigorous evaluation. Here, ...we evaluated components of a scenario-based habitat suitability model, the Murray Flow Assessment Tool, by correlating model outputs against fish monitoring data collected since its development. Overall, we detected a low correlation between habitat suitability scores for fish and fish assemblages during low-flow conditions, including when lags in fish response to hydrological inputs were introduced. Scores specific to fish functional groups were also poorly correlated with data for those groups. Finally, model outcomes were highly sensitive to methods used to combine both individual indices and weightings for each component. Thus, we recommend using constant weightings, simple and consistent combination methods and reconsidering the number of fish functional groups as simplifications to this model and in the development of similar habitat suitability models elsewhere.
•We evaluated a widely-used ecological model to predict fish assemblages.•Model scores were poorly correlated with fish data collected from the same basin.•Functional-group specific scores did not correspond with fish composition.•Scores were highly sensitive to weighting and preference-curve combination methods.•There is a need for ongoing model evaluation and refinement as new data are available.
Sjögren's syndrome is a common autoimmune disease (affecting ∼0.7% of European Americans) that typically presents as keratoconjunctivitis sicca and xerostomia. Here we report results of a large-scale ...association study of Sjögren's syndrome. In addition to strong association within the human leukocyte antigen (HLA) region at 6p21 (Pmeta = 7.65 × 10(-114)), we establish associations with IRF5-TNPO3 (Pmeta = 2.73 × 10(-19)), STAT4 (Pmeta = 6.80 × 10(-15)), IL12A (Pmeta = 1.17 × 10(-10)), FAM167A-BLK (Pmeta = 4.97 × 10(-10)), DDX6-CXCR5 (Pmeta = 1.10 × 10(-8)) and TNIP1 (Pmeta = 3.30 × 10(-8)). We also observed suggestive associations (Pmeta < 5 × 10(-5)) with variants in 29 other regions, including TNFAIP3, PTTG1, PRDM1, DGKQ, FCGR2A, IRAK1BP1, ITSN2 and PHIP, among others. These results highlight the importance of genes that are involved in both innate and adaptive immunity in Sjögren's syndrome.

NASA’s Transiting Exoplanet Survey Satellite (TESS) is an all-sky survey mission designed to find transiting exoplanets orbiting nearby bright stars. It has identified more than 329 ...transiting exoplanets, and almost 6000 candidates remain unvalidated. In this manuscript, we discuss the findings from the ongoing Validation of Transiting Exoplanets using Statistical Tools (VaTEST) project, which aims to validate new exoplanets for further characterization. We validated 11 new exoplanets by examining the light curves of 24 candidates using the
LATTE
and
TESS-Plot
tools and computing the false-positive probabilities using the statistical validation tool
TRICERATOPS
. These include planets suitable for atmospheric characterization using transmission spectroscopy (TOI-2194b), emission spectroscopy (TOI-3082b and TOI-5704b) and for both transmission and emission spectroscopy (TOI-672b, TOI-1694b, and TOI-2443b). Our validated planets have one super-Earth (TOI-2194b) orbiting a bright (
V
= 8.42 mag), metal-poor (Fe/H = −0.3720 ± 0.1) star, and one short-period Neptune-like planet (TOI-5704) in the hot-Neptune desert. In total, we validated one super-Earth, seven sub-Neptunes, one Neptune-like, and two sub-Saturn or super-Neptune-like exoplanets. Additionally, we identify five likely planet candidates (TOI-323, TOI-1180, TOI-2200, TOI-2408, and TOI-3913), which can be further studied to establish their planetary nature.
The corona virus disease (COVID-19) pandemic disrupted daily life worldwide, and its impact on child well-being remains a major concern. Neighborhood characteristics affect child well-being, but how ...these associations were affected by the pandemic is not well understood. We analyzed data from 1039 children enrolled in the Environmental influences on Child Health Outcomes Program whose well-being was assessed using the Patient-Reported Outcomes Measurement Information System Global Health questionnaire and linked these data to American Community Survey (ACS) data to evaluate the impacts of neighborhood characteristics on child well-being before and during the pandemic. We estimated the associations between more than 400 ACS variables and child well-being t-scores stratified by race/ethnicity (non-Hispanic white vs. all other races and ethnicities) and the timing of outcome data assessment (pre-vs. during the pandemic). Network graphs were used to visualize the associations between ACS variables and child well-being t-scores. The number of ACS variables associated with well-being t-scores decreased during the pandemic period. Comparing non-Hispanic white with other racial/ethnic groups during the pandemic, different ACS variables were associated with child well-being. Multiple ACS variables representing census tract-level housing conditions and neighborhood racial composition were associated with lower well-being t-scores among non-Hispanic white children during the pandemic, while higher percentage of Hispanic residents and higher percentage of adults working as essential workers in census tracts were associated with lower well-being t-scores among non-white children during the same study period. Our study provides insights into the associations between neighborhood characteristics and child well-being, and how the COVID-19 pandemic affected this relationship.

