This paper examines the major minerals producing countries and the potential that local energy mix and renewable energy resources have for lowering the impact of the industry, with respect to carbon ...dioxide emissions. Furthermore, it seeks to estimate the potential for new technologies, energy efficiency and demand-side measures to reduce emissions. Of total carbon dioxide emissions, 71% arise from fossil fuels used in thermal applications and 29% from indirect electricity generation. We find the highest theoretical potential for reduction of emissions from thermal applications is from the increased use of renewable biofuels and charcoal instead of fossil fuels (up to 46% of total industry net emissions), while conversion of various applications to use renewable hydrogen may reduce emissions by 28%. Electricity generation emissions could be “substantially reduced” by switching to electricity generation from cleaner sources, with hydropower a key potential contributor (reduction of 22% of total industry emissions). Solar thermal and electrical production may only be capable of 2–7% reduction in the industry's emissions.
Early integration of sustainability considerations into the design of industrial operations offers great potential for the reduction of risks associated with a project. Recent experience in the ...development of a practical methodology for undertaking such “design for sustainability” has illustrated this point clearly. This paper examines the generic and specific risk reduction potential of applying a sustainability-based design process. This is illustrated through the analysis of two case studies. The case studies involved base metals extraction and processing and were located in low-mid development countries. By utilizing an overarching sustainability model, it was possible to elicit risks that could affect the sustainability performance of the resulting minerals operation and consequently identify potential initiatives or strategies that could mitigate these risks. As both case study projects are at the early stages of project development, the risk mitigation initiatives or strategies can be developed in concert with the normal project development activities. As the opportunities for reducing sustainability-associated risks are closely related to the project phase, the earlier (e.g. concept or pre-feasibility) such a methodology is applied in the life of a project the better. Progressing down the project life cycle (e.g. basic or detailed design), elements of flexibility are lost, and therefore the potential to derive greater sustainability benefits and minimize risk at least cost is diminished. Risk reduction through early assessment and integration of sustainability in design makes a significant contribution to the social license to operate the resulting operation.

Setting
The Institute for Dementia Research and Prevention at the Pennington Biomedical Research Center.
Participants
Ten independently-living, cognitively-healthy older adults (mean age: 67.3 + 2.01 years). Intervention: Daily consumption of an investigational product (trade name “Cerbella TM”) consisting of an emulsified liquid combination of standardized fish oil, panax ginseng extract, and green tea catechins in a flavored base of lecithin phospholipids optimized to maximize bioavailability of the active ingredients.
Measurements
Before and after supplementation with the investigational product or placebo, participants completed cognitive tests including the Mini Mental State Exam (MMSE), Stroop test, Digit Symbol Substitution Test (DSST), and Immediate and Delayed Recall tests, as well as functional magnetic resonance imaging (fMRI) during a standard cognitive task switching paradigm.
Results
Performance on the MMSE, Stroop test, and DSST increased significantly over one month of supplementation with the investigational product (one-sample t tests, p<.05) although differences between these changes and corresponding changes during supplementation with placebo were not significant (two-sample t tests, p>.05). During supplementation with the investigational product, brain activation during task performance increased significantly more than during supplementation with placebo in brain regions known to be activated by this task (anterior and posterior cingulate cortex). Functional connectivity during task execution between task regions (middle frontal gyrus and anterior cingulate cortex) increased significantly during supplementation with the investigational product, relative to placebo. Functional connectivity during rest between task regions (precentral gyrus and middle frontal gyrus) and default mode network regions (medial frontal gyrus and precuneus) decreased during supplementation with the investigational product relative to placebo, suggesting greater segregation of task and rest related brain activity.
Conclusion
One-month supplementation with a combination of omega 3 essential fatty acids, green tea catechins, and ginsenosides was associated with suggestive changes in cognitive functioning as well as modification of brain activation and brain functional connectivity in cognitively healthy older adults.
A catalogue of molecular aberrations that cause ovarian cancer is critical for developing and deploying therapies that will improve patients' lives. The Cancer Genome Atlas project has analysed ...messenger RNA expression, microRNA expression, promoter methylation and DNA copy number in 489 high-grade serous ovarian adenocarcinomas and the DNA sequences of exons from coding genes in 316 of these tumours. Here we report that high-grade serous ovarian cancer is characterized by TP53 mutations in almost all tumours (96%); low prevalence but statistically recurrent somatic mutations in nine further genes including NF1, BRCA1, BRCA2, RB1 and CDK12; 113 significant focal DNA copy number aberrations; and promoter methylation events involving 168 genes. Analyses delineated four ovarian cancer transcriptional subtypes, three microRNA subtypes, four promoter methylation subtypes and a transcriptional signature associated with survival duration, and shed new light on the impact that tumours with BRCA1/2 (BRCA1 or BRCA2) and CCNE1 aberrations have on survival. Pathway analyses suggested that homologous recombination is defective in about half of the tumours analysed, and that NOTCH and FOXM1 signalling are involved in serous ovarian cancer pathophysiology.

Penicillium marneffei is a thermally dimorphic fungus that causes severe human immunodeficiency virus‐related opportunistic infection in endemic areas of Southeast Asia and has rarely been reported ...in solid organ transplant (SOT) recipients. We report here the case of an Australian renal transplant patient who presented with disseminated P. marneffei infection shortly after a 10‐day holiday to Vietnam, and review all previously published cases of penicilliosis associated with renal transplantation. This is the first reported case, to our knowledge, of P. marneffei infection in an SOT recipient acquired during travel to an endemic country, and highlights the importance of an accurate travel history when opportunistic infection is suspected, as well as giving appropriate health advice to transplant patients who travel.
