Abstract
Typical black hole binaries in outburst show spectral states and transitions, characterized by a clear connection between the inflow on to the black hole and outflows from its vicinity. The ...transient stellar mass black hole binary V404 Cyg apparently does not fit in this picture. Its outbursts are characterized by intense flares and intermittent plateau and low-luminosity states, with a dynamical intensity range of several orders of magnitude on time-scales of hours. During the 2015 June–July X-ray outburst a joint Swift and INTEGRAL observing campaign captured V404 Cyg in one of these plateau states. The simultaneous Swift/XRT + INTRGRAL/JEM-X + INTEGRAL/IBIS-ISGRI spectrum is reminiscent of that of obscured/absorbed active galactic nuclei (AGN). It can be modelled as a Comptonization spectrum, heavily absorbed by a partial covering, high column density material (N
H ≈ 1–3 × 1024 cm−2), and a dominant reprocessed component, including a narrow iron Kα line. Such spectral distribution can be produced by a geometrically thick accretion flow able to launch a clumpy outflow, likely responsible for both the high intrinsic absorption and the intense reprocessed emission observed. Similarly to what happens in certain obscured AGN, the low-flux states might not be (solely) related to a decrease in the intrinsic luminosity, but could instead be caused by an almost complete obscuration of the inner accretion flow.

No studies have evaluated the potential of egg oral immunotherapy (egg‐OIT) to induce sustained unresponsiveness after discontinuing therapy following short‐term treatments.

We assessed the efficacy of short‐course egg‐OIT to induce sustained unresponsiveness.
Methods
Sixty‐one egg‐allergic children, 5 to 17 years old, with positive double‐blind placebo‐controlled food challenge (DBPCFC) to dehydrated egg white (EW) were randomized to receive egg‐OIT (OITG) for 3 months (maintenance dose one undercooked egg every 48 hours) or to continue egg avoidance diet (control group, CG) for 4 months. Children who completed egg‐OIT avoided egg for 1 month. At 4 months, both groups underwent a DBPCFC. OITG participants who passed this challenge were instructed to add egg to their diet ad libitum. Immune markers were studied at different time points.
Results
Ninety‐three percent (28/30) of OITG children were desensitized in a median of 32.5 days (IQR, 14 days). At 4 months, 1/31 (3%) in CG passed DBPCFC and 11/30(37%) of OITG (95% CI, 14 to 51%; P = 0.003), all of them were consuming egg at 36 months. A decrease in EW, OVA and OVM skin test results and OVA‐specific IgE (sIgE) levels was observed on OITG at 4 months (P = 0.001). EW‐, OVA‐ and OVM‐sIgE levels prior to the start of egg avoidance diet were lower in OITG children who passed DBPCFC at 4 months than in those who did not pass it. EW‐ and OVM‐sIgE showed the best diagnostic performance in predicting DBPCFC result at 4 months. Levels above optimal EW‐sIgE cut‐off of 7.1 kU/L indicated 90% probability of failing DBPCFC.
Conclusion
This is the first demonstration of sustained unresponsiveness with a three‐month egg‐OIT protocol. Almost all treated subjects were desensitized and 37% achieved sustained unresponsiveness. EW‐sIgE levels at the end of treatment predicted sustained unresponsiveness. This protocol shows a new approach to OIT for egg‐allergic children.
Abstract
The black hole (BH) binary V404 Cyg entered the outburst phase in 2015 June after 26 yr of X-ray quiescence, and with its behaviour broke the outburst evolution pattern typical of most BH ...binaries. We observed the entire outburst with the Swift satellite and performed time-resolved spectroscopy of its most active phase, obtaining over a thousand spectra with exposures from tens to hundreds of seconds. All the spectra can be fitted with an absorbed power-law model, which most of the time required the presence of a partial covering. A blueshifted iron-Kα line appears in 10 per cent of the spectra together with the signature of high column densities, and about 20 per cent of the spectra seem to show signatures of reflection. None of the spectra showed the unambiguous presence of soft disc–blackbody emission, while the observed bolometric flux exceeded the Eddington value in 3 per cent of the spectra. Our results can be explained assuming that the inner part of the accretion flow is inflated into a slim disc that both hides the innermost (and brightest) regions of the flow, and produces a cold, clumpy, high-density outflow that introduces the high absorption and fast spectral variability observed. We argue that the BH in V404 Cyg might have been accreting erratically or even continuously at Eddington/super-Eddington rates – thus sustaining a surrounding slim disc – while being partly or completely obscured by the inflated disc and its outflow. Hence, the largest flares produced by the source might not be accretion-driven events, but instead the effects of the unveiling of the extremely bright source hidden within the system.

