Abstract A randomized, double-blind, placebo-controlled clinical trial was conducted to evaluate the safety, reactogenicity and the immunogenicity of a 2 × 109 CFU dose of the 638 lyophilized live ...attenuated cholera vaccine for oral administration, formulated and produced at Finlay Institute, City of Havana, Cuba. Thirty-six healthy female and male adult volunteers from 18 to 40 years old were involved, clinically examined and laboratory tested after the informed consent signature. Adverse events were monitored and seroconversion rates and geometrical mean titer (GMT) of vibriocidal antibodies were tested in volunteer's sera samples. Neither serious adverse events nor other damages to the volunteers due to vaccine or placebo feeding were reported during the clinical follow-up period of this study; none of the adverse events registered within the first 72 h after inoculation were life-threatening for volunteers. Neither severe nor moderate adverse events were reported. Sixty-one percent of subjects showed mild expected adverse events in an interval lower than 24 h up to the first 72 h, 75% of these in the vaccinated group and 18% in the placebo group. Fourteen days after inoculation the GMT of vibriocidal antibodies in the vaccine group significantly increased in comparison to the placebo group. All subjects in the vaccine group (24) seroconverted (100%). Results show that this vaccine is safe, well tolerated and immunogenic in healthy female and male volunteers.
ObjectivesTo compare the effectiveness of oral versus intramuscular (IM) vitamin B12 (VB12) in patients aged ≥65 years with VB12 deficiency.DesignPragmatic, randomised, non-inferiority, multicentre ...trial in 22 primary healthcare centres in Madrid (Spain).Participants283 patients ≥65 years with VB12 deficiency were randomly assigned to oral (n=140) or IM (n=143) treatment arm.InterventionsThe IM arm received 1 mg VB12 on alternate days in weeks 1–2, 1 mg/week in weeks 3–8 and 1 mg/month in weeks 9–52. The oral arm received 1 mg/day in weeks 1–8 and 1 mg/week in weeks 9–52.Main outcomesSerum VB12 concentration normalisation (≥211 pg/mL) at 8, 26 and 52 weeks. Non-inferiority would be declared if the difference between arms is 10% or less. Secondary outcomes included symptoms, adverse events, adherence to treatment, quality of life, patient preferences and satisfaction.ResultsThe follow-up period (52 weeks) was completed by 229 patients (80.9%). At week 8, the percentage of patients in each arm who achieved normal B12 levels was well above 90%; the differences in this percentage between the oral and IM arm were −0.7% (133 out of 135 vs 129 out of 130; 95% CI: −3.2 to 1.8; p>0.999) by per-protocol (PPT) analysis and 4.8% (133 out of 140 vs 129 out of 143; 95% CI: −1.3 to 10.9; p=0.124) by intention-to-treat (ITT) analysis. At week 52, the percentage of patients who achieved normal B12 levels was 73.6% in the oral arm and 80.4% in the IM arm; these differences were −6.3% (103 out of 112 vs 115 out of 117; 95% CI: −11.9 to −0.1; p=0.025) and −6.8% (103 out of 140 vs 115 out of 143; 95% CI: −16.6 to 2.9; p=0.171), respectively. Factors affecting the success rate at week 52 were age, OR=0.95 (95% CI: 0.91 to 0.99) and having reached VB12 levels ≥281 pg/mL at week 8, OR=8.1 (95% CI: 2.4 to 27.3). Under a Bayesian framework, non-inferiority probabilities (Δ>−10%) at week 52 were 0.036 (PPT) and 0.060 (ITT). Quality of life and adverse effects were comparable across groups. 83.4% of patients preferred the oral route.ConclusionsOral administration was no less effective than IM administration at 8 weeks. Although differences were found between administration routes at week 52, the probability that the differences were below the non-inferiority threshold was very low.Trial registration numbersNCT 01476007; EUDRACT (2010-024129-20).
This paper aims to develop an approach for the reconfiguration of a parallel
kinematic manipulator (PKM) with four degrees of freedom (DoF) designed to
tackle tasks of diagnosis and rehabilitation in ...an injured knee. The original
layout of the 4-DoF manipulator presents Type-II singular configurations within
its workspace. Thus, we proposed to reconfigure the manipulator to avoid such
singularities (owing to the Forward Jacobian of the PKM) during typical
rehabilitation trajectories. We achieve the reconfiguration of the PKM through
a minimization problem where the design variables correspond to the anchoring
points of the robot limbs on fixed and mobile platforms. The objective function
relies on the minimization of the forces exerted by the actuators for a
specific trajectory. The minimization problem considers constraint equations to
avoid Type-II singularities, which guarantee the feasibility of the active
generalized coordinates for a particular path. To evaluate the proposed
conceptual strategy, we build a prototype where reconfiguration occurs by
moving the position of the anchoring points to holes bored in the fixed and
mobile platforms. Simulations and experiments of several study cases enable
testing the strategy performance. The results show that the reconfiguration
strategy allows obtaining trajectories having minimum actuation forces without
Type-II singularities.
To determine the dilution titles at antinuclear antibodies (ANA) by indirect immunofluorescence observed in cell substrate HEp-2 and its association with the diagnosis of systemic connective tissue ...disease in ANA test requested by a Rheumatology Unit.
Samples of patients attended for the first time in the rheumatology unit, without prior ANA test, between January 2010 and December 2012 were selected. The dilution titers, immunofluorescence patterns and antigen specificity were recorded. In January 2015 the diagnosis of the patients were evaluated and classified in systemic disease connective tissue (systemic lupus erythematosus, Sjögren's syndrome, systemic sclerosis, undifferentiated connective, antiphospholipid syndrome, mixed connective tissue and inflammatory myophaty) or not systemic disease connective tissue.
