Introduction. Currently the prevalence of pneumococcal coinfection in patients with COVID-19 is unknown. In this work we present its clinical characteristics, evolution and treatment. Material and ...methods. Retrospective data collection from August to October 2020 in two hospitals in the Murcia region. Results. Eighteen patients had COVID-19 diagnosed by PCR and pneumococcal infection confirmed by antigenuria, which represented a prevalence of 2%. A total of 88% had radiological alterations upon admission (two patients had an X-ray within normality) and 29% had elevated procalcitonin. Mortality in our series was 12%. Conclusions. It could be reasonable to consider the start of antimicrobial therapy in those cases in which there is a moderate or high suspicion of bacterial coinfection, being essential the early suspension of antibiotic treatment if it is not confirmed
The aim of this study is to evaluate the relationship between psycho-social functioning and symptoms, cognitive function, and premorbid adjustment, in patients with a first-episode psychosis. Method: ...Clinical data were obtained from 90 patients, who were assessed with the Disability Assessment Scale (DAS-sv), the Positive and Negative Syndrome Scale (PANSS), the Premorbid Adjustment Scale (PAS-S) and with a battery of cognitive tests including Trail Making Tests A and B (TMTA- B), Continous Performance Test (CPT), some subscales of the Wechler Adult Intelligence Scale (WAIS), and the Verbal Learning Test España-Complutense (TAVEC). Results: The results of the study suggest that psycho-social functioning in first-episode psychosis is significantly related to: positive, negative, excitative, affective and disorganized symptoms, social premorbid adjustment, cognitive flexibility, working memory, short term and long term memory. Of these, those which best explained psycho-social functioning are the positive and excitative symptoms, premorbid adjustment, flexibility and memory. Conclusions: These findings highlight the importance early intervention on cognitive and clinical variables to help provide a better psycho-social functioning in people with a first-episode of psychosis.
●Social functioning is related to cognition, pre-morbid adjustment and psychotic symptoms.●Memory functions and flexibility explain social disability in first-episode psychosis.●Psychological interventions are crucial to achieve satisfactory social functioning.

Each year it is estimated in the United States an approximate of 8 million fractures; 5 to 10% develop delayed union or absence of periosteal new bone. There are several factors that can cause delay ...in fracture healing, among the well known, is the use of prophylactic antithrombotic therapy for deep vein thrombosis (DVT). DVT appears in 40 to 60% of the patients undergoing orthopedic surgery without prophylactic antithrombotic therapy. The goal of this study was to assess whether there is a difference in time of bone healing in lower limb fractures (femur and tibia) comparing rivaroxaban to enoxaparin as the prophylactic antithrombotic management.
We present a retrospective observational and analytic study in a sample of cases. It is a cross-sectional study with patient data from the database of the American British Cowdray (ABC) Medical Center. We included patients with femur and tibia fractures under antithrombotic prophylactic management with rivaroxaban or enoxaparin during the period of January 2011 to December 2012. Our sample included 32 patients separated into two groups. Students t-test was used for comparing parametric variables and the Mann-Whitney U test for nonparametric variables. Linear regression model was preformed considering the variables related to the time it took the fracture to heal.
All fractures consolidated in a time of 13 and 14 weeks for rivaroxaban and enoxaparin respectively (p = 0.67).
We found no difference in bone healing time for lower limb fractures in patients receiving antithrombotic prophylaxis treatment comparing rivaroxaban with enoxaparin.
This paper explores the relationship between levels of teacher education and socioeconomic characteristics of students with quality education that they themselves receive; the analysis takes place in ...the higher education sector over the period 2007-2012 in Colombia. By using data obtained from institutions, such as DANE and ICFES; together with the methodology of quantile regressions, descriptive statistics and basic econometrics, It manages to give a quantitative answer to the effect of a particular type of teacher in the quality of a student, emphasizing the importance of the role of masters and doctors in our institutions of higher education, and some clear public policy suggestions regarding investment in education and also an invitation to the scientific community to explore the special cases we came across. Abstract
Thermal recovery techniques such as CSS (Cyclic Steam Stimulation) induce significant changes in the stress path at the reservoir formation due to the induced thermal stresses in the ...wellbore and surrounding area. The understanding of phenomena regarding these stress paths is crucial to ensure safe operation and optimization of well productivity. Therefore, stress path analysis in CSS entails a challenge related to the number of focal points throughout the reservoir in which the stress paths should be studied. The reduction on the number of focal points is an advantageous step to ensure the appropriate analysis in extensive reservoirs with multiple sands and layers with different geomechanical and yield parameters. In the proposed grouping strategy, temperature and pore pressure are found to be key variables when determining the sets of stress paths with a common phenomenological trending behavior. In the current case study located in Middle Magdalena Valley basin, this grouping strategy allows a comprehensive analysis of 3 sets of stress path strategically located on the borehole wall, inside and outside the steam front with the aid of numerical simulation software (CMG). Considering that stress paths usually represent a single focal point behavior within the reservoir, once this grouping strategy is implemented, an analysis of the whole reservoir is simultaneously performed during CSS.
Rationale
First episode patients and patients with schizophrenia exhibit increased rates of cannabis use compared to the general population. Contrary to what has been reported in studies with healthy ...people, most of the published studies so far have reported no impairments or even beneficial effects on neurocognition associated with cannabis consumption in psychotic patients. However, these studies did not address the effects of very high cannabis consumption.
Objectives
Our aim in this study was to assess the effects on neurocognition of medium and heavy cannabis consumption in first psychotic episode patients.
Methods
A total of 74 patients were included in the study and assigned to three different groups according to their mean cannabis consumption during the last year (non-users, medium users, and heavy users). Participants were administered verbal memory and other neurocognitive tasks.
Results
Heavy cannabis users were significantly impaired in all the verbal memory measures with respect to non-users, including immediate (
p
= .026), short-term (
p
= .005), and long-term (
p
= .002) memory. There were no significant differences between medium and non-users. Moreover, non-users performed better than all cannabis users in the arithmetic task (
p
= .020). Heavy cannabis consumption was associated with more commission errors in the continuous performance task (CPT) (
p
= .008) and more time to complete trail making test A (TMT-A) (
p
= .008), compared to the group of medium users.
Conclusions
Heavy cannabis consumption seems to impair verbal memory in first psychotic episode patients. Heavy users also perform worse than medium users in other neurocognitive tasks. Based on the results and the available evidence, a dose-related effect of cannabis consumption is suggested.