The Iowa Gambling Task (IGT) is one of the most widely used paradigms for assessing decision-making. An impairment in this process may be linked to several psychopathological disorders, such as obsessive–compulsive disorder (OCD), substance abuse disorder (SUD) or attention-deficit/hyperactivity disorder (ADHD), which could make it a good candidate for being consider a transdiagnostic domain. Resting-state functional connectivity (rsFC) has been proposed as a promising biomarker of decision-making. In this study, we aimed to identify idiosyncratic decision-making profiles among healthy people and impulsive-compulsive spectrum patients during the IGT, and to investigate the role of frontoparietal network (FPN) rsFC as a possible biomarker of different decision-making patterns. Using functional near-infrared spectroscopy (fNIRS), rsFC of 114 adults (34 controls; 25 OCD; 41 SUD; 14 ADHD) was obtained. Then, they completed the IGT. Hybrid clustering methods based on individual deck choices yielded three decision-makers subgroups. Cluster 1 (n = 27) showed a long-term advantageous strategy. Cluster 2 (n = 25) presented a maladaptive decision-making strategy. Cluster 3 (n = 62) did not develop a preference for any deck during the task. Interestingly, the proportion of participants in each cluster was not different between diagnostic groups. A Bayesian general linear model showed no credible differences in the IGT performance between diagnostic groups nor credible evidence to support the role of FPN rsFC as a biomarker of decision-making under the IGT context. This study highlights the importance of exploring in depth the behavioral and neurophysiological variables that may drive decision-making in clinical and healthy populations.

which is a software tool to assess evolutionary algorithms for Data Mining problems of various kinds including as regression, classification, ...unsupervised learning, etc. It includes evolutionary learning algorithms based on different approaches: Pittsburgh, Michigan and IRL, as well as the integration of evolutionary learning techniques with different pre-processing techniques, allowing it to perform a complete analysis of any learning model in comparison to existing software tools. Moreover, KEEL has been designed with a double goal: research and educational.
Aims. Hard X-ray spectra of black hole binaries (BHB) are produced by Comptonization of soft seed photons by hot electrons near the black hole. The slope of the resulting energy spectra is governed ...by two main parameters: the electron temperature (Te) and optical depth (τ) of the emitting plasma. Given the extreme brightness of V404 Cyg during the 2015 outburst, we aim to constrain the source spectral properties using an unprecedented time resolution in hard X-rays, and to monitor the evolution of Te and τ over the outburst. Methods. We have extracted and analysed 602 X-ray spectra of V404 Cyg obtained by the IBIS/ISGRI instrument on-board INTEGRAL during the 2015 June outburst, using effective integration times ranging between 8 and 176 000 s. We fitted the resulting spectra in the 20–200 keV energy range. Results. We find that while the light curve and soft X-ray spectra of V404 Cyg are remarkably different from those of other BHBs, the spectral evolution of V404 Cyg in hard X-rays and the relations between the spectral parameters are consistent with those observed in other BHBs. We identify a hard branch in which the Te is anti-correlated with the hard X-ray flux, and a soft flaring branch in which the relation reverses. In addition, we find that during long X-ray plateaus detected at intermediate fluxes, the thermal Comptonization models fail to describe the spectra. However, the statistics improve if we allow NH to vary freely in the fits to these spectra. Conclusions. We conclude that the hard branch in V404 Cyg is analogous to the canonical hard state of BHBs. V404 Cyg never seems to enter the canonical soft state, although the soft flaring branch bears resemblance to the BHB intermediate state and ultra-luminous state. The X-ray plateaus are likely the result of absorption by a Compton-thick outflow (NH ≳ 1024 cm-2) which reduces the observed flux by a factor of about 10. Variable covering of the central source by this Compton-thick material may be the reason for the complicated light curve variability, rather than intrinsic source variability.
Advances in skin tissue engineering have promoted the development of artificial skin substitutes to treat large burns and other major skin loss conditions. However, one of the main drawbacks to ...bioengineered skin is the need to obtain a large amount of viable epithelial cells in short periods of time, making the skin biofabrication process challenging and slow. Enhancing skin epithelial cell cultures by using mesenchymal stem cells secretome can favor the scalability of manufacturing processes for bioengineered skin. The effects of three different types of secretome derived from human mesenchymal stem cells, e.g. hADSC-s (adipose cells), hDPSC-s (dental pulp) and hWJSC-s (umbilical cord), were evaluated on cultured skin epithelial cells during 24, 48, 72 and 120 h to determine the potential of this product to enhance cell proliferation and improve biofabrication strategies for tissue engineering. Then, secretomes were applied in vivo in preliminary analyses carried out on Wistar rats. Results showed that the use of secretomes derived from mesenchymal stem cells enhanced currently available cell culture protocols. Secretome was associated with increased viability, proliferation and migration of human skin epithelial cells, with hDPSC-s and hWJSC-s yielding greater inductive effects than hADSC-s. Animals treated with hWJSC-s and especially, hDPSC-s tended to show enhanced wound healing in vivo with no detectable side effects. Mesenchymal stem cells derived secretomes could be considered as a promising approach to cell-free therapy able to improve skin wound healing and regeneration.
