This research aims to determine whether the combination of epidemiological and clinical features can predict malaria. Diagnostic investigation detected 22.3% of individuals with Plasmodium vivax (P. ...vivax) malaria, with significant predominance of the male gender. The malaria triad (fever, chills and headache) had a more expressive frequency (81.1%) in individuals with positive thick blood than those with negative thick blood smear (65.1%), although there was no statistical significance. Among the variables analysed as predictive for positive thick blood smear, it was observed that personal history of travel to an endemic malaria area and past malaria infection (PMI) were significantly associated with malaria, even in multiple logistic regression. Fever had the higher sensitivity (94.6%) and past malaria history had the greater specificity (68.2%), with accuracy of 23.5% and 67.5%, respectively. In combined analysis, fever with chills had the highest sensitivity (91.9%), but low accuracy (38.5%). High specificity (91.5%) was found in the association of malaria triad, PMI and history of travel to endemic malaria area (which along with anorexia, was higher 94.6%), with good accuracy (80.7%), suggesting that the screening of patients for performing thick blood smear can be based on these data. The epidemiological features and the malaria triad (fever, chills and headache) can be predictors for identification of malaria patients, concurring to precocious diagnosis and immediate treatment of individuals with malaria.
Introdução/Objetivo: A Síndrome pós-COVID-19 trata-se de uma doença complexa com sintomas heterogêneos que podem persistir ou aparecer após a fase aguda da infecção. Entre as alterações induzidas ...nessa síndrome, está a disfunção metabólica, evidenciada através do desiquilíbrio de alguns parâmetros laboratoriais como marcadores lipídicos e glicêmicos. Tais alterações podem contribuir para o agravo no estado nutricional, colaborando com a comprometimento da qualidade de vida desses indivíduos. Dessa forma, o objetivo desse trabalho é analisar o estado nutricional e alteração metabólica em pacientes com Síndrome pós-COVID-19. Métodos: O estudo foi constituído por adultos, atendidos no Núcleo de Medicina Tropical da Universidade Federal do Pará, residentes na região metropolitana de Belém, que tenha tido COVID-19 diagnosticada por um teste laboratorial positivo e que estivessem a 30 dias ou mais tempo recuperados após a fase ativa da doença. A coleta de dados foi realizada no período de março de 2022 a abril de 2023, com aplicação de questionário sociodemográfico estruturado com questões relacionadas a saúde, realização de exame bioquímico contemplando as concentrações lipídicas, glicemia de jejum e investigação do estado nutricional. Para as análises dos dados coletados utilizou-se o Excel 2010 para estatística descritiva. Resultados: A população de estudo totalizou 156 pacientes, a faixa etária mais recorrente foi de 50 a 59 anos (34%), a população foi predominantemente composta pelo sexo feminino (82%), em que houve a maior frequência de indivíduos com ensino médio completo (40%) e renda prevalente foi de 1 a 2 salários-mínimos (44%). Referente ao estado nutricional o índice de massa corporal predominante foi o de pacientes com excesso de peso (72%). Em relação a razão cintura-estatura (82%) dos pacientes obtiveram risco aumentado para doenças cardiometabólicas. Quanto aos exames bioquímicos foi observado níveis aumentados de glicemia (66%) e de triglicerídeos (51%) entre os participantes do estudo. Conclusão: Muitos pacientes mesmo após a cura da COVID-19 continuaram a apresentar anormalidade no perfil metabólico, como alteração glicídica, lipídicas e excesso de peso, o que dificulta a reabilitação desses indivíduos. Assim, tais achados reforçam a importância de um acompanhamento nutricional e endócrino nos pacientes após a cura da COVID-19, com intuito de controlar, reverter essas alterações e contribuir para melhora na qualidade de vida.
