Context.
In the past few years, there has been a rise in the detection of streamers, asymmetric flows of material directed toward the protostellar disk with material from outside a star’s natal core. ...It is unclear how they affect the process of mass accretion, in particular beyond the Class 0 phase.
Aims.
We investigate the gas kinematics around Per-emb-50, a Class I source in the crowded star-forming region NGC 1333. Our goal is to study how the mass infall proceeds from envelope to disk scales in this source.
Methods.
We use new NOEMA 1.3 mm observations, including C
18
O, H
2
CO, and SO, in the context of the PRODIGE MPG – IRAM program, to probe the core and envelope structures toward Per-emb-50.
Results.
We discover a streamer delivering material toward Per-emb-50 in H
2
CO and C
18
O emission. The streamer’s emission can be well described by the analytic solutions for an infalling parcel of gas along a streamline with conserved angular momentum, both in the image plane and along the line-of-sight velocities. The streamer has a mean infall rate of 1.3 × 10
−6
M
⊙
yr−
1
, five to ten times higher than the current accretion rate of the protostar. SO and SO
2
emission reveal asymmetric infall motions in the inner envelope, additional to the streamer around Per-emb-50. Furthermore, the presence of SO
2
could mark the impact zone of the infalling material.
Conclusions.
The streamer delivers sufficient mass to sustain the protostellar accretion rate and might produce an accretion burst, which would explain the protostar’s high luminosity with respect to other Class I sources. Our results highlight the importance of late infall for protostellar evolution: streamers might provide a significant amount of mass for stellar accretion after the Class 0 phase.
Abstract Background Multicenter longitudinal objective data for survival into adulthood of patients who have undergone Fontan procedures are lacking. Objectives This study sought to describe ...transplant-free survival and explore relationships between laboratory measures of ventricular performance and functional status over time. Methods Exercise testing, echocardiography, B-type natriuretic peptide, functional health assessment, and medical history abstraction were repeated 9.4 ± 0.4 years after the Fontan Cross-Sectional Study (Fontan 1) and compared with previous values. Cox regression analysis explored risk factors for interim death or cardiac transplantation. Results From the original cohort of 546 subjects, 466 were contacted again, and 373 (80%) were enrolled at 21.2 ± 3.5 years of age. Among subjects with paired testing, the percent predicted maximum oxygen uptake decreased (69 ± 14% vs. 61 ± 16%; p < 0.001; n = 95), ejection fraction decreased (58 ± 11% vs. 55 ± 10%; p < 0.001; n = 259), and B-type natriuretic peptide increased (median interquartile range 13 7 to 25 pg/mol vs. 18 9 to 36 pg/mol; p < 0.001; n = 340). At latest follow-up, a lower Pediatric Quality of Life Inventory physical summary score was associated with poorer exercise performance (R2 adjusted = 0.20; p < 0.001; n = 274). Cumulative complications since the Fontan procedure included additional cardiac surgery (32%), catheter intervention (62%), arrhythmia treatment (32%), thrombosis (12%), and protein-losing enteropathy (8%). Since Fontan 1, 54 subjects (10%) have received a heart transplant (n = 23) or died without transplantation (n = 31). The interval risk of death or/cardiac transplantation was associated with poorer ventricular performance and functional health status assessed at Fontan 1, but it was not associated with ventricular morphology, the subject’s age, or the type of Fontan connection. Conclusions Interim transplant-free survival over 12 years in this Fontan cohort was 90% and was independent of ventricular morphology. Exercise performance decreased and was associated with worse functional health status. Future interventions might focus on preserving exercise capacity. ABSTRACT The finding of residual gas in the large central cavity of the HD 142527 disk motivates questions regarding the origin of its non-Keplerian kinematics and possible connections with planet ...formation. We aim to understand the physical structure that underlies the intra-cavity gaseous flows, guided by new molecular-line data in CO(6-5) with unprecedented angular resolutions. Given the warped structure inferred from the identification of scattered-light shadows cast on the outer disk, the kinematics are consistent, to first order, with axisymmetric accretion onto the inner disk occurring at all azimuths. A steady-state accretion profile, fixed at the stellar accretion rate, explains the depth of the cavity as traced in CO isotopologues. The abrupt warp and evidence for near free-fall radial flows in HD 142527 resemble theoretical models for disk tearing, which could be driven by the reported low-mass companion, whose orbit may be contained in the plane of the inner disk. The companion's high inclination with respect to the massive outer disk could drive Kozai oscillations over long timescales; high-eccentricity periods may perhaps account for the large cavity. While shadowing by the tilted disk could imprint an azimuthal modulation in the molecular-line maps, further observations are required to ascertain the significance of azimuthal structure in the density field inside the cavity of HD 142527.