Baryon Acoustic Oscillations (BAOs) provide a 'standard ruler' of known physical length, making it one of the most promising probes of the nature of dark energy (DE). The detection of BAOs as an ...excess of power in the galaxy distribution at a certain scale requires measuring galaxy positions and redshifts. 'Transversal' (or 'angular') BAOs measure the angular size of this scale projected in the sky and provide information about the angular distance. 'Line-of-sight' (or 'radial') BAOs require very precise redshifts, but provide a direct measurement of the Hubble parameter at different redshifts, a more sensitive probe of DE. The main goal of this paper is to show that it is possible to obtain photometric redshifts with enough precision ( sigma z ) to measure BAOs along the line of sight. There is a fundamental limitation as to how much one can improve the BAO measurement by reducing sigma z . We show that sigma z ~ 0.003(1 + z) is sufficient: a much better precision will produce an oversampling of the BAO peak without a significant improvement on its detection, while a much worse precision will result in the effective loss of the radial information. This precision in redshift can be achieved for bright, red galaxies, featuring a prominent 4000 A break, by using a filter system comprising about 40 filters, each with a width close to 100 A, covering the wavelength range from ~4000 to ~8000 A, supplemented by two broad-band filters similar to the Sloan Digital Sky Survey u and z bands. We describe the practical implementation of this idea, a new galaxy survey project, PAU16Physics of the Accelerating Universe (PAU): http://www.ice.cat/pau., to be carried out with a telescope/camera combination with an etendue about 20 m2 deg2, equivalent to a 2 m telescope equipped with a 6 deg2 field of view camera, and covering 8000 deg2 in the sky in four years. We expect to measure positions and redshifts for over 14 million red, early-type galaxies with L > L and iAB 22.5 in the redshift interval 0.1 < z < 0.9, with a precision sigma z < 0.003(1 + z). This population has a number density n 10 -3 Mpc -3 h 3 galaxies within the 9 Gpc3 h -3 volume to be sampled by our survey, ensuring that the error in the determination of the BAO scale is not limited by shot noise. By itself, such a survey will deliver precisions of order 5% in the dark-energy equation of state parameter w, if assumed constant, and can determine its time derivative when combined with future cosmic microwave background measurements. In addition, PAU will yield high-quality redshift and low-resolution spectroscopy for hundreds of millions of other galaxies, including a very significant high-redshift population. The data set produced by this survey will have a unique legacy value, allowing a wide range of astrophysical studies.
This paper analyses how gender-based disparities in the Euro-Area affect women’s job satisfaction using the EWCS (2015), and the Global Gender Gap Index introduced by the World Economic Forum. ...Heckman's two-stage estimates show that women have a higher probability of job satisfaction than their male colleagues, which endorses the paradox of the female contented worker. There does not seem to be an equalization of job satisfaction as higher educational levels and lower age groups are considered. In those settings where the situation of women is more unfavourable than that of men, the probability for women to be more satisfied at work is lower. Therefore, the adaptive expectations hypothesis, by which individuals would internalize the difficulties they face and, ceteris paribus, would experience greater satisfaction than their counterparts, in this case males, is not corroborated.
BACKGROUNDPassive immunotherapy with convalescent plasma (CP) is a potential treatment for COVID-19. Evidence from controlled clinical trials is inconclusive.METHODSWe conducted a randomized, ...open-label, controlled clinical trial at 27 hospitals in Spain. Patients had to be admitted for COVID-19 pneumonia within 7 days from symptom onset and not on mechanical ventilation or high-flow oxygen devices. Patients were randomized 1:1 to treatment with CP in addition to standard of care (SOC) or to the control arm receiving only SOC. The primary endpoint was the proportion of patients in categories 5 (noninvasive ventilation or high-flow oxygen), 6 (invasive mechanical ventilation or extracorporeal membrane oxygenation ECMO), or 7 (death) at 14 days. Primary analysis was performed in the intention-to-treat population.RESULTSBetween April 4, 2020, and February 5, 2021, 350 patients were randomly assigned to either CP (n = 179) or SOC (n = 171). At 14 days, proportion of patients in categories 5, 6, or 7 was 11.7% in the CP group versus 16.4% in the control group (P = 0.205). The difference was greater at 28 days, with 8.4% of patients in categories 5-7 in the CP group versus 17.0% in the control group (P = 0.021). The difference in overall survival did not reach statistical significance (HR 0.46, 95% CI 0.19-1.14, log-rank P = 0.087).CONCLUSIONCP showed a significant benefit in preventing progression to noninvasive ventilation or high-flow oxygen, invasive mechanical ventilation or ECMO, or death at 28 days.