The purpose of this study was to examine the effect of repeated doses of caffeine on repeated exercise endurance. METHODS Nine male caffeine users performed exercise rides (ER) to exhaustion at 80% ...VO(2max) after ingesting a placebo, 5 mg x kg-1 of caffeine, or 2.5 mg x kg-1 of caffeine 1 h before the ER. Two ER were performed weekly on the same day once in the morning (AM) and 5 h later in the afternoon (PM). There were four treatments containing either caffeine or placebo, i.e., trial A representing 5-mg x kg-1 caffeine in the AM and 2.5-mg x kg-1 caffeine in the PM; trial B, which was placebo in both AM and PM; trial C representing 5-mg x kg-1 caffeine in the AM and placebo in the PM; and trial D representing a placebo in the AM and 5-mg x kg-1 caffeine in the PM. The order of the treatment trials was double blind and randomized.
Caffeine ingestion significantly increased exercise time to exhaustion in the AM (trial A 24.9 +/- 10.2 min and trial C 21.8 +/- 4.9 vs trial B 18.0 +/- 6.4 min and D 17.7 +/- 4.3 min). This effect was maintained in the PM and greater than placebo (B 18.3 +/- 4.8 min) regardless of whether redosing (trial A 21.5 +/- 8.6 min) or placebo (trial C 21.0 +/- 6.8) followed the initial morning dose. Caffeine dosing in the PM (trial D 22.4 +/- 7.2 min) also increased ER after placebo trial D in the AM.
It was concluded that redosing with caffeine after exhaustive exercise in the AM was not necessary to maintain the ergogenic effect of the drug during subsequent exercise 6 h later.
This paper reviews the tools and methodologies used for incorporating sustainability considerations into the design of mineral processing operations. It was found that while there is a range of tools ...and methodologies that contribute to Design for Sustainability, there is no consistent, integrated approach to support the mineral industry in incorporating a greater level of sustainability into the design process. This paper identifies the required elements in such an approach and discusses the ways in which its development would progress the industry towards sustainability.
Catheter-related blood stream infection (CR-BSI) in patients with pulmonary hypertension (PH) receiving intravenous iloprost via an indwelling central line has previously not been fully described. ...Recent studies have suggested a link between the pH of prostanoid infusions and the rate and nature of CR-BSI. We have investigated CR-BSI in patients receiving intravenous iloprost at our unit. Databases and hospital records were interrogated for all patients receiving intravenous iloprost between September 2007 and June 2012. Fifty-nine patients received intravenous iloprost via an indwelling central catheter with a total of 23,072 treatment days. There were 15 episodes of CR-BSI, identified using a systematic screening protocol, involving 11 patients giving an overall CR-BSI rate of 0.65/1,000 treatment days. CR-BSI rate for Gram-positive organisms was 0.26/1,000 treatment-days and for Gram-negative organisms was 0.39/1,000 treatment-days. The pH of iloprost in typical dosing regimens was comparable to the pH used in standard-diluent treprostinil and dissimilar to alkaline epoprostenol infusions. The proportion of Gram-negative CR-BSI was similar to that reported for standard-diluent treprostinil. CRP was normal on admission in 33 % of cases of confirmed CR-BSI and remained normal in 13 % of cases. CR-BSI rates with intravenous iloprost are comparable to those observed for other prostanoids. The high proportion of Gram-negative organisms observed and the neutral pH of iloprost infusions support the previously hypothesised link between pH and antimicrobial activity. Although usually elevated during a CR-BSI, CRP may be normal in early infection and a normal result cannot completely exclude infection.
The purpose of the present study was to examine the duration of caffeine's ergogenic effect and whether it differs between users and nonusers of the drug. Twenty-one subjects (13 caffeine users and 8 ...nonusers) completed six randomized exercise rides to exhaustion at 80% of maximal oxygen consumption after ingesting either a placebo or 5 mg/kg of caffeine. Exercise to exhaustion was completed once per week at either 1, 3, or 6 h after placebo or drug ingestion. Exercise time to exhaustion differed between users and nonusers with the ergogenic effect being greater and lasting longer in nonusers. For the nonusers, exercise times 1, 3, and 6 h after caffeine ingestion were 32.7 +/- 8.4, 32.1 +/- 8.6, and 31.7 +/- 12.0 min, respectively, and these values were each significantly greater than the corresponding placebo values of 24.2 +/- 6.4, 25.8 +/- 9.0, and 23.2 +/- 7.1 min. For caffeine users, exercise times 1, 3, and 6 h after caffeine ingestion were 27.4 +/- 7.2, 28.1 +/- 7.8, and 24.5 +/- 7.6 min, respectively. Only exercise times 1 and 3 h after drug ingestion were significantly greater than the respective placebo trials of 23.3 +/- 6.5, 23.2 +/- 7.1, and 23.5 +/- 5.7 min. In conclusion, both the duration and magnitude of the ergogenic effect that followed a 5 mg/kg dose of caffeine were greater in the nonusers compared with the users.
Deglutition-induced atrial tachycardia is a rare arrhythmia with a poor response to medical therapy. Electrophysiological study is challenging due to the dependence of induction on swallowing. We ...present a novel approach to management of deglutition-induced atrial tachycardia arising from right superior pulmonary vein. Use of minimal conscious sedation and repeated swallow challenge inductions, together with contact force-guided mapping were key determinants of success. We review published cases, discussing potential mechanisms including oesophageal distension and neural reflexes.