ABSTRACT
Radial substructure in the form of rings and gaps has been shown to be ubiquitous among protoplanetary discs. This could be the case in exo-Kuiper belts as well, and evidence for this is ...emerging. In this paper, we present ALMA observations of the debris/planetesimal disc surrounding HD 206893, a system that also hosts two massive companions at 2 and 11 au. Our observations reveal a disc extending from 30 to 180 au, split by a 27 au wide gap centred at 74 au, and no dust surrounding the reddened brown dwarf (BD) at 11 au. The gap width suggests the presence of a 0.9MJup planet at 74 au, which would be the third companion in this system. Using previous astrometry of the BD, combined with our derived disc orientation as a prior, we were able to better constrain its orbit finding it is likely eccentric ($0.14^{+0.05}_{-0.04}$). For the innermost companion, we used radial velocity, proper motion anomaly, and stability considerations to show its mass and semimajor axis are likely in the ranges 4–100MJup and 1.4–4.5 au. These three companions will interact on secular time-scales and perturb the orbits of planetesimals, stirring the disc and potentially truncating it to its current extent via secular resonances. Finally, the presence of a gap in this system adds to the growing evidence that gaps could be common in wide exo-Kuiper belts. Out of six wide debris discs observed with ALMA with enough resolution, four to five show radial substructure in the form of gaps.
While most of the known debris discs present cold dust at tens of astronomical unit (au), a few young systems exhibit hot dust analogous to the Zodiacal dust. ... Corvi is particularly interesting as ...it is old and it has both, with its hot dust significantly exceeding the maximum luminosity of an in situ collisional cascade. Previous work suggested that this system could be undergoing an event similar to the Late Heavy Bombardment (LHB) soon after or during a dynamical instability. Here, we present ALMA observations of ... Corvi with a resolution of 1.2 arcsec (~22 au) to study its outer belt. The continuum emission is consistent with an axisymmetric belt, with a mean radius of 152 au and radial full width at half-maximum of 46 au, which is too narrow compared to models of inward scattering of an LHB-like scenario. Instead, the hot dust could be explained as material passed inwards in a rather stable planetary configuration. We also report a 4s detection of CO at ~20 au. CO could be released in situ from icy planetesimals being passed in when crossing the H2O or CO2 ice lines. Finally, we place constraints on hidden planets in the disc. If a planet is sculpting the disc's inner edge, this should be orbiting at 75-100 au, with a mass of 3-30 M... and an eccentricity <0.08. Such a planet would be able to clear its chaotic zone on a time-scale shorter than the age of the system and scatter material inwards from the outer belt to the inner regions, thus feeding the hot dust. Objective To investigate the presence and severity of real-world impairments in executive functioning—responsible for children's regulatory skills (metacognition, behavioral regulation)—and its ...potential impact on school performance among pediatric survivors of complex congenital heart disease (CHD). Study design Survivors of complex CHD aged 8-16 years (n = 143) and their parents/guardians from a regional CHD survivor registry participated (81% participation rate). Parents completed proxy measures of executive functioning, school competency, and school-related quality of life (QOL). Patients also completed a measure of school QOL and underwent IQ testing. Patients were categorized into 2 groups based on heart lesion complexity: 2-ventricle or single-ventricle. Results Survivors of complex CHD performed significantly worse than norms for executive functioning, IQ, school competency, and school QOL. Metacognition was more severely affected than behavioral regulation, and metacognitive deficits were more often present in older children. Even after taking into account demographic factors, disease severity, and IQ, metacognition uniquely and strongly predicted poorer school performance. In exploratory analyses, patients with single-ventricle lesions were rated as having lower school competency and school QOL, and patients with 2-ventricle lesions were rated as having poorer behavioral regulation. Conclusions Survivors of complex CHD experience greater executive functioning difficulties than healthy peers, with metacognition particularly impacted and particularly relevant for day-to-day school performance. Especially in older children, clinicians should watch for metacognitive deficits, such as problems with organization, planning, self-monitoring, and follow-through on tasks.