Disease can be a powerful driver of population and community dynamics, as well as evolutionary processes. Disease is also emerging at increasing rates, resulting in massive impacts on populations, ...communities, and ecosystems. However, assessing these impacts requires foundational knowledge of disease agents and hosts, which is often lacking, particularly in aquatic insects.
We describe a recent disease outbreak in caddisflies, suggesting potential consequences for population and community dynamics of the host. We use a series of complementary studies to develop a cohesive foundation of information about this disease, including identification using genomic methods, observational prevalence studies, laboratory experiments to establish transmissibility and fitness consequences, and laboratory and field investigations to infer transmission mechanisms.
We identified the infection as being caused by the oomycete Saprolegnia—the first time this parasite has been noted in insect eggs. Prevalence surveys found high prevalence (up to 36%) with variation across space, time, and host species. We demonstrated increasing egg mortality with increasing infection within an egg mass (every 10% increase in infection rate doubles odds of mortality), thereby confirming disease. We established transmissibility and show that transmission occurs through both direct contact with infected egg masses and from background sources, which probably interact to create complex patterns of disease.
Taken together, our findings show that conditions necessary for population and community consequences are present. Specifically, increased mortality rates almost certainly occurred during the outbreak, yielding lower larval numbers and potentially altering community interactions. Transmission by contact between egg masses combined with observed species‐specific prevalence suggest shifts in the relative performance of different species because of interactions between host and parasite life histories.
Outside extreme examples such as chytrid fungus, disease has traditionally received less interest than resource competition or predation in community ecology, although disease ecology is advancing rapidly. One major hurdle is a lack of foundational knowledge characterising disease processes in natural communities, particularly in aquatic insects. Our findings highlight the importance of investigating diseases in insect eggs and provide the foundation for further investigations of how these processes play out at the population and community scale.
Chronic homelessness is a problem characterised by longstanding inability to attain or maintain secure accommodation. Longitudinal research with homeless populations is challenging, and randomised ...controlled trials that evaluate the effectiveness of intensive, case management interventions aimed at improving housing and health-related outcomes for chronically homelessness people are scant. More research is needed to inform programmatic design and policy frameworks in this area. This study protocol details an evaluation of the Journey to Social Inclusion - Phase 2 program, an intervention designed to reduce homelessness and improve outcomes in chronically homeless adults.
J2SI Phase 2 is a three-year, mixed methods, multi-site, RCT that enrolled 186 participants aged 25 to 50 years between 07 January 2016 and 30 September 2016 in Melbourne. The intervention group (n = 90 recruited) receives the J2SI Phase 2 program, a trauma-informed intervention that integrates intensive case management and service coordination; transition to housing and support to sustain tenancy; and support to build social connections, obtain employment and foster independence. The comparison group (n = 96 recruited) receives standard service provision. Prior to randomisation, participants completed a baseline survey. Follow-up surveys will be completed every six months for three years (six in total). In addition to self-report data on history of homelessness and housing, physical and mental health, substance use, quality of life, social connectedness and public service utilisation, linked administrative data on participants' public services utilisation (e.g., hospitalisation, justice system) will be obtained for the three-year period pre- and post-randomisation. Semi-structured, qualitative interviews will be conducted with a randomly selected subset of participants and service providers at three time-points to explore changes in key outcome variables and to examine individual experiences with the intervention and standard service provision. An economic evaluation of the intervention and associated costs will also be undertaken.
Results of this trial will provide robust evidence on the effectiveness of J2SI Phase 2 compared to standard service provision. If the intervention demonstrates effectiveness in improving housing, health, quality-of-life, and other social outcomes, it may be considered for broader national and international dissemination to improve outcomes among chronically homeless adults.

Suicide is a major public health problem around the world, with an estimated 800,000 deaths from suicide each year. Research suggests that the ever-growing use of the Internet is contributing to and ...facilitating suicide. A thematic analysis was conducted on the final communications of three individuals aged between 18 and 21 to obtain an understanding of why people commit suicide online, how the Internet is involved, and how individuals who are online can influence a person’s choice of life or death. The analysis found that all three suicides were encouraged and baited by their audience, often as the audience doubted the legitimacy of the events, which resulted in pride issues for the victim. The issue of excitement from the audience was also present in all three cases, suggesting that online communications feel more like entertainment than real life. Other themes include social disorders, hesitation, manipulation, and attention seeking behaviour related to low self-esteem. The finding showed that those who die online by suicide have moments of hesitation, which are relevant for suicide-prevention strategies. The limitations of this study in terms of generalisability are discussed, and recommendations for the future of public awareness projects and research are given.