Introdução/Objetivos: A Síndrome Pós-COVID-19 tem sido comumente relatada, os sintomas podem aparecer mesmo um ano pós a infecção pelo SARS-CoV-2. As características da Síndrome Pós-COVID-19 se ...diferenciam de outras infecções, pois apresentam sintomas multissitêmicos, complicações e sequelas a longo prazo. Dentre as sequelas a perda de memória está entre os sintomas mais prevalentes. Sendo assim, o objetivo desse estudo é avaliar a perda de memória em pacientes com Síndrome Pós-COVID-19 através de ferramentas de rastreio cognitivo. Métodos: Participaram do estudo um total de 160 pacientes com Síndrome Pós-COVID-19, cadastrados e atendidos no Núcleo de Medicina Tropical, no período de março de 2022 a maio 2023, que relataram queixa de perda de memória. Primeiramente foi aplicado o questionário do Miniexame do estado mental (MEEN), e posteriormanete o Consortium to Establish a Registry for Alzheimer's Disease (CERAD), ambos para rastreio cognitivo (perda de memória). Os dados obtidos foram inseridos em planilhas e analisados pelo programa Microsoft Excel 2010 para a análise descritiva. Resultados: Do total de 160 avaliações, 124 (78%) eram mulheres com idade que varia entre 21 a 77 anos e 36 (23%) homens com idade 19 a 82 anos. Em relação a autodeclaração de cor, a mais frequente com 95 (59%) eram pardos, 47 (29%) brancos e 18 (11%) pretos. No que se refere ao grau de escolaridade, a maior parte 61 (38%) possuía o ensino médio completo. Em relação ao teste cognitivo do MEEN 128 (80%) dos pacientes foram classificados sem perda cognitiva (escore > 25), 27 (17%) apresentaram perda cognitiva leve (escore entre 21 a 24) e 5 (3%) moderada (escore 10 a 20). Referente a avaliação do CERAD, 116 (73%) apresentaram normalidade nos parâmetros do teste (escore > 25) e 44 (28%) ficaram abaixo da normalidade (escore < 14). Conclusão: Embora as investigações com os testes para avaliar a função cognitiva tenham apresentado normalidade na maioria dos pacientes, observou-se que houve casos leves e moderados sugestivos de comprometimento de perda de memória pelo MEEN. Assim como, na avaliação do CERAD que também demonstrou alteração no comprometimento da perda da memória. O que demostra a importância de mais investigações acerca do assunto, contribuindo para melhora na qualidade de vida e prevenção dos agravos das sequelas da Síndrome Pós-COVID-19.
Some studies have suggested the importance of vitamin D 25(OH)D in malaria clinical practice. The prevalence of 25(OH)D deficiency in the Amazon population is not well known, and there are few ...studies in patients with malaria. This study aimed to evaluate 25(OH)D serum levels in patients with malaria and determine their relationships with epidemiological, clinical, laboratory, and parasitemia data.
An analytical cross-sectional study of 123 patients with malaria and 122 individuals without malaria was performed in Itaituba, Pará, Brazil, from January 2018 to October 2019, by evaluating sociodemographic, clinical-epidemiological, parasitological, and laboratory data and adopting a 5% significance level. Parametric tests (Student's t-test), non-parametric tests (Mann-Whitney U), and Spearman's correlation (rs, for non-parametric variables) were used according to the nature of the distribution of the variables. For the qualitative variables, Pearson's chi-square test, Fisher's exact test, and the G test were used. Spearman's correlation was used to compare the results of the 25(OH)D levels and blood counts performed among patients and the control group.
Malaria patients residing in a mining area had 25(OH)D serum levels that were significantly lower than those in the control group residing in the mining area, though both were within normal levels. Red blood cell counts had an inverse correlation with parasitemia (Plasmodium falciparum), and platelet levels had an inverse correlation with parasitemia (Plasmodium vivax). 25(OH)D deficiency was evidenced in Itaituba, in the state of Pará, which is an endemic area of malaria in the Amazon region.

Children and adolescents are at great risk for developing iron deficiency anaemia worldwide. In the tropical areas, malaria and intestinal parasites may also play an important role in anaemia ...pathogenesis. This study aimed at evaluating clinical and immunological aspects of anaemia in children and adolescents with Plasmodium vivax malaria, in the Pará State, Brazil. A longitudinal study was performed in two Reference Centers for malaria diagnosis in the Brazilian Amazon in children and adolescents with malaria (n = 81), as compared to a control group (n = 40). Patients had blood drawn three times before treatment (D0), after treatment (D7) and at the first cure control (D30) and hemogram, autoantibody analysis (anticardiolipin, antibodies against normal RBC membrane components) and cytokine studies (TNF and IL-10) were performed. Stool samples were collected for a parasitological examination. Malaria patients had a 2.7-fold greater chance of anaemia than the control group. At D0, 66.1% of the patients had mild anaemia, 30.5% had moderate and 3.5% had severe anaemia. Positivity to intestinal helminths and/or protozoa at stool examinations had no influence on anaemia. Patients had significantly lower levels of plasmatic TNF than control individuals at D0. Low TNF levels were more prevalent among patients with moderate/severe anaemia than in those with mild anaemia and among anaemic patients than in anaemic controls. TNF levels were positively correlated with the haemoglobin rates and negatively correlated with the interval time elapsed between the onset of symptoms and diagnosis. Both plasma TNF levels and haemoglobin rates increased during the follow-up period. The IL-10 levels were lower in patients than in the controls at day 0 and decreased thereafter up to the end of treatment. Only the anti-anticardiolipin autoantibodies were associated with moderate/severe anaemia and, possibly by reacting with the parasite glycosylphosphatidylinositol (a powerful stimulator of TNF production), may have indirectly contributed to decrease the TNF levels, which could be involved in the malarial vivax anaemia of these children and adolescents. More studies addressing this issue are necessary to confirm these findings and to add more information on the multifactorial pathogenesis of the malarial anaemia.
