The incidence of tuberculosis (TB) is currently increasing in HIV-infected patients living in Africa and Asia, where TB endemicity is high, reflecting the susceptibility of this group of patients to ...mycobacteria belonging to the TB group. In this population, extension of multiple resistance to anti-tuberculous drugs is also a matter of anxiety. HIV-induced immunosuppression modifies the clinical presentation of TB, resulting in atypical signs and symptoms, and more frequent extrapulmonary dissemination. The treatment of TB is also more difficult to manage in HIV-infected patients, particularly with regard to pharmacological interactions secondary to inhibition or induction of cytochrome P450 enzymes by protease inhibitors with rifampicin or rifabutin, respectively. Finally, immune restoration induced by highly active anti-retroviral therapy (HAART) in developed countries may be responsible for a paradoxical worsening of TB manifestations.

Nous rapportons l’observation d’un patient immunocompétent vivant en métropole ayant présenté une occlusion intestinale secondaire à une infestation massive par des ankylostomes compliquée d’une septicémie plurimicrobienne par translocation digestive d’évolution fatale par état de choc septique réfractaire.
Il s’agit d’un cas non rapporté jusqu’alors à notre connaissance d’abdomen aigu chirurgical avec état de choc réfractaire sur septicémie par translocation digestive secondaire à l’atteinte iléale massive à ankylostome.

We report a case of acute surgical abdominal presentation with septicemia and refractory shock syndrome due to ileal translocation secondary to massive hookworm infestation. To the best of our knowledge, such a case has not yet been reported.
L’acidose lactique induite par la metformine (MALA) est définie par un Ph < 7,35 et taux de lactate supérieure à 5 mM. Elle doit être distinguée des autres acidoses lactiques habituellement ...secondaires à un choc septique. Le pronostic fonctionnel rénal et de mortalité ne sont pas clairement séparés dans des publications et beaucoup font l’amalgame entre les deux entités avec une mortalité estimée de 30 à 50 %. Les données sur le pronostic rénal sont rares. Nous avons fait une étude observationnelle rétrospective, incluant les patients admis consécutivement dans l’unité de soins intensifs pour MALA entre janvier 2010 et juillet 2015 au centre hospitalier de Saint-Denis : 17 patients, 9 hommes ; âge moyen 60 ans. La cause présumée de MALA était l’insuffisance rénale par déshydratation chez 7, la septicémie chez 7, et une l’intoxication volontaire chez 3. Le pH initial moyen était de 7,05 (extrêmes 6,58 à 7,35), la lactacidémie : 10,33 mM (extrême 5,12 à 15,2), la créatinine était de 551 uM (extrêmes 56–1338), la glycémie : 1,62 g/L (extrêmes 0,29–4,5). Le séjour aux soins intensifs était de 10,2 jours (extrêmes 1–26) ; 2/17 sont décédés de choc septique ; 14/15 ayant initialement une clairance de la créatinine anormale < 60 mL/min, ont retrouvé une fonction rénale normale ; 11/17 patients ont bénéficié d’une dialyse pour prévenir l’hypocalcémie, la surcharge cardiaque, hyperosmolalité et dialyser la metformine. Ainsi, à l’opposé de l’acidose lactique due à d’autres causes, la MALA représente une situation avec un bon pronostic rénale rapide par l’utilisation de la dialyse.

Among 36 cases of Escherichia coli native valve endocarditis (NVE) that met Duke criteria (31 cases in the literature between 1909 and 2002, and five cases seen in Paris, France), the urinary tract ...was the most common portal of entry. The majority (72.2%) of cases developed in elderly females. Overall, the proportion of patients aged > 70 years rose from 5.3% in 1982 to 22.9% in 2002. Persistent E. coli bacteraemia in the elderly in the absence of cardiac risk-factors may be a sign of NVE and should prompt an investigation by echocardiography.
Les encéphalopathies au 5-fluoro-uracile (5FU) sont rares et d’évolution habituellement régressive.
Une femme de 45 ans traitée par une chimiothérapie associant du 5FU et du cisplatine a développé 6 ...jours après la première cure, une encéphalopathie aiguë avec troubles de conscience et crises convulsives. L’imagerie par résonance magnétique a montré des hypersignaux de la substance blanche à prédominance ventriculaire. L’évolution en réanimation a été rapidement défavorable malgré un traitement symptomatique et la perfusion de thiamine. La toxicité du 5FU a été confirmée par un dosage sanguin élevé.
Les encéphalopathies au 5FU sont rares et le plus souvent d’évolution favorable après arrêt du traitement. Deux mécanismes physiopathologiques sont décrits pour expliquer les encéphalopathies au 5FU: soit un déficit d’activité de la dihydropyrine déhydrogénase entraînant une augmentation du taux d’uracile sanguin avec toxicité digestive et muqueuse associée, soit une interaction des catabolites du 5FU avec diverses voies métaboliques intracérébrales.

Six days after her first course of chemotherapy combining 5FU and cisplatin, a 45-year-old woman developed acute encephalopathy with altered consciousness and convulsions. MRI showed hyperintense signals of the white matter, mainly ventricular. Despite symptomatic treatment and thiamine perfusion in intensive care, the patient deteriorated and resuscitation failed. Blood tests confirmed 5FU toxicity.
5FU encephalopathies are rare and most often resolve favorably after treatment is stopped. Two pathophysiologic mechanisms may explain these encephalopathies: a deficit of dihydropyridine dehydrogenase, leading to an increased level of serum uracil with digestive and mucosal toxicity, or interaction of 5FU catabolites with various intracerebral metabolic pathways.

Retrospective multicentre study between 1996 and 2000 through the French Cryptococcosis Database.
Subsequent occurrence of IRIS examined in 120 HIV-infected adult patients treated with HAART and experiencing a first episode of culture-confirmed cryptococcosis.
Ten patients developed IRIS during the study period, giving an incidence of 10/239, or 4.2/100 person-years 95% confidence interval (CI), 2.2-7.8. IRIS consisted of acute symptoms consistent with inflammation occurring within a median of 8 months (range, 2-37) after the diagnosis of cryptococcosis in the context of negative cultures and immunological and/or virological response to HAART. Radiology and histopathology detected features compatible with inflammation. Symptom severity required transfer into intensive care units for three patients and use of anti-inflammatory drugs for four. Three patients with evolutive IRIS died. Compared with patients without IRIS for whom complete clinical and microbiological information were available at baseline, previously unknown HIV infection odds ratio (OR), 4.8; 95% CI, 1.0-21.7, CD4 cell count < 7 x 10 cells/l (OR, 4.0; 95% CI, 0.9-17.2), fungaemia (OR, 6.1; 95% CI, 1.1-35.2) and HAART initiation within 2 months of cryptococcosis diagnosis (OR, 5.50; 95% CI, 1.0-29.6) were independently associated with the risk of subsequent IRIS.
IRIS-related cryptococcosis was observed more frequently in severely immunocompromised patients with disseminated infection and HAART initiation soon after the diagnosis.