ABSTRACT
Bright debris discs can contain large amounts of CO gas. This gas was thought to be a protoplanetary remnant until it was recently shown that it could be released in collisions of ...volatile-rich solids. As CO is released, interstellar UV radiation photodissociates CO producing CI, which can shield CO allowing a large CO mass to accumulate. However, this picture was challenged because CI is inefficient at shielding if CO and CI are vertically mixed. Here, we study for the first time the vertical evolution of gas to determine how vertical mixing affects the efficiency of shielding by CI. We present a 1D model that accounts for gas release, photodissociation, ionization, viscous evolution, and vertical mixing due to turbulent diffusion. We find that if the gas surface density is high and the vertical diffusion weak (αv/α < H/r2) CO photodissociates high above the mid-plane, forming an optically thick CI layer that shields the CO underneath. Conversely, if diffusion is strong (αv/α > H/r2) CI and CO become well mixed, shortening the CO lifetime. Moreover, diffusion could also limit the amount of dust settling. High-resolution ALMA observations could resolve the vertical distribution of CO and CI, and thus constrain vertical mixing and the efficiency of CI shielding. We also find that the CO and CI scale heights may not be good probes of the mean molecular weight, and thus composition, of the gas. Finally, we show that if mixing is strong the CO lifetime might not be long enough for CO to spread interior to the planetesimal belt where gas is produced.
Pulmonary vascular dysfunction, pulmonary hypertension (PH), and resulting right ventricular (RV) failure occur in many critical illnesses and may be associated with a worse prognosis. PH and RV ...failure may be difficult to manage: principles include maintenance of appropriate RV preload, augmentation of RV function, and reduction of RV afterload by lowering pulmonary vascular resistance (PVR). We therefore provide a detailed update on the management of PH and RV failure in adult critical care.
A systematic review was performed, based on a search of the literature from 1980 to 2010, by using prespecified search terms. Relevant studies were subjected to analysis based on the GRADE method.
Clinical studies of intensive care management of pulmonary vascular dysfunction were identified, describing volume therapy, vasopressors, sympathetic inotropes, inodilators, levosimendan, pulmonary vasodilators, and mechanical devices. The following GRADE recommendations (evidence level) are made in patients with pulmonary vascular dysfunction: 1) A weak recommendation (very-low-quality evidence) is made that close monitoring of the RV is advised as volume loading may worsen RV performance; 2) A weak recommendation (low-quality evidence) is made that low-dose norepinephrine is an effective pressor in these patients; and that 3) low-dose vasopressin may be useful to manage patients with resistant vasodilatory shock. 4) A weak recommendation (low-moderate quality evidence) is made that low-dose dobutamine improves RV function in pulmonary vascular dysfunction. 5) A strong recommendation (moderate-quality evidence) is made that phosphodiesterase type III inhibitors reduce PVR and improve RV function, although hypotension is frequent. 6) A weak recommendation (low-quality evidence) is made that levosimendan may be useful for short-term improvements in RV performance. 7) A strong recommendation (moderate-quality evidence) is made that pulmonary vasodilators reduce PVR and improve RV function, notably in pulmonary vascular dysfunction after cardiac surgery, and that the side-effect profile is reduced by using inhaled rather than systemic agents. 8) A weak recommendation (very-low-quality evidence) is made that mechanical therapies may be useful rescue therapies in some settings of pulmonary vascular dysfunction awaiting definitive therapy.
This systematic review highlights that although some recommendations can be made to guide the critical care management of pulmonary vascular and right ventricular dysfunction, within the limitations of this review and the GRADE methodology, the quality of the evidence base is generally low, and further high-quality research is needed.
ABSTRACT
The question of what determines the width of Kuiper belt analogues (exoKuiper belts) is an open one. If solved, this understanding would provide valuable insights into the architecture, ...dynamics, and formation of exoplanetary systems. Recent observations by ALMA have revealed an apparent paradox in this field, the presence of radially narrow belts in protoplanetary discs that are likely the birthplaces of planetesimals, and exoKuiper belts nearly four times as wide in mature systems. If the parent planetesimals of this type of debris disc indeed form in these narrow protoplanetary rings via streaming instability where dust is trapped, we propose that this width dichotomy could naturally arise if these dust traps form planetesimals whilst migrating radially, e.g. as caused by a migrating planet. Using the dust evolution software dustpy, we find that if the initial protoplanetary disc and trap conditions favour planetesimal formation, dust can still effectively accumulate and form planetesimals as the trap moves. This leads to a positive correlation between the inward radial speed and final planetesimal belt width, forming belts up to ∼100au over 10 Myr of evolution. We show that although planetesimal formation is most efficient in low-viscosity (α = 10−4) discs with steep dust traps to trigger the streaming instability, the large widths of most observed planetesimal belts constrain α to values ≥4 × 10−4 at tens of au, otherwise the traps cannot migrate far enough. Additionally, the large spread in the widths and radii of exoKuiper belts could be due to different trap migration speeds (or protoplanetary disc lifetimes) and different starting locations, respectively. Our work serves as a first step to link exoKuiper belts and rings in protoplanetary discs.