A total of 1282 ANA tests requested by the Rheumatology Unit in subjects without previous study, 293 were positive, predominance of women (81.9%). Patients with systemic connective tissue disease were recorded 105, and 188 without systemic connective tissue disease. For 1/640 dilutions the positive predictive value in the connective was 73.3% compared to 26.6% of non-connective, and for values ≥1/1280 85% versus 15% respectively. When performing the multivariate analysis we observed a positive association between 1/320 dilution OR 3.069 (95% CI: 1.237–7.614; P=.016), 1/640 OR 12.570 (95% CI: 3.659–43.187; P=.000) and ≥1/1280 OR 42.136 (95% CI: 8.604–206.345; P=.000).
These results show association titles dilution ≥1/320 in ANA's first test requested by a Rheumatology Unit with patients with systemic connective tissue disease. The VPP in these patients was higher than previous studies requested by other medical specialties. This may indicate the importance of application of the test in a targeted way.
Determinar los niveles en los títulos de anticuerpos antinucleares (ANA) observados por inmunofluorescencia indirecta en sustrato de célula HEp-2, y su asociación con el diagnóstico de enfermedad del tejido conectivo sistémica en las pruebas solicitadas por una Unidad de Reumatología.
Se seleccionaron muestras de pacientes que acudían por primera vez a consulta de reumatología, sin prueba de ANA previa, durante el periodo comprendido entre enero de 2010 y diciembre de 2012. Se registró el título de dilución, patrón y especificidad antigénica. En enero de 2015 se valoraron los diagnósticos de los pacientes y se clasificaron en conectivopatías sistémicas (lupus eritematoso sistémico, síndrome de Sjögren, esclerosis sistémica, conectivopatía indiferenciada, síndrome antifosfolípido, enfermedad mixta del tejido conectivo y miopatía inflamatoria) o no conectivopatía sistémica.
De un total de 1.282 pruebas solicitadas por la Unidad de Reumatología en sujetos sin estudio previo 293 resultaron positivas, predominando las mujeres (81,9%). Con conectivopatía sistémica se registraron 105 pacientes y 188 sin conectivopatía. En diluciones 1/640 el valor predictivo positivo en las conectivopatías fue de 73,3% frente al 26,6% de las no conectivopatías, y para valores ≥1/1.280, 85% frente al 15% respectivamente. Al realizar el análisis multivariante se observó una asociación positiva entre las diluciones 1/320 OR 3,069 (IC 95%: 1,237-7,614; p=0,016), 1/640 OR 12,570 (IC 95%: 3,659-43,187; p=0,000) y ≥1/1.280 OR 42,136 (IC 95%: 8,604-206,345; p=0,000).
Estos resultados muestran asociación de títulos de dilución ≥1/320 para la primera prueba de ANA realizada en una Unidad de Reumatología con pacientes con conectivopatía sistémica. El VPP en estos pacientes resultó superior a estudios previos desarrollados por otras especialidades médicas. Esto puede indicar la importancia de una solicitud de la prueba de forma dirigida.
El embarazo ectópico se considera por algunos autores, según su comportamiento en este decenio, como enfermedad epidémica muchas veces mortal para las mujeres que lo padecen, por lo cual nos ...propusimos revisar y analizar la evidencia científica disponible en la literatura mundial para determinar las manifestaciones clínicas más frecuentes de esta enfermedad, así como los medios diagnóstico más empleados en la actualidad. Lo anterior puede concluirse planteando que los antecedentes más importantes referidos por éstas fueron la enfermedad inflamatoria pélvica, el uso de dispositivos intrauterinos, abortos inducidos, embarazo ectópico anterior y el uso de técnicas de reproducción asistida. La asociación de dolor bajo vientre y trastornos menstruales con dolor a la palpación abdominal o al tacto vaginal en presencia de masa pélvica son altamente sugestivos de embarazo ectópico. La combinación de ultrasonido transvaginal con determinación de fracción beta de gonadotropina coriónica muestran una alta especificidad y sensibilidad. Mediante la ecografía vaginal con imagen de flujo en color con transductor de alta frecuencia es posible establecer un diagnóstico más preciso y rápido del embarazo ectópico. La laparoscopia continúa siendo un medio sencillo para llegar al diagnóstico definitivo.
Se realizó un estudio observacional descriptivo para determinar los procedimientos y medios diagnósticos aplicados en la detección del embarazo ectópico en el Hospital Ginecobstétrico Docente ...Provincial "Ana Josefa Betancourt de Mora" de Camagüey, en el período comprendido desde el 1 de enero de 1998 hasta el 30 de junio de 1999. De las 651 pacientes que ingresaron presuntamente con este diagnóstico, a 214 se les confirmó por estudio anatomopatológico y para procesar la información se emplearon métodos de estadística descriptiva, distribución de frecuencia y porcientos. La mayoría de las féminas contaban con edades entre 26 y 35 años, la enfermedad inflamatoria pélvica y los abortos inducidos fueron los antecedentes ginecológicos más comunes en el 29.44 y 13.8 % respectivamente, se destacó la asociación de dolor pélvico y retraso menstrual unido a exámenes abdominal y vaginal dolorosos con tumoración anexial. De las ecografías realizadas, el 99.08 % arrojó imágenes sugestivas de gestación ectópica; la culdocentesis resultó positiva en el 85.16 % y la laparoscopia alcanzó el 100 % de positividad. Se demostró relación entre el mayor tiempo transcurrido para el diagnóstico (pasados siete días) y la forma accidentada del embarazo ectópico.
